양악 전방분절골절단술과 이부 성형술을 통한 개방교합의 치험례 (TREATMENT OF ANTERIOR OPEN BITE WITH BIMAXILLARY ANTERIOR SEGMENTAL OSTEOTOMY AND GENIOPLASTY)

  • Skeletal anterior open bite is a difficult problem to correct in orthodontic treatment. To treat adult patients who have skeletal anterior open bite, we considered two methods. Combination treatment of orthodontics & surgery and camouflage orthodontic treatment. In adults, treatment of severe skeletal anterior open bite consists mainly of surgically repositioning the maxilla or the mandible. However, camouflage therapy is often the treatment of choice for skeletal open bite patients who have mild to moderate skeletal discrepancies when growth modification is no longer possible. But excellent results generally require careful coordination of the orthodontic and surgical phases of treatment. This is a case report of a skeletal anterior open bite patients who were treated with orthodontic treatment and orthognathic surgery. First case was diagnosed as skeletal class I malocclusion & bimaxillary protrusion with anterior open bite, and finally treatment ended for removal of open bite with orthodontic procedure and bimaxillary anterior segmental osteotomy surgery. Second case was diagnosed as skeletal class II malocclusion with open bite & mandibular retrusion, and was treated with only camouflage orthodontics because she feared to have a surgery. In a regular follow up visit after debonding we proposed to the patient advanced genioplasty, and in her agreement her facial esthetics was improved through the surgery.

측두하악장애와 교합상태와의 관계에 대한 연구 (Relationship between Temporomandibular Disorders and Occlusal States Dental Students)

  • In this study, 88 dental students were examined to evaluate the relationship between occlusal states and TM disorders for the epidemiologic study of TM disorders. The clinical evaluation were composed of mandibular movement, TMJ noise, occlusal states and muscle palpation. The following results were disclosed. 1. The frequencies of pain on mandibular movement were 3.4% on maximum opening, 1.13% on protrusion and no pain on laterotrusion. 2. The frequencies on TMJ sound were 21.6% in click, 1.13% in crepitus. 3. The frequency of tenderness on palpation was 12.5% on extra oral, intraoral and neck muscles, tenderness on palpation of TMJ capsule were reported 5 cases, and 4 of them were female. 4. The distribution of Angle's classification was found 79.5% in class I, 4.5% in clasII-div.1 and 15.9% in class III. There was no significant differences on TM disorders between Angles classifications. 5. There was no significant differences on TM disorders between the subjects of canine guided occlusion and group function occlusion, and also for the differences between the subjects of nonworking side interferences and no interferences on laterotrusion. 6. There was no significant differences on TM disorders between the subjects of anterior teeth trauma in C.C. and no anterior teeth trauma, but there were significant differences between the subjects of posterior protrusive contact and no posterior protrusive contact.

상악 제 2 대구치 발거에 의한 교정치료의 효과 (A STUDY ON TREATMENT EFFECTS OF MAXILLARY SECOND MOLAR EXTRACTION CASES)

  • Orthodontic treatment in conjunction with second-molar extraction has been a controversial issue among orthodontists over many decades. The aim of this study was to investigate the treatment effects of upper second molar extraction cases. The sample included 19 upper second molar extraction orthodontic cases(ten Angle's Class I's and nine Class II's, average age=13Y 6M) cared at Kyung-Hee University Department of Orthodontics. Lateral cephalometric radiographs were taken before and immediately after treatment. Seventy-nine points were digitized on each cephalogram and 38 cephalometric parameters were computed comprising 22 angular measurements, 13 linear measurements, and 3 facial proportions. The data obtained from each malocclusion group were analyzed by paired t-test. The statistical results disclosed that there was no significant change in skeletal pattern after treatment except for that accountable by growth while there was statistically significant change in dentoalveolar and soft tissue patterns. There were no significant changes in Bjork sum, posterior facial height /anterior facial height and lower anterior facial height /anterior facial height. No significant changes in anteroposterior position of maxilla and palatal plane were manifested. Although facial axis and lower facial height was slightly increased and the mandible was rotated backward and downward, there was no remarkable change in the mandibular plane. There were statistically significant changes in distal movement of upper first molar, molar key correction and overjet reduction while there was no change in the occlusal plane. The upper lip was slightly retracted simultaneously with slight increase in nasolabial angle. These results signify that distalization of upper dentition with the second molar extraction does change occlusal relationship without gross modifications in the craniofacial skeletal configurationson. Henceforth the second molar extracted would be recommended to treat severe anterior crowding and protrusion with minor skeletal discrepancy.

Effectiveness of autologous leukocyte-platelet-rich fibrin on the rate of maxillary canine retraction, rotation, pain, and soft tissue healing: A split-mouth randomized controlled trial

  • Objective: To assess the effectiveness of leukocyte-platelet-rich fibrin (L-PRF) compared with conventional treatment on canine retraction, rotation, pain, and soft tissue healing. Methods: Sixteen adult patients aged 18-25 years (10 females, and 6 males; mean age 22.25 ± 2.26 years) with Class I bimaxillary protrusion and Class II div 1 malocclusion participated in this single-center, split-mouth randomized controlled trial at the Orthodontics Department of a single hospital in SCB Dental College and Hospital, Cuttack, India. Randomization was performed using a computer-assisted function with a 1:1 allocation ratio. The intervention included the placement of L-PRF on the experimental side and follow-up for 90 days. The primary outcome measures were canine retraction, rotation, pain, and soft tissue healing. The range of tooth movement was evaluated at 15-day intervals: 0th day (T0), 15th day (T1), 30th day (T2), 45th day (T3), 60th day (T4), 75th day (T5), and 90th day (T6). Canine rotation was assessed at T0 and T6, and pain and soft tissue healing were evaluated on the 3rd, 7th, and 15th days of the treatment. Results: Cumulatively, the L-PRF group demonstrated a significantly greater tooth movement as compared to conventional treatment group (P < 0.001). Overall, canine retraction was 1.5 times greater on the L-PRF side than on the control side. Canine rotation showed no significant relationship, whereas pain and soft tissue healing were significantly better on the L-PRF side than on the control side. Conclusions: Local administration of L-PRF amplifies canine retraction while improving pain and soft tissue repair.

Study of soft tissue changes in the upper lip and nose after backward movement of the maxilla in orthognathic surgery

  • Objectives: This study evaluates soft tissue changes of the upper lip and nose after maxillary setback with orthognathic surgery such as Le Fort I or anterior segmental osteotomy. Materials and Methods: All 50 patients with bimaxillary protrusion and skeletal Class II malocclusion underwent Le Fort I or anterior segmental osteotomy with backward movement. Soft and hard tissue changes were analyzed using cephalograms collected preoperatively and 6 months postoperatively. Results: Cluster analysis on the ratios shows that 2 lines intersected at 4 mm point. Based on this point, we divided the subjects into 2 groups: Group A (less than 4 mm, 27 subjects) and Group B (more than 4 mm, 23 subjects). Also, each group was divided according to changes of upper incisor angle (≥4°=A1, B1 or <4°=A2, B2). The correlation between A and B groups for A'/ANS and Ls/Is (P<0.001) was significant; A'/A (P=0.002), PRN/A (P=0.043), PRN/ANS (P=0.032), and St/Is (P=0.010). Variation of nasolabial angle between the two groups was not significant. There was no significant correlation of vertical movement and angle variation. Conclusion: The ratio of soft tissue to hard tissue movement depends on the amount of posterior movement in the maxilla, showing approximately two times higher rates in most of the midface when posterior movement was greater than 4 mm. The soft tissue changes caused by posterior movement of the maxilla were little affected by angular changes of upper incisors. Interestingly, nasolabial angle showed a different tendency between A and B groups and was more affected by incisal angular changes when horizontal posterior movement was less than 4 mm.

Double keyhole loop에 의한 상악 6전치의 후방견인시 치아이동양상에 관한 측모두부방사선계측학적 연구 (A Cephalometric study on tooth movement pattern of maxillary 6 anteriors with double keyhole loops)

  • 일반적으로 치조성 양악전돌증의 치료는 환자의 나이나 골격 부조화의 원인, 심도 등에 따라 다른데, 환자의 성장이 이미 완료되고 골격적 부조화의 점도가 심하지 않은 경우에는 치아의 이동만으로 문제를 해소하는 교정적인 절충치료를 행하게 되며 심미성의 증진을 위해서는 소구치, 특히 제1소구치 발치가 적절한 선택일 수 있다. 제1소구치의 발치 후 전치부의 En Masse 견인시 이용되고 있는 double keyhole loop(이하 DKHL로 약함)에는 4개의 루프가 있어 발치 공간 폐쇄시 견치 치관의 원심경사를 조절하고 구치 치관이 근심측으로 경사되는 것을 막아 교합평면의 굴곡을 방지하는 효과가 있는 한편 견치의 회전을 조절할 수 있도록 설계되어 있다. 이 연구는 I급 치조성 양악전돌증 및 II급 1류 부정교합이 있는 성인환자를 대상으로 상, 하악 6전치를 후방이동시킴으로써 연조직의 변화를 가져와 심미적으로 좋은 치료의 결과를 얻기 위해 상악 제1소구치를 발거하여 DKHL에 의해 치료가 완료된 환자의 측모두부방사선사진에서 치료 전, 후의 골격적인 변화와 치아치조영역의 변화를 알아보기 위해 시행되었고, 다음의 결론을 얻었다. 1. DKHL에 의한 치료 전, 후의 골격적인 변화는 SNB의 감소경향(0.49 ${\pm}$ 0.87$^{\circ}$ , p<0.05)과 PTFH의 증가경향(0.50 ${\pm}$ 0.8 mn, p<0.05)을 제외하고는 통계적으로 유의한 변화를 나타내지 않았고 주된 변화는 치아치조영역에서 일어났다. 2. 치료 후 치아치조영역에서는 절치간각을 제외한 모든 계측항목에서 통계적으로 유의할 만한 감소를 보였다. 3. 상하순의 돌출도 모두 유의성 있는 감소를 나타내었다. 4. 상악 전치의 치관은 통계적으로 유의할 만한 후방이동(7.08 ${\pm}$ 2.14 mm, p<0.01)을 보였고, 상악 전치의 치근에서는 통계적으로 유의할 만한 수직고경의 감소(2.38 ${\pm}$ 1.15 mm, p<0.01)를 보였다. 5. 상악 구치의 치관, 치근 모두에서 통계적으로 유의할 만한 전방이동(2.48 ${\pm}$ 0.99 mm, 2.05 ${\pm}$ 0.91 mm,p<0.01)을 보였다.

삼차원적 안모형태의 미적분석 (MAXILLOFACIAL ESTHETICS BY THREE DIMENSIONAL FACIAL MORPHOMETRICS)

  • This study was performed to investigate the average anthropometric value of normal Korean men and women and to compare the preceding literatures. Additionally, average Korean profilogram was made to serve as a template for diagnosis of facial form. Eighty five Korean subjects(41 men, 44 women) aged between 21 to 26 were selected by members of the author's department on the basis of intact dentition and Class I occlusion without facial asymmetry. Frontal and lateral photographs were taken under standardized condition with digital camera. The image was magnified and adjusted according to the FH plane of cephalometric radiographs and digitized using personal computer. To compare the Western beauty, 25 esthetically pleasing female was selected to measure various angle and distance of the face. 1. It was possible to calculate the mean coordinate value of Korean normal samples which enables the direct visualization and comparison with the use of template. The method in this study was easy to applicate under the Microsoft $Windows^{(R)}$ bases. 2. Maxillary vertical hypoplasia, upper and lower lip protrusion was characteristics of Korean norms and relatively narrow alar base distance, less conspicuous nasal projection was observed. As the vermilion exposure and upper lip length was more than western norm, chin looks shorter than western. To compare the Korean and Western esthetically pleasing profile, facial convexity and nose was less conspicuous in Korean women.

Mandibular Kinesiograph를 이용한 측두하악장애환자의 하악운동 분석 (Analysis on Mandibular Movement of Temporomandibular Disorder Patients using Mandibular Kinesiograph)

  • The purpose of this study was to estimate primary diagnosis, prediction of prognosis and recognition fo treatment progress for treatment of TMD patients through measuring the various ranges of mandibular movement in normal and TMDs patients using Mandibular Kinesiograph K-6 Diagnostic system. In normal groups, 20 adults were selected, who have normal or class I molar relationship, and have no symptoms on TMJ and masticatory muscles, and have restorations less than 3 surfaces on each tooth, and have no other prosthetic restoration. In Patients group, we selected 31 outpatients who were confirmed to TMDs with clinical examination and radiographic findings. The obtained results were as follows : 1. In maximal opening, patient group was showed the limitation of vertical movement range (P<0.01) and lager lateral deviation than in normal group (P<0.05). And actual dimensional displacement of opening was calculated larger in normal group (P<0.05). 2. In protrusive movement, patients group was showed the limitation of anteroposterior movement range (P<0.001) and larger deviation than in normal group (P<0.01). And actual 3 dimensional displacement of protrusion was calculated larger in normal group (P<0.001). 3. In lateral maximum excursion, compared with normal group patient group was no significant differences to affected side, but was showed the limitation of lateral movement to unaffected side (P<0.001). 4. There was no significant difference in movement velocity of opening and closing in both groups. 5. Mandibular movement from physiologic rest position to centric occlusion was moved more anteroposteriorly in patient group. 6. Mandibular movement from centric relation to centric occlusion was no significant difference in both groups.

  • Opposed to following the contours of the human body, the voluminous enlargement in costume, which characterizes the distinguished enlargement in space rather than the contour of human body, mean the enlargement aspect involving the vertical protrusion and the expansion of shape and volume as well as the extension of length. The costume enlargement as a different method of expression is a symbol showing a meaning of something and an aesthetic expression containing man's will. This voluminous enlargement of costume, as an aesthetic expression, has different formative characteristicsand immanent meanings according to ideals and thoughts as well as social and cultural background of each age Accordingly, the aesthetic consciousness also differaccording to the change of the times. To study the aesthetic consciousness of costume's voluminous enlargement, focous had been given to milieus that show comparatively conspicuous voluminous enlargement Periods that have been subjected to this study include costumes of the ancient Egypt, the Gothic period in the Middle age, the Renaissance, the Baroque Rococo of the recent times, and the modern era (Empire, Romantic, Art Nouveau. etc) With focus given to the principle of design obtained through this study were used to analyze the aesthetic characteristics, Futhermore, based on the spirit of the times and the socio-cultural symbolism, research on immanent meanings, as supported by objectivity and universal validity, was also made, the enlargement beauty of costume had been placed under the aesthetic category and, by interpreting the analogies of presented in aesthetic consciousness, the true nature of the voluminous enlargement in costume had been traced. Evaluation of Tooth Movement and Arch Dimension Change in the Mandible Using a New Three-dimensional Indirect Superimposition Method

  • Purpose: To analyze the amount and pattern of tooth movement and the changes in arch dimension of mandibular dentition after orthodontic treatment using a new three-dimensional (3D)-indirect superimposition method. Materials and Methods: The samples consisted of fifteen adult patients with class I bialveolar protrusion and minimal anterior crowding, treated by extraction of four first premolars with conventional sliding mechanics. After superimposition of 3D-virtual maxillary models before and after treatment using best-fit method, 3D-virtual mandibular model at each stage was placed into a common coordinate of superimposition using 3D-bite information, which resulted in 3D-indirect superimposition for mandibular dentition. The changes in mandibular dental and arch dimensional variables were measured with Rapidform 2006 (INUS Technology). Paired t-test was used for statistical analysis. Result: The anterior teeth moved backward, displaced laterally, and inclined lingually. The posterior teeth showed statistically significant contraction toward midsagittal plane. The amounts of backward movement of anterior teeth and forward movement of posterior teeth showed a ratio of 6 : 1. Although the inter-canine width increased slightly (0.8 mm, P<0.05), the inter-second premolar, inter-first molar, and inter-second molar widths decreased significantly with similar amounts (2.2 mm, P<0.05; 2.3 mm, P<0.01; 2.3 mm, P<0.001). The molar depth decreased (6.7 mm, P<0.001) but canine depth did not change. Conclusion: A new 3D-indirect superimposition of the mandibular dentitions using best-fit method and 3D-bite information can present a guideline for virtual treatment planning in terms of tooth position and arch dimension.
