Smoking Initiation and Continuation - A Qualitative Study among Bruneian Male Adolescents

  • Background: Cigarette smoking is one of the leading global causes of premature and preventable death. In Brunei Darussalam, smoking-related diseases have been a primary cause of mortality for the past three decades. Despite the increasing efforts that have been made in recent years to reduce the consumption of tobacco products in Brunei, the prevalence of adolescent smoking cigarette, however has risen alarmingly, from 8.9% in 2013 to 11.4% in 2014, with a higher prevalence found in males (17.8%) than in females (4.8%). In response to the need for more effective smoking prevention programmes in Brunei, this study sought to explore factors that influence Bruneian male adolescents to start and continue smoking. Materials and Methods: A qualitative study using focus group discussions (FGDs) as the data collection method was conducted from October to November 2015. A total of 43 studentss, comprising 31 smokers and 12 non-smokers, aged 13-17 years, from two government secondary schools in Bandar Seri Begawan, participated in six FGDs. Discussions were recorded and translated. Transcripts were entered into NVivo10, before thematic analysis was conducted. Results: We identified three themes under the core construct of 'factors influencing smoking initiation' ('family as teachers', 'overt pressure from peers' and 'perceived smoking has many advantages') and three themes under the core construct of 'factors influencing smoking continuation' ('craving and addiction', 'smoking as a 'social activity' and 'easy accessibility of cigarettes'). Conclusions: Based on the findings, it is recommended that future prevention activities should be embedded in a comprehensive approach, involving all stakeholders within a community, and should be focused towards bringing a change in smoking and parenting behavior of parents, social norms within the culture towards all population levels, and at strengthening the existing non-smoking policies in schools and other public places where young people congregate.

흡연경고문이 대학생의 흡연태도에 미치는 영향에 대한 연구 (A Study on the Effects of Smoking Warning Statement for Smoking Attitudes of College Students)

  • 본 논문은 담뱃갑에 표기된 흡연경고문구가 대학생의 흡연에 대한 태도에 미치는 영향을 연구하였으며 흡연경고문구의 실효성을 검증하기 위하여 2가지의 연구가설을 설정하고 가설을 검증하기 위하여 중앙대학교 학생 129명의 통계 자료를 이용하였다. 조사는 흡연경고문의 유무에 따른 흡연 태도의 차이를 알아보기 위해 흡연경고문이 제시된 것과 제시가 되지 않은 것, 두 종류의 설문지를 작성하였다. 수집한 자료 분석을 위하여 통계 프로그램 SPSS 18.0을 이용하였으며 빈도분석, t-test, 일원배치 분산분석과 다중회귀분석을 시행하였다. 흡연 경고문 목격 유무가 흡연 태도 점수에 유의미한 정적 영향을 주고 있는 것으로 나타났으며 성별과 부모님의 흡연 여부는 유의미한 영향을 주고 있지 않은 것으로 나타났다.

우리나라 성인의 흡연형태별 구강건강상태 및 구강건강관리와의 관련성 (Relationship between Oral Health Status and Oral Health Management by Smoking Type in Korean Adults)

  • 본 연구는 2017년 지역사회건강조사 자료를 이용하여 우리나라 만 19세 이상 성인을 대상으로 흡연형태별 구강건강 상태 및 관리와의 관련성을 평가하여 특히 전자담배가 구강건강에 미치는 영향을 알아보고자 하였다. 비 흡연, 궐련, 전자담배, 궐련과 전자담배 병용 4그룹으로 나눠 무응답과 응답거부 등 결측값 24명을 제외하고 최종 228,357명을 분석대상으로 하였다. 구강건강 상태 및 관리를 종속변수로 하여 로지스틱회귀분석을 시행한 결과 비 흡연군에 비해 궐련군과 혼합군은 다변량 회귀분석에서 유의한 차이를 보였다. 전자담배군은 연령과 성별을 보정한 결과변수에서만 유의한 차이를 보였으나, 다변량 회귀분석에서 스켈링 무경험에서만 0.72배 오즈비가 통계적으로 유의하였을 뿐, 다른 모든 결과 변수는 유의한 차이가 없었다. 본 연구에서 전자담배군과 구강건강과의 관련성이 나타나지 않았지만, 전자담배의 흡연량, 흡연강도 등 흡연패턴을 고려한 후속 연구가 필요한 것으로 사료된다.

  • Transfer of nicotine into the mails stream of smote in Korean commercial cigarettes was investigated. The mass transfer of nicotine into tile main stream of smote and accumulation of nicotine in cigarette filler got higher as the puffing number increased. Only about 14oyo of nicotine ill a cigarette was puffed into mouth end. and the rest of the nicotine went to side stream of smote or converted to other volatile bases.

유도결합 플라스마 질량분석기를 이용한 담배 제품의 무기물 반정량 분석 (Semiquantitative Analysis of metal for Cigarette Product by Inductively Coupled Plasma Mass Spectrometry)

  • Semiquantitative analysis by ICP-MS has proven to be a powerful tool for fast screening, in addition, it does not require the element of interest to be present in the calibration standard, making it especially useful for the analysis of unknown samples. In this study, seven cigarette samples were analyzed by the rapid semi-quantitative analysis method based on the ICP-MS. For each cigarette sample, cut tobacco, cigarette paper, filter (before and after smoking), and smoke condensate were analyzed. The accuracy of the analysis technique was evaluated by comparing results obtained from Calibration Check Standard(CCS) and calibration method. Relative Percentage Error(RPE) value of all elements measured for three CCS showed a stable result of less than ${\pm}20%$. Compared to full quantitative analysis by calibration method, the results for cigarette samples showed average error within ${\pm}15%$.

흡연, 음주와 운동습관의 군집현상을 통한 건강행태의 고위험군: 국민건강영양 조사 (High Risk Groups in Health Behavior Defined by Clustering of Smoking, Alcohol, and Exercise Habits: National Heath and Nutrition Examination Survey)

  • Objectives: We investigated the clustering of selected lifestyle factors (cigarette smoking, heavy alcohol consumption, lack of physical exercise) and identified the population characteristics associated with increasing lifestyle risks. Methods: Data on lifestyle risk factors, sociodemographic characteristics, and history of chronic diseases were obtained from 7,694 individuals ${\geq}20$ years of age who participated in the 2005 Korea National Health and Nutrition Examination Survey (KNHANES). Clustering of lifestyle risks involved the observed prevalence of multiple risks and those expected from marginal exposure prevalence of the three selected risk factors. Prevalence odds ratio was adopted as a measurement of clustering. Multiple correspondence analysis, Kendall tau correlation, Man-Whitney analysis, and ordinal logistic regression analysis were conducted to identify variables increasing lifestyle risks. Results: In both men and women, increased lifestyle risks were associated with clustering of: (1) cigarette smoking and excessive alcohol consumption, and (2) smoking, excessive alcohol consumption, and lack of physical exercise. Patterns of clustering for physical exercise were different from those for cigarette smoking and alcohol consumption. The increased unhealthy clustering was found among men 20-64 years of age with mild or moderate stress, and among women 35-49 years of age who were never-married, with mild stress, and increased body mass index (>$30\;kg/m^2$). Conclusions: Addressing a lack of physical exercise considering individual characteristics including gender, age, employment activity, and stress levels should be a focus of health promotion efforts.

노인의 흡연상태에 따른 식이섭취 패턴 (Dietary Patterns of Elderly People by Smoking Status)

  • It has been proposed that differences in diet between smokers and nonsmokers may partially explain the positive association between cigarette smoking and chronic diseases such as cancer and coronary heart disease. To investigate the potential for this confounding, the authors studied the relation between cigarette smoking and dietary pattern including nutrient intake in 152 Korean elderly people aged 59∼84years. Dietary intake was determined by a 24-hour recall method and a food frequency questionnaire. Male smokers were lighter than nonsmokers and had a lower body mass index(BMI)(p<0.05). But there were no differences in body weight and BMI between female smokers and nonsmokers. Female smokers had lower intakes of energy and carbohydrates than nonsmokers(p<0.05), but in male, smokers and nonsmokers did not show the difference in energy intake. The mean intake of fiber, protein, fat, minerals, and vitamins did not vary significantly by smoking status. However, both male and female smokers consumed fewer fruits than nonsmokers and ate fresh fruits less often than nonsmokers. The total food frequency score which reflect dietary quality of smokers was lower than those of nonsmokers(p<0.05). Male exsmokers' diet were similar to those of smokers, but in female, dietary patterns of exsmokers were similar to those of nonsmokers. These results confirm that elderly smokers have partially different nutrient and food intake patterns from nonsmokers. The decreased ingestion of fruits and less balanced diet by cigarette smokers could contribute to their increased risks of chronic disease compared with those by nonsmokers. The increased ingestion of nutrients and improved nutritional balance in the diet of exsmokers may help to reduce their risks as well.

Inhibitory effect of Se-Yeon-Eum on nicotine- and cigarette smoke extract induced cytotoxicity in human lung fibroblast

  • There are over 4,000 different chemicals in cigarette smoke, including nicotine and tar. These compounds influence on lung tissue directly or indirectly. In this study, we have examined whether an aqueous extract of Se-Yeon-Eum (SYE), composed of Oriental medicine that has been known to be effective to symptom by smoking, inhibits nicotine- or cigarette smoke extract (CSE)-induced cytotoxicity in human embryonic lung fibroblast, MRC-9. Assessment of cell viability using 3-(4,5-dimethythiazol-2-yl)-2,5-diphenyltetrazolium bromide colorimetric assay indicated that SYE inhibited not only nicotine-induced cytotoxicity but also CSE-induced cytotoxicity. These results suggest the possibility that the use of SYE may be useful for improvement of many symptoms by smoking.

담배연기 노출량 평가 생체지표 (Biomarkers of Exposure for Cigarette Smoke)

  • Biomarkers could be critical and useful tools for assessing the biological effects of smoking and detecting differences between potentially reduced exposure product (PREP) and conventional cigarettes. Smoking-related biomarkers can be classified into three categories as biomarkers of exposure, biomarkers of effects, and biomarkers of potential harm. When compared with the biomarkers of effects or harm, the biomarkers of exposure for chemical constituents of cigarette smoke are well established and characterized. In addition, they could offer the important information in understanding how cigarette smoke interacts with biological molecules and causes the disease to human. Therefore, we provide an overview of 6 biomarkers of exposure (Nicotine and nicotine metabolites, Carboxyhaemoglobin, NNAL (4-(methylnitrosoamino)-1-(3-pyridyl)-1-butanol) and NNAL - glucuronide, 3-Hydroxypropyl-mercapturic acid, and Monohydroxy-butenyl-mercapturic acids, and Urine mutagenicity) which were validated through extensive research and clinical experience. These reliable biomarkers could help identify the efficacy of PREP by predicting early toxicological effects and lead to improve it.

PREVENTION OF CIGARETTE SMOKE INDUCED LUNG CANCER BY LOW LET IONIZING RADIATION

  • Lung cancer is the most prevalent global cancer, ${\sim}90%$ of which is caused by cigarette smoking. The LNT hypothesis has been inappropriately applied to estimate lung cancer risk due to ionizing radiation. A threshold of ${\sim}1\;Gy$ for lung cancer has been observed in never smokers. Lung cancer risk among nuclear workers, radiologists and diagnostically exposed patients was typically reduced by ${\sim}40%$ following exposure to <100 mSv low LET radiation. The consistency and magnitude of reduced lung cancer in nuclear workers and occurrence of reduced lung cancer in exposed non-worker populations could not be explained by the HWE. Ecologic studies of indoor radon showed highly significant reductions in lung cancer risk. A similar reduction in lung cancer was seen in a recent well designed case-control study of indoor radon, indicating that exposure to radon at the EPA action level is associated with a decrease of ${\sim}60%$ in lung cancer. A cumulative whole-body dose of ${\sim}1\;Gy$ gamma rays is associated with a marked decrease in smoking-induced lung cancer in plutonium workers. Low dose, low LET radiation appears to increase apoptosis mediated removal of $\alpha$-particle and cigarette smoke transformed pulmonary cells before they can develop into lung cancer.
