Type Distribution of Lymphomas in Lebanon: Five-Year Single Institution Experience

  • Background: Lymphomas represent the fifth most frequent cancer in Lebanon. However, little is known concerning epidemiologic characteristics and distribution of lymphoid neoplasms according to the 2008 WHO classification. Materials and Methods: We conducted a retrospective study of lymphoma cases diagnosed from 2008 till 2012 at $H\hat{o}tel$-Dieu de France University Hospital. Results: A total of 502 new cases of lymphoma were diagnosed at our institution during a five year period: 119 cases (24%) were Hodgkin lymphomas (HL) and 383 cases (76%) were non-Hodgkin lymphomas (NHL). HLs were equally distributed in both sexes with a mean age at diagnosis of 30 years. Among NHL, 87% (332 cases) were B cell lymphomas, 9% (34 cases) were T cell lymphomas and 4%(17 cases) were classified as precursor lymphoid neoplasms. Among B cell lymphomas, 44% (147 cases) were diffuse large B cell lymphomas (DLBCL), 20% (65 cases) follicular lymphomas and 8% (27 cases) mantle cell lymphomas. DLBCL were equally distributed in both sexes with a mean age of 58 years. Follicular lymphomas were characterized by a male predominance (57%) and a mean age of 60 years. Mantle cell lymphomas showed a pronounced male predominance (85%) with a mean age of 60 years in men and 70 years in women. Some 72% of patients having T cell lymphomas were men, with a mean age of 57 years in men and 45 years in women, while 65% of patients having precursor lymphoid neoplasms were women with a mean age of 22 years in women and 30 years in men. Conclusions: The lymphoma subtype distribution in Lebanon is unique when compared to other countries from around the world. In fact, Hodgkin and follicular lymphomas are more frequent than in most Far Eastern, European and American countries, while T-cell lymphomas and DLBCL are less frequent.

  • The design of underground cavern is basically governed by the mechanical properties of ground mass distributed around excavation. It is seldom possible to consider all the factors of ground mass properties in the evaluation of ground mass behavior as well as to classify those factors to a simple category. Until computer sciences have developed to calculate complex and laborious mechanical simulation of underground openings, ground behavior was quantitatively and qualitatively evaluated using empirical classification system. In this paper, analysis methods of ground behavior for underground cavern using the prediction of loosening zone, empirical method derived from rock mass classification and element stress analysis are described with chronological sequence.

SPIRAL ARM MORPHOLOGY IN CLUSTER ENVIRONMENT

  • We examine the dependence of the morphology of spiral galaxies on the environment using the KIAS Value Added Galaxy Catalog (VAGC) which is derived from the Sloan Digital Sky Survey (SDSS) DR7. Our goal is to understand whether the local environment or global conditions dominate in determining the morphology of spiral galaxies. For the analysis, we conduct a morphological classification of galaxies in 20 X-ray selected Abell clusters up to z~0.06, using SDSS color images and the X-ray data from the Northern ROSAT All-Sky (NORAS) catalog. We analyze the distribution of arm classes along the clustercentric radius as well as that of Hubble types. To segregate the effect of local environment from the global environment, we compare the morphological distribution of galaxies in two X-lay luminosity groups, the low-$L_x$ clusters ($L_x$ < $0.15{\times}10^{44}$erg/s) and high-$L_x$ clusters ($L_x$ > $1.8{\times}10^{44}$erg/s). We find that the morphology-clustercentric relation prevails in the cluster environment although there is a brake near the cluster virial radius. The grand design arms comprise about 40% of the cluster spiral galaxies with a weak morphology-clustercentric radius relation for the arm classes, in the sense that flocculent galaxies tend to increase outward, regardless of the X-ray luminosity. From the cumulative radial distribution of cluster galaxies, we found that the low-$L_x$ clusters are fully virialized while the high-$L_x$ clusters are not.

Combined Cytogenetic and Molecular Analyses for the Diagnosis of Prader-Willi/Angelman Syndromes

  • Prader-Willi (PWS) and Angelman (AS) are syndromes of developmental impairment that result from the loss of expression of imprinted genes in the paternal (PWS) or maternal (AS) 15q11-q13 chromosome. Diagnosis on a clinical basis is difficult in newborns and young infants; thus, a suitable molecular test capable of revealing chromosomal abnormalities is required. We used a variety of cytogenetic and molecular approaches, such as, chromosome G banding, fluorescent in situ hybridization, a DNA methylation test, and a set of chromosome 15 DNA polymorphisms to characterize a cohort of 27 PWS patients and 24 suspected AS patients. Molecular analysis enabled the reliable diagnosis of 14 PWS and 7 AS patients, and their classification into four groups: (A) 6 of these 14 PWS subjects (44%) had deletions of paternal 15q11-q13; (B) 4 of the 7 AS patients had deletions of maternal 15q11-q13; (C) one PWS patient (8%) had a maternal uniparental disomy (UPD) of chromosome 15; (D) the remaining reliably diagnoses of 7 PWS and 3 AS cases showed abnormal methylation patterns of 15q11-q13 chromosome, but none of the alterations shown by the above groups, although they may have harbored deletions undetected by the markers used. This study highlights the importance of using a combination of cytogenetic and molecular tests for a reliable diagnosis of PWS or AS, and for the identification of genetic alterations.

Comparison between Korean Triage and Acuity Scale and Injury Severity Scoring System in Emergency Trauma Patients (외상환자의 한국형 중증도 분류와 손상중증도 점수체계의 비교)

  • Purpose: We compared the Korean Triage and Acuity Scale (KTAS), Injury Severity Score (ISS), and Revised Trauma Score (RTS) determined the validity of KTAS for classifying trauma patients. Methods: A retrospective chart review of 10,865 trauma patients (aged ≥15 years) who visited a single regional trauma and emergency medical center from January 1, 2016, to December 31, 2020, was conducted. Data were collected from the Korean Trauma Data Bank. Based on KTAS classification, the rates of intensive care unit admission, surgery and intervention, transfusion, emergency room (ER) and hospital mortality, and ER stay time were investigated. Data were analyzed using Chi-square test, Pearson's correlation coefficient, receiver operating characteristic curve, and area under the ROC curve. Results: In the KTAS, severe trauma patients (ISS ≥16) were classified as Level 1 (79.6%), 2 (44.8%), 3 (15.5%), 4 (4.0%) and 5 (7.6%). The following were the predictive powers of KTAS, ISS, and RTS for different parameters: surgery and intervention rate, KTAS (.71), ISS (.70), and RTS (.63); transfusion rate within 4h, KTAS (.82), ISS (.82), and RTS (.74); ER stay time within 90 min, KTAS (.72), ISS (.62), and RTS (.56); and ER mortality, KTAS (.84), ISS (.72), and RTS (.88). These findings were statistically significant (p<.001). The sensitivity and specificity of KTAS for trauma patients were .88 (.87~.90), and .38 (.37~.39), respectively. Conclusion: KTAS is a useful classification system that can predict the clinical outcomes of patients with trauma, and effectively triage acutely ill trauma patients, thus provide appropriate treatment.

Clinical usefulness of teleradiology in general dental practice

  • Purpose: This study was performed to investigate the clinical usefulness of teleradiology in general dental practice. Materials and Methods: Two hundred and seventy five cases were submitted for inquiry to the case presentation board of the website of The Korean Academy of Oral and Maxillofacial Radiology for a 5 year periods. The diagnosis results of those cases were analyzed according to the disease classification, the correlation with the patient's chief complaint, the necessity of additional examinations or treatments, the image modalities, and the number of dentists inquiring. Results: Differential diagnoses of normal anatomic structures were the most frequently submitted cases, covering 15.6% of all cases. Among 275 cases, 164 cases required no additional treatments or examinations. Panoramic radiographs were the most frequently submitted images, accounting for 248 inquiries. The 275 cases were submitted by 96 dentists. Fifty-two dentists wrote one inquiry, and 44 inquired 2 or more times. The average inquiry number of the latter group was 5.0 cases. Conclusion: A teleradiology system in general dental practice could be helpful in the differential diagnosis of common lesions and reduce unnecessary costs.

Two-Step Neural Network Approach for Determining EDM(Electrical Discharge Machining) Parameters in Low Tool Erosion (전극 저소모 방전조건 결정을 위한 2단계 신경망 접근)

  • Two-step neural network is designed for determining electrical discharge machining parameters in low erosion. The first neural network, which is used as a classification network, checks whether the current conditions are appropriate to electrical discharge machining in low tool erosion. If the conditions are appropriate to EDM in low erosion, suitable EDM parameters are generated by the second neural network. Theoretically known EDM conditions are produced and also utilized for training the second neural network. The trained neural network is tested how well suitable EDM machining conditions are generated under unknown machining situations Experimental result shows that the proposed two-step neural network approach could be effectively used for determining EDM parameters in low tool erosion. The results also have a practical contribution to EDM area in that it could be applied for maintaining low tool wear as well as obtaining maximum machining rates simultaneously.

Automatic Extraction of Rescue Requests from Drone Images: Focused on Urban Area Images (드론영상에서 구조요청자 자동추출 방안: 도심지역 촬영영상을 중심으로)

  • In this study, we propose the automatic extraction method of Rescue Requests from Drone Images. A central object is extracted from each image by using central object extraction method[7] before classification. A central object in an images are defined as a set of regions that is lined around center of the image and has significant texture distribution against its surrounding. In this case of artificial objects, edge of straight line is often found, and texture is regular and directive. However, natural object's case is not. Such characteristics are extracted using Edge direction histogram energy and texture Gabor energy. The Edge direction histogram energy calculated based on the direction of only non-circular edges. The texture Gabor energy is calculated based on the 24-dimension Gebor filter bank. Maximum and minimum energy along direction in Gabor filter dictionary is selected. Finally, the extracted rescue requestor object areas using the dominant features of the objects. Through experiments, we obtain accuracy of more than 75% for extraction method using each features.

A Study on the Propriety of the Medical Insurance Fee Schedule of Surgical Operations - In Regard to the Relative Price System and the Classification of the Price Unit of Insurance Fee Schedule - (수술수가의 적정성에 관한 연구 - 상대가격체계와 항목분류를 중심으로 -)

  • In Korea, fee-for service reimbursement has been adopted from the begining of medical insurance system in 1977, and the importance of the relative value unit is currently being investigated. The purpose of this study was to find out the level of propriety of the difference in the fees for different surgical services, and the appropriateness of the classification of the insurance fee schedule. For the purpose of this study, specific subjects and the procedural methodology is shown as follows: 1. The propriety of the Relative Price System(RPS). 1) Choice of sample operations. In this study, sample operations were selected and classified by specialists in general surgery, and the number of items they classified were 32. For the same group of operations the Insurance Fee Schedule(IFS) classified the operations into 24 separate items. In order to investigate the propriety of the RPS, one of the purpose of this study, was to examine the 24 items classified by the IFS. 2) Evaluation of the complexity of surgery. The data used in this study was collected The data used in this study was collected from 94 specialists in general surgery by mail survey from November I to 15, 1986. Several independent variables (age, location, number of bed, university hospital, whether the medical institution adopt residents or not) were also investigated for analysis of the characteristics of surgical complexity. 3) Complexity and time calculations. Time data was collected from the records of the Seoul National University' Hospital, and the cost per operation was calculated through cost finding methods. 4) Analysis of the propriety of the Relative Price System of the Insurance Fee Schedule. The Relative Price System of the sample operation was regressed on the cost, time, comlexity relative ,value system (RVS) separately. The coefficient of determination indicates the degree of variation in the RPS of the Insurance Fee Schedule explained by the cost, time, complexity RVS separately. 2. The appropriateness of the classification of the Insurance Fee Schedule. 1) Choice of sample operations. The items which differed between the classification of the specialist and the classification of medical, Insurance Fee Schedule were chosen. 2) Comparisons of cost, time and complexity between the items were done to evaluate which classification was more appropriate. The findings of the study can be summarized as follows: 1. The coefficient of determination of the regression of the RPS on-cost RVS was 0.58, on time RVS was 0.65, and on complexity RVS was 0.72. This means that the RPS of Insurance Fee Schedule is improper with respect to the cost, time, complexity separately. Thus this indicates that RPS must be re-shaped according to the standard element. In this study, the correlation coefficients of cost, time, complexity Relative Value System were very high, and this suggests that RPS could be reshaped I according to anyone standard element. Considering of measurement, time was thought to be the most I appropriate. 2. The classifications of specialist and of the Insurance Fee Schedule were compared with respect to cost, time, and complexity separately. For complexity, ANOVA was done and the others were compared to the different values of different classifications. The result was that the classification of specialist was more reasonable and that the classification of Insurance Fee Schedule grouped inappropriately several into one price unit.

Changes in Potential Distribution of Pinus rigida Caused by Climate Changes in Korea (기후변화에 따른 리기다소나무림의 잠재 생육적지 분포 변화 예측)

  • In this research, it was intended to examine the vulnerability of Pinus rigida to climate changes, a major planting species in Korea. For this purpose, the distribution of Pinus rigida and its changes caused by climate changes were estimated based on the 'A1B' climate change scenario suggested by IPCC. Current distribution of Pinus rigida was analyzed by using the $4^{th}$Forest Type Map and its potential distribution in the recent year (2000), the near future (2050) and the further future (2100) were estimated by analyzing the optimized ranges of three climate indices - warmth index(WI), minimum temperature index of the coldest month (MTCI) and precipitation effectiveness index(PEI). The results showed that the estimated potential distribution of Pinus rigida declines to 56% in the near future(2050) and 15% in the further future (2100). This significant decline was found in most provinces in Korea. However, in Kangwon province where the average elevation is higher than other provinces, the area of potential distribution of Pinus rigida increases in the near future and the further future. Also the result indicated that the potential distribution of Pinus rigida migrates to higher elevation. The potential distributions estimated in this research have relatively high accuracy with consideration of classification accuracy (44.75%) and prediction probability (62.56%).
