Patterns of Antimicrobial Resistance and Genotyping of Carbapenemase-producing Imipenem-nonsusceptible Pseudomonas aeruginosa (Imipenem 비감수성 Carbapenemase 생성 Pseudomonas aeruginosa에 의한 항생제 내성유형과 분자생물학적인 특성)

  • Pseudomonas aeruginosa are important nosocomial pathogens. Their resistance to carbapenem is increasing and causing concerns in Korea. An increasing prevalence of carbapenem resistance mediated by acquired carbapenemase is being reported. Over a 10 month-period from July 2007 to April 2008, 32 strains of imipenem-nonsusceptible P. auruginosa were isolated from Kangwon National University Hospital. To determine the prevalence and genotypes of the carbapenemase-producing clinical isolates, the antibiotic susceptibility was determined by Microscan Walkaway 96 SI System and the carbapenem activity was detected by the modified Hodge test and the imipenem-EDTA-SMA double-disk synergy test. The metallo-${\beta}$-lactamase gene and OXA-type ${\beta}$-lactamase gene reported in Korea were detected by PCR. As for the result of PCR, 30 isolates of P. aeruginosa were found to have $bla_{IMP-1}$-like and 1 isolate was found to have $bla_{IMP-1}$-like and $bla_{IMP-2}$. No clinical isolates were found to have $bla_{SIM-1}$, $bla_{OXA-23}$-like and $bla_{OXA-24}$-like. Random amplified polymorphic DNA (RAPD)-PCR and dendrogram for genetical similarity to band patterns of each clinical isolates were examined. P. aeruginosa were grouped into 7 clusters of up to 50% of similarity index. In the P. aeruginosa group, PS3 was resistant to the most antibiotics, PS1 was susceptible to the most antibiotics. PS7 was resistant to aztreonam unlike other groups. This is the first report of prevalence of carbapenemase in Chuncheon.

A Survey Study on Clinical Practice of Clinical Laboratory Science Students (임상병리과 학생들의 임상실습에 관한 의식조사 연구)

  • This study was conducted to investigate an attitude related to clinical laboratory science students' clinical practice. Data for this analysis was gathered from March to May, 2004 by the questionnaires of 348 students who are in their third year in 5 colleges and have experienced clinical practice. The students were satisfied with the term of clinical practice (70.4%) and the first factor influencing them in clinical practice was sincerity (45.1%), and the second was basic knowledge (19.8%). In the contents of clinical practice, 53.2% were satisfied but 43.3% of them showed negative responses because their practice was limited and they were simply assistants. 74.1% were satisfied after experiencing clinical practice and the first factor was improvement of understanding and interest in their major (65.9%) and the second was the gain of confidence (20.1%). Among anxieties in the clinical practice, a shortage of self-confidence (47.7%) was the highest. 81.3% of them wanted to maintain medical technologist's life continuously. It appeared that the more satisfied with their major, the more satisfied with the clinical practice (p<0.05). As a result, the students were satisfied with the clinical practice and fresh experience generally. But they were relatively unsatisfied with the practicality, and connection with the lecture. As a part of efforts to formulate an effective system for clinical practice, it is necessary to establish concrete goals and detail check lists to guide these students among professors, trainers and students.

Comparison of Histopathological Changes on the Three Drugs of Carbon Tetrachloride, Dimethylnitrosamine, Thioacetamide, and Bile Duct Ligation used for Induction of Liver Fibrosis in Rat

  • This study was carried out to compare the histopathological differences of liver lesions in carbon tetrachloride ($CCI_4$), dimethylnitrosamine (DMN), thioacetamide (TAA) and bile duct ligation (BDL)-induced rats. $CCl_4$, DMN and TAA were administered intraperitoneally and conducted bile duct ligation for 4 weeks to induce hepatic fibrosis. Indices of liver cell injury (steatosis, hydropic degeneration, bile duct hyperplasia, hemorrhage & hemosiderin deposition), the extent of liver fibrosis (fibrotic area) and the rate of regeneration (number of PCNA-positive cells) were investigated in each group. Liver tissues were stained with hematoxylin-eosin (HE), sirius red, prussian blue and immunostained with ${\alpha}$-smooth muscle actin (${\alpha}$-SMA), transforming growth factor-${\beta}1$ (TGF-${\beta}1$), proliferative cell nuclear antigen (PCNA), and quantified using a computerized image analysis system. Liver cell steatosis was significantly increased in $CCl_4$ and TAA groups, and hydropic degeneration and bile duct hyperplasia were significantly increased in TAA and BDL groups when compared with that in normal control, respectively. Fibrosis area was significantly increased in all four groups, especially in $CCl_4$ group. Correlation between ${\alpha}$-SMA and TGF-${\beta}1$ expressions in four groups was good. Hemorrhage area in liver parenchyma was significantly increased in DMN group only when compared with that in normal control, while hemosiderin deposition area was significantly increased in TAA and BDL groups as well as DMN group. The Number of PCNA-positive cells was significantly increased in all four groups, especially in TAA group. These results indicate that the duration and methods of hepatotoxic drug treatment are very important factors to make plans for animal experimentation on the induction of hepatic fibrogenesis in rats.

Relationship between AdeABC Efflux Pump Genes and Carbapenem in Multidrug-resistant Acinetobacter baumannii

  • Multidrug-resistant strain of Acinetobacter baumannii (MDRAB) is an emerging pathogen in health care facilities, preventing MDRAB is a public health concern. We conducted this experiment on a clinical isolate of A. baumannii with two main goals: the role of the efflux pump system in the stress provision of carbapenem and the response to the transcription level of the efflux pump gene. A total of 34 strains of A. baumannii was isolated from the Yangsan Hospital of Pusan National University. First, when we compared and observed the expression of the efflux pump gene and antibacterial resistance to carbapenem, a strong correlation was observed between carbapenem resistance and overexpression of adeB (P=0.0056). Second, a correlation between the efflux pump and concentration gradient and tolerance to carbapenem stress at the AdeABC efflux pump genes transcription level was confirmed. Our results revealed that the expression of the AdeABC efflux pump is an important resistance determinant in obtaining antibiotic resistance of the carbapenem group in A. baumannii.

  • Transporting clinical samples for microbiological testing requires a proper transport medium that guarantees the survival of microorganisms. Therefore, the aim of the study was to determine the ability of Amies Transport Medium (ATM) to maintain the viability of microorganisms in clinical specimens and its suitability as a transport medium for microbiological testing. This study evaluated the performance of swab provided by KS Co., Ltd. for three groups of bacteria comprising aerobic and facultative anaerobic bacteria, anaerobic bacteria, and fastidious bacteria, according to the Clinical and Laboratory Standard Institute (CLSI) 8.11.2. The ATM stability test was conducted by dividing the medium into two groups based on the product expiration date of use. All tested media, A and B (the date of manufacture and expiration date are different) showed ≥5 CFU, and there was no significant difference in the result values of Category A and Category B with different serial numbers for each test. The results of this experiment when cross-checked with the guidelines suggest that ATM is a suitable transport medium for microbiological testing, as it maintains the viability of microorganisms and is suitable for overgrowth trials. In addition, compared to the number of CFUs at the origin, the number of CFUs did not increase by more than 1 log after storage. These results have important implications for the development of transport media that can guarantee the survival of microorganisms in clinical specimens.

A Comparison of Genospecies of Clinical Isolates in the Acinetobacter spp. Complex Obtained from Hospitalized Patients in Busan, Korea

  • Of the Acinetobacter spp., A. baumannii (genospecies 2) is the most clinically significant in terms of hospital-acquired infections worldwide. It is difficult to perform Acinetobacter-related taxonomy using phenotypic characteristics and routine laboratory methods owing to clusters of closely related species. The ability to accurately identify Acinetobacter spp. is clinically important because antimicrobial susceptibility and clinical relevance differs significantly among the different genospecies. Based on the medical importance of pathogenic Acinetobacter spp., the distribution and characterization of Acinetobacter spp. isolates from 123 clinical samples was determined in the current study using four typically applied bacterial identification methods; partial rpoB gene sequencing, amplified rRNA gene restriction analysis (ARDRA) of the intergenic transcribed spacer (ITS) region of the 16~23S rRNA, the $VITEK^{(R)}$ 2 system (an automated microbial identification system) and matrix-assisted laser desorption/ionization-time of flight mass spectrometry (MALDI-TOF MS). A. baumannii isolates (74.8%, 92/123) were the most common species, A. nosocomialis (10.6%, 13/123) and A. pittii isolates (7.5%, 9/123) were second and third most common strains of the A. calcoaceticus-A. baumannii (ACB) complex, respectively. A. soli (5.0%, 6/123) was the most common species of the non-ACB complex. RpoB gene sequencing and ARDRA of the ITS region were demonstrated to lead to more accurate species identification than the other methods of analysis used in this study. These results suggest that the use of rpoB genotyping and ARDRA of the ITS region is useful for the species-level identification of Acinetobacter isolates.

Current Status of Ophthalmic Optometry Laboratory Personnel in Korea, Japan, and the United States (한국, 일본, 미국 안과검사인력의 현황)

  • The education and training system, the ISCO (International Standard Classification of Occupations), and the legal systems of Japan and the United States consider all ophthalmic optometry laboratory personnel as ophthalmologists. They encompass optometrists, orthoptists, optometric technicians, and ophthalmic medical technicians. Data retrieved from the KOSIS (Korean Statistical Information Service) revealed that the number of opticians associated with the department of ophthalmology in 2022 could be appraised by classifying their medical institutions; contrarily, the number of clinical laboratory technologists could not be assessed. However, the current research investigated a general tertiary hospital and determined that clinical laboratory technologists outnumber opticians. Classification in Korea is based on ophthalmic optometry laboratory personnel, ISCO, ISCED (International Standard Classification of Education), the medical service act, the act on medical service technologists, and the higher education act. These results cannot be compared to the optometrists evaluated in the United States. Ophthalmology is a suitable profession for optometric technologists and technicians who perform under the instructions of ophthalmologists and optometrists. The field of eye healthcare would be benefitted by assigning the management based on their qualification according to the requirement of the job title, such as 'Clinical Optometry Technologist' to be given to clinical laboratory technologists and opticians who work in the ophthalmic optometry laboratories after obtaining a private qualification endowed by the Korean Ophthalmological Society and the Korean Optometry Society.

A Study on Spatial and Physical Environment Satisfaction of Clinical Laboratory Scientists (진단검사의학과 검사실의 공간 환경만족도 조사 연구 - 근무자 중심으로 -)

  • The purpose of this study is to provide the basic guidelines of spatial and physical environment for the planning and design of clinical laboratory by analysing the extent of satisfaction of clinical laboratory scientists. The data for this analysis was gathered from March to May 2005 by the questionnaires of 208 clinical laboratory scientists who work in the 13 hospitals. All the collected data was analyzed by the SPSSWIN program. In this study, the satisfaction measurement tool was composed with 8 items and 5 score scale. The mean score of satisfaction for spatial and physical environment was 2.89 out of 5.0, "noise" was the lowest 2.40, "temperature and moisture of the lab." was 2.72, "lab. area, service area and administration area" was 2.77, "passageway space" was 2.94, circulation of workers was 2.94, "color of finish" was 3.19, "lighting of lab." was the highest 3.39. In conclusion, various factors, noise, temperature and moisture, clinical lab area, were evaluated to moderate dissatisfaction. Noise was especially the first serious problem in clinical lab. Considering the high growth of the number of tests, the planning of the clinical laboratory size should be considered not only to provide the optimal size but how it will correspond to the growth of the number of tests. Therefore the arrangement of each section need to be flexible in arrangement which is inevitable in expansion and reconstruction in the future.

Transient Non-Regenerative Anemia in a Dog with Granulomatous Meningoencephalitis Following Leflunomide Treatment

  • A 10-year-old, spayed female Maltese dog was tentatively diagnosed with granulomatous meningoencephalitis (GME) on magnetic resonance imaging. The meningoencephalitis was classified as aseptic GME because cerebral fluid analysis did not reveal an infectious aetiology. Two months after leflunomide treatment (Arava; Sanofi; 4 mg/kg/day), the patient developed non-regenerative, macrocytic, and normochromic anemia. As the patient's anaemia began after the administration of leflunomide, and other differentials for anaemia had been ruled out, the leflunomide was determined to be the cause and this treatment ended. After 15 days, the anaemia resolved spontaneously. This is the first report of reversible aplastic anaemia following treatment with leflunomide in a canine patient with GME.

Comparison of Clinical Biochemicals in Sera of Senior Patients with Hyperglycemia

  • We analyzed the concentration of Aspartate Aminotransferase (AST), Alanine Aminotransferase (ALT), Alkaline Phosphatase (ALP), Gamma Glutamyl Transferase (GGT), total protein (TP), albumin, total cholesterol (TC) and triglyceride in sera of patients (n=455) aged 60 years and older with hyperglycemia (${\geq}120mg/dL$ in serum). The average concentration of biochemicals in patient group was compared with those in reference group. Our data showed that the average concentrations of AST and TC in sera of senior patients with hyperglycemia were significantly increased by 27.5% (P<0.05) and 82% (P<0.05) in total group, 38.5% (P<0.05) and 75.0% (P<0.05) in sixty years' group, 16.0% (P<0.05) and 89.7% (P<0.05) in seventy years' group and 27.0% (P<0.05) and 79.5% (P<0.05) in eighty years' group, respectively. Patients with hyperglycemia showed a significant decrease in albumin level by 6.7% (total group, P<0.05), 4.5% (sixty years' group, P<0.05), 8.9% (seventy years' group, P<0.05) and 4.5% (eighty years' group, P<0.05), respectively. In conclusion, the high concentration of glucose in the senior patients with hyperglycemia may be a cause of severe liver function and lipid metabolism disorder.
