The Clinical Effects Following Discontinuation of Chungsangboha-tang(Qingshangbuxia-tang) Treatment in Patients with Controlled Asthma (청상보하탕 치료중단이 기관지 천식환자에게 미치는 영향)

  • Background : Nowadays asthma is considered to be an inflammatory disease characterized by airway hyperresponsiveness and pulmonary eosinophilia. Production of cytokines by bronchial epithelial cells may contribution to the local accumulation of inflammatory cells in patients with bronchial asthma. Chungsangboha-tang is the herbal treatment of choice in persistent asthma patients. It has been recognized that cessation of treatment with Chungsangboha-tang evokes a recurrence of symptoms in patients with controlled asthma. This study was designed to evaluate the long-term effect of Chungsangboha-tang. Materials and Methods : The subjects consisted of 24 patients with asthma who had been treated with Chungsangboha-tang for four weeks. Chungsangboha-tang is an herbal decoction which has been used as the traditional therapeutic agent for asthma. PFT, QLQAKA, blood eosinophils, serum IgE, Serum IL-4, IL-5, $IFN-{\gamma}$ were checked before treatment, before withdrawal and 3 months after cessation of treatment with Chungsangboha-tang. Results : Treatment with Chungsangboha-tang for four weeks resulted in significant increase in FEV1.0%, PEFR%, and QLQAKA. The patients were treated with Chungsangboha-tang for four weeks with no significant difference in the blood eosinophils, serum IgE, IL-4 and IL-5. The serum $IFN-{\gamma}$ in asthmatic patients decreased significantly after 4 weeks of treatment. Discontinuation of treatment with Chungsangboha-tang resulted in significant drops in QLQAKA. Others measures in asthmatic patients 3 months after discontinuation of treatment with Chungsangboha-tang showed no significant difference. Conclusion : This study demonstrates that asthma can be exacerbated by discontinuation of treatment with Chungsangboha-tang in patients with asthma. Obviously further research concerning this is still necessary.

The Antioxidant Effect, Inhibition of Interleukin-4 and the Effect on the Gene Expression by Using cDNA Chip of Chungsangboha-tang(Qingshangbuxia-tang) (청상보하탕의 항산화 효과, Interleukin-4 억제 및 cDNA chip을 이용한 유전자발현에 미치는 영향)

  • Backgrounds & Objectives: In many recent studies, molecular biological methods have been used to investigate the role of cytokines in pathogenesis and new therapeutic targets of asthma. Recently, as a method of research on the gene expression, they are applying another method which assays multiple gene expressions at the same time by the microarray. In this study, the antioxidant effect, the inhibitory effect against interleukin-4 and the effect on the CD/cytokine gene expression in PBMC (peripheral blood mononuclear cells) was evaluated by using cDNA microarray chip of Chungsangboha-tang. Methods: Experimental studies were performed for the antioxidant effect of Chungsangboha-tang on DPPH (1, 1-diphenyl-2-picrylhydrazyl) solution, for the IL-4-inhibiting effect on BALB/c mouse spleen, and for the gene expression effect on PBMC (peripheral blood mononuclear cells) with microarray. Results: Chungsangboha-tang showed antioxidant effect dose-dependently. Chungsangboha-tang inhibited interleukin-4 dose-dependently and showed significant difference in 10ug/ml and 100ug/ml of test groups. There was no 2 more times upregulated genes than in the control group by using cDNA microarray chip of Chungsangbohn-tang, but there were 140%-200% upregulated genes. There was no 2 more times downregulated genes than in the control group by using cDNA microarray chip of Chungsangboha-Tang, but there was 50%-75% downregulated genes. Conclusions: This study showed that Chungsangboha-tang has an antioxidant effect and inhibition of Interleukin-4, but further studies are necessary with microarray.

The Effects of Kamichungsangboha-tang on the Respiratory Patterns and Tracheal Tissues in Allergic Asthma (가미청상보하탕이 Allergy성 기관지천식에 미치는 실험적 연구)

  • This study was aimed to find out Kami-Chungsangboha-tang's effects on allergic asthma using Egg-White Implant(EWI) method model recommended by Siqueira et al. Kami-Chungsangboha-tang has widely been prescribed and used in Oriental Medicine for controlling and curing of the symptoms of Asthma(dyspnea, wheezing, cough, sputum, chest discomfort). The Previous studies have demonstrated that Kami-Chungsangboha-tang had analgesic, sedative, anticonvulsive and antihistamine effects. But it was uncertain whether Kami-Chungsangboha-tang could inhibit the allergic reaction, so alleviate the symptoms of allergic asthma. With such aim, the respiratory patterns and eosinophil infiltration in the tracheal mucosa of rats with experimentally induced allergic asthma (using EWI method) were mainly observed. The results are as follows ; 1. Kami-Chungsangboha-tang decreased the number of malformed respiration with respect to control group (p<0.02). 2. Kami-Chungsangboha-tang decreased the number of eosinophils infiltrated in tracheal mucosa with respect to control group (p<0.05). Depending upon above results, it is considered that Kami-Chungsangboha-tang has the inhibitory effects on the process of allergic asthma and suggested that it could be used in relieving patients of the symptoms caused by allergic asthma.

A Case of Cough and Sputum Production Treated with Chungsangboha-tang(Qingshangbuxia-tang) in Stroke Patient (중풍환자의 해수, 객담에 대한 청상보하탕 투여 1례)

  • The subject was a 70-year-old man who was suffering from productive cough after stroke. His symptoms were night cough, thick sputum production, reddened tongue and night sweat. The subject was diagnosed as having a deficiency of Yin, and was treated with Chungsangboha-tang(Qingshangbuxia-tang). A decrease in the severity of the subject's cough and sputum were observed, and $O_2$ saturation increased. In this case Chungsangboha-tang(Qingshangbuxia-tang) was effective on the patient with night cough, thick sputum production, reddened tongue and night sweat due to deficiency of Yin after stroke.

A Clinical Observation of Chungsangboha-tang's Effects on Asthmatic Patients (기관지천식 환자의 효과적인 치료를 위한 청상보하탕(淸上補下湯)의 임상적 관찰)

  • Objectives : Asthma is considered an inflammation disease characterized by airway hyperresponsiveness and pulmonary eosinophilia. Chungsang boha-tang is a herbal medication which has been frequently used in traditional therapeutic agents for asthma. We aimed to observe the clinical effects of Chungsangboha-tang on blood eosinophil, serum IgE, AST, ALT and pulmonary function in asthmatic outpatients. Methods : The subjects consisted of 66 asthmatic patients who had been treated with Chungsangboha-tang among all the asthmatic patients who visited the 5th Internal Medicine of the Oriental Medicine Hospital of Kyung Hee University from November,2004 to November,2009. We observed blood eosinophil, serum IgE, AST, ALT levels and pulmonary function test results as well as medical records of patients to evaluate the effects. Results : Pulmonary function significantly improved after 8 weeks of treatment, and blood eosinophil and serum IgE level significantly decreased after 8 weeks of treatment. AST, ALT levels changed in normal ranges. Chungsangboha-tang was significantly effective on the male patients who had a smoking history and worse initial lab findings. We also observed a significant improvement of pulmonary function and decrease of eosinophil and serum IgE levels in the patient group who had a single treatment of Chungsangboha-tang without steroid therapy. Conclusions : This study shows that Chungsangboha-tang has effects on the improvement of pulmonary function and decrease of eosinophil and serum IgE levels as well as improvement of symptoms. And, 8 weeks of long-term treatment showed no negative effects such as significant increase of AST, ALT levels.

Development of an animal model for chronic asthma using Chungsangboha-tang (만성해수 치료제의 개발 연구)

  • Chronic asthma is considered as an incurable disease in modern society. This study focused on development of an animal model for the chronic asthma to investigate new drugs from traditional herbal medicine. And we tested the animal model with a typical prescription, Chungsangboha-tang and tried to find biochemical markers such as catecholamines and cAMP in serum, and as densities of beta-receptor in lung tissues. SD rats were actively sensitized by exposure to ovalbumine (OA). Ten days after sensitization, rats were challenged with OA aerosol by nebulizer six times every three days. Mucin was increased in bronchoalveolar lavages (BALs) after antigen (OA) challenge. Serum concentration of epinephrine was decreased significantly although there were not changed much in serum concentration of cAMP and the densities of beta-receptor in lung tissues. Chungsangboha-tang (5 g/kg/day) was given orally to ovalbumin-sensitized rats (n=8) for 15 days. Mucin in bronchoalveolar lavages (BALs) was increased significantly after treatment of Chungsangboha-tang although concentrations of epinephrine and cAMP were not changed significantly. The densities of beta-receptor in lung tissues were not different from those of controls. These results suggest that the ovalbumin-sensitized rats can be a good animal model of chronic asthma and Chungsangboha-tang is a possible drug in the treatment of chronic asthma.

A Case of Wei Symptom Treated with Chungsangboha-tang (청상보하탕(淸上補下湯)으로 호전된 위증환자 치험(治驗) 1례(例))

  • Wei symptom is muscle relaxation to the point of no contraction and occurs in the limbs. Several cases have led to death. This is a clinical report of a Wei symptom patient diagnosed with impairment of body fluid due to lung heat(肺熱傷津). This patient was treated with Chungsangboha-tang(Qingshangbuxia-tang) and had significant improvement in Wei symptom. Results suggest that oriental medicine is an effective treatment for Wei symptom. But more clinical case reports are needed.

A Retrospective Study of Chronic Pulmonary Disease Patients Treated with Chungsangboha-tang : IgE, eosinophil, PFT (청상보하탕(淸上補下湯)을 복용한 천식, COPD 등 만성호흡기환자에 대한 후향적 연구)

  • Objective: This study was designed to analyze the treatment effects of Chungsangboha-tang through retrospective chart reviews.Methods: Fifty-one outpatients who had visited the Allergy, Immune & Respiratory System Division at the Kyung Hee Korean Medicine Hospital and who had taken Chungsangboha-tang from February 1, 2006, to February 1, 2016, had their basic medical records and examinationsretrospectively reviewed with respect to IgE, eosinophil, AST, and ALT. The PFT results of 11 patients were also investigated with respect to FEV1, FVC, and FEV1/FVC.Results: The percentage of males and females was 49.02% and 50.98%, respectively. The past histories of patients included asthma (66.67%), unspecified cough (21.57%), COPD (9.80%), allergic rhinitis (7.84%), and others. The subjective symptoms included cough (82.35%), sputum (39.22%), dyspnea (37.25%), and others. The most numerous pattern identification was wheezing dyspnea. IgE was significantly reduced, and eosinophil had a reduced tendency after 116.76±160.40 days of taking Chungsangboha-tang. PFT results also significantly increased after 213.09±266.62 days, while AST and ALT results showed a reduced tendency. In the asthmatic group, IgE also showed a reduced tendency. In particular, IgE was significantly reduced in patient groups taking medicine for more than 12 weeks.Conclusions: The conditions of patients with chronic pulmonary disease such as asthma and COPD significantly improved with Chungsangboha-tang after more than 12 weeks.

Effect of Chungsangboha-tang on LPS induced Anti-inflammatory in THP-1 cells (LPS로 유발된 대식세포의 염증반응에 대한 청상보하탕(淸上補下湯)의 효과)

  • Background and Objective : Chungsangboha-tang (CSBHT) has analgesic, sedative, anti-convulsive and anti-histamine effects, so it alleviates the symptoms of asthma. For the comparison of anti-inflammatory effect(s) on CSBHT, PD098059 was used as a negative control. Materials and Methods : This study emphasized THP-1 cells, which had been well characterized as a human monocytic leukemic cell line. The cells resemble monocytes with respect to several criteria and can be differentiated into macrophage-like cells by treatment with PMA. By using the MTS assay, it was possible to prove the safety of CSBHT. Results : Results shows that the CSBHT did not affected cell survival within $10^{1}$ ng/ml to $10^{5}$ ng/ml. Especially, $10^{5}$ ng/ml CSBHT treated cells show 70% deduction of $TNF-{\alpha}$ gene expression against that of LPS treated group. Furthermore, $IL-1{\beta}$, IL-6, IL-8, IL-10 and $TNF-{\alpha}$ levels are down-regulated when treated with CSBHT with concentrations up to 100 ug/ml on monocyte-derived macrophages. Interestingly, CSBHT-treated samples showed that overall transcriptional activities were down-regulated to 20% of that of PD098059 ($TNF-{\alpha}$ inhibitor). At protein level, the expression of $TNF-{\alpha}$ showed similar results as that of transcriptional activity. Results show that the protein level decreased more in the CSBHT-treated group (487 ${\pm}$ 87 pg/ml) than in the LPS-treated group (703 ${\pm}$ 103 pg/ml). In addition, the protein level of IL-8 in the CSBHT treated-group (9.84 ${\pm}$ 3.28 ng/ml) decreased similar as the expression of the control and PD098059-treated groups. Conclusion : CSBHT affects immune response, especially allergic responses and suppression of inflammatory reaction. The results provide us an alternative way to care for clinical inflammatory diseases, not only asthma but also the other possible general inflammatory and allergic diseases.

  • Object: By process of treatment for a case which diagnosed as Churg-Strauss Syndrome and admitted from the 11th, pril, 2002 to the eight, May, 2002, the results are as follows. Methods: Clinical observation and analysis about a case of Churg-strauss Syndrome was done, who visited College of Oriental Medicine, Kyungsan University. This case was managed by integrated therapy of oriental Medicine. This Patient was diagnosed as deficiency of Yin and coagulation of phlegm(陰虛痰疽) and medicated chungsangboha-tang(淸上補下湯) and gamiyunpe-tang (加味潤肺湯). As acupuncture and moxibustion and oriental medication, we could get the effective results. Conclusion: We could get the effective results that case of Churg-Strauss Syndrome with dyspnea, sputum, cough, fatigue, sweating sign, was treated with oriental medicine, acupuncture, moxibustion.
