  • Visceral fat accumulation is known to be an evident clinical index for the insulin resistance related with obesity. Patients with excessive accumulation of visceral fat frequently suffered from metabolic disorder, such as hyperlipidemia, hypertension, and glucose intolerance. However, molecular mechanism for the pathogenesis of obesity-accompanied metabolic disorders has not been fully elucidated. It has been clarified that adipocytes in visceral fat area have different functions from subcutaneous fat area, and these differences might contribute the pathological significance of excessive accumulation of visceral fat for the accompanied insulin resistance and hyperinsulinemia.

  • It was attempted in the present study to determind the effect of Korean ginseng on acute viral(B type) hepatitis by double blind test clinically and the results were as follows. 1. The double blind test of 4 week period showed no statistical significance of the effect of ginseng on the disease. 2. The double blind test of 2 week period, however, showed a significant effect in such clinical symptoms as appetite, stomach ache, headache, chillness dizziness, nausea and vomitting, stool habit change and jaundice. Improvement of the blood serum level of transaminases, bilirubin. alkaline phosphatase and cholesterol Chillness feeding period was observed.

  • The parathyroid gland is probably the simplest endocrine organ in the body. The only cells of clinical significance are the parathyroid or chief cells. The primary signal that these cells listen to is calcium. Primary hyperparathyroidism is due to a parathyroid adenoma. The most common cause of hypercalcemia in veterinary medicine is hypercalcemia of malignancy associated with variety of neoplasms. Secondary hyperparathyroidism is due to a disease process, most commonly associated with renal and nutritional hyperparathyroidism. Primary and secondary hyperparathyroidism are markedly different in their clinical, laboratory, and pathogenic mechanism.

  • The significance of the enactment of the 「Act On The Safety Of And Support For Advanced Regenerative Medicine And Advanced Biological Products」 is to break away from the regulation of the Pharmaceutical Affairs Act and expand patient treatment opportunities through a medical technology approach to regenerative medicine, which is essentially a medical practice called 'transplantation'. However, more than a year after the law was enacted, clinical study has not been activated, with not a single high-risk study approved by the Ministry of Food and Drug Safety being approved. The reason is that despite the legal purpose of expanding patient treatment opportunities, the data requirements for clinical study approval are set in connection with drug development despite the insufficient legal basis, making it difficult for many researchers to meet the data requirements. Prior to the enactment of the Act, submitted data for clinical study on cell therapy products within the Pharmaceutical Affairs Act were cosiderably exempted from quality and non-clinical test data, but with the enforcement of the Advanced Regenerative Bio Act, quality and non-clinical test data are required in accordance with pharmaceuticals when applying for approval of a clinical study plan. To rectify this, when considering the identity of clinical study on advanced regenerative medicine to expand treatment opportunities, recognize that there are limitations in connection with drug development. And it is necessary to preserve the identity of clinical study on advanced regenerative medicine, and on the other hand, in the case of drug product approval, clinical study results should be utilized while specifying usage requirements. Therefore, with the power of the market and the voluntary motive of the clinical researcher, it is necessary to prepare the necessary data by themselves rather than the basic requirements for clinical study approval.

Critical Thinking of Clinical Nurses (임상간호사의 비판적 사고)

  • Introduction: Critical thinking involves identifying problem(s), assessing resources, and generating possible solutions and allows clinical nurses to decide which solution is the most reasonable under the given circumstances, taking into consideration the "hat ifs" and how they will affect the end result. This research was conducted to further understanding and identification of subjective factors in critical thinking in clinical nurses. Methods: The research design was a Q-Methodological Approach. Q-population was formulated from a non-structured questionnaire and interviews from 17 experienced clinical nurses. Thirty selected Q-statements were sorted by 30 experienced clinical nurses. Results: Four factors for critical thinking were identified: (1) Deductive reasoning based on causal relation, (2) Construction of an effective model based on patients' responses, (3) Formulating categories based on priorities for effective interventions, and (4) Judging validity of the situational significance on clinical performances. Conclusion: Critical thinking is an attitude and reasoning process. From this study, the frame of reference for clinical nurses in formulating critical thinking within the context of clinical settings is identified and indicates the way nurses utilize thinking skills when they care for patients and areas that need further exploration as nurses and faculty develop education systems to advance clinical performance competency.

Evaluation of Effects of a Clinical Reasoning Course among Undergraduate Nursing Students (시뮬레이션을 적용한 임상추론 교과목의 적용효과: 일 대학의 예를 중심으로)

  • Purpose: To evaluate undergraduate nursing students' ability in clinical competence, critical thinking, and problem solving following enrollment in a clinical reasoning course. Methods: A clinical reasoning course utilizing a human patient simulator and scenarios was offered to 22 senior students at a College of Nursing in Seoul. Students' clinical competence was measured with a checklist of 15 items by analyzing students' performance recorded on video tapes for eight scenarios. Critical thinking disposition and problem solving were measured by a self-administered questionnaire before and after the course. Data were analyzed using descriptive statistics and Wilcoxon signed-rank test. Results: The high scored items of clinical competence were: 'obtain relevant subjective/objective data', 'interpret vital signs', 'communicate with healthcare providers', and 'utilize standard precautions including handwashing.' Students' critical thinking and problem solving scores following the course were increased with statistical significance. Conclusion: A clinical reasoning course utilizing a human patient simulator creates a realistic clinical environment for nursing students and provides the opportunity to obtain clinical competence, critical thinking, and problem solving skills.

Image of Nurse Perceived by Nursing Student Before and After Their First Clinical Practice (간호학생이 지각한 첫 임상실습 전·후의 간호사 이미지)

  • Purpose: The purpose of this study was to identify changes in image of nurses of nursing students before and after their first clinical practice. Methods: The survey was performed to 60 nursing students in one university in Goseong-gun, Gangwon-do. All participants had their first clinical practice for 8 weeks at four general hospitals in Seoul. The data were collected by questionnaires and were analyzed with frequency, independent t-test, one-way ANOVA using SPSS 19.0. Results: After the first clinical practice, the mean score ($4.05{\pm}0.85$) of the image of nurse was slightly higher than that of the image ($4.03{\pm}0.93$) before the clinical practice. However, there was no statistical significance in the scores between before and after the clinical practice (t=-0.55, p=.589). In four subcategories, before the first clinical practice, professional image was the highest followed by traditional, personal, and social image in order but after the clinical practice, professional image was the highest followed by traditional, social, and personal image in the mean scores of the image of nurse. Conclusion: The findings of the study may be a basis for expanding nursing practice and education related to image of nurse.

A Study on Evaluation of Clinical Practice of Dept. of Dental Technology - Construction of items and grading of the evaluation table at clinical practice - (치기공과 임상실습평가에 관한 연구 - 임상실습평가표 항목구성내용과 배점을 중심으로 -)

  • This study is for the purpose of studying the validity of the construction of items and grading of the evaluation table at clinical practice used as the method of evaluation when clinical practice of Dept. of Dental Technology. 205 dental technicians in 4 cities were questioned to analyze the relation between items of the evaluation table at clinical practice and its grading. The collected materials were inspected by SAS V8 for Windows. The results are as follows: 1. Making up all the items and grading seem to be appropriate because they had the marks more than midium(more than 3.28). 2. Making up detailed items and grading seem to be trusted because they had the marks more than 3.30. 3.The significance level of the relation between items of the evaluation at clinical practice and its grading by t-inspection is p<0.0001 and so it appears to be meaningful. 4. It proved to be meaningful in result of regression analysis of the influence on the construction of the evaluation at clinical practice and its grading. With this result, further study is needed for the better methods of the evaluation in its adjustment and graduated grading among the items.

Satisfaction of Practice and Clinical Skill in Participation Learning and Observation Learning (참여학습과 관찰학습의 실습만족도 및 임상수기술)

  • Objective: The purpose of this study was to compare satisfaction of practice and clinical skill in different participation learning and observation learning, and to offer the basic data to promote quality of nursing education. Method: The study used a quasi-experimental, nonequivalent control group post-test design. The subjects were 62 nursing students of K college in Jeonbuk, and the period of data gathering was limited from 19 Nov. 2006 to 30 Nov. 2006. Experience group was taught by hand-on education and control group was taught by conventional education. The instrument tools included self-efficacy, satisfaction of practice and clinical skill. The collected data were analyzed by SPSS-PC programme. Result: The results of this study were as follows: There was statistically a significance difference between the two group about satisfaction of practice(t=2.011 p=.043), and clinical skill(t=11.997, p=.000). Self-efficacy showed a significantly positive correlation with satisfaction of practice(r=.476, p=.000) and clinical skill(r=.178, p=.014). Also, satisfaction of practice showed a significantly positive correlation with clinical skill(r=.l82, p=.000). Conclusion: In conclusion, this study suggests that hand-on education is an effective learning method to nursing students. By utilizing hand-on education, makes nursing students plan self-directed nursing performance and improve their clinical skills.

Clinical management of amelogenesis imperfecta in primary dentition

  • Amelogenesis imperfecta patients suffered common clinical problems of poor esthetics, teeth sensitivity, and loss of occlusal vertical dimension. Amelogenesis imperfecta is a group of inherited disorders primarily affecting dental enamel. Variants of amelogenesis imperfecta generally classified hypoplastic, hypocalcified, or hypomaturation types based on the primary enamel defects. The mildest problems were found in the pitted hypoplastic type whereas the most severe problems were encountered in the hypocalcified type amelogenesis imperfecta. Management stragies include composite resin veneer and jacket crowns for anterior teeth as well as steel crowns for posterior teeth. Knowledge of the clinical features and dental complications of each variants if amelogenesis impecta helps in the diagnosis of the condition and allows institution of early preventive measures. The objective of this paper is to provide a review of the current concepts of the wide spectrum of etiological factors involved in the pathogenesis of this significance clinical entity in the primary dentition.
