  • Sternocostoclavicular hyperostosis is a relatively uncommon disease characterized by hyperostosis and soft tissue ossification in the clavicle, sternum and the anterior part of the upper ribs. Although the cause of this disease is unknown, radiologic feature is diagnostic. We report one case of sternocostoclavicular hyperostosis with pain and limitation of motion on shoulder.

  • Sternoclavicular joint posterior dislocations are considered a very uncommon, and type of injury where if esophagus or airway injury occurs behind the clavicle, it poses a high risk to the patient. In addition, if epiphyseal fracture occurs as a result of the sternoclavicular joint posterior dislocation, surgical treatment is often required. However, in the absence of a complete ossification of the clavicle, it is difficult to differentiate between a simple dislocation and epiphyseal fracture-dislocation solely based on simple radiographs or computed tomography scans. In this case report, the authors present a case in which a sternoclavicular joint posterior dislocation was diagnosed in a 14-year-old male athlete. The case report discusses how the posterior dislocation without epiphyseal fracture was diagnosed using an ultrasound and subsequently treated with successful outcomes using manual reduction. The case report presents our findings along with discussion that includes a literature review of relevant research.

  • Upper extremity deep vein thrombosis (DVT) is an unusual condition compared to lower extremity DVT, and it represents about 10% of all DVTs. We report a case of upper extremity DVT after clavicle fracture and immobilization.

  • The management of the exposed cardiac pacemaker or its lead is a new challenge to the plastic surgeon. This complication is not rare. Coburn et al. [1972] reported less than 5 percent, but Sowton et al. [1974] showed that over a period of 19 years, in a series of 372 patients, the pacing system had to be removed because of local wound breakdown or infection in 10 percent of the cases. The methods used to treat exposure may vary from removal and re-introduction at anterior site to the rotation of local flaps to cover the exposed pacemaker. Recently we have experienced 6 times of migration and recurrent skin ulcerations without pyogenic infection overlying the pacemaker in one patient. We developed a new technique, anchoring the pacemaker to the clavicle by a wire through the hole of clavicle and by creating a pocket under the pectoralis major muscle. Then we would like to emphasize this operating method could be choice of treatment to prevent the migration of pacemaker and the ulceration of skin when complication of implantation of pacemaker is occurred.

  • Acromioclavicular joint cyst is a extremely rare condition, usually occuring in the presence of a wide communication between glenohumeral and acromioclavicular joint in patients with a full thickness rotator cuff tear. Removal of cyst only is reported with high recurrence rate. There was no case previously reported in Korea. We report a case of recurred acromioclavicular joint cyst with a massive rotator cuff tear after simple excision, which was treated by excision of cyst, acromioplasty and resection of the lateral end of the clavicle.

  • Reported in this article is a case report of a patients who developed limb threatening ischemia as a consequence of a subclavian artery thrombosis resulting from screws. The subclavian artery thrombosis after open reduction and internal fixation of clavicle fracture, when it occurs, should be treated promptly by plate removal and claviculectomy. It can be prevented by placing screws in locations away from the underlying neurovascular structures.

흉쇄관절 전방변위 추나치료 1례 (Anterior Translation of Sternoclavicular Joint Chuna Treatment: A Case Report)

  • Objectives : This report is to investigate clinical effects of Chuna treatment on anterior translation of sternoclavicular joint. Methods : The female Patient, 50 years old, had anterior translation of sternoclavicular joint and the local pain. We treated the patient with Chuna treatment. In order to evaluate the improvement, Degrees of pain were measured with Visual Analogue Scale(VAS), and the change of anterior translation was assessed with anterior height of clavicle head. Results : After 3 weeks treatment, In VAS, Degrees of pain decreased 10 to 2. and Anterior height of clavicle head decreased 15mm to 5mm. Conclusions : Chuna treatment is effective on treating anterior translation of sternoclavicular joint and the local pain.

비관통성 흉부손상 410예 보고 (Nonpenetrating Chest Trauma 410 Cases)

  • From Jan. 1985 to Sep. 1989, four hundred and ten patients were admitted to the department of Thoracic and Cardiovascular Surgery, Yonsei University College of Medicine with a diagnosis of nonpenetrating chest trauma. The most common cause of injury were motor vehicle accidents(75.1%) with the prevalent age group being their forties. The four most common findings were rib fracture(83.4%), hemothorax(50.7%), pneumothorax(24.6%) and clavicle fracture(12.0%), Fifty piratory insufficiency and the ventilation time was correlated with 6 fators : 1) shock 2) endotracheal intubation or tracheostomy 3) hemothorax 4) clavicle fracture 5) flail chest 6) more than three combined injuries or combined head injury. Ventilatory care was also related with mortality and complications such as pneumonia sepsis and empyema thoracis(p<0.05). The most common organism of post-traumatic infection were Pseudomonas and Staphylococcus.

  • A FE model is to develop a personalized biomechanical model of the scoliotic spine that will allow the design of clinical test providing optimal estimation of the post-operation results. A flexible multi-body model of the spine including rib cage, clavicle, and scapular was developed to simulate several mobility simulations. Vertebrae, clavicle and scapular were represented using rigid bodies and ribs and sternum were modeled as flexible bodies. Kinematical Joints and spring elements were used to represent the intervertebral disc and ligaments respectively. Postero-anterior and lateral radiographics of a scoliotic spine were used to represent a 3D reconstruction. CT data for same patient were also used to verify vertebrae rotation driven from postero-anterior and lateral radiographic images. Simulated results showed good reducibility almost uniformly distributed along the spinal segments. It was also found that boundary and loading conditions, required to mimic the operation procedures, were proven to be very sensitive parameters to its results rather than its mechanical properties

  • The techniques of joint mobilization and traction are used to improve joint mobility or to decrease pain by restoring accessory movements to the shoulder joints and thus allowing full, nonrestriced, pain-free range of motion. In the glenohumeral joint, the humeral head would be the convex surface, while the glenoid fossa would be the concave surface. The medial end of the clavicle is concave anterioposteriorly and convex superioinferiorly, the articular surface of the sternum is reciprocally curved. The acromioclavicular joint is a plane synovial joint between a small convex facet on lateral end of the clavicle and a small concave facet on the acromion of the scapula. The relationship between the shape of articulating joint surface and the direction of gliding is defined by the convex-concave rule. If the concave joint surface is moving on a stationary convex surface, gliding occur in the same direction as the rolling motion. If the convex surface is moving on a stationary concave surface, gliding will occur in an opposite direction to rolling. Hypomobile shoulder joint are treated be using a gliding technique.
