Purpose: This study was aimed to evaluate the performance of a community-based home care services model. Method: The subjects were 138 patients who have been enrolled during a 6 month's period, nursing records and nursing service bills. The data was collected by self report and chart review. The mean cost per visit was compared with those of hospitalization & clinic visit in the data from National Health Insurance Corporation. Result: A significant number of patients were bed ridden(63.8%) and unconscious (27.5%), and most of the patients had complex chronic diseases. Except nursing assessment, bed sore care was the most frequent nursing treatment(25.1%) in home care services. The mean cost per visit of home care services was 34,665 won, which was lower than those of hospitalization & clinic visit for medical aid. The patients were highly satisfied with the services by visiting nurse specialists. Conclusion: Community-based home care services provided cost-effective and satisfied services. Community-based home care services needs systematic supports to expand it's domain for promoting community health.
Objectives: The purpose of the study was to investigate the correlation between personal temperament, dental visit, and dental fear among adults. Methods: A self-reported questionnaire was completed by 400 adults visiting dental clinics from April to June, 2016. The questionnaire consisted of general characteristics of the subjects, dental fear, and temperament. Data were analyzed by t test, chi-square test, chi-square test, and pearson's correlation analysis using SPSS v. 23.0. Results: The first impression of dental clinics had an impact on dental fear (p<0.001). The negative image on the dental clinics increased dental fear of the visitors. There was a significant difference between the purpose of dental visit and dentla fear. Conclusions: The positive image of the dental clinics can solve the dental fear due to treatment in the adults. The dental consumers should try to have the comfortable attitude toward dental clinic in case of visit.
The purpose of this study was to collect basic data for the development of oral health education program of parents and to encourage their actual interest and participation in school oral health program. The authors surveyed parents' awareness and behaviors about the oral health of their children. The subjects in this study were 193 parents of elementary school children in Seoul. The collected data were analyzed, the obtained results were as follows: 1. Thirty-five percent of the parents investigated paid a regular visit to a dental clinic. The most common tooth brushing time among them was after dinner, the second was after breakfast, and this was followed by before bedtime and before breakfast 2. Ninety-four percent of the parents responded that prevention is necessary, 92.2% had a plan to visit a dental clinic to prevent dental caries, and 60.1% experienced receiving pit and fissure sealants. Ninety-seven percent responded that they did check their children's dental caries. 54.9% checked their children's oral status after tooth brushing. The parents who visit a dental clinic on a regular basis were more likely to check their children's teeth after tooth brushing than those who don't(P<0.05). 3. Eighty eight percent of respondents wanted to keep the fluoride mouth rinse program. 4. It is recommended that an oral health education program be developed for parents to visit dental clinics on a regular basis, to educate as to the right tooth brushing time, and to check out their children's oral status after tooth brushing.
Background: The use of telemedicine for postoperative visits is increasing, especially in rural areas. Few studies have investigated its use for arthroscopic shoulder patients. This study aims to evaluate patient satisfaction with telemedicine for postoperative clinic visits following arthroscopic shoulder procedures in a rural setting. Methods: Patients were prospectively enrolled using the following exclusion criteria: <18 years, open procedures, and non-compliance follow-up at 6 weeks postoperatively. All patients completed a 13-question satisfaction survey, while telemedicine patients completed an additional, separate seven-question survey. Patients who switched groups completed a four-question prompt to determine the reasons for switching. Differences between groups were evaluated by either Student t-test or Mann-Whitney U-test. Results: The study enrolled 32 patients, with five patients following up by telemedicine and 27 in person. Age and distance from clinic were similar between patients who were assigned to the telemedicine group, completed the telemedicine visit, and opted for in-person visits (all p>0.05). Patient satisfaction did not vary significantly based on care by the surgeon, concerns being addressed, thoroughness of visit, overall clinical assessment at a prior visit, and improvements in pain and physical function (all p>0.05). Among patients who opted out of telemedicine visits, the most common reason was a preference to meet in-person but these patients agreed that telemedicine visits are a good idea. Conclusions: Regardless of type of follow-up, individuals reported similar levels of satisfaction with treatment during the visit and improvements in pain and physical function.

Objectives : The purpose of this study is to report the effectiveness of a Korean Medicine Treatment on 89 severe atopic dermatitis patients all through 12 months. Methods : The degree of atopic dermatitis was measured in Objective SCORAD Index (OSI) with photos of patients. Then, the OSI score was estimated according to gender, age and intensity. Results : 1. The ratio of male to female was 41.57 : 58.43. The 24.72% of patients were in 0~11 year-old group, 16.85% were in 12~18 year-old group and 58.43% were in the group above 19 years old. 2. The initial OSI score was 64.43 and the final was 37.03 after 12 months. After 3, 6, 9, 12 months, the OSI score was significantly lower than the initial visit. 3. There was no significant difference between male and female patients on age, OSI scores and OSI improvements. 4. The patients who treated acupuncture were significantly older then who not treated. The OSI improvement had no significant difference whether treated with acupuncture or not. 5. There was no significant difference among 3 different age groups on initial OSI scores. The OSI improvement of 0~11 year-old group was significantly higher than the other groups after 12 month treatment. 6. The OSI score of 97.75% was decreased after 12 month treatment compared to initial visit. 7. After 12 month treatment, the distribution of severe atopic dermatitis was decreased from 100% to 37.08%. 8. There was no significant difference betweent steroid group and non-steroid group on OSI scores at initial visit and after 12 month treatment. Conclusions : The OSI Score of severe atopic dermatitis patients was decreased on the significant rate with Korean medicine treatment for 12 months.
The main purpose of this study was to identify the service quality of some dental clinics located in Gwangju and find an association between service quality and patient's intention to re-visit the same dental clinic. The search objects were 197 patients from five dental clinics, and data collected by self-administered questionnaire which was composed general characteristics, characteristics relating to visit of clinics, quality of service, patients satisfaction and intentions to re-visit. The used statistical analysis to find the association between quality of service and intention to re-visit was multiple regression analysis. This study looked at relations between quality of dental service, service satisfaction and intentions to visit again, and found that quality of service including six service areas had a statistically significant positive correlations with satisfaction and intentions of re-visit. When this study conducted a multiple regression analysis to identify variables that influence intentions of re-visit, it found that patients revisited the dental clinic only when quality of service was included. In conclusion, quality of service evaluated by patients influenced on their intention of re-visit and especially. Therefore, this study concludes that improvement in satisfaction and intentions of re-visit relies on Accessibility, that is, providing of medical information, consultant telephone, how delivery the dental medical system is. This study examined and analyzed the demographical characteristics of the study target and their expectancy level on the factors in selecting dental medical institutes (basic elements of dental clinics, traffic convenience, physical environment of dental clinic and explicit qualification of dentist) with the questionnaire survey method with 222 questionnaires answered and returned (return rate 88.8%) out of 250 copies distributed to the 1st, 2nd and 3rdyear college students attending technical colleges located in Cheongwon-Gun, Chungbuk, Korea from March 26 to April 7, 2009. The results of this study are as follows: 1. The demographical characteristics of the study target group consisting of 225 college students include 92.3% of age 20s (205 students); 123 males (55.4%) and 99 females (44.6%); the number of those who had at least one experience to visit dental clinic was 199 (89.6%) and 23 (10.4%) of those with no-visiting experience. 2. The descriptive statistics for the selection factor showed that basic elements, traffic convenience, physical environment and explicit qualification of dentist were regarded important as in order. In particular, the factors considered in selecting dental clinic was ordered from most to least in their importance beginning with cleanness of dental clinic as the most important factor, dentist's trustworthiness, kindness of medical staffs, low medical fee, dentist's knowledge level, modernized medical equipments and convenience in using medical facilities as the least. On the contrary, brand name of dental clinic, age, college, sex and appearance of dentist were rated low in order in terms of their importance in selecting dental clinic. 3. As for the difference in the selection factors across demographical characteristics, while male students placed importance on traffic convenience and physical environment of dental clinic, female students placed more importance on dentist's qualification and basic elements of dental clinic. With respect to the difference in the selection factors across the visiting experiences, while those with visiting experience regard traffic convenience more importantly, those without visiting experience regarded physical environment of dental clinic, dentist's qualification and basic elements of dental clinic. 4. In terms of pearson correlation coefficient, the result showed that all aspects in selecting dental clinic were positively correlated. Particularly, the correlation between physical environment of dental clinic and dentist's explicit qualification were correlated highest. Significant positive correlations were found high in traffic convenience, physical environment of dental clinic, dentist's explicit qualification and basic elements of dental clinic in order.
The contents of prescription service were comparatively analysed between health centers(HC) and private clinics(PC). Medical chart review was done for 330 otu-patients diagnosed with upper respiratory tract infection(UR) of 120 adults and 90 children, and gastritis or duodenitis of 120 adults. Emphasis on comparison was the prime cost of medication which used in prescription service. The results were as follows; 1. The prime costs fro the medication per visit of HC group were significantly higher than PC group in all three diseases, and the out of pocket payments of patients per visit were significantly lower in the HC group than PC group. 2. The reason for high prime costs of medication per visit of HC in adult case of URI were due to the idverse use of medication and long prescription period per visit. And high medication costs in children cases of URI in HC group were due to the longer prescription day. In cases of gastritis, the prime cost of medication was also higher because of longer prescription period and the higher prime cost of medication. The proportions of medications for injection in the HC and PC groups showed similar features. 3. In depth analysis of the prescription services showed the differences of the contents of medication. In adults cases of URI, the averaged cost of oral medication was significantly lower in HC group, but that of medication for injection was higher in HC group. In children cases of URI, the averaged cost of oral medication and medication for injection was lower in HC group than in PC group. But in the cases of gastritis it was was higher in HC group than in PC group. The prescription periods were longer in HC group than in PC group in all three diseases. As a conclusion prime medication cost and quality of prescription services of HC group were higher than PC group. In terms of health care the cost containment and quality assurance in physician visit for common disease, public sector utilization is good option for those perspectives. But it should not be generalized unless future study about structure and outcome research for quality assurance.
Background: As herpes zoster progresses via postherpetic neuralgia (PHN) to well-established PHN, it presents its recalcitrant nature to the treatment. At this point, the well-established PHN is fixed as a non-treatable, but manageable chronic painful neuropathic disorder. This study evaluated the incidence of complete relief from PHN according to PHN duration at their first visit, and the other factors influencing it. Methods: A retrospective chart review was performed on patients with PHN at a university-based pain clinic over 7 years. The responders were defined as patients who had complete relief from pain after 1 year of active treatment. Age, sex, PHN duration at their first visit, dermatomal distribution, and underlying disorders were compared in the responder and non-responder groups. Responders were also compared according to these factors. Results: Among 117 PHN patients (M/F = 48/69), 35 patients (29.9%) had complete relief from PHN. Mean ages were $64.3{\pm}10.6$ and $66.9{\pm}10.7$ years, numbers of male to female patients were 11/24 and 37/45, and mean durations of PHN at their first visit were $8.5{\pm}6.3$ and $15.3{\pm}10.7$ months in the responder and non- responder groups, respectively. In addition, PHN patients who visited the clinic before 9 months showed a better result. Dermatomal distribution and underlying disorders did not show significant differences. Conclusions: Almost 30% of PHN patients received complete relief. Those who sought treatment in a pain clinic before 9 months received a better result.
Objectives : This study was conducted to improve oral heath behaviour of child by analyzing child's mother oral health behaviour. Methods : A total of 361(95.0%) 3-year-old-child's mothers who sent their child to child care institution in Daegu were analyzed from October 1, 2009 to October 31, 2009. Results : 1. In relation to general nature and experience on oral health education of child's mother, mother with higher monthly income, over than graduation from university and professional job had high oral health education experience, which was statistically related(p<0.01)(p<0.001). 2. In the case of mother with oral health education experience, this mother had higher dental clinic visit for 1 year, higher experience on scaling, higher good food preference for teeth health and higher use of floss, which was statistically related(p<0.05)(p<0.01). 3. Regarding child's oral management behaviour by mother's oral health importance, in the case that oral health was important, flouride toothpaste use, restriction on cavity inducting food and toothbrushing before sleep were higher only, which was statistically related(p<0.05)(p<0.001). Conclusion : In case of mother with oral health education experience, this mother had experience on dental clinic visit for prevention of cavity, dental clinic visit for treatment and restriction on food in which their child's oral health behaviour were satisfactory, which was statistically related.
