Using a Cellular Automaton to Extract Medical Information from Clinical Reports

  • An important amount of clinical data concerning the medical history of a patient is in the form of clinical reports that are written by doctors. They describe patients, their pathologies, their personal and medical histories, findings made during interviews or during procedures, and so forth. They represent a source of precious information that can be used in several applications such as research information to diagnose new patients, epidemiological studies, decision support, statistical analysis, and data mining. But this information is difficult to access, as it is often in unstructured text form. To make access to patient data easy, our research aims to develop a system for extracting information from unstructured text. In a previous work, a rule-based approach is applied to a clinical reports corpus of infectious diseases to extract structured data in the form of named entities and properties. In this paper, we propose the use of a Boolean inference engine, which is based on a cellular automaton, to do extraction. Our motivation to adopt this Boolean modeling approach is twofold: first optimize storage, and second reduce the response time of the entities extraction.

Comparison of Two pMDIs in Adult Asthmatics: A Randomized Double-Blind Double-Dummy Clinical Trial

  • Background: Only a few studies directly compared the therapeutic efficacy and safety of two pressurized metered-dose inhalers (pMDIs) in asthma. We analyzed the asthma treatment outcomes, safety, and patient preferences using formoterol/beclomethasone (FORM/BDP), a pMDI with extra-fine particles, compared with formoterol/budesonide (FORM/BUD), another pMDI with non-extra-fine particles. Methods: In this randomized, double-blind, double-dummy parallel group study, 40 adult asthmatics were randomized to FORM/BDP group (n=18; active FORM/BDP and placebo FORM/BUD) or FORM/BUD group (n=22; active FORM/BUD and placebo FORM/BDP). During the two visits (baseline and end of 8-week treatment), subjects were asked to answer questionnaires including asthma control test (ACT), asthma control questionnaires (ACQ), and Quality of Life Questionnaire for Adult Korean Asthmatics (QLQAKA). Lung function, compliance with inhaler, and inhaler-handling skills were also assessed. Results: Ten subjects in the FORM/BDP group and 14 in the FORM/BUD group completed follow-up visits. ACT, ACQ, QLQAKA (a primary outcome), and adverse events did not differ between two groups. We found that the increase in forced expiratory volume in 1 second/forced vital capacity and forced expiratory flow at 25% to 75% of the pulmonary volume in the FORM/BDP group was higher than in the FORM/BUD group. Regarding preference, subjects responded that the flume velocity of FORM/BDP was higher, but more adequate than that of FORM/BUD. They also answered that FORM/BDP reached the trachea and bronchus and irritated them significantly more than FORM/BUD. Conclusion: The use of pMDI with extra-fine particles may relieve small airway obstruction more than the one with non-extra-fine particles despite no significant differences in overall treatment outcomes. Some asthmatics have a misconception about the adequacy of high flume velocity of pMDIs.

Research Design for the Verification of Constitutional Prescription Determinants (체질처방 결정요인 확인을 위한 연구설계)

  • Objectives The purpose of this study is to design a research to find clinical factors involved in the decision-making process for determining the constitutional prescription based on prospective clinical data. Methods We have created a draft of the case report form. Therefore, seven constitutional experts was interviewed for additional items to be important used to determine the constitutional prescription in clinic. Interviews were done from December 2012 to March 2014, experts per person 2-3 times, took time of about 120 minutes per interview. Since then, we developed the final case report form through the expert meeting. At the same time, the developing the electronic case report form (eCRF) and the protocol to collect constitutional treatment cases was also discussed. Results & Conclusions The items of the case report form were subject general, lifestyle, health measurement, record of expert, prescription and evaluation after medication. The part of the clinical symptoms of the record of expert allowed to be recorded in the 5-point scale for the collection of quantitative data as much as possible. Assuming a re-visit of the patient, if necessary, twice the recording were to be possible. At the same time, the eCRF and the protocol to collect constitutional treatment cases were also developed. In this study, it will be able to more objectively standardize the medical decision making process that the experts of constitutional prescription decision. As a result, it will be possible to provide the standardized constitutional medical services.

A study on the effect of tooth ash and hydroxylapatite root implantation on preservation of alveolar ridge (치아회분(齒牙灰粉) 및 인공(人工) 수산화(水酸化) 아파타이트 치근(齒根)이 치조제(齒槽堤) 유지(維持)에 미치는 영향(影響))

  • Biocompatibility of dense synthetic hydroxylapatite is well known and the direct bond with adjacent bone developed. The purpose of this study was to evaluate the potential of clinical application of tooth ash for preservation of alveolar ridge. For this purpose the author performed an experimental implantation of the particulate and root form of both pure dense hydroxylapatite and tooth ash in alveolar sockets immediately after extraction. The pure dense hydroxylapatite was particulate form and root form made by Calciteck Inc. The tooth ash was prepared by incineration at $950^{\circ}C$, and the syrindrical form of the tooth ash was sintered and trimmed to fit the size of the each extraction socket of 10 mongrel dogs. After sugery the clinical, roentgenographical, and histological observation was carried out. The results obtained were as follows; 1. Clinical observation disclosed no dehiscence and exfoliation due to tissue rejection. 2. Vertical resorption of alveolar bone occurred in all experimental sockets as well as the control sites on the roentgenograph. 3. Osteoclastic activity appeared at the inner surfaces of the crestal alveolar bone on the 1st week but disappeared on the 2nd week. 4. There were macrophages in the particulate form on the 1st and 2nd week after surgery but no macrophages appered in the root form. S. New bone formation was developed from the bony wall of experimental sockets and grew to bond with the implant materials. In particulate form the new bone formation did not occur in central zone independently. 6. Tooth ash implant sites were covered with the newly formed bony trabeculation from third week, but Calcitite particles were covered with soft tissue. 7. Generally the healing occurred more rapidly in control sites than in implant sites.

A Case of Childhood-onset Form of Nemaline Myopathy (소아형 Nemaline 근병증 1예)

  • Nemaline myopathy is one of the congenital myopathy, which is characterized by histological findings of nemaline bodies (rods) and type 1 fiber hypotrophy and predominance. It can be classified into three forms according to the age of onset and clinical features: congenital form, childhood-onset form, and adult-onset form. We report an 18-year-old patient with generalized muscle weakness and dysmorphic features who was diagnosed as typical childhood-onset nemaline myopathy.

Reliability of Visual Gait Analysis according to Clinical Experience Level of Physical Therapists (임상 물리치료사의 경험에 따른 시각적 보행 분석의 신뢰도 연구)

  • Purpose: Visual gait analysis plays a pivotal role in determining the important gait problem of patients. A few studies have been published and have received little attention regarding visual gait analysis on patients with orthopedic problems. The purposes of this study were to investigate the difference of reliability levels according to experience of clinical physical therapists. Methods: Thirty-five clinical physical therapists, 5 high experienced, 15 experienced, and 15 inexperienced, were recruited and individually evaluated these videotaped gait patterns of the participants, and filled up the structured gait analysis form. The gait of nine participants was videotaped. Reliability levels were calculated by the Intraclass Correlation Coefficients (ICC). Results: The inter-rater reliability of high experienced group (ICC=0.56; 95% CI: 0.50-0.62) was comparable to that of the experienced raters (ICC=0.48; 95% CI: 0.43-0.53) and inexperienced group (ICC=0.42; 95% CI: 0.38-0.46). High experienced group reached a higher inter-rater reliability level. The average intra-rater reliability of the high experienced group was 0.70 (ICCs ranging from 0.54 to 0.82). The experienced group reached an average intra-rater reliability of 0.61 (ICCs ranging from 0.47 to 0.81). The inexperienced group attained average ICC values of 0.53 (ICCs ranging from 0.30 to 0.74). Conclusion: Use of a structured gait analysis form as described in this study was found to be moderately reliable. Clinical experience appears to increase the reliability of visual gait analysis.

Clinical managements of implant periapical lesions: a report of three cases with five to twelve years of follow-up (임플란트 근단 병소의 임상적 접근 방법: 5 - 12년간의 증례 보고)

  • The purpose of this report is to suggest clinical managements of implant periapical lesions by presenting three clinical cases managed by either the infected form or the inactive form with the follow-up period of five to twelve years. One patient with no clinical symptom was regarded as inactive form. Two patients having pain were regarded as infected form and have been under the systemic antibiotic therapy. In one patient, the symptom subsided and the size of radiolucent lesion decreased. However, the other patient showed increased size of lesion causing the implant unstable, which leaded to remove the implant and to replace it. There was neither additional increase of the lesion nor functional problem for all three. It is important to detect implant periapical lesion in early stage before jeopardizing the stable implant and manage properly using systemic antibiotic therapy and surgical approach if needed, depending on infected form and inactive form.

Development of Case Review Form for Detecting Adverse Events (위해사건 확인을 위한 증례검토지 개발)

  • Objectives: The purpose of this study is to develop a case review form for detecting adverse events through a medical records review in hospitalized patients in South Korea. Methods: To develop the case review form, several literatures were reviewed, first. Through the clinical expert meeting, screening criteria were selected and case review form was developed. Result: The Korean version of case review form consisted of the review form-1 for adverse event screening and form-2 for adverse event identification. The applied methodology for the case review form is determined according to the previous studies. For example, the method used in the first stage review is nurse review. Furthermore, the National Coordinating Council for Medication Errors Reporting and Prevention index is used to categorize disability, and a scale of 1 to 6 was used in the causation scores and preventability scores, respectively. Through the clinical expert meeting, a total of 41 screening criteria were selected. Conclusion: The Korean specific case review form was developed for detecting adverse events in hospitalized patients. The results from this study can be used in a large-scale study regarding the nationwide incidence of adverse events.

Relationship of Self-Directedness and Practice Satisfaction to Clinical Practice in Nursing Students: The Mediating Effect of Clinical Competence (간호학생의 임상실습에 대한 자기주도성과 임상실습 만족도: 임상수행능력의 매개효과)

  • Purpose: This study was done to identify the relationship of self-directedness, clinical competence and practice satisfaction to clinical practice for nursing students and to investigate the mediating effect of their clinical competence between self-directedness and clinical practice satisfaction. Method: The data were collected form 291 nursing students in clinical practice using self-report questionnaire on April 25 and June 20, 2011. Results: There were statistically positive relationships between self-directedness, clinical competence and practice satisfaction to clinical practice in nursing students. Clinical competence was found to completely mediate the relationship between self-directedness and clinical practice satisfaction. Conclusion: This study shows that self-directedness to clinical practice for nursing students have no direct effect on clinical practice satisfaction. It has an indirect effect by clinical competency.

  • Toxoplasmosis is a parasitic infection caused by the obligate parasite, Toxoplasma gondii, that is distributed throughout the world. Infection is produced by contact with animal like dog, cat, pig, cow or horse. It is also possible to produce clinical disease in the form of oropharyngeal infection through consumption of raw or incompletely cooked meat. Handling of contaminated cat litter has been found to be the major route of transmission. There are two clinical manifestations of toxoplasmosis: the congenital and the acquired form. The congenital form is characterized by hydrocephalus, chorioretinitis, convulsion, and intracerebral calcifications in the newborn. The acquired form is further subdivided into the disseminated and the lymphadenopathic types, the latter being the more common form of the disease in men. This is a case report of a Korean 8 year 2 months female congenital toxoplasmosis child. Clinical feature, patient's medical history, radiographic evaluation and dental problems are discussed in this report.
