A CLINICAL STUDY OF THE NASAL MORPHOLOGIC CHANGES FOLLOWING LEFORT I OSTEOTOMY (상악골 수평골절단술 후 비외형 변화에 관한 임상적 연구)

  • The facial esthetics are much affected by nasal changes due to especially its central position in relation to facial outline and so appropriately evaluated should be the functional and esthetic aspects of the nose associated with the facial appearance. Generally, a maxillary surgical movement is known to induce the changes of nasolabial morphology secondary to the skeletal repositioning accompanied by muscular retraction. These changes can be desirable or undesirable to individuals according to the direction and amount of maxillary repositioning. We investigated the surgical changes of bony maxilla and its effects to nasal morphology through the analysis of the lateral cephalogram in the Le Fort I osteotomy. Subjects were 10 patients(male 2, female 8, mean age 22.3 years) and cephalograms were obtained 2 weeks before surgery(T1) and 6 months after surgery(T2). The surgical maxillary movement was identified through the horizontal and vertical repositioning of point A. Soft-tissue analysis of the nasal profile was performed employing two angles: nasal tip projection(NTP), columellar angle(CA). Also, alar base width(ABW) was assessed directly on the patients with a slide gauge. The results were as follows; 1. Both anterior and superior movement above 2mm of maxilla rotated up nasal tip above 1mm. Either anterior or superior movement above 2mm of maxilla made prediction of the amount & direction of NTP changes difficult. Especially, a correlation between horizontal movement of maxilla and NTP rotated-up was P<0.01. 2. Both much highly anterior and superior movement of maxilla is accompanied by more CA increase than either highly. Especially, the correlation between horizontal movement of maxilla and CA change was P<0.05. 3. Anterior and/or superior movement of maxilla was accompanied by the unpredictable ABW widening. 4. The amount of changes of NTP, CA, and ABW is not in direct proportion to amout of anterior and/or superior movement of maxilla. 5. Nasal morphologic changes following Le Fort I osteotomy are affacted by not merely bony repositioning but other multiple factors.

Methylation of p16 and E-cadherin in ameloblastoma (법랑아세포종에서 p16과 E-cadherin의 메틸화)

  • Introduction: Ameloblastic carcinoma is a rare malignant lesion, and may arise from either carcinoma ex-ameloblastoma or de novo carcinoma. Aberrant promoter hypermethylation of the tumor-associated genes leading to their inactivation is a common event in many cancer types. The p16/CDKN2/INK4A gene and p16 5 protein are involved directly in regulating the cell cycles. Cadherins are cell adhesion molecules that modulate the epithelial phenotype and regulate tumor invasion. The aim of this study was to evaluate the roles of p16 and E-cadherin methylation and loss of p16 and E-cadherin expression in the malignant transformation of an ameloblastoma. Materials and Methods: Eight cases of ameloblastoma, including 4 benign ameloblastomas without recurrence, 2 benign ameloblastomas with recurrence and 2 carcinoma ex-ameloblastomas, were examined. The promoter hypermethylation profile of the p16 and E-cadherin genes was studied using methylation-specific polymerase chain reaction (MSP) and immunohistochemical staining for p16 and E-cadherin expression. Results: 1) Aberrant CpG island methylation of the p16 gene was detected in 3 of the 4 benign ameloblastomas without recurrence and 1 of the 2 benign ameloblastomas with recurrence. 2) Aberrant CpG island methylation of the E-cadherin gene was found in 1 of the 4 benign ameloblastomas without recurrence. 3) A loss of p16 expression was noted in 1 of 4 benign ameloblastomas without recurrence and 1 of 2 carcinoma ex-ameloblastomas. 4) A loss of E-cadherin expression was noted in 2 of the 4 benign ameloblastomas without recurrence, 1 of the 2 benign ameloblastomas with recurrence and 2 of the 2 carcinoma ex-ameloblastomas. 5) A loss of p16 expression was observed in 1 of the 4 cases showing aberrant methylation of the p16 gene. 6) A loss of E-cadherin expression was observed in 3 benign ameloblastoma case showing aberrant methylation of the E-cadherin gene. Conclusion: These results suggest that loss of E-cadherin expression related to the other genetic pathway (not methylation) might be an adjuvant indicator predicting the malignant transformation of an ameloblastoma. However, the number of samples in this study was too small and the relationship between the treatment methods and clinical course were not defined. Therefore, further study will be needed.

  • Hidden caries is a subtype of the occlusal pit and fissure caries type and is defined as a dentinal caries lesion near the occlusal surface of the tooth seen on a radiograph. In visual examination, the occlusal enamel is seen intact or is minimally perforated. Covert caries, Occlult caries or Fluoride syndrome are used as synonym. The percentage of occlusal dentin lesions that are clinically undetected ranges from 1.4-50%. Little is known about the mechanisms involved in the development of hidden caries. But it is thought that extensive use of topical fluoride or the special bacteriological profile has been a major factor. This case report is about detection and treatment of hidden caries of two children who visited the department of pediatric dentistry, Yonsei University Dental Hospital. The color of caries dentin found in hidden caries lesion is lighter than cavity forming caries, which makes it more difficult to detect caries by visual examination. Therefore diagnosis of hidden caries is often accomplished after clinical sign is recognized by patients. The use of advanced caries detection aids such as Diagnodent.. with periodic radiographic examination is seemed to be helpful for early detection of hidden caries.

Human Cord Serum as a Fetal Bovine Serum Substitute for the Culture of Human Amnion-Derived Stem Cells (인간의 양막유래 줄기세포의 체외 배양 시 소태아혈청 대체제로서의 인간제대혈청)

  • Mesenchymal stem cells (MSC) are promising candidates for cell-based therapies. One major obstacle for their clinical use is the unsafety of fetal bovine serum (FBS), which is a crucial part of all media currently used for the culture of MSC. We investigated the effect of human cord serum (HCS) on the growth response, mRNA and protein expressions of human amnion-derived stem cells (HAM). HAM were isolated from the amnion after a Caesarean section and cultured in DMEM supplemented with 10% FBS, 5% HCS or 10% HCS. During culture, their biological characteristics at earlier and later passages were analyzed using RT-PCR and immunocytochemistry. Regardless of serum sources, HAM showed the prominent expression of Oct-4, Rex-1, SCF, FGF-5, BMP-4, nestin, GATA-4, NCAM and HLA ABC genes. The expression profile was observed even at later passages. Similarly, HAM cultured in either FBS or HCS exhibited the distinct protein expression of collagen I, II, III and XII, fibronectin, $\alpha$-smooth muscle actin, vimentin, CK18, CD54, FSP, TRA-1-60, SSEA-3, -4 and HLA ABC. However, desmin expression was only observed in HAM cultured in medium supplemented with FBS and vWF expression was only found in HAM cultured in medium supplemented with HCS. Overall pattern of gene and protein expression of HAM was typical of known adult stem cells such as bone marrow-derived MSC. In conclusion, HCS could be as effective as FBS for the culture of HAM.

Neurocognitive Function in Patients with Hwa-byung and with Major Depressive Disorder (화병과 주요우울증 환자의 신경인지기능)

  • Objectives:Hwa-byung has been studied clinically for several years and introduced as Korean Culture- Bound Syndrome. However, the definition and the diagnostic method are not yet clarified, and there has not been any sufficient comparative study on this disease entity. This study was conducted to determine the clinical symptoms and the profile of the neurocognitive functions in Hwa-byung(HB) and Major Depressive Disorder(MDD), and We wish to identify any critical factors that differentiate the disorders. Methods:A total of 102 participants were examined, including 34 participants with MDD, 34 with HB, and 34 healthy controls. The MDD and HB patients were recruited from among inpatients and outpatients at the National Medical Center for the period from May to December of 2004. As a major diagnostic tool of MDD, diagnostic reference of DSM-IV-TR was used and as HB's diagnostic tool, We used computerized neurocognitive function test. Psychiatric symptomatology was evaluated by the Beck Depression Inventory(BDI), and Symptom Checkist-90-Revision(SCL-90-R). Oneway ANOVA, Scheffe post-hoc test and Chi-Squre Tests were used for statistical analysis. Results:The participants in three groups did not differ in terms of age, sex, and education. Assessment of BDI indicated that the MDD group had significantly higher total score than the HB group. MDD and HB groups showed significantly higher total scores on the SCL-90-R in comparison to the controls. The MDD group was found to have significantly more symptoms of depression than the HB group, based on the depression subscale of the SCL-90-R. The computerized neurocognitive function test suggest several results 1) Within the memory domain, it was found that one of the two memory tests in MDD and HB groups were significantly impaired in comparison to the control group. 2) Within the attention domain, it was found that only the MDD group was significantly impaired in comparison to the control group. 3) Within the higher cortical function domain, it was found that significant impairment exist in MDD group and HB group compared to the control group; the severity of impairment was found to be more profound in the MDD group than in the HB group. Conclusion:These results suggest that both HB group and MDD group have significantly decreased neurocognitive function than the control group, and neurocognitive function of the HB group is better than that of the MDD group.

INFLUENCE OF HEATED SPREADER ON THE SEALING ABILITY OF LATERAL CONDENSATION (측방가압충전시 Heated Spreader 사용유무가 근관밀폐에 미치는 영향)

  • The purpose of this study was to evaluate the effect of heated spreader on the sealing ability of lateral condensation, compared with regular cold spreader. Forty two extracted human teeth with single canal were randomly placed into 3 experimental groups, and four additional teeth were used as positive and negative controls. Each group was prepared with Ni-Ti Profile #40 using step-down technique and obturated with standardized colored gutta-percha cone by standard(cold) lateral condensation technique, warm lateral condensation technique with Endotec and hot spreader soaked in glass bead sterilizer, each with Sealapex sealer. Control groups were not obturated, but prepared. After 2 days in 2% methylene blue, the teeth were invested and made into transparent resin blocks. And then, each block was sectioned horizontally with microtome at 1, 2, 3, 4, 5 mm levels from the apex. The linear extent of dye penetration was examined with stereomicroscope at ${\times}$20 magnification. At each of 5 levels, ratio of the area of gutta-percha was obtained by calculating the area of gutta-percha to the total area of the canal. The data collected were then analyzed statistically using an analysis of variance(ANOVA) and Scheffe test. The results were as follows ; 1. All experimental groups produced the apical microleakage. 2. The mean leakage was 1.57${\pm}$0.76mm for cold spreader group, 0.86${\pm}$0.95mm for Endotec spreader group, and 0.64${\pm}$0.93mm for hot spreader group. The difference between hot spreader group and cold spreader group was statistically significant(p<0.05). 1. At the 1 mm level, the mean ratio of area of gutta-percha was 74.58${\pm}$13.15(%) for cold spreader group, 65.42${\pm}$14.62(%) for Endotec spreader group, and 80.72${\pm}$14.63(%) for hot spreader group. There was statistically significant difference between hot spreader group and Endotec spreader group(p<0.05). 2. At the 2mm level, the mean ratio of area of gutta-percha was 87.86${\pm}$11.22(%) for cold spreader group, 66.55${\pm}$14.02(%) for Endotec spreader group, and 92.93${\pm}$7.24(%) for hot spreader group. There was statistically significant difference between Endotec spreader group and other two spreader groups(p<0.05). 3. At the level 3, 4, 5 mm, there was no statistically significant difference between each group. Within the limits of the results of this experiment, warm lateral condensation technique with hot spreader soaked in a glass bead sterilizer demonstrated favorable apical sealing effect and improved density of gutta-percha mass. Thus, it is thought that this obturation technique is effective for clinical use and beneficial to reduce condensation forces, also economical and easy. Deconvolution of Detector Size Effect Using Monte Carlo Simulation (몬데카를로 시뮬레이션을 이용한 검출기의 크기효과 제거)

  • The detector size effect due to the spatial response of detectors is a critical source of inaccuracy in clinical dosimetry that has been the subject of numerous studies. Conventionally, the detector response kernel contains all the information about the influence that the detector size has on the measured beam profile. Various analytical models for this kernel have been proposed and studied in theoretical and experimental works. Herein, a method to simply determine the detector response kernel using the Monte Carlo simulation and convolution theory has been proposed. Based on this numerical method, the detector response kernel for a Farmer type ion chamber embedded in a water phantom has been obtained. The obtained kernel shows characteristics of both the pre-existing parabolic model proposed by Sibata et al. and the Gaussian model used by Garcia-Vicente et al. From this kernel and deconvolution technique, the detector size effect can be removed from measurements for 6MV, 10${\times}$10 $\textrm{cm}^2$ and 0.5${\times}$10 $\textrm{cm}^2$photon beams. The deconvolved beam profiles are in good agreements with the measurements performed by the film and pin-point ion chamber, with the exception of in the tail legion.

  • In this study, we have fabricated a one-dimensional fiber-optic dosimeter for electron beam therapy dosimetry. Each fiber-optic dosimeter has an organic scintillator with a plastic optical fiber and it is embedded and arrayed in the plastic phantom to measure one-dimensional high energy electron beam profile of clinical linear accelerator. The scintillating lights generated from each sensor probe are guided by plastic optical fibers to the multi-channel photodiode amplifier system. We have measured one-dimensional electron beam profiles in a PMMA phantom according to different field sizes and energies of electron beam. Also, the isodose and three-dimensional percent depth dose curves in a PMMA phantom are obtained using a one-dimensional fiber-optic dosimeter with different electron beam energies.

Increased Frequency of Apolipoprotein E4 Genotype in Childhood Minimal Change Nephrotic Syndrome (MCNS) (소아 미세변화 신증후군 환자에서 Apolipoprotein E4 유전자형에 관한 연구)

  • Purpose: We studied to find out apo-E genotype polymorphism in minimal change nephrotic syndrome(MCNS) and IgA nephropathy(IgAN) and to determine the relationship between apo-E genotype and clinical course of MCNS. Materials and Method: 43 MCNS patients and 15 IgAN patients were examined for apo-E polymorphism. 50 healthy blood donors were examined for apo-E genotype as control. Genomic DNA was prepared front peripheral blood leukocytes according to standard procedures. Results: As compared with control group, e4 allele frequency was significantly increased in MCNS (p<0.01). However, in IgAN e2 allele frequency, however, was 2.6 times higher than normal control (P<0.01). The frequency of e4 allele of frequent relapser group was 4.6 times higher than normal control and was 2 times higher than infrequent relapser group. Conclusion: We think that apo-E typing might be one of the parameters, which should be considered to predict the course of MCNS in children. MCNS with risky HLA profile and E4/4 genotype could indicate the need for a longer steroid administration. And apo-E genotype needs to be considered for the evaluation of therapeutic responses to other drugs. (J, Korean Soc Pediatr Nephrol 2001 ; 5 : 87-99)

Evaluation of short-term Hypolipidemic Effect and Safety of Simvastatin($Zocor^{(R)}$) in Patients with Hyperlipidemia (고콜레스테롤혈증 환자에서 Simvastatin($Zocor^{(R)}$) 단기 투여후 효과와 안정성 평가)

  • Background : Hyperlipidemia is the one of the major risk factors causing the atherosclerosis of coronary arteries. Treatment of hyperlipidemia with drugs has been confirmed the effects of therapy showing a decreased incidence of coronary artery disease. Simvastatin is a new drug of HMG-CoA reductase inhibitors and the short-term hypolipidemic effects and safety of simvastatin is evaluated in patients with hyperlipidemia. Methods: We studied 63 patients (39 males and 24 females, mean age 58) for 12 weeks whose plasma levels of total cholesterol were higher than 240 mg/dL or higher than 220 mg/dL with ischemic heart disease. Simvastatin was administered 20 mg/day and measured lipid profile at 12 week interval. Result: 1) Simvastatin significantly reduced the level of the plasma total cholesterol(-29.3%), LDL-cholesterol(-36.9%) and triglyceride(-13%)(p<0.05) but the level of HDL-cholesterol was not changed after 12 weeks simvastatin therapy. 2) the clinical symptoms and laboratory examination before and after simvastatin treatment showed no particular abnormal findings in short term follow up. Conclusion: These results suggested that short-term simvastatin therapy in patients with hyperlipidemia seeems to be very effective and safe.
