  • Background: This study was performed to review the efficacy of national and international randomized controlled trials (RCT) investigating Chuna manipulative treatment for ankle sprains. Methods: Online databases (PubMed, Cochrane, EMBASE, CNKI, NDSL, OASIS), were searched for studies where Chuna treatment was performed for ankle sprains up to October 12th, 2017. Only RCT were selected that fulfilled the inclusion/exclusion criteria. Data were analyzed using the Cochrane risk of bias tool. Results: There were 676 studies retrieved from the databases, resulting in analysis of 24 RCT. There was an average of 7 treatment visits over a 7 day period and the most frequent evaluation tool used was efficacy rate, with drug therapy being the most common control used in the trials. In 15 RCT, several Chuna methods were used in combination, amongst which, the osteopathic technique was most common. Statistically significant improvement in evaluation indices was reported in 19 RCT, and in 3 RCT, statistically significant improvement was reported, but not for all indices. In the remaining 2 RCT, there were no significant differences in any of the evaluation indices. No adverse reactions were reported in any of the RCT, although it was unknown whether all the trial protocols indicated that adverse reactions should be monitored, and for this reason, the risk of bias was unclear. Conclusion: The review of 24 studies suggest that Chuna manipulative treatment for ankle sprains was effective in most cases, although, potential bias in these studies was difficult to evaluate.

  • Objectives : The purpose of this study is to explore the trends of Chuna(manipulation) treatment on neck pain and cervical disc displacement. Methods : We set up the two searching strategies and investigated the latest clinical trials on Chuna(manipulation) therapy for Neck disorders and cervical disc herniation through PubMed search. The searched papers were analyzed and the year, journals, article types of literatures were examined. Of the articles, Randomized controlled trials(RCTs) are assessed by Jadad scale. Results : Studies on Chuna(manipulation) treatment to cervical disc herniation were very few. Articles on Chuna(manipulation) treatment to neck pain were relatively more than those of cervical disc herniation. The most frequently published journal was J Manipulative Physiol Ther. The main design of study was RCT. The limited quality assessment of RCT was conducted by Jadad scale and the average Jadad score was 1.73, which indicates low quality in terms of Jadad evaluation criteria. Conclusions : International trends of studies on Chuna therapy of cervical disease is increasingly reviewed. Although this review has several limitations, analyzing the latest trends of studies on Chuna(manipulation) treatment on neck pain and cervical disc displacement is necessary for further studies.

  • Objectives : The aim of this study is to analyze the actual investigation and classification of osteopathic manipulation by investigation of the various literature of the inside and outside of the country, and to present the application plan of osteopathy in chuna manual therapy for meridian sinew system. Methods : I referred to the domestic and foreign books about osteopathy and chiropractic. In order to investigate domestic dissertations, I searched 4 Korean medical databases and 4 Korean medical journals of osteopathy. Search terms used were osteopathy, osteopathic, fascia, proprioreceptor, mechanoreceptor, muscle spindle, golgi tendon organ, osteopathic manipulation technics. And I classified all the searched studies into principle and region and etc. In order to investigate foreign dissertations, I search 'NCBI pubmed'. Search terms used were osteopathy, osteopathic technique, osteopathic manipulative technique. Results : 1. Osteopathy do not regard the systems which compose the human body in individual territory, but regard whole. It is diagnosis, prevention and medicine which treats 2. Osteopathic manipulation techniques are classified into direct techniques, indirect techniques, and compound techniques. 3. Osteopathic manipulation techniques are classified into fascia, muscle, ligament-joint in applied region. 4. I could search clinical cases in domestic and foreign study. I found cases about myofascial release technique(MFR), postisometric relaxation(PIR), proprioceptive neuromuscular fascilitation(PNF), muscle energy technique(MET), joint mobilization in domestic studies, and strain-counterstrain technique(SCS), MET, AK in foreign studies. Conclusions : Osteopathic manipulation techniques can be used in diagnosis and treatment of meridian muscle theory, because osteopathy and the oriental medicine have many similarities in theoretical background. So osteopathic manipulation technique can be useful in oriental medicine treatment techniques.

  • Objectives This study aims to comprehensively review research utilizing ultrasonography for assessing manipulative treatment on spinal diseases, with the goal of promoting the wider integration of ultrasound imaging into clinical practice. Methods A systematic search was conducted on three international databases (Embase, PubMed, Cochrane) up to July 23, 2023. The search included key terms such as ultrasonography, manipulation, and skeletal muscle. The inclusion criteria narrowed down the selection to studies specifically related to lumbar and cervical vertebrae. Results Eleven studies were included in the review, with 10 focusing on lumbar vertebrae and one on cervical vertebrae, all employing spinal manipulation treatment. Among the 11 selected studies, nine primarily focused on ultrasound imaging to measure muscle thickness, while two utilized shear wave elastography to assess muscle stiffness. Also, rigorous measures were taken to ensure the reliability of the ultrasonography data. Conclusions This scoping review highlights the limited but growing evidence supporting the use of ultrasonography to assess manipulative treatment for spinal diseases. Despite a scarcity of studies in South Korea, it is crucial to recognize the potential of ultrasonography in becoming a widely used and practical tool for evaluating the effectiveness of manipulative treatments in the near future.

  • Objectives : This study aims to educate clinical doctors of the valuable practice of Manipulative Therapy(MT) as an opportunity for evidence-based medicine. Consequently it also serves to review the effectiveness and safety of MT. Methods : The literature studies of overseas were done by Cochrane Library and Medline website; those of domestic researches were completed by utilizing the sources which are gained from KERIS, KISS, DBpia, Kisti, and domestic institutes related with MT. Results : Eight papers related to the effectiveness of MT were published in South Korea, which concluded with the positive effects of MT. None of them are, however, well-designed randomized controlled trials(RCT). On the other hand, fifteen cases of nine articles indicate the adverse reaction of MT, and numbers of the researches in overseas revealed the side effects of MT in order of cervical, lumbar, and thoracic vertebrae. A vascular adverse reaction such as vertebral and carotid artery dissection was a highly reported cervical adverse reaction; in the thoracic and lumbar regions, neurological adverse reaction in terms of disc hemiation was frequently discovered. Conclusions : Henceforth, highly qualitative studies are required developing the effective outcomes and preventing any possible complications of MT. Therefore, systemic curriculums in institutions and sufficient clinical training in the filed are strongly recommended.
