Phonological Error Patterns: Clinical Aspects on Coronal Feature (음운 오류 패턴: 설정성 자질의 임상적 고찰)

  • The purpose of this study is to investigate two phonological error patterns on coronal feature of children with functional articulation disorders and to compare them with those of general children. We tested 120 children with functional articulation disorders and 100 general children from 2~4 years of age with 'Assessment of Phonology & Articulation for Chidren(APAC)'. The results were as follows: (1) 37 disordered children substituted [+coronal] consonants for [-coronal] consonants (fronting of velars) and 9 disordered children substituted [-coronal] consonants for [+coronal] consonants (backing to velars). (2) Theses two phonological patterns were affected by the articulatory place of following phoneme. (3) The fronting pattern of children with articulation disorders was similar with that of general children, but their backing pattern was different with that of general children. These results show the clinical usefulness of coronal feature in phonological pattern analysis, the need of articulatory assessment with various phonetic context, and the importance of error contexts in clinical judgment.

Clinical courses and degradation patterns of absorbable plates in facial bone fracture patients

  • Background: Absorbable plates are widely used in open reduction and internal fixation surgeries for facial bone fractures. Absorbable plates are made of polyglycolic acid (PGA), polylactic acid (PLA), polydioxane (PDS), or various combinations of these polymers. The degradation patterns of absorbable plates made from different polymers and clinical courses of patients treated with such plates have not been fully identified. This study aimed to confirm the clinical courses of facial bone fracture patients using absorbable plates and compare the degradation patterns of the plates. Methods: A retrospective chart review was conducted for 47 cases in 46 patients who underwent open reduction and internal fixation surgery using absorbable plates to repair facial bone fractures. All surgeries used either PLA/PGA composite-based or poly-L-lactic acid (PLLA)/hydroxyapatite (HA) composite-based absorbable plates and screws. Clinical courses were confirmed and comparisons were conducted based on direct observation. Results: There were no naturally occurring foreign body reactions. Post-traumatic inflammatory responses occurred in eight patients (nine cases), in which six recovered naturally with conservative treatment. The absorbable plates were removed from two patients. PLA/PGA compositebased absorbable plates degraded into fragments with non-uniform, sharp surfaces whereas PLLA/HA composite-based absorbable plates degraded into a soft powder. Conclusion: PLA/PGA composite-based and PLLA/HA composite-based absorbable plates showed no naturally occurring foreign body reactions and showed different degradation patterns. The absorbable plate used for facial bone fracture surgery needs to be selected in consideration of its degradation patterns.

Molecular Identification Patterns of Clinical Isolates from Korean Patients Infected with Dermatopbytes

  • The aim of this study was to compare the results of morphological phenotypic examination and molecular biological method, randomly amplified polymorphic DNA (RAPD) using clinical isolates from Korean patients infected with dermatophytes. A total of 44 clinical isolates have been collected by Korean Collection Medical Fungi (KCMF) and those were subjected for the RAPD analysis. The results showed that the fungal strains that have RG1 band patterns were identified as Trichophyton rubrum by the neighbour-joining analysis. Additionally, other fungal strains that have RG3 band patterns were identified as Epidermophyton floccosum and RG5 band patterns were identified as Micorsporum gypseum. Even though molecular biological method such as RAPD are not necessary, it might be useful when some strains have similar morphological characteristics or when same species has phenotypic variations. In this study, therefore, we targeted internal transcribed spacer (ITS) regions of fungal rDNA, performed RAPD and then compared with morphological phenotypic examination.

A Clinical Study on the Ordinary Sleeping Patterns of Soyangin (소양인(少陽人) 수면 습관에 관한 연구)

  • Objectives : This study is for discovering the clinical Features of the sleep in ordinary symptoms based on the Sasang Constitution. The result of this study could be helpful to understand and to identify the patients as Soyangin in contrast with the other constitutions -Soeumin, Taeumin and Taeyangin. Methods : There were 1,229 patients(700 female), who answered the questionnaire about their ordinary sleeping patterns. They had been diagnosed, including their clinical Sasang Constitution, by the Sasang Constitution specialist at Bundang Oriental Hospital of Dongguk University. By applying the binary logistic regression analysis, we can measure the characteristics and the influence of ordinary sleeping patterns to the dependent variable(Sasang Constitution). Results : As a result of the binary logistic analysis on the observed questionnaire, we found the characteristics of the ordinary sleeping patterns on Soyangin in contrast with the other constitutions. Firstly, Soyangin has a tendency that he wouldn't dreams more, when he sleeps in contrast with the others. Secondly, Soyangin has a tendency that he wouldn't sleep longer than 6-7hours. Thirthly, Soyangin has a tendency that he will sleep well in contrast with the others. Conclusion : This study will be used to identify patients as Soyangin in contrast with the others-Soemin, Taeumin and Taeyangin.

  • Acupuncture is a complex intervention that manifests varied theories, treatment methods, diagnostic methods and diagnostic patterns. Traditionally based systems of acupuncture (TBSAs) often have their own diagnostic approaches and patterns. Despite the wide variety that can be found amongst TBSAs, is it possible that they share a common background in clinical observation and practice? Research has shown that multiple physiological pathways and mechanisms can be triggered by different acupuncture techniques and methods. It is highly likely that clinicians will have observed some of the effects of these responses and used those observations as feedback to help construct the patterns of diagnosis and their associated treatments. This review briefly examines this possibility. Pattern identification will have developed out of a complex interaction of factors that include; theories current at the time of their development, historical theories, personal choices and beliefs, training, practice methods, clinical observations and the natural feedback that comes from observing how things change once the treatment is applied. Researchers investigating TBSAs and pattern identification need to be more explicit about the systems they have investigated in order to understand the biological basis of pattern identification and their treatments.

Development of Speech-Language Therapy Program kMIT for Aphasic Patients Following Brain Injury and Its Clinical Effects (뇌 손상 후 실어증 환자의 언어치료 프로그램 kMIT의 개발 및 임상적 효과)

  • MIT has been applied for nonfluent aphasic patients on the basis of lateralization of brain hemisphere. However, its applications for different languages have some inquiry for aphasic patients because of prosodic and rhythmic differences. The purpose of this study is to develop the Korean Melodic Intonation Therapy program using personal computer and its clinical effects for nonfluent aphasic patients. The algorithm was composed to voice analog signal, PCM, AMDF, Short-time autocorrelation function and center clipping. The main menu contains pitch, waveform, sound intensity and speech files on window. Aphasic patients' intonation patterns overlay on selected kMIT patterns. Three aphasic patients with or without kMIT training participated in this study. Four affirmative sentences and two interrogative sentences were uttered on CSL by stimulus of ST. VOT, VD, Hold and TD were measured on Spectrogram. In addition, articulation disorders and intonation patterns were evaluated objectively on spectrogram. The results indicated that nonfluent aphasic patients with kMIT training group showed some clinical effects of speech intelligibility based on VOT, TD values, articulation evaluation and prosodic pattern changes.

Morphological classification, anatomical variations, innervation patterns, musculocutaneous nerve relation of the coracobrachialis muscle: anatomical study and clinical significance

  • The anatomical variations of coracobrachialis muscle (CBM) are of great clinical importance. This study aimed to elucidate the morphological variations, innervation patterns and musculocutaneous nerve (MCN) relation to CBM. Upper limbs of fifty cadavers (30 males and 20 females) were examined for proximal and distal attachments, innervation pattern of CBM and its relation to MCN. Four morphological types of CBM were identified according to number of its heads. The commonest type was the two-headed (63.0%) followed by the single belly (22.0%), three-headed (12.0%) and lastly four-headed (3.0%) type. Moreover, an abnormal insertion of CBM was observed in four left limbs (4.0%); one inserting into the medial humeral epicondyle, the second into the upper third of humeral shaft, the third one in the common tendon of biceps, and the fourth one showing a bifurcated insertion. Also, four different innervation patterns of CBM were identified including MCN (80.0%), lateral cord (14.0%), lateral root of median nerve (4.0%), and median nerve itself (2.0%). The course of MCN was superficial to the single belly CBM (19.0%) and in-between the heads in the other types (71.0%). Measurements of the length and original distance of CBM muscular branches originating from MCN revealed no sex or side significant difference. Awareness of the anatomic variations, innervation patterns, and MCN relation of CBM is imperative in recent diagnostic and surgical procedures to obtain definite diagnosis, effective management and good outcome.

Molecular Epidemiology of Listeria monocytogenes by Ribotyping

  • Ten Listeria monocytogenes were isolated from clinical specimens and mussels, and their physio-biochemical characters were compared with the type strains. Ribotyping was used as a taxonomic tool to determine molecular epidemiological marker. Chromosomal DNA was cleaved with restriction enzymes HindIII and EcoRI. The fragment were subjected to Southern blot hybridization with 165 rDNA from B. subtilis by PCR. EcoRI patterns of Listeria strains showed 6 to 8 bands ranging from 0.75 kb to 11 kb band and they were classified into 6 groups. In comparison, HindIII patterns revealed that 5 to 7 bands ranging from 2.75 kb to 7.75 kb band and they classified into 5 groups. The various patterns of Listeria strains were observed within genus, species and isolated sources. 165 rRNA gene restriction patterns (ribotyping) are useful in epidemiological and taxonomic study.

A Survey on Korean Medicine Treatment for Traffic Injuries: A Second On-line In-depth Survey (교통사고 상해 증후군의 한의 임상진료 현황 조사: 2차 온라인 심층 설문 조사)

  • Objectives To understand current status of clinical practice patterns on Korean Medicine treatment practice of traffic injury patients by 2nd on-line survey for developing Korean Medicine treatment clinical pratice guideline (CPG) for traffic injuries. Methods A revised questionnaire about current practice patterns of Korean Medicine treatments for traffic injuries was distributed by e-mail survey to 19,385 respondents of Korean Medicine doctors from May 21th in 2018 to June 21th in 2018. All data were statistically analysed. Results The response rate was 3.5%. Most of the respondents felt necessity of the development of Korean Medicine CPG for traffic injuries. The results showed the detailed usage and trend of current Korean Medicine treatment such as acupuncture, pharmacopuncture, moxibustion, Chuna manual therapy, cupping, Korean Medicine physiotherpy for traffic injury patients. Conclusions This survey helps to determine current Korea Medicine treatments' clinical practice patterns for traffic injury patients. All data from the survey will provide basic data for future clinical studies and adaptation for the revision of Korean Medicine CPG for traffic injuries.

An Analysis on Prescribing Patterns of Alzheimer's Dementia Treatment and Choline Alfoscerate using HIRA Claims Data (건강보험 청구자료를 이용한 알츠하이머 치매 치료제와 콜린알포세레이트의 처방 양상 분석)

  • Background: Alzheimer's dementia is the most common dementia. However, recently, choline alfoscerate is prescribed for treating Alzheimer's dementia, although it is not a treatment for this disease. Purpose: To analyze the prescription patterns of choline alfoscerate as a dementia treatment for patients with Alzheimer's disease and to analyze, as well as the factors affecting choline alfoscerate prescription. Method: The 2016 HIRA-NPS data was used in this study. The code of Alzheimer's dementia is F00 in the ICD-10 disease classification code. We analyzed the demographic, clinical, and regional characteristics associated with donepezil, rivastigmine, galantamine, memantine, and choline alfoscerate prescriptions. All statistical and data analyse were conducted by SAS 9.4 and Excel. Results: For patients with Alzheimer's disease, choline alfoscerate was the second most prescribed after donepezil. Analysis results showed that choline alfoscerate was more likely to be prescribed to men than to women, and more likely to be prescribed by local health centers than by medical institutions. Moreover, choline alfoscerate was highly likely to be prescribed at neurosurgical departments, among medical departments. Conclusions: This study confirmed that choline alfoscerate was prescribed considerably for patients with Alzheimer's dementia. Further studies valuating its clinical validity should be performed to clarify whether choline alfoscerate prescription is appropriate for treating Alzheimer's dementia.
