총의치 제작에 적용 가능한 디지털 기술과 임상적 고찰 (Application and consideration of digital technology for removable complete denture)

  • Digital technology has changed various aspects of the clinical dentistry. The intraoral scanner and Computer-aided design / Computer-aided manufacturing (CAD-CAM) technology are widely used in fabricating fixed prostheses and in implant surgery. These technologies greatly improved the efficiency of clinical and laboratory procedures. With all newly introduced software, devices, and clinical studies, digital technology has been actively applied in removable prostheses. It is now possible to fabricate the removable prostheses more quickly and easily through subtractive and additive manufacturing. Various clinical and laboratory protocols were introduced by many manufacturers. The purpose of this review is to summarize the literature on digital technology for fabricating complete denture with current status and future perspectives.

White matter injury following rotavirus infection in neonates: new aspects to a forgotten entity, 'fifth day fits'?

  • That rotavirus infection can cause neurological symptoms in young children has been well established. However, it is surprising why rotavirus infection has been overlooked as a cause of neonatal seizures for many years, despite significant research interest in neonatal rotavirus infection. Neonates are the age group most vulnerable to seizures, which are typically attributed to a wide range of causes. By contrast, because rotavirus infection is usually asymptomatic, it has been difficult to identify an association between this virus and neonatal seizures. The conventional wisdom has been that, although neonates are commonly infected with rotavirus, neurological complications are rare in this age. However, recent studies using diffusion-weighted imaging (DWI) have suggested a connection between rotavirus infection and neonatal seizures and that rotavirus infection can induce diffuse white matter injury without direct invasion of the central nervous system. The clinical features of white matter injury in rotavirus-infected neonates include the onset of seizures at days 4-6 of life in apparently healthy term infants. The recent findings seem to contradict the conventional wisdom. However, white matter injury might not be a completely new aspect of rotavirus infection in neonates, considering the forgotten clinical entity of neonatal seizures, 'fifth day fits'. With increased use of DWI in neonatal seizures, we are just starting to understand connection between viral infection and white matter injury in neonates. In this review, we discuss the historical aspects of rotavirus infection and neonatal seizures. We also present the clinical features of white matter injury in neonatal rotavirus infection.

의료현장에서의 설명동의에 대한 윤리적 고찰 (An Ethical Appraisal of Informed Consent in the Clinical Setting)

  • Purpose: This study attempted to analyze problems of informed consent in the clinical setting and appraise ethical aspects inherent in such issues in order to boost awareness of informed consent and its implementation among healthcare professionals. Methods: Study methods included identifying ethical meanings of informed consent in the clinical setting based on the principal ethics, and exploring the process of informed consent utilizing communicative ethics and feminine care ethics Results: The ethical basis of informed consent encompasses not only respect for autonomy but also prohibiting malice, practicing beneficience, and establishing justice. These principles, however, are limited in illustrating the ethical aspects of communicative ethics and care ethics that are entailed in informed consent within clinical settings. The ethical meaning of informed consent involves a communicative and caring process between healthcare professionals, patients, and family built on mutual respect. Conclusion: Healthcare professionals must fully understand the ethical meanings of informed consent and in turn respect and protect the clients' right to know and making decisions. Nurses especially, must take on the role of mediator and advocate throughout the process of obtaining informed consent, and practice ethical caring by facilitating communication grounded in mutual understanding among the physician, patient, and family members.

  • Marfan syndrome which is complicated with spontaneous pneumothorax is a rare chance we can contact usually. There are many aspects of the clinical features in the Marfan syndrome such as ectopia lentis, cardiovascular anomalies, and skeletal anomalies. With the use of this paper, we report a rare case, Marfan syndrome complicated with spontaneous pneumothorax.

Endocrine problems in children with Prader-Willi syndrome: special review on associated genetic aspects and early growth hormone treatment

  • Prader-Willi syndrome (PWS) is a complex multisystem genetic disorder characterized by hypothalamic-pituitary dysfunction. The main clinical features include neonatal hypotonia, distinctive facial features, overall developmental delay, and poor growth in infancy, followed by overeating with severe obesity, short stature, and hypogonadism later in development. This paper reviews recent updates regarding the genetic aspects of this disorder. Three mechanisms (paternal deletion, maternal disomy, and deficient imprinting) are recognized. Maternal disomy can arise because of 4 possible mechanisms: trisomy rescue (TR), gamete complementation (GC), monosomy rescue (MR), and postfertilization mitotic nondisjunction (Mit). Recently, TR/GC caused by nondisjunction at maternal meiosis 1 has been identified increasingly, as a result of advanced maternal childbearing age in Korea. We verified that the d3 allele increases the responsiveness of the growth hormone (GH) receptor to endogenous GH. This paper also provides an overview of endocrine dysfunctions in children with PWS, including GH deficiency, obesity, sexual development, hypothyroidism, and adrenal insufficiency, as well as the effects of GH treatment. GH treatment coupled with a strictly controlled diet during early childhood may help to reduce obesity, improve neurodevelopment, and increase muscle mass. A more active approach to correct these hormone deficiencies would benefit patients with PWS.

돌발성 난청 환자 3례에 대한 임상적 고찰 (The Clinical Study of Three Patients Hospitalized Due To Sudden Sensorineural Hearing Loss)

  • Backgrounds : sudden sensorineural hearing loss is defined as a disease that has abrupt onset of sensorineural hearing loss without definite cause. In some cases, this disease cause tinnitus, vertigo, headache addition to hearing loss. In traditional oriental medicine, sudden sensorineural hearing loss is could be diagnosed as deficiency of Yang-Gi(陽氣). Objects & Methods : This study is clinical observation about three sudden sensorineural hearing loss cases. For diagnosing in these cases, pure tone audiometry was used. For treatment, acupuncture, herbal-medicine and moxibustion therapy were used. Results & Conclusions : as follows. 1. Generally, in this disease, the shorter the whole period from the time that occurs to the time that starts to treat is, the more the results are better. The research shows that short-term case is more efficient than the other long-term two cases in the aspects of symptoms and pure tone audiometry . 2. In making a diagnosis of sudden sensorineural hearing loss and measuring the effect, subjective symptoms generally correspond to the improvement of pure tone audiometry, but not completely. Therefore, in treating patients, the doctor must consider these both aspects and keep the balance.

일개지역 근무 간호사의 한방의학에 대한 인식, 경험 및 태도별 한방간호 관심도에 관한 조사 연구 (Interests on Oriental Nursing by Perception, Practice, and Attitude of Oriental Medicine of Nurses Working in One Region (Survey))

  • Purpose: This study investigated the perception, practice and attitude of nurses towards oriental nursing. Methods: Nurses (N=162) from a localized region were surveyed during June, 2008. Collected data was statistically analyzed using t-test, one-way ANDVA, and Sheffe test. Results: The perception of surveyed nurses to the following aspects of oriental nursing varied significantly: "chances to hear of oriental nursing", "treatment function of oriental medicine", "necessity of clinical application", "treatment method", and "educational demand on oriental nursing". Nurses' interest in the following aspects of oriental nursing was significantly different: "experiences undertaken in the treatment of oriental medicine", "treatment result", and "clinical result from the patients who were under the oriental medicine treatments". Nurses' attitude toward oriental nursing varied significantly in response to the following: "recommendation by other nurses or neighbors" and "experiences being questioned by patients on oriental nursing question". Conclusions: Support for oriental nursing and systematic education for nurses would be beneficial in light of a changing medical environment.

Image Guided Radiation Therapy

  • Over the past decades, radiation therapy combined with imaging modalities that ensure optimal image guidance has revolutionized cancer treatment. The two major purposes of using imaging modalities in radiotherapy are to clearly delineate the target prior to treatment and set up the patient during radiation delivery. Image guidance secures target position prior to and during the treatment. High quality images provide an accurate definition of the treatment target and the possibility to reduce the treatment margin of the target volume, further lowering radiation toxicity and improving the quality of life of cancer patients. In this review, the various types of image guidance modalities used in radiation therapy are distinguished into ionized (kilovoltage and megavoltage image) and nonionized imaging (magnetic resonance image, ultrasound, surface imaging, and radiofrequency). The functional aspects, advantages, and limitation of imaging using these modalities are described as a subsection of each category. This review only focuses on the technological viewpoint of these modalities and any clinical aspects are omitted. Image guidance is essential, and its importance is rapidly increasing in modern radiotherapy. The most important aspect of using image guidance in clinical settings is to monitor the performance of image quality, which must be checked during the periodic quality assurance process.

소아청소년 양극성 장애의 임상 경과 (Clinical Course of Bipolar Disorder in Children and Adolescents)

  • Objectives : The early onset of mood symptoms in bipolar disorder has been associated with poor outcomes in many studies. However, aspects of the clinical course of bipolar disorder in children and adolescents are controversial. The goal of this article is to review the clinical characteristics and longitudinal course of children and adolescents with bipolar disorders. Methods : Searches were conducted in MedLine, PsycINFO, KISS, and RISS using the terms phenomenology, clinical course, outcome, BPD, pediatric, children and adolescents. Twenty-one reports were selected : either original articles reporting symptoms and clinical characteristics of subjects (ages 5-18 years), or published articles in reviewed journals about bipolar disorder in children and adolescents. Results : Approximately 70% of subjects with bipolar disorder recovered from their index episode, and 50% had at least 1 syndromal recurrence, particularly depressive episodes. For 60% of the follow-up time, subjects had syndromal or subsyndromal symptoms with numerous changes in symptoms and shifts of polarity. Approximately 20% of BP-II subjects converted BP-I. Conclusion : Bipolar disorders in children and adolescents are characterized by episodic illness with subsyndromal and syndromal episodes with mainly depressive and mixed symptoms and rapid mood changes. Extensive follow-up time is needed to evaluate the continuity of bipolar disorder symptoms from childhood to adulthood.
