Orthodontic Traction of Impacted Tooth (매복된 치아의 교정적 견인을 통한 치험예)

  • Tooth impaction may lead to malocclusion, root resolution, cyst or aberrant changes in adjacent teeth. Clinical and radiographic examinations are used to locate the impaction, and appropriate treatment plans must be made to relocate the impacted tooth. When surgically exposing the impacted tooth, periodontal considerations to conserve maximum amount of soft and hard tissue are used. Oral hygiene instructions are emphasized to maintain sound periodontal health. Securing enough space for the impacted tooth and proper anchorage is important. Proper use of force and mechanics is crucial to prevent such complications as root resolution. Various patterns of orthodontic traction may be employed as situation permits. Most impaction cases can be managed with orthodontic traction to restore function and esthetics, provided that early detection and proper diagnosis and treatment planning are made.

Changes in maximum lip-closing force after extraction and nonextraction orthodontic treatments

  • Objective: The aims of the present study were to evaluate the changes in the maximum lip-closing force (MLF) after orthodontic treatment with or without premolar extractions and verify the correlation of these changes with dentoskeletal changes. Methods: In total, 17 women who underwent nonextraction orthodontic treatment and 15 women who underwent orthodontic treatment with extraction of all four first premolars were included in this retrospective study. For all patients, lateral cephalograms and dental models were measured before (T0) and after (T1) treatment. In addition, MLF was measured at both time points using the Lip De Cum LDC-110R® device. Statistical analyses were performed to evaluate changes in clinical variables and MLF and their correlations. Results: Both groups showed similar skeletal patterns, although the extraction group showed greater proclination of the maxillary and mandibular incisors and lip protrusion compared to the nonextraction group at T0. MLF at T0 was comparable between the two groups. The reduction in the arch width and depth and incisor retroclination from T0 to T1 were more pronounced in the extraction group than in the nonextraction group. MLF in the extraction group significantly increased during the treatment period, and this increase was significantly greater than that in the nonextraction group. The increase in MLF was found to be correlated with the increase in the interincisal angle and decrease in the intermolar width, arch depth, and incisor-mandibular plane angle. Conclusions: This study suggests that MLF increases to a greater extent during extraction orthodontic treatment than during nonextraction orthodontic treatment.

A Study on Shu Points Selection of the Four Seas in Lingshu Hailun (『영추(靈樞)·해론(海論)』의 사해(四海) 수혈(輸穴) 선정에 대한 고찰)

  • Objectives : To study the shu points selection of the Four Seas in the 「Hailun」 chapter of 『Lingshu』. Methods : The effects and main indications of each shu point, their anatomical position, characteristics along with similarities of disease patterns according to excessiveness and deficiency of the Four Seas were examined. Results : The selection of shu points of the Four Seas were deeply related to the effects and main indications, and the common similarity among conditions due to excessiveness and deficiency was the presence of psychological aspects. Also, when the Four Seas are connected to the Jing-Qi-Shen and Blood, marrow sea[髓海] is connected to Jing & Shen, qi sea[氣海] is connected to Qi, blood sea[血海] is connected to Blood, and sea of water and grain[水穀之海] replenishes Jing-Qi-Shen and Blood. When connected to the Four Qi Intersections, the marrow sea is connected to 'head qi with intersection', qi sea is connected to 'chest qi with intersection', sea of water and grain or blood sea is connected to 'stomach qi with intersection', otherwise sea of water and grain is connected to 'shin qi with intersection'. Conclusions : The Four Seas are designated to the shu points due to their accumulation of Qi when the main elements of Jing-Qi-Shen and Blood are deficient or excessive, allowing for the most convenient management and manipulation of the condition of these main elements. In clinical practice, the shu points of the Four Seas will likely increase treatment efficacy for conditions that include psychological aspects.

Trends in the Prescribing of Antimanic Agents for Acute Manic Inpatients (급성 조증 입원환자에 대한 항조증약물의 처방 경향)

  • Objective:This study was performed to investigate the prescribing patterns of antimanic agents in the treatment of acute bipolar disorder inpatients in Korea from 1990 through 2000. The results will serve as the basic data for the practice guideline for the pharmacotherapy of bipolar disorder patients in Korea. Method:Retrospective chart review of bipolar disorder inpatients of Soonchunhyang Medical Center in Seoul and Chun-An was conducted for each of the year 1990, 1995, and 2000. The following data are collected ; 1) demographic data, 2) history of bipolar disorder, 3) length of hospital stay, 4) detailed drug titration records of antimanic agents and antipsychotic agents. Results:During the last decade, the frequency of lithium monotherapy was decreased obviously. Instead, more than half of the patients in 2000 were on combination therapy of lithium and anticonvulsants. Lithiumvalproate combination was the preferred strategy and the use rate of carbamazepine has been decreased. In addition, most of the patients were given antipsychotic agents during the last 10 years. And recently, atypical antipsychotics were increasingly prescribed. These changes in the field of pharmacology of bipolar disorder have resulted neither in shorter hospital stays nor lower dosages of concurrent neuroleptics. Conclusions:The results indicate the trends in the prescribing of antimanic agents for the treatment of bipolar disorder in Korea across the past 10 years. Mostly, the change seems to correspond to the international practice guideline. More systematic research is needed to find out the clinical benefits of the anticonvulsants in the real practice of treatment of bipolar disorder.

A THREE-DIMENSIONAL FINITE ELEMENT ANALYSIS ON STRESS AND DISPLACEMENT RELATED TO ISTHMUS WIDTH OF GOLD INLAY CAVITY (금인레이 와동의 폭경이 응력분포와 변위에 미치는 영향에 관한 3차원 유한요소법적 연구)

  • The purpose of this study was to evaluate the fracture resistance of tooth restored with gold inlay. A profound understanding of the isthmus width factor, which is one of the several parameters of cavity designs, would facilitate the appropriate cavity preparation in a specific clinical situation. In this study, the cavities for gold inlay were prepared in maxillary left first premolar. A three-dimensional model was designed using I-DEAS program. The model was composed of 2515- nodes and 2172 isoparametric brick elements. In the model isthmus width was varied into 1/4, 1/3 and 1/2 of intercuspal width respectively, and numeric values of the material properties of enamel, dentin and gold was set. Three types of load : concentrated load, divided load and distributed load was 500N. The empty cavities in the model were also examined using divided load and distributed load. The three - dimensional Finite Element Method was used to analysis the displacement and stress distribution. The results were as follows : 1. All of the experimental models which were filled with gold inlay revealed similar direction of displacement to that of the natural tooth model under the same load type. But in the models with empty cavities, as the isthmus width increased, the degree of displacement increased in the case of divided load type. 2. All experimental models which were filled with gold inlay showed stress concentration at load points, but in the models with empty cavities at divided load type, as isthmus width increased, stress was concentrated at the comer of the pulpal floor. 3. In the models with empty cavities at divided load type, tooth fracture was expected regardless of isthmus width, but all experimental models which were filled with gold inlay after cavity preparation were not susceptible to fracture. 4. In all experimental models which were filled with gold inlay after cavity preparation, displacement patterns were similar under both concentrated and divided load types. In the models with empty cavities, a divided load resulted in a bucco-lingual cuspal displacemenat in both sides, but a distributed load resulted in a lingual displacement of the tooth.

Medical Parameter Extraction Using Time-Density Data in Contrast-Enhanced Ultrasound Image Sequence (조영증강 초음파영상에서 밀도변화 데이터를 이용한 진단 파라미터 추출 기법)

  • In medical ultrasonography, transit time and contrast enhancement patterns are considered as important parameters to analyze liver diseases. In many recent researches, time-intensity curves(TIC) have been used for calculating the transit time of the contrast agents. However, the intensity curve may include the variations which are caused by the micro-bubble effect of contrast agents. In this paper, we propose a complementary approach to diagnostic parameter extraction which utilizes a density information as well as the intensity data. The proposed technique improves the accuracy in extraction of the transit time and velocity of contrast agents for detection and characterization of focal liver lesions. Through the experiments using a set of clinical data, we show that the proposed methods can improve the reliability of the parametric image data.

Preparation and Evaluation of Meloxicam-loaded Poly(D,L-lactic acid) Microspheres (멜록시캄 함유 poly (D,L-lactic acid) 미소립자의 제조 및 평가)

  • Meloxicam-loaded microspheres were prepared with poly(D,L-lactic acid)(PLA) by a solvent-emulsion evaporation method. The morphology, particle size, drug loading capacity, drug entrapment efficiency (EE) and release patterns of drug were investigated in vitro. Various batches of micro spheres with different size and drug content were obtained by changing the ratio of meloxicam to $PLA^{\circ}{\AE}s$ with different molecular weight, PLA concentration in the dispersed phase and stirring rate. Meloxicam crystals on microsphere surface, which were released rapidly and could act as a loading dose, were observed with increasing drug content. The release rate was increased with increase in drug contents and decrease in the molecular weight of PLA. Microspheres prepared with smaller molecular weight produced faster drug release rate. The release rate of meloxicam for long-acting injectable delivery system in vitro, which would aid in predicting in vivo release profile, could be controlled by properly optimizing various factors affecting characteristics of microspheres. Blood concentration-time profile of meloxicam after intramuscular injection of meloxicam-loaded microspheres in rabbits showed possibility of long term application of this system in clinical settings.

Molecular Characterization of Salmonella Genomic Island 1 in Proteus mirabilis Isolates from Chungcheong Province, Korea

  • The emergence and dissemination of Salmonella genomic island 1 (SGI1) are strongly associated with the occurrence of multidrug-resistant (MDR) enterobacteria in humans and animals. Diverse SGI1s have been reported among Salmonella enterica and Proteus mirabilis in several countries. We aimed to characterize SGI1 in P. mirabilis isolates from humans and chickens in Chungcheong Province, Korea. A total of 44 P. mirabilis isolates were recovered from humans (n = 20) and chickens (n = 24). Antimicrobial susceptibility was determined by disk diffusion assay. To detect and characterize SGI1s, PCR amplification and PCR mapping experiments were performed. Repetitive extragenic palindromic-PCR (REP-PCR) was performed to assess the clonality of the isolates. The four P. mirabilis strains (16.7%) from chicken harbored a SGI1, whereas none of the isolates from clinical specimens contained SGI1. The SGI1s detected in our study were all confirmed as SGI1-PmABB harboring aminoglycoside-resistant genes (aacCA5 and aadA7). In P. mirabilis isolates, the presence of SGI1-PmABB was significantly correlated with high resistance rates of the isolates to antimicrobial agents, such as gentamicin, streptomycin, and spectinomycin. Moreover, the four SGI1-bearing isolates showed the same REP-PCR patterns and that suggested both horizontal and clonal spread of the isolates. This study is the first attempt to determine SGI1s in P. mirabilis isolates in Korea. We confirmed that P. mirabilis isolates carrying SGI1-PmABB were distributed at poultry farms in Korea. The present study emphasizes the need for continuous monitoring of SGI1s to prevent spreading of the MDR genomic islands among P. mirabilis isolates from humans and animals.

The opportunities of virtual reality in the rehabilitation of children with attention deficit hyperactivity disorder: a literature review

  • Attention deficit hyperactivity disorder (ADHD) is one of the most common psychiatric disorders in childhood. This disorder, in addition to its main symptoms, creates significant difficulties in education, social performance, and personal relationships. Given the importance of rehabilitation for these patients to combat the above issues, the use of virtual reality (VR) technology is helpful. The aim of this study was to highlight the opportunities for VR in the rehabilitation of children with ADHD. This narrative review was conducted by searching for articles in scientific databases and e-Journals, using keywords including VR, children, and ADHD. Various studies have shown that VR capabilities in the rehabilitation of children with ADHD include providing flexibility in accordance with the patients' requirements; removing distractions and creating an effective and safe environment away from real-life dangers; saving time and money; increasing patients' incentives based on their interests; providing suitable tools to perform different behavioral tests and increase ecological validity; facilitating better understanding of individuals' cognitive deficits and improving them; helping therapists with accurate diagnosis, assessment, and rehabilitation; and improving working memory, executive function, and cognitive processes such as attention in these children. Rehabilitation of children with ADHD is based on behavior and physical patterns and is thus suitable for VR interventions. This technology, by simulating and providing a virtual environment for diagnosis, training, monitoring, assessment and treatment, is effective in providing optimal rehabilitation of children with ADHD.

Development and Application of the Constipation Assessment Tool for Patients with Terminal Cancers (말기 암환자의 변비 사정도구 개발 및 적용)

  • Purpose: The purposes of this study were to develop the constipation assessment tool for identifying their bowel movement patterns, and to provide basic information for the assessment and intervention protocol on constipation for patients with terminal cancers. Methods: The study followed the steps: the first step was to build a conceptual framework based on literature review; the second step was to develop a tentative instrument by analyzing the conceptual framework and existing instruments; the third step was to test content validity and reliability; and the final step was to apply the tool to patients with terminal cancers (N=112). Results: The constipation assessment tool was consisted of total nine items; under the categories of subjective and objective data for diagnosis had 4 items, and under the initial assessment category which includes stool type, physical examination, and abdomen X-ray had 5 items. Conclusion: The constipation assessment tool developed in this study is very easy to use and useful in nursing practice, especially in hospice and palliative care setting. Particularly this tool has items on patient assessment which would be considered as an evidence for choosing nursing interventions. Based on the constipation assessment tool, the development and application of intervention protocol on constipation for patients with terminal cancers is warranted in future research.
