Purpose: Gaucher disease is caused by a ${\beta}$-glucocerebrosidase (GBA) deficiency. The aim of this study is to investigate the clinical and genetic characteristics according to subtypes of Gaucher disease in the Korean population. Methods: Clinical findings at diagnosis, $GBA$ mutations, and clinical courses were reviewed in 20 patients diagnosed with Gaucher disease. Results: Eleven patients were diagnosed with non-neuronopathic type, 2 with acute neuronopathic type, and 7 with chronic neuronopathic type. Most patients presented with hepatosplenomegaly, thrombocytopenia, and short stature. In the neuronopathic group, variable neurological features, such as seizure, tremor, gaze palsy, and hypotonia, were noted at age $8.7{\pm}4.3$ years. B cell lymphoma, protein-losing enteropathy, and hydrops fetalis were the atypical manifestations. Biomarkers, including chitotriosidase, acid phosphatase, and angiotensin-converting enzyme, increased at the initial evaluation and subsequently decreased with enzyme replacement treatment (ERT). The clinical findings, including hepatosplenomegaly, thrombocytopenia, and skeletal findings, improved following ERT, except for the neurological manifestations. L444P was the most common mutation in our cohort. One novel mutation, R277C, was found. Conclusion: Although the clinical outcome for Gaucher disease improved remarkably following ERT, the outcome differed according to subtype. Considering the high proportion of the neuronopathic form in the Korean population, new therapeutic strategies targeting the central nervous system are needed, with the development of a new scoring system and biomarkers representing clinical courses in a more comprehensive manner.
Objectives : Sleep Disorders are very common in clinical stages and there are many reports and papers regarding this subject. I try to understand the present state of sleep research in JON (The Journal of Oriental Neuropsychiatry) for the benefits of clinical practice. Methods : We searched and read every article on JON from 1990 to 2012 and selected ones that are related to Sleep Disorder; then analyzed the data to 5 categories, like literature study, case study, Clinical Data-Analysis study, experimental study, and clinical trial. Results : 1. We Found 41 articles relating to sleep disorder in JON form 1999 to 2012, and there were 9 literature studies, 13 case studies, 13 Clinical Data-Analysis studies, 4 experimental studies, and 2 clinical trials. 2. There were Sa-Am Acupuncture Method, Pharmacopuncture, electroacupuncture, and Auricular acupuncture to treat Sleep Disorder. 3. There were many additional treatments like Purgative Therapy, etc. 4. They reported the ordinary sleep pattern of Soeumin and Soyangin, too. 5. In the experimental study, they reported several herbs, herbal prescription on the expression of melatonin receptors, and Punsimgeimgamibang on insomnia, anxiety, emotionality in rats. 6. There were 2 clinical trials, one for Hwabyung-patient with insomnia by acupuncture treatment and another for insomnia in the elderly by ETF-I program. 7. There were 2 case of other sleep disorder (enuresis, sleepwalking) and 2 literature study of dream. Conclusions : In JON regarding sleep disorder, the majority was due to insomnia. Therefore, we also have to expand our sight into other sleep diseases. We need more clinical trials and experimental researches for the construction of EBM Sleep Disorder in Oriental Neuropsychiatry.
Background : Over the last thirty years, majority of researches on clinical effectiveness of acupuncture have been explanatory (or experimental) randomized controlled trial. The benefits of acupuncture in clinical trials are still controversial and most studies concluded that further control studies were required. Standardized combinations of acupuncture points for all the experimental subjects in various past studies have been criticized because such treatments do not reflect current routine clinical treatment. Objective : This paper aims to review pragmatic clinical trials on the effect of acupuncture treatment and to develop the ideal clinical research methodology of acupuncture study. Method : Clinical studies of acupuncture relevant with pragmatic or individualized trials were searched mainly in Pubmed and Science direct databases. All articles were fully reviewed by researchers, and data were evaluated by usage of a standardized form. Results & Suggestion : Pragmatic acupuncture researches were tried for various symptoms (eg. low back pain, hypertension, depression during pregnancy, sleep quality in HIV disease, chronic poststroke leg spasticity, headache, etc). Individualized acupuncture treatments based on oriental disease pattern diagnosis reflexes practical treatments which is more effective than unified and fixed acupuncture treatments without any theoretical basis of oriental medical philosophy. Conclusion : To overcome the controversies and limitations of past explanatory acupuncture trials, more individualized and tailored acupuncture trials with the theoretical basis of oriental medical diagnosis is highly recommended. Also clear definition and categorization of pattern identification should be established for further active clinical researches and applications of acupuncture.
This study was conducted to identify the perceived stress, self-esteem and clinical practice stress of nursing students and how these variables affect clinical practice stress. Participants consisted of 164 nursing students who were in the third and fourth years who had experienced clinical practice at two nursing universtities located in Gyonganam and Jeonnam, and agreed to the survey. Data were collected using questionnaires form April 1 to April 30, 2021. Collected data were analyzed using the SPSS 22.0 Win statistics program with descriptive statistics, an t-test, ANOVA, scheffe's test, and multiple regression. The study found that clinical preactice stress was positively correlated with perceived stress but negatively correlated with self-esteem and the factors affecting clinical practice stress wre perceived stress, self-esteem, major satisfaction and interpersonal relationships and their explanatory power was 27.0%. This suggests that it is necessary to develop and apply a program to reduce perceived stress and improve self-esteem in order to reduce clinical practice stress of nursing students.

■ Objectives The purpose of this case study is to report the effect of Gami SSanghwa-tang on a patient with central post-stroke pain. ■ Methods The patient was treated with herbal medicine Gami SSanghwa-tang, acupuncture, pharmaco-acupuncture, and moxibustion. The treatment effect was evaluated by Numerical Rating Scale(NRS), Neuropathic Pain Symptom Inventory(NPSI), and 36-item Short-form Health Survey(SF-36). ■ Results After the treatment, the NRS score of pain intensity was reduced from moderate to mild degree. The total NPSI score and subscores also decreased, as the various features of the pain were relieved. The SF-36 score increased, as the patient's quality of life improved. ■ Conclusion This case study suggests that Gami SSanghwa-tang, could be effective in reducing pain and improving quality of life of patients suffering from central post-stroke pain.
Objectives : This paper studies the pulse diagnosis as found in Youksimanpil, which is a series of medical charts containing 150 diagnosis records of Yi Suki, a doctor who was active in Joseon during the 17-18th centuries. Through this effort, the paper aims to shed light on how pulse was utilized in the Korean medicine, and in process tries to reveal the essence of Korean medicine's treatment method. Methods : 60 charts where pulse method was used are selected in Youksimanpil and a table is created with them. Figures are drawn to explain four steps of pulse-sensing from the simple method to highly advanced method. Charts are presented with the corresponding original texts and their translations. With these efforts, the paper attempts to reveal the broad understanding of the doctor of Joseon period who consistently kept to the most basic principle of pulse diagnosis. Results : The efficiency of pulse diagnosis depends on the unity and simplicity in diagnosis and prescription. There were continued efforts between the doctors in Joseon to collect and compare the experiences they gained from clinical practices in order to organize their findings and form a system. These are: (1) individual pulse, (2) patternized pulse, (3) balance between left and right pulses, (4) balance between pulse and body, and (5) the doctor's extemporaneous diagnosis. In that efforts, they protect the principle of holistic diagnosis, which is one of Korean medicine's core principles. Conclusions : Thanks to the existence of medical charts that presents in detail how the texts of Donguibogam were applied in real clinical practices, today we can see Korean medicine's highly advanced synergy between textual knowledge and clinical experiences as recorded in the form of charts.
Purpose: The aim of this study was to investigate if the 7-item Berg balance scale (BBS) 3-point, which is a short form of the BBS (SFBBS), has compatible psychometric properties in comparison with the original BBS, and also to study the concurrent validity using a 10-meter walk test (10mWT) and a timed up and go test (TUG), which are widely used with SFBBS in clinical settings. Methods: A total of 255 patients who had experienced stroke participated in this cross-sectional study. We used results obtained from 188 patients who completed both 10mWT and TUG. The three levels in the center of the BBS were collapsed to a single level (i.e.,0-2-4) to form the SFBBS. The concurrent validity was assessed by computing the Spearman coefficients for correlation among outcome measures and in between each outcome measure and the SFBBS. As there were four outcomes, the corrected p-value for significant correlation was 0.013 (0.05/4). Results: Spearman coefficients for correlations and evaluation instruments for concurrent validity revealed significantly high validity for both of SFBBS and BBS (r=0.944). 10mWT and TUG were -0.749 and -0.770 respectively, which are in the high margin and are statistically significant (p>0.000). Conclusion: SFBBS has sound psychometric properties for evaluating patients with stroke. Thus, we recommend the use of SFBBS in both clinical and research settings.
This study was carried out to evaluate the diagnosis of acute clinical mastitis (ACM) which was based on the vital signs and complete blood count (CBC) tests in dairy cows. Twenty eight dairy cows diagnosed with ACM, were selected for the study between Jan 2003 and July 2006 in the National Institute of Animal Science. Based on their vital signs (rectal temperature, depression, rumen contraction and, dehydration status), ACM was divided into three different classes; mild, moderate and severe forms. In addition, ACM cows were subjected to CBC tests for further diagnosis of ACM. Of the 27 dairy cows diagnosed with ACM, 3 cows were determined to have a mild form, while moderate and sever forms were each observed in twelve cows. Among of them, 4 cows died, 5 cows were culled and 18 cows were recovered. In the mild form, all haematological parameters were comparable with normal values. However, leukopenia, due to neutropenia and lymphocytopenia, appeared characteristically in the moderate and severe forms. Using the observation of vital signs in conjunction with CBC tests, the diagnosis of ACM is more accurate, and is helpful in making decisions of whether treatment or culling of dairy cows infected with ACM is most appropriate.
Purpose: Increased bone regeneration has been achieved through the use of stem cells in combination with graft material. However, the survival of transplanted stem cells remains a major concern. The purpose of this study was to evaluate the viability of transplanted mesenchymal stem cells (MSCs) at an early time point (24 hours) based on the type and form of the scaffold used, including type I collagen membrane and synthetic bone. Methods: The stem cells were obtained from the periosteum of the otherwise healthy dental patients. Four symmetrical circular defects measuring 6 mm in diameter were made in New Zealand white rabbits using a trephine drill. The defects were grafted with 1) synthetic bone (${\beta}$-tricalcium phosphate/hydroxyapatite [${\beta}-TCP/HA$]) and $1{\times}10^5MSCs$, 2) collagen membrane and $1{\times}10^5MSCs$, 3) ${\beta}-TCP/HA+collagen$ membrane and $1{\times}10^5MSCs$, or 4) ${\beta}-TCP/HA$, a chipped collagen membrane and $1{\times}10^5MSCs$. Cellular viability and the cell migration rate were analyzed. Results: Cells were easily separated from the collagen membrane, but not from synthetic bone. The number of stem cells attached to synthetic bone in groups 1, 3, and 4 seemed to be similar. Cellular viability in group 2 was significantly higher than in the other groups (P<0.05). The cell migration rate was highest in group 2, but this difference was not statistically significant (P>0.05). Conclusions: This study showed that stem cells can be applied when a membrane is used as a scaffold under no or minimal pressure. When space maintenance is needed, stem cells can be loaded onto synthetic bone with a chipped membrane to enhance the survival rate.

Objectives In order to extend the bounds of korean medical instruments, we studied on the clinical application of BUDDEUM (the remedy of the synchronus combination of cupping boil and moxibustion) by the industry-academic cooperation. Methods The korean clinic, enterprise and university participated in jointly an educational-industrial complex. We studied on the clinical application of BUDDEUMI according to the structural character, functional efficiency and clinical case. And therefore we confirmed clinically the application of BUDDEUMI. Results The results was as follows. 1. The effects of BUDDEUMI are observed on the detoxification of blood waste material, & the form and appearance of effete matter are various. 2. The BUDDEUMI massage treatment operated in the abdominal and the back in order to recover energy and increase immunity. 3. The heating treatment by BUDDEUMI operated in sickly region, in order to pour into heat of moxibustion. Conclusion According to the results, by the set up the exclusive therapy room and the arrange of the exclusive human agency of BUDDEUMI, the newly and various therapies of korean medicine are possible to develop.
