Herniated Lumbar Disc in a Patient Undergoing Lumbar Chuna Manipulation Treatment: A Case Report (요추 추나치료 후 악화된 요추 추간판 탈출증 1례 보고)

  • Cauda equina syndrome(CES) and disc hemiation have been implicated as a potential complication of Chuna Manipulation Treatment. We reported a man who developed sudden complication alter lumbar Chuna Manipulation Treatment. It is supposed that this patient suffered a disc herniation secondarily due to a lumbar Chuna Manipulation Treatment. Therefore, doctors should be aware that neurological complication may occur as a result of lumbar Chuna Manipulation Treatment.

Cervical Dural Tear induced by Cervical Chuna Manipulation Treatment : A Case Report (경추추나치료 후 발생한 경막파열 환자 1례 보고)

  • Objectives : A rare case of dural tear ensuing after a cervical Chuna Manipulation Treatment leading to cerebrospinal fluid (CSF) leakage in the lower cervical and upper thoracic spine was found, so we report it. Methods : A 32-year-old woman presented with back and neck pain in 1 days earlier. The patient undertook a cervical Chuna Manipulation Treatment. After this maneuver, the patient complained of an orthostatic headache with nausea. The patient's headache worsened, and lying down gave the only measure of limited relief. In Brain CT and MRI study, nonspecific finding was detected. In Cistemography study, CSF leakage at lower cervical or upper thoracic area was detected. Results and Conclusions : It is supposed that this patient suffered a dural tear and CSF leakage secondarily due to a cervical Chuna Manipulation Treatment. From this case, we can understand the etiology of dural tear to some extent and consider the complication of Chuna Manipulation Treatment. In the future, more study, research and prospective trial for complications of a cervical Chuna Manipulation Treatment is needed.

Indication Analysis Study through Literature Review of Clinical Trials on Visceral Chuna Manipulation (내장기추나요법 임상연구 문헌고찰을 통한 적응증 분석 연구)

  • Objectives The purpose of this study is to analyze the trend and indication of clinical research about Visceral Chuna Manipulation and to suggest the direction of further study. Methods We searched 9 electronic databases (MEDLINE Pubmed, EMBASE, CENTRAL, CiNii, CNKI, KMbase, OASIS, NDSL, KISS) using the search terms "visceral manipulation", "visceral manual therapy" and identified relevant literature that investigated visceral manipulation as a intervention for various subjects. Results Among 142 studies were searched and screened, 16 met designated criteria. Visceral Chuna Manipulation showed the effectiveness in musculoskeletal disease, digestive disease, psychiatry, obstetrics, circulatory and oncology. Conclusions This study shows possibility of Visceral Chuna Manipulation as a treatment for many diseases. Better designed further study is necessary to establish the evidence of clinical efficacy of visceral manipulation and to suggest standard of techniques and treatment protocols for diverse patients.

Domestic and Foreign Research Trend on the Pediatric Chuna Treatment (소아 추나에 대한 국내·외 연구 동향)

  • Objectives The purpose of this review is to investigate the domestic and foreign studies of pediatric Chuna treatment and propose the directions of future studies and clinical applications. Methods We searched for the study at RISS, KISS, DBPIA, Pubmed, CNKI by keywords, '추나', 'Osteopathic', 'Chiropractic', 'Manipulation', '推拿', '導引', '按摩', After 2010. Results 1. Selected 3 domestic studies were categorized as 1 survey study and 2 case reports. Selected 41 foreign studies from Pubmed were categorized as 15 systemic reviews, 8 survey studies, 12 case reports and 6 control studies. Selected 82 foreign studies from CNKI were categorized as 10 systemic reviews, 22 case reports and 50 control studies. 2. 2 clinical domestic studies researched on idiopathic Scoliosis. The foreign clinical studies from Pubmed are 18 cases, and those studies were categorized into Premature baby care (3), Infant colic (2), ADHD (2), Congenital talipes equinovarus (1), Somatic dysfuntion (1), Nonsynostotic occipital plagiocephaly (1), Conversion disorder (1), Lower back pain (1), Chronic bilateral dorsal foot pain and stiffness (1), plantar fasciitis (1), Migraine headaches (1), Cyclic vomiting syndrome (1), Acute otitis media (1) and Cerebral palsy (1). The other 72 foreign clinical studies were from CNKI, and they studied 39 different diseases. Systematically, they studied about digestive diseases (25), respiratory diseases (20), fever (6), musculoskeletal diseases (5), nervous system diseases (5), dermatology diseases (2) and other disease states. The Chuna treatment was used in variety of studies. 3. 2 clinical domestic studies adopted techniques of Osteopathy Chuna. The foreign clinical studies from Pubmed adopted techniques of Osteopathic manipulation (10) and Chiropractic manipulation (8). The other foreign clinical studies from CNKI adopted techniques of Acupressure (69), Abdominal manipulation (23), Spinal manipulation (21), Thoracic manipulation (11), Traction manipulation (2), Muscular manipulation (2), Squeezing Sha manipulation (1), Spine correction (1), Joint manipulation (1) and Fascia manipulation (1). Conclusions In addition to musculoskeletal disorders, variety of pediatric diseases could be treated with Chuna treatment instead of acupuncture.

  • Objectives: The purpose of this case study was to report the effect of Chuna therapy include Space Spinal Conduction Exercise and Manipulation Treatment for Functional dyspepsia patients. Methods: A 32-years-old male patients with functional dyspepsia visited the clinic ten times from March 17th 2023 to April 22th 2023. To assess the treatment outcomes, we used the Standard tool for pattern identification of Functional dyspepsia and Nepean Dyspepsia Index-Korean version (NDI-K) symptom score. Result: After treatment, Standard tool for pattern identification of Functional dyspepsia and Nepean Dyspepsia Index-Korean version (NDI-K) score decreased. Conclusion: Chuna therapy include Space Spinal Conduction Exercise and Manipulation Treatment was effective in the treatment of patient with Functional dyspepsia.

  • Objectives : The os odontoideum is an uncommon abnormality which is due to non fusion of axis and odontoid process that exists as a separate ossicle apart from a hypoplastic dens. Its clinical significance lies in its potential to produce serious neurologic symptoms after cervical Chuna Manipulation Treatment. We experienced a patient diagnosed as os odontoideum which corresponds to absolute contra-indications of chuna manipulation treatment and report it. Methods : The patient was diagnosed as os odontoideum through cervical spine CT and MRI. Results and Conclusion : During cervical physical examination, we encountered L'hermitte's phenomenon from the patient and suspected one of contra-indications of chuna manipulation treatment. We could diagnose the patient as os odontoideum through cervical spine CT, MRI and prevent him from severe sequelae caused by chuna manipulation treatment.

  • The objective of this study is to report the effect of Chuna manipulation and foot orthosis treatment on juvenile, adolescent idiopathic scoliosis by observing four clinical case studies. Pre-and post-treatment, we investigated the changes in Cobb's angle, pelvic height and walking pattern by using the standing full spine X-ray. After application of a Chuna manipulation and foot orthosis treatment, Cobb's angle was reduced in 4 cases. Difference of pelvic height was reduced in 3 cases, and other 1 case was increased rather. And walking balance was improved in the case of measuring walking balance pattern. This study showed that Chuna manipulation and foot orthosis treatment has meaningful effect on juvenile, adolescent idiopathic scoliosis and more researches should be followed.

The Trend review of Chuna Therapy on Neck disorders and Cervical disc Herniation in Pubmed (경추추간판탈출증과 경부통증에 대한 추나치료에 관한 해외연구동향분석 -Pubmed 검색중심으로-)

  • Objectives : The purpose of this study is to explore the trends of Chuna(manipulation) treatment on neck pain and cervical disc displacement. Methods : We set up the two searching strategies and investigated the latest clinical trials on Chuna(manipulation) therapy for Neck disorders and cervical disc herniation through PubMed search. The searched papers were analyzed and the year, journals, article types of literatures were examined. Of the articles, Randomized controlled trials(RCTs) are assessed by Jadad scale. Results : Studies on Chuna(manipulation) treatment to cervical disc herniation were very few. Articles on Chuna(manipulation) treatment to neck pain were relatively more than those of cervical disc herniation. The most frequently published journal was J Manipulative Physiol Ther. The main design of study was RCT. The limited quality assessment of RCT was conducted by Jadad scale and the average Jadad score was 1.73, which indicates low quality in terms of Jadad evaluation criteria. Conclusions : International trends of studies on Chuna therapy of cervical disease is increasingly reviewed. Although this review has several limitations, analyzing the latest trends of studies on Chuna(manipulation) treatment on neck pain and cervical disc displacement is necessary for further studies.

  • Objectives : The aims of this study were to introduce the theory of traditional Chinese orthopedic theory(整脊理論) and to analyze the guideline of diagnosis and treatment of common spinal disorders by treating Chinese spinal manipulation technique(常見病中醫整脊指南). Methods : We have searched the web sites of traditional Chinese orthopedics association (整脊學會), a branch of the China academy of traditional Chinese medicine (TCM), and related articles. Additionally, we analyzed the guideline of diagnosis and treatment of common spinal disorders by treating Chinese spinal manipulation technique which was introduced by that association at 7th academic conference in China in 2011. Results : The guideline was prepared by 2 year project leaded by professor Yi-zhong Wei who was a person in charge of the spinal manipulation guideline of national standardization committee of TCM in China. It was a summary of the whole theory of China spinal manipulation therapy in TCM and contained 22 common spinal disorders by the treatment of traditional Chinese Tuina methods. Spinal manipulation theory was summarized by one doctrine, two theories, and two important view points. The treatment method was abstracted as 8 major Tuina techniques. Conclusions : The guideline will be a way of advance of Chinese spinal manipulation technique. This will be a good reference to Korea Chuna manipulation for improving their techniques.

Stroke Following Cervical Chuna Treatment by Nonprofessional Therapist : A Case Report (경추 추나치료 후 발생한 뇌졸중 1례)

  • The occurrence of neurologic symptoms after cervical manipulation(Chuna treatment) seems to be an uncommon event considering the large number of manipulations performed. The precise incidence is unknown, for only patients with serious complications seek medical care. We recently experienced a 3D-year-old woman who developed sudden neurologic complications after cervical Chuna treatment by a nonprofessional therapist. Whether the manipulation is the major causative factor in the subsequent stroke can be disputable and must be considered in each case. Physicians and patients, however, should be aware that cerebrovascular accidents may occur as a result of cervical manipulation.
