Job Analysis of Korean Oncology Advanced Practice Nurses in Clinical Workplace -Using the DACUM Method- (임상에서 활동 중인 종양전문간호사의 직무분석-DACUM 기법 이용-)

  • Purpose: The purposes of this study were to identify job definition, duties and tasks of Korean oncology Advanced Practice Nurses (APNs) and assess frequency, importance, and difficulty for each of duty and task using the DACUM method. Methods: The DACUM committee was organized with nine oncology APNs. The committee documented the job definition, duties and tasks based on their daily job descriptions. Validity of the elicited duties and tasks was tested by a study team and 6 oncology APNs. Data were collected using a questionnaire survey from 53 oncology APNs. Results: The job description of oncology APNs was identified 12 duties and 66 tasks. Among the twelve duties, 'education for patients/families' was the most frequently performed and was the most important duty. 'Case management' was the most difficult duty. Among 66 tasks, 'providing education for patients/families' was most frequently performed. 'Providing education for patients/families' and 'assessing patient condition comprehensively' were the most important tasks. 'Implementation research' as the principal investigator was perceived as the most difficult task. Conclusion: In this study, Korean oncology APNs' job performances were identified and analyzed. It is recommended that job description for oncology APNs needs to be standardized and reshaped in advanced way in future.

The Effect of Sa-am acupuncture on Knee Osteoarthritis

  • Objective : Acupuncture has been widely used throughout the world for the treatment of knee osteoarthritis (knee OA). This study investigated whether acupuncture, particularly Sa-am acupuncture, could be effective in relieving pain and improving the symptoms of knee OA. Method : This study was conducted as a prospective, randomized, controlled, and patient- and investigator-blinded clinical trial. Forty volunteers with knee OA participated in the study. All participants were screened through an inclusion and exclusion criteria. Thirty four participants completed the clinical trial. In total, forty subjects were randomly selected to receive Sa-am acupuncture. Eight sessions of acupuncture were given at the contralateral side of the problematic knee for 4 weeks. Korean translation of Western Ontario and McMaster Universities Osteoarthritis Index (KWOMAC) scores were measured twice: at the beginning and end of the clinical trial period. Both the Patient Global Assessment and physical health scores based on the 36-Item Short-Form Health Survey were also used to measure the results. Results : Compared to the pre-trial scores, the Sa-am acupuncture group (n=34) showed a significant decrease in KWOMAC total scores according to a paired t-test. The Sa-am acupuncture group also showed significant improvement in the Patient Global Assessment when compared to the pre-trial. Conclusions : Sa-am acupuncture for knee OA resulted in an improved KWOMAC total score. This was mostly driven by the function component score that was greatly affected by acupuncture. However, further studies with expanded designs are needed to solidify this finding with scientific rigor.

STUDY OF THE EFFECT OF COMPREHENSIVE NURSING CARE ON THE ADJUSTMENT OF CHILDREN TO HOSPITALIZATION (유.소아를 위한 포괄적 간호가 그들의 병원생활 적응에 미치는 영향에 관한 연구)

  • The goal of modern nursing is to provide comprehensive nursing care to patients. If comprehensive nursing care to children (within the hospital setting) is to be provided, consideration of the stage of growth and development of the child is especially important. From clinical observation, it appeared that nurses often disregarded individual requirements of children in giving nursing care. Therefore, the purpose of this study is to show that comprehensive nursing care which is based on an understanding of the growth and development of the child contributes to both the child and the mother's adaptability to the child's hospitalization. Method: Sixty children, three to three year of age, hospitalized at the Yonsei University Pediatric Ward ware studied. From April 1, 1973 to May 5, 1973, children admitted to the hospital were assigned to either an experimental or a compare groups. There were 30 children in each group. The sex and age of the children in each group was similar. In both groups were more male than female children. In the experimental group, each mother stayed with hot child continuously during his hospitalization. In the compare groups, the mother or some other member of the family stayed with the child. Each day on the child's admission the investigator visited the ward from 1-2 P.M. to 9-10 P.M., in order to provide comprehensive care for the experimental -group. The assistance given the nurses by the investigator was in the form of conferences regarding care and in giving direct care to the child and his mother. The compare group of children received nursing care as usually provided by the hospital. The instruments used to obtain the data for analysis were as follows: 1. The fear and anxiety reaction of the child was recorded by observation of the investigator for four areas: 1) separation from parent and relatives 2) reaction to Doctor and Nurse with white gowns 3) reaction to nursing care 4) reaction to injection and tests, etc. 2. Regression in area of eating, sleeping, and elimination were recorded by the investigator by questioning the mother and by observation. 3. Adaptability to the hospitalization was recorded by direct questioning of the children for areas of emotional and social adjustment. For children older than 3 years of age or children not seriously ill, using the simple I. Q. test this was possible for only 35 of the total 60 children. Result: 1. 55 percents of the total 60 children had been prepared by their parents for hospitalization. The children who had received prior preparation accepted hospitalization more readily than those who had received no preparation. (χ²=4.6 Ρ<0.05) 2. On admission 31.7 percent of the children expressed verbal fear of their discase or treatment. 25 percent felt that the disease was due to their mistake. 3. There was a significant difference in the reaction of the child to separation from the parent or relatives between the two groups. The experimental groups showed less anxiety due to separation than the compare group. (χ²=4.34 Ρ<0.05) In both groups there was less anxiety due to separation among school age (6-12 years) children than among preschool age (3-5 years) children. (χ²=9.22 Ρ<0.05) 4. More than half of the children in both groups reacted with fear and avoidance to doctor and/or nurses wearing white gowns. (χ²=0.06 Ρ<0.05) 5. The experimental group reacted more favorably to nursing in general than the compare group. (χ²=4.8 Ρ<0.05) 6. There was no difference in the fear and refused reaction to special tests and/or such as X-rays and injections, etc. between the groups. (χ²=3.77 Ρ<0.05) 7. More children in the compare group showed regressive tendencies in eating, sleeping, and elimination habits than in experimental groups. (χ²=2.3 Ρ<0.05 χ²=3.88 Ρ<0.05 χ²=4.9 Ρ<0.05) 8. There was a significant difference in the adaptability to hospitalization between the two groups. The experimental groups adapted more readily. (χ²=2.02 Ρ<0.05) 9. For children who had higher I.Q. s the adaptability to hospitalization was better regardless of the group. (χ²=5.03 Ρ<0.05) However, because of the small number of cases (60), this finding cannot be extrapolated without further verification. The date demonstrates that there was a greater adaptability to hospitalization by the child when comprehensive nursing care was given. By planning care and applying knowledge of growth and development to meet, nurses are in a position to prevent some of the psychological trauma associated with hospitalization.

Study of the New Patients of Acupuncture and Moxibustion Department in Dongeui Oriental Medical Hospital. (주소증(主訴症)으로 본 침구과(鍼灸科) 초진환자(初診患者)의 분석(分析))

  • Purpose In order to profits to investigator who want to know acupuncture and moxbustion treatments are available to what symptoms today and how goes on the clinical lecture for oriental medical students, this research was carried out. Methods The following conclusion were derived from 745 of patients who treated in acupunture & moxibustion department of Dongeui Oriental Medical hospital from the July in 2006 till January in 2007. Results 1. There were 203(27.25%) persons of Lumbago, 101(13.56%) persons of Bel's palsy, 94(12.62%) persons of Omalgia, 80(10.74%) persons of paralysis of limb, 74(9.94%) persons of disordinary sense of limb and etc. 2. There were 395 females(53.02%) and 350 males(46.98%). 3. The patients over 60th were 26.72%, the 50th were 23.08%, the 40th were 22.14%, the 30th were 15.44%, 20th were 8.45%. So the older the more patients. Conclusion In accordance with the above results, acupuncture & moxibustion teatments are good to use degenerative musculoskeletal disease, pain, paralysis and etc.

The Study of an Entrance Experience into Major Subject of a Student Nurse (간호학생의 전공교육 입문경험에 관한 연구)

  • This research was an attempt to find out an entrance experience into major subject of student nurse. The purpose of this study was to understand an lived learning experience of student nurse. Method of the study was applied a phenomenological approach used by Colaizzi's analytic technique. Participants were 33 sophomore student nurse from TC University. The investigator conducted participated observations and in-depth unstructured interviews which were audio taped under the permission of student nurse, formulating meanings, themes and theme clusters. Results of the study were revealed 6 categories, 14 theme clusters, and 31 themes. Six categories include 'feeling anxious about clinical application', 'making superficial class', 'evaluation for ability of memory', 'deficit of academic evaluation criteria', 'insufficiency of textbook' and 'deficit for uniqueness of nursing science'. This findings confirm the fact that the student nurse's entrance experience was complicated and diversely influenced by their learning experiences. In conclusion, this study will prove helpful not only in understanding student nurse's entrance experience about their specialities but also in developing curriculum for nursing education.

Development and evaluation of continuing education course in renal nutrition

  • BACKGROUND/OBJECTIVE: Competent renal dietitians are crucial for better patient compliance and clinical outcomes, specifically in critical settings. The aim of this study was to develop and evaluate an evidence-based course in renal dietetics for dietitians working in health care systems where dietetic specialization is absent. SUBJECTS/METHODS: Fifteen licensed dietitians working with hemodialysis patients in Lebanon were randomly recruited to participate in the course. The latter was developed by the study's primary investigator, according to evidence-based practice guidelines, and focused on all aspects of renal nutrition. Total course duration was 28 hours spread over a 2 month period. Dietitians' knowledge in renal nutrition was tested pre- and post-training through a 23-item questionnaire; the total score was expressed in percentage (< 60% score indicated insufficient knowledge). Paired-samples t test was used for statistical analysis. RESULTS: Overall knowledge of the dietitians significantly improved post-training and reached satisfactory levels (pre: $38.75{\pm}17.20%$, post: $62.08{\pm}21.85%$). Sub-analysis of the change in the knowledge showed significant and satisfactory improvement only in 3 topics: 1) correct body weight use in calculations, 2) energy estimation method and 3) phosphorus management. Knowledge in the fluid management significantly improved but did not achieve a satisfactory level. CONCLUSION: The course significantly improved dietitians' knowledge in renal nutrition. If adopted as part of the continuing education of dietitians in countries that lack dietetic specializations, it may serve the first step towards improving health care practice.

Diagnostic Accuracy of Dermoscopy for Scabies

  • The diagnostic accuracy of dermoscopy (DS) for scabies, a highly contagious parasitic disease, remains disputed. This study aimed to assess the diagnostic accuracy of DS in scabies, analyze the factors influencing DS, and explore its role in post-treatment evaluation. Patients with suspected scabies were randomly divided into 2 groups: 71 patients in the skin scraping (SS) group and 73 patients in the DS group. The diagnostic efficiencies of SS and DS in these groups were calculated. We also analyzed the influence of body part and investigator competence on the accuracy of DS. Then 16 body parts with typical signs of scabies were monitored by DS 2 and 4 day after sulfur ointment treatment. The sensitivity and specificity of DS were 98.3% and 88.5%, respectively. Hands, arms, and the abdomen had higher positivity rates than other body parts (P<0.001). The accuracy of dermatologists' interpretations of images negative for scabies in the intermediate- and high-level groups was higher than that in the low-level group (P<0.001). At follow-up, the mites were still visible on 43.8% to 62.5% of the skin lesions 2 and 4 day after sulfur ointment treatment. These results showed that DS could significantly increase the accuracy of diagnosing scabies owing to its high sensitivity and specificity. Therefore, it may be useful for monitoring clinical responses to anti-parasitic treatment.

A Study on Self-efficacy of Course Decision and Major Field Satisfaction in Dental Hygiene Students (치위생과 학생의 진로결정 자기 효능감과 전공만족 관계 연구)

  • Purpose. This study was purposed to seek for a way to enhance the self-efficacy of students in dental hygiene department at deciding their careers by analyzing the relationship between the self-efficacy and the major satisfaction. Methods. The investigator conducted a survey targeting 253 students attending on dental hygiene departments in two 3-year-graduate colleges and 1 university located in Busan from May 1st to May 30th, 2013. The used research instruments were the major satisfaction questionnaire and the Career Decision-Making Self-Efficacy Scale Short Form(CDMSES-SF). The survey was conducted in self-administered questionnaire. The data was analyzed with SPSS (ver 12.0) and also conducted t-test, ANOVA and multi-regression analysis. Results. It was found that self-efficacy level of students in dental hygiene department was $3.22{\pm}0.38$ point, and their major satisfaction level was $3.27{\pm}0.47$ point. The factors enhancing the self-efficacy of students in dental hygiene department at deciding their careers were identified as the curriculum satisfaction, the relationship satisfaction, and the recognition satisfaction. It was identified that more these elements were increased, the higher the self-efficacy level was enhanced. Conclusions. In order to enhance the self-efficacy of students in dental hygiene department at deciding their careers, it is considered that the curriculum should be organized carefully under reasonable plan, and there should be more active communication between professors and students as well as students each other. Besides there needs some educational programs to inspire the pride and sense of duty for the dental hygiene department.

Perceptions held by Investigators, IRB Members and IRB Administrators on the Bioethical Oversight System of National R&D Projects (국가연구개발과제 생명의학윤리 감독체계에 대한 인식조사 및 제언)

  • Purpose: Aim of this study was to gather and evaluate perceptions of investigators, IRB members, and IRB administrators on the appropriateness of IRB review process and bioethical oversight system of national R&D (NR&D) projects. Method: Investigators, IRB members, and IRB administrators at 17 different institutions were surveyed using convenience sampling and survey questionnaires were partially group-specialized to consider any differences between the groups. Results: Participants included 29 investigators, 37 IRB members, and 17 administrators with response rate of 100% (83 of 83). According to the responses obtained, insufficient preparation time for constructing protocol and gaining IRB approval was one of the main problems in the IRB review process (investigator 79.3%, IRB administrator 88.2%). Also, discrepancy between NR&D and IRB's protocol formats was another major issue (IRB members 96.4%, IRB administrator 100%) and most investigators (89.7%) had to modify the original NR&D protocol to obtain IRB approval. Moreover, it was reported that 13.8% of investigators and 31.3% of IRB administrators did not submit midyear reports to IRB and for bioethical issues of NR&D projects, 17.2% of investigators did not include information on project status and safety issues in the annual reports. Conclusion: In conclusion, for successful and ethical completion of R&D projects, revision of both IRB review process and NR&D project protocol formats as well as implementation of appropriate bioethical oversights are necessary.

A Study on Influential Factors Related to the Smartphone Addiction Tendencies of Some Dental Hygiene Students (일부 치위생과 학생의 스마트폰 중독경향과 관련된 영향요인 연구)

  • Purpose: The purposes of this study are to investigate the actual uses of a smartphone and tendencies for smartphone addiction among dental hygiene students, examine the effects of life stress, self-esteem, communication ability and interpersonal relationship ability considered to have correlations with addiction, and is provided basic data to solve the problems. Methods: The investigator analyzed data collected from 332 students in the Dental Hygiene Department at J University in Gyeongnam Province with SPSS 18.0. Results: The students scored mean 2.34±0.40 points on the four-point scale of smartphone addiction tendencies, 1.70±0.43 on the life stress scale, 2.99±0.32 on the interpersonal relationship ability scale, 2.80±0.42 on the self-esteem scale, and 2.83±0.39 on the Communication ability scale. There were differences in their smartphone addiction tendencies according to such general characteristics as the grade(p<.05), perceptions of a smartphone(p<.001), and hours of using a smartphone(p<.001). They had differences in life stress according to the grade(p<.01). Their smartphone addiction tendencies increased according to higher percentage of a smartphone(β=.360, p<.001), longer hours of using a smartphone(β=.225, p<.001), and greater life stress(β=.297, p<.001). The higher their smartphone addiction tendencies were, the lower their self-esteem was(β=.-210, p<.001). Conclusions: The study provides data needed for dental hygiene students to recognize the problems of smartphone addiction, find improvement measures, and make effective use of a smartphone.
