Adaptation of the Evidence Based Nursing Practice Guideline (근거중심 급성 뇌졸중 간호 가이드라인 수용개작)

  • Purpose: This methodological study translated, adapted, and validated the evidence based guideline of RNAO (Registered Nurses Association of Ontario) in Canada, for acute stroke care to domestic circumstances. Methods: Quality of the RNAO guideline was evaluated using AGREE tool. Then the guideline was translated into Korean and a preliminary guideline was established After checking content validation by an expert group, the Korean version of the guideline was applied to clinical settings to see its applicability. Results: The RNAO guideline in Korea was deemed appropriate. The Korean version of the guideline was drawn up with 56 items in 3 areas in stroke system related nursing, nursing of acute stroke inpatients, and stroke nursing assessment. A questionnaire survey was performed on appropriateness, enforceability, and applicability of those recommendations. A total of 54 recommendations were finalized after deleting 2 items below CVI 0.7 and correcting 3 items by taking professional advices. After trial application of the guideline to 40 stroke patients hospitalized at a ward of a hospital in Seoul, its performance was improved but was not statistically significant. Conclusion: This guideline is expected to contribute to improving nursing quality by offering it as a guide to evidence based practices for acute stroke care in Korea.

Effects of Internal Oriented Policy, Social Support, and Job Embeddedness by Turnover Intention of Nurses (간호사의 내부지향적 정책, 사회적지지 및 직무배태성이 이직의도에 미치는 영향)

  • This study is a descriptive research to investigate the effects of nurses' internal orientation, social support and job orientation on turnover intention. The subjects were two university hospital nurses in D metropolitan city who agreed to participate in the study. Data collection was conducted from October 2, 2018 to March 20, 2019. The data were analyzed using SPSS 22.0 program, frequency and percentage, mean and standard deviation, correlation analysis, t-test, ANOVA, Pearson's correlation, and multiple regression. The study results were 3.04 ± .62 points for internal orientation policy, 3.08 ± .55 points for social support, 3.81 ± .59 points for peers, 3.67 ± .60 points for family members, and 2.98 ± 0.43 points for job placement and intention to turnover. The degree of turnover decreased with higher internal orientation, social support and job orientation. An examination of the general characteristics of the factors affecting turnover intention showed significant results with clinical experience and working department. The higher the clinical experience, the higher the turnover intention, and the department of work showed the higher turnover intention in the order of surgical ward, medical ward, ICU, and OR. These results show the need to actively develop and educate programs to improve nurses' intention to turnover, as well as internal hospital-oriented policies, social support, and job placement.

Effect of Post Operative Daily Activity on Post-dural Puncture Headache after Spinal Anesthesia (척추마취수술 환자의 수술 후 일상활동이 경막천자 후 두통 발생에 미치는 효과)

  • Purpose: The purpose of this study was to compare and analyze differences in effects of postoperative daily activity on headaches in patients who underwent surgery under spinal anesthesia. Methods: The study was conducted with 219 adults, 20 years or older. Official approval (Approval number: KMC IRB 1434-01) was received from K university hospital clinical trials review board. The research design was a nonequivalent control group non-synchronized design with a daily activities group and the bed rest group. Data was collected after receiving written consent from the participants. Results: There were no participants in either group who experienced headaches. Changes in a physiological index were also not significantly different between the daily activities group and the bed rest group. Conclusion: The results indicate that allowing daily activities in the ward, rather than maintaining bed rest for 6 hours, the existing method of nursing care for the prevention of postoperative headaches, in spinal anesthesia patients, is not detrimental to these patients post operatively.

Relationship between Uncertainty, Sick Role Behaviors, and Quality of Life of Rehospitalized Patients underwent Percutaneous Coronary Intervention (관상동맥중재술을 받고 재입원한 환자의 불확실성, 환자역할행위 및 삶의 질의 관계)

  • Purpose: This study was conducted to investigate the relationship between uncertainty in illness and the future, sick role behavior with what diet, weight control, no smoking, abstinence, doctor visits, medications, etc, and quality of life of rehospitalized patients after percutaneous coronary intervention in a cardiology ward. Methods: A total of 120 patients participated in the study. Data were collected using a questionnaire and analyzed using t-test, ANOVA, $Scheff{\grave{e}}$ test, and Pearson's Correlation Coefficient. Results: The mean score for uncertainty was $3.45{\pm}1.08$. Sick role behavior of the patients showed a moderate value with a mean of $3.68{\pm}0.79$. The mean score for quality of life was $3.52{\pm}0.64$. Uncertainty in illness and the future was significantly correlated to sick role behavior with that diet, weight control, no smoking, abstinence, doctor visits, medications, etc (r=-.27, p=.002), and quality of life (r=-.35, p<.001), and sick role behaviors were significantly correlated to quality of life (r=.62, p<.001). Conclusion: The results implicate that there is a need to decrease the levels of uncertainty and reinforce positive behaviors by patients in order to improve their quality of life.

Levels of Partnership between Nurses and Parents of Hospitalized Children and the Quality of Pediatric Nursing Care as Perceived by Nurses

  • Purpose: This study investigated the relationship of the partnership between nurses and parents of hospitalized children with the quality of pediatric nursing care as perceived by pediatric nurses. Methods: A cross-sectional descriptive study was conducted among 90 nurses. The data collected were analyzed using the independent t-test and the Pearson correlation coefficient. Results: The level of partnership between nurses and parents showed significant differences by nurses' age (t=2.22, p=.029) and marital status (t=2.54, p=.013). The quality of pediatric nursing care was found to significantly differ by the nurses' age (t=2.62, p=.013), marital status (t=4.17, p<.001), whether nurses had children (t=2.53, p=.020), pediatric ward work experience (t=2.43, p=.020), and total work experience (t=2.51, p=.016). The level of partnership between nurses and parents was positively correlated with the quality of pediatric nursing care (r=.48, p<.001). Conclusion: To improve the quality of nursing care for hospitalized children and their parents, it is important to consider the specific characteristics of pediatric nurses, whose clinical skills should be improved.

Characteristics and Mortality Risk Factors in Geriatric Hospital Patients visiting One Region-wide Emergency Department (일 권역응급의료센터를 방문하는 요양병원 환자들의 특성 및 사망 위험요인)

  • Purpose: This study was to examine the clinical characteristics and mortality risk factors of geriatric hospital patients who visited one region-wide emergency department (ED). It's basically meant to develop criteria for the patient management of geriatric hospitals and to provide related information. Methods: A retrospective research study was implemented using electronic medical records. The subjects in this study included 484 geriatric hospital patients who were selected from among 15,994 patients that visited one region-wide ED between January 1, 2014, and December 31, 2015. Results: There were significant differences in death, a change for the better and no change for the better, which were results of treatment, according to the length of stay in hospital, hospitalization in a general ward, not having an operation or surgery, the presence or absence of malignant neoplasm, the insertion of foley catheter, intubation, ventilator and the insertion of central venous catheter. Conclusion: The results of this study suggest that a patient management system is necessary in geriatric hospitals and that competent healthcare workers who can properly respond to emergencies are required as well.

  • Purpose: The purpose of this study was to investigate nurse images and likability that were perceived by various groups in the general population. Methods: Using cross-sectional descriptive design, six groups of people were invited to participate in the survey. The participants included nurses, physicians, nurse aids/technicians, patients/caregivers, college students, and the general public. A total of 1,530 individuals were drawn from 42 hospitals nationwide. Bhargava's likability questionnaires and the Ward Nurse Image tool were used. Results: Compared with other groups, patients and caregivers had higher scores for likability and better nurse image than other groups. Most respondents perceived nurses are clean and caring, but many individuals still had the image that nurses are submissive. Conclusion: To improve nurse image, perceptions from various groups of people need to be considered and programs develop to show the positive aspects of nursing and nurses.

Effects of Doin Gigong Exercise on Recovery from Facial Paralysis, Pain and Anxiety of Bell's palsy Patients (도인기공체조(導引氣功體操)가 구안와사(口眼喎斜)환자의 안면마비 회복정도, 통증 및 불안에 미치는 효과)

  • Purpose: This study was done to evaluate the effects of Doin Gigong Exercise on the Recovery from Facial Paralysis, pain and anxiety in patients with Bell's palsy. Methods: A nonequivalent control group posttest-only non-synchronized design was used for this study. Participants were contacted at the oriental medicine ward of D hospital in Busan and assigned to either the experimental group (25) or control group (25). Collected data were analyzed for changes in recovery from facial paralysis, in pain and in anxiety between pre and post Doin Gigong Exercise. Data analysis was done using t-test, Fisher's exact test, $x^2-test$ with the SPSS 12.0 Win program. Results: There were significant decreases in the lip paralysis status pain and in anxiety between pre and post Doin Gigong Exercise. Conclusion: The results indicate that Doin Gigong Exercise is effective for patients with Bell's palsy to decrease lip paralysis, pain and anxiety and therefore, an effective intervention for use with patients with Bell's palsy.

Saliva Supernatant miR-21: a Novel Potential Biomarker for Esophageal Cancer Detection

  • Objective: To identify whether saliva supernatant miR-21 can serve as a novel potential biomarker in patients with esophageal cancer (EC). Methods: 32 patients with EC and 16 healthy controls were recruited in this study. Total RNA was extracted from saliva supernatant samples for measurement of miR-21 levels using RT-qPCR and relationships between miR-21 levels and clinical characteristics of EC patients were analyzed. Results: miR-21 was significantly higher in the EC than control groups. The sensitivity and specificity were 84.4% and 62.5% respectively. Supernatant miR-21 levels showed no significant correlation with cancer stage, differentiation and nodal metastasis. Conclusions: Saliva supernatant miR-21 may be a novel biomarker for EC.

The Effects of Foot Reflexology Massage on the Central Pain, Fatigue and Sleep in Stroke Patients (발반사 마사지가 뇌졸중환자의 중추성 통증, 피로감 및 수면에 미치는 효과)

  • Purpose: This study was to evaluate the effects of foot reflexology massage on the central pain, fatigue and sleep in stroke patients. Methods: The study utilized a non-equivalent control group pretest and posttest design. Subjects were contacted at the oriental medicine ward of D hospital in B city for strokes. The subjects were 25 in the experimental group and 23 in the control group. Collected data were analyzed central pain, fatigue and the change of sleep between pre and post foot reflexology massage using a t-test, $x^2-test$, Fisher's exact test by SPSS 12.0 Win program. Results: There were significant decrease in the central pain, fatigue and change of sleep between pre and post foot reflexology massage. Conclusion: The results proved that foot reflexology massage was an effective stroke patients to decrease in the central pain, fatigue and change of sleep. Therefore, this foot reflexology massage should be effective as a stroke patients.
