Effects of Self-Efficacy and Job Stress on Organizational Commitment among Clinical Dental Hygienists

  • Background: This study aimed to provide basic data to establish a foundation for efficient operation of the organization by assessing the effects of self-efficacy and job stress (measured by self-efficacy, job demand, and job autonomy) on organizational commitment among clinical dental hygienists. Methods: This was a cross-sectional survey study that used a self-administered questionnaire to collect data. After institutional review board approval, a survey was conducted from January to May 2017, targeting dental hygienists working in dental clinics and hospitals. The final 199 questionnaires were analyzed with PASW 18.0 for Windows (IBM Corp.). The data were analyzed using mean and/or standard deviation t-test, one-way ANOVA, and multiple regression analysis. Results: The means for self-efficacy, job demand, job autonomy, and organizational commitment were $55.94{\pm}9.65$, $26.55{\pm}2.29$, $20.24{\pm}3.88$, and $49.49{\pm}8.39$, respectively. Along with self-efficacy, the other factors of organizational commitment among dental hygienists that were statistically significant included job autonomy, mean salary (2,500~2,990 thousand Korean won [KRW] and ${\geq}3,000$ thousand KRW), and employee welfare (good), which are sub-areas of the surface acting. In other words, it was found that the higher the mean salary, the better the employee welfare, the higher self-efficacy, and the higher the organizational commitment, and the explanatory power of the model was approximately 42.1%. Conclusion: These results suggest that the higher the self-efficacy, job autonomy, mean salary, and employee welfare, the higher the organizational commitment. In order to improve job demand among dental hygienists, it is necessary to establish an effective plan to improve job welfare, self-efficacy, and job autonomy.

Evaluation of Pain Reduction and Clinical Efficacy of Feedback-Controlled Ultrasonic Scaler

  • Background: Recently, a piezoelectric ultrasonic scaler based on a feedback control mechanism was introduced for pain relief. This study aimed to investigate the effects of a new ultrasonic scaler in reducing pain and discomfort in adults. Methods: A newly introduced ultrasonic scaler (Master 700®) was used as the test device and a conventional ultrasonic scaler device (PIEZON®) was used as the control device. Forty-one healthy adults visited the dental clinic for dental scaling but did not undergo scaling or periodontal treatment within 6 months. Intraoral examinations were performed before scaling and 3 months later; before scaling, both devices were randomly assigned on the left or right side of each dentition (split-mouth model) and scaling was performed by a registered dental hygienist. The levels of pain and discomfort during scaling were evaluated subjectively and objectively using the visual analog scale (VAS) and physiological monitoring of the heart rate (HR), respectively. Time was measured for each device. Results: All clinical indicators, except bleeding on probing, significantly improved with both devices. The treatment times were 7 minutes, 13 minutes (control) and 6 minutes, 59 minutes (test). VAS scores for pain were 4.89±2.12 (control) and 4.58±2.77 (test) points out of 10; for noise, these were 4.68±2.33 (control) and 4.55±2.55 (test), and for vibration, the values were 4.26±2.0 (control) and 4.18±2.48 (test). HR averages were 72.34±3.39 (control) and 75.97±9.78 (test) beats/min. No statistically significant differences were observed between the devices. Conclusion:The pain, discomfort levels, and scaling time of the new piezoelectric ultrasonic scaler did not differ from those of the conventional device. Further research and development are necessary for more prominent pain-relief effects of scaling devices.

한국과 미국의 치위생학과 교육과정 운영실태 (The actual condition of operating dental hygiene curriculum between Korea and America)

  • Objectives : The purpose of this study is to train talents with high quality in dental hygienists by suggesting problems and proposing basic data available for applying to development in dental hygiene curriculum, through analyzing contents of dental hygiene curriculum. Methods : To analyze curriculum of educational institutions for dental hygienists between America and Korea, it utilized materials that were notified on each university homepage, analyzed documents, analyzed curricular content analysis, comparatively analyzed America's ASDHEP field, and comparatively analyzed Biomedical Science field, dentistry field, prevention and public field, clinical dental hygiene field, integrated curriculum, and teaching course between domestic university and American university. Results : 1. The basic field of ASDHEP was indicated the average credit in 28.6 for 4-year university with establishment and the average credit in 26.9 for 3-year university with establishment. The curricular subject field of ASDHEP except the basic field was indicated the average credit in 29.45 for 4-year university with establishment and the average credit in 30.68 for 3-year university with establishment. The prevention and public field by university was indicated the totally average credit in 27.5 for 4-year university and in 26.2 for 3-year university. The clinical dental hygiene field by university was indicated the totally average credit in 27.5 for 4-year university and in 35.0 for 3-year university. The integrated curriculum was indicated to be established dental hygiene practice(3 universities), dental clinics practice for Dept. of clinical dental hygiene(2 universities), and comprehensive dental hygiene practice(1 university) for 4-year university, and to be established clinical dental hygiene(2 universities), comprehensive dental hygiene practice, dental hygiene practice, and dental clinics practice(1 university, respectively) for 3-year university. Comparing the teaching courses by university, they were established diversely from 8 credits to 2 credits. Conclusions : Seeing the above findings, a plan for dental hygiene curriculum needs to be continuously developed so that dental hygienists can enter upon a professional career. Development in the international standard curriculum proper for global era is considered to be desperately needed.

치위생(학)과 학생들의 임상만족도와 수행경험과의 관계 (Relationship between Degree of Clinical Satisfaction and Experience on Performance for Dental Hygiene Students)

  • 본 연구는 치위생과학생의 실습특성에 따른 임상실습만족도와 임상수행경험과의 관계를 규명하기 위해 2014년 11월에서 12월까지 현장실습 경험이 있는 치위생(학)과 2~4학년에 재학 중인 480명을 대상으로 연구의 목적을 설명을 하고 동의한 자에 의해 조사를 실시하여 분석한 결과 다음과 같은 결론을 얻었다. 임상실습기관별 만족도에서는 저학년보다 고학년으로 갈수록 임상실습기관만족도(p<0.001)와 임상수행경험(p<0.001)에서의 만족도도 높은 것으로 나타났고, 전공에 대한 만족도도 비교적 높게 나타났다. 임상실습만족도와 임상수행경험은 기관별로는 치과병(의)원이, 실습기관 결정에서는 치과병(의)원일 경우 학교에서 결정할 경우 점수가 높게 나타났고(p<0.002), 실습기간은 종합병원이나 대학병원인 경우 '늘여야 한다'(p<0.001)가, 실습지도는 종합병원에서는 전담치과위생사일 경우(p<0.009)가 치과병(의)원에서는 기관장(p<0.010)이 점수가 높게 나타났다. 임상실습만족도와 임상수행경험에 영향을 미치는 요인으로는 전공만족(p<0.001), 실습기간(p<0.01)으로 나타났다. 학생들의 임상실습에 대한 만족도를 높이기 위해서는 임상실습기관별 특성을 고려하는 실습운영 시스템을 구성하고 교육을 전담하는 치과위생사로 하여금 능동적인 실습기간으로 운영될 수 있도록 하며 기관에서는 학교와의 지속적인 연계를 통해 끊임없이 교육내용을 개선 및 보완을 해야 할 것으로 생각된다.

임상 치과위생사의 언어폭력 경험 및 대처방안 (A Study on Verbal Abuse Experience and Coping Strategies of Dental Hygienist)

  • 본 연구는 임상 치과위생사가 경험한 언어폭력 실태를 조사하고자 하였으며 다음과 같은 결론을 얻었다. 치과 근무 중에 언어폭력 경험이 있는 치과위생사 177명(61.2%)으로 나타났으며, 최근 1달간 언어폭력 경험한 경우는 없음이 57.1%, 1회 15.7%, 2회 13.6%, 3회 이상이 13.6%로 나타났다. 언어폭력의 가해자는 환자가 67.9%로 가장 많은 것으로 나타났으며, 치과의사 21.1%, 선임 또는 동료 치과위생사 10.0%, 타 직종 직원 1.0% 순이었다. 치과위생사가 자각한 언어폭력의 이유에는 '의료서비스에 대한 불만을 나에게 화풀이 한다'가 17.0%로 가장 많았으며, 그 다음은 '환자나 보호자가 질병과 관련된 신체적 정서적 고통을 나에게 화풀이 한다' 14.1%, '치과위생사를 치료자가 아닌 아랫사람으로 인식한다' 12.6% 순으로 나타났다. 치과위생사가 자각한 언어폭력의 유형에는 '반말을 한다'가 21.7%로 가장 많았고, 그 다음으로 '큰 소리를 지른다', 16.3%, '비아냥거리는 말을 한다' 11.3% 등의 순으로 나타났다. 언어폭력을 경험한 응답자의 대처는 '참는다' 12.3%로 가장 많았고, 그 다음으로 '무시한다' 8.2%로 소극적 대처 양상이 많았다. 근무 특성에 따른 언어폭력 경험의 빈도 차이를 분석한 결과, 총 임상경력, 주 업무, 직위에 따른 언어폭력 경험은 차이가 있는 것으로 나타났다. 언어폭력 경험과 최근 1달간 언어폭력경험 횟수에 따른 자아존중감 및 직업 만족도의 차이를 검증한 결과 자아존중감은 통계적으로 유의하지 않았으나, 직업 만족도는 통계적으로 유의하였다. 이상의 결과를 종합해 보았을 때, 치과위생사의 언어폭력의 가해자는 대부분 환자이며, 언어폭력 내용은 의료서비스에 대한 불만, 신체적 정신적 고통을 화풀이하는 등 치과위생사가 경험하고 있는 언어폭력의 심각성을 알 수 있었다. 임상에서 발생할 수 있는 사례중심별 언어폭력을 구체적으로 분석하고, 언어폭력 방지를 위한 예방 프로그램 개발과 지속적인 연구가 필요할 것으로 생각된다. 또한 치과 내에서 발생하는 언어폭력을 예방하고 대처할 수 있는 방안을 고안하여 안전하고 협력적인 치과 근무환경을 조성하여야 할 것으로 생각된다.

일부지역 임상치과위생사의 전문직 자아개념과 조직사회화의 상관관계 (Correlation between the self-concept of clinical dental hygiene professionals and organizational socialization)

  • Objectives: The purpose of this study was to review the correlation between self-concept and organization of socialization in professional settings among dental hygienists. Methods: Statistical analysis was carried out on the collected data to identify any correlation between frequency and percentages. Cronbach's alpha value and one-way ANOVA were conducted. SPSS 12.0 was used to conduct a correlation analysis on self-concept and socialization in professional settings. Validity of the research tools was also confirmed. Results: There was a correlation between the identity and job satisfaction, with the figures showing great variance. Satisfaction was correlated with the nature of the corporation and job performance. In addition, communication had a negative correlation between job characteristics and identities, job performance, while, organizational commitment and positive correlation showed a statistically significant difference. Conclusions: The study reviewed the impact on organizational socialization. The dental hygienist is believed to be on going research into developing more effective and systematic training programs so that they can be effectively socialized society organizations to improve the professional self-concept.

임상 치과위생사와 치과기공사의 윤리적 가치관과 직업윤리 (Ethical Values and Work Ethic of Clinical Dental Hygienists and Dental Technician)

  • 본 연구는 치과위생사와 치과기공사 면허증을 발급받았으며 현재 병원에서 근무하는 대상자로 편의 표본추출하여 자료를 분석하였다. 회수된 설문자료 가운데 75부의 치과위생사 설문지와 90부의 치과기공사 설문지를 이용하여 최종자료로 선정하였다. 분석결과로는 두 집단 모두 윤리적 가치관이 높게 나타났고, 봉사기회가 주어진다면 봉사 할 의사가 있음으로 조사되어 직업적 직업적 사명의식도 높은 것으로 나타났다. 또한 조사대상자 두 집단 모두 임상활동에서 윤리적 문제가 대두 될 것임을 과반수 이상에서 응답했고, 이에 따른 직업윤리 교육이 절실히 필요하다고 응답했다. 이에 따른 윤리적 기반이 확립된 지속가능한 전문직업적 교육과 프로그램 개발로 직업적 전문성 함양을 위한 방안이 모색되어야 한다.

치위생(학)과 학생들의 성격유형이 임상실습에 미치는 영향 (The Effects of Personality Types of Dental Hygienist Students on Clinical Practice)

  • 본 연구는 부산광역시, 울산광역시 및 경상남도 일부 지역 대학교 433명의 학생을 대상으로 성격유형을 파악하고 임상실습 만족도에 미치는 영향을 조사하였다. 주요 변인들과의 관련성 결과, 외향성 및 저신경증적 경향의 대상자에서 임상실습 만족의 하위요인들에서 유의미한 결과를 보였다. 외향성 요인에 영향을 미치는 요인으로는 지도만족(p<0.001), 환경만족과 실습만족(p<0.05)이 유의한 수준으로 나타났고, 신경증적 경향성에 영향을 미치는 요인으로는 내용만족(p<0.05), 지도만족(p<0.001), 대인관계만족(p<0.05) 및 실습만족(p<0.05)이 유의한 수준으로 나타났다. 따라서, 치위생(학)과 학생들의 임상실습의 만족을 높이기 위해서는 재학 기간 중 성격유형 검사가 반드시 선행되어야 하며, 검사 결과를 토대로 맞춤형 상담프로그램의 강화와 대인관계 능력을 향상시킬 수 있는 다양한 프로그램이 개발되어야 할 것이다.

치과위생사의 임시치관제작 기술 숙련에 대한 인식도 (Recognition on technical skills of temporary resin jacket of dental hygienist)

  • Objectives : In order to manage to reformative tendency of the educational whole with change request of dental hygienics education which is diversificated and professionalized the manufacturing practice of a temporary resin jacket which is educated with from much dental hygiene is become accomplished, so far also the recognition which relates with the manufacturing practice of a temporary resin jacket of dental hygiene students there is not researched. The present research carried out the practice evaluation with the target of students of a dental hygiene department, who finished a manufacturing practice of a temporary resin jacket, analyzed and investigated the material, reported its result, and enforced a research. Methods : This research selected students of 6 dental hygiene departments located in the Daegu Gyeongbuk region, who have manufacturing experience of a temporary resin jacket, by a convenient sampling method, and then a questionnaire was executed by a self-administration method, and a total of 290 copies were statistically treated. The content of the questionnaire are composed of 2 questions on an attitude survey before and after manufacturing practice of a temporary resin jacket, 3 questions on an educator for practice of a temporary resin jacket, 7 questions on recognition of technical skills for manufacturing a temporary resin jacket, 9 questions on the practice content evaluation of manufacturing of a temporary resin jacket and recognition on manufacturing practice of a the temporary resin jacket. Results : 1. The average of recognition on technical skills for manufacturing a temporary resin jacket of a dental hygienist was 3.82 points, the average of evaluation on practice content of manufacturing a temporary resin jacket being 3.75 points, and the average of recognition on the practice of manufacturing a temporary resin jacket was 3.71 points. 2. Currently, as the educator for the practice of manufacturing a temporary resin jacket, dental hygienists were 188 persons(64.8%) and dental technicians was 102 persons(35.2%), and as educators they want in the future, dental hygienists were 97 persons(33.3%) and dental technicians was 193 persons(66.7%). Persons hoping reeducation for manufacturing a temporary resin jacket were 219 persons(85.9%). 3. There was a significant difference in consciousness of students before and after practice of manufacturing a temporary resin jacket(p<.001). 4. There was a significant difference in practice evaluation and practice recognition according to the educational subject(p<.001). 5. There was a significant reverse correlation in the educational subject, practice evaluation and practice recognition(p<0.01). Conclusions : The practice for manufacturing a temporary resin jacket in education of dental hygiene is necessary to improve job performance ability in more various and specialized dental treatment and to solve desire for improvement of professional technical skill ability of students, and accordingly. About study process of the future students observation investigation or student objective interview investigation etc. seeks a problem in the become research lead, the succeeding research which gropes the plan which is the possibility of solving the problem which discovered is thought will be necessary.

치과위생사 역할갈등 경험에 관한 질적연구 (A Qualitative Research on Role Conflict Experiences of Clinical Dental Hygienists)

  • 임상에 근무하는 치과위생사가 경험하는 역할갈등의 유발요인과 대처에 대한 본질을 탐색하고자 9명의 임상치과 위생사를 대상으로 심층면담을 실시하였다. 면담내용을 분석한 결과 업무 범위, 역할갈등, 역할갈등에 대한 대처와 한계로 이루어진 3개의 범주가 도출되었다. 첫 번째 범주는 업무 범위로 치과위생사가 인식하고 있는 업무 범위, 법적 업무 범위 및 실제 수행하는 업무 범위가 다소 차이를 보였다. 두 번째 범주는 역할갈등으로 대인관계, 업무, 업무환경, 역량, 전문직으로서의 정체감으로 인한 역할갈등을 경험하고 있었다. 세 번째 범주는 역할갈등에 대한 대처와 한계이며 치과위생사는 다양한 노력을 통해 역할갈등 상황을 해결하려 노력하였으며 그러한 노력에도 불구하고 해결되지 않는 현실에 직면할 경우 한계를 느끼고 이직을 선택하고 있었다. 본 연구결과를 토대로 치과위생사의 역할갈등에 관한 수준을 정량적으로 측정할 수 있는 도구의 개발 및 역할갈등 상황별 대처 방안의 효과에 대한 후속 연구가 필요하다.
