A Systemic Review of Traction Therapy for the Patients of Cervical, Lumbar Vertebral Disease (경추 및 요추의 통증에 사용되는 견인요법에 대한 문헌 고찰)

  • Objectives : This study aims to evaluate domestic/foreign clinical studies related to the traction therapy and analyse points to consider about cervical/lumbar traction therapy. Methods : Seven databases were searched for related articles about cervical/lumbar traction therapy from 1950 to 2014. Fourteen clinical studies and several systemic reviews were included among 144 studies searched. Out of fourteen clinical studies, four were case series and ten were controlled trials. Results : Most of included studies reported favorable effects of traction group compared to baseline of controlled group. The various mechanical factors most relevant to traction are 1) angle of pull and direction, 2) traction force, 3) duration of traction and frequency of treatment. Conclusions : We found various mistake in the applications of statistical methodologies of traction therapy targeting patients of cervical, lumbar vertebral disease. It is necessary for more randomized controlled trials to evaluate effect of cervical/lumbar traction therapy targeting patients of cervical, lumbar vertebral disease.

Analysis of the Foot Indices Using Dynamic and Static Check of Postgraduate School Students in Yangsan (양산지역 대학원 일개 학년 학생을 대상으로 동적검사 및 정적검사를 통한 족부지표 분석)

  • Objectives : To evaluate the foot indices using dynamic and static checking on healthy adult people. Methods : Foot indices analysis was performed for 46 students. Static checking practiced for Foot Pressure(FP) and Toe Pressure Difference(TPD) and dynamic checking was used for Vertical Axis Angle(VAA). Results : On gender, there was generally no statical significant difference, except RFP(Right Foot Pressure). The RFP of male was bigger than that of female. On age, in addition, there was similar tendency, only RFP had a positive correlation (r=0.35, p<0.05). Comparing foot indices on right and left, we found TPD and VAA had a tendency of pronation on left, but FP had a opposite tendency. On consistency of each foot index, discrepancy between TPD and VAA (39.47%) was lesser than combination of the others. Conclusions : According to foot indices among 46 healthy student, we found out that there is partially correlation between dynamic and static checking. We hope that the data we found will contribute to standardization of podiatry indices which provide medical guidance for cure of foot disease, supporting further podiatry research.

The Domestic Trends of Traditional Korean Medicine Treatments on Frozen Shoulder (동결견의 한의학적 치료에 대한 국내 연구 동향)

  • Objectives : The purpose of this review is to investigate the trends of traditional korean medicine treatments on frozen shoulder. Methods : We investigated the studies on traditional korean medicine treatments for frozen shoulder via searching 6 Korean web databases. As a result, 32 research papers were found to be analyzed according to their published year, the titles of journals, published institution, the types of study, the number of cases, the types of treatments, the instruments for assessment and ethical approvals. Results : 32 papers were published since 1989. The studies on traditional korean medicine treatments about frozen shoulder were mainly published in The journal of korean acupuncture & moxibustion medicine. 10 case series, 7 reviews, 6 non-randomized controlled trials, 5 randomized controlled trials and 4 case reports had been under research. In most of the research, the number of the cases were not enough. In 15 cases of the studies, various traditional korean medicine treatments were used to treat the symptoms. In 8 cases of the studies, one method was used to treat the symptoms. Range of motion(ROM) and visual analogue scale(VAS) were used as primary assessments. Among the 25 clinical studies, 5 of them were accepted by institutional review board(IRB). Conclusion : In this study, we analyzed the trends of traditional korean medicine treatments on frozen shoulder. Reviewing the domestic trends of studies on traditional korean medicine treatments for frozen shoulder and examining the strong and weak points of those treatments are essential for the future studies.

  • Objectives : To identify the association between cross-sectional area of lumbar regional muscle and stability(ST), center of pressure(COP) assessed by tetrax. Methods : Patients(n=55) who had taken L-SPINE MRI and Tetrax(Sunlight, Israel) were analyzed retrospectively. To evaluate the cross-sectional area of lumbar regional muscle, L-SPINE MRI was used. Data of ST, COP were accumulated by using dynamic equilibrium analysis by Tetrax. Of the lumbar regional muscles, the Psoas muscles and the Paraspinalis muscles were examined. Using Pearson correlation, we analyzed COP, ST and the difference between the cross-sectional area of lumbar regional muscles. Results : The variance of cross-sectional area of both sides of Psoas muscle and COP had moderate positive correlation(r=0.621). Between variance of cross-sectional area of both sides of Paraspinalis and lumbar regional muscle and COP there was low positive correlation(r=0.287, r=0.329) ST also had low correlation with variance of cross-sectional area of both sides of Psoas muscle. Conclusion : The variance of cross-sectional area between both sides of Psoas muscle had moderate correlation with COP.

  • Objectives This research aimed to investigate recent clinical treatment of herbal medicine fumigation for plantar fasciitis. Methods We searched recent clinical studies regarding herbal medicine fumigation in traditional Chinese medical journals on plantar fasciitis through the China Academic Journal (CAJ). Clinical articles published from 2012 to July 2022 were analyzed. This study examined the types of study, periods, herbal medicine, assessment, and risk of bias and conducted a comparative evaluation of herbal medicine fumigation added to other treatments. Results Ten studies were selected from 52 studies. Eight studies were classified as randomized controlled trial. Various assessment scales were used. Most treatments were administered for approximately 20 to 30 min once daily. Most studies revealed that herbal medicine fumigation is effective for plantar fasciitis. Conclusions Most studies showed that herbal medicine fumigation is effective for plantar fasciitis. More studies are needed for Korean medicine development.

  • Objectives : The aim of this study was to analyze changes in the gait pattern of a unilateral knee pain patient after treatment with Korean Medicine therapy. Methods : We conducted gait analysis 4 times over 11 days to analyze changes in gait parameters using gait analysis systems. Results : Velocity, cadence, step length, and stride length were all increased. On the collateral side, both stance phase and double support (%) decreased. These results are similar to those of hemiplegic patients with improvement patterns. Conclusions : Gait analysis could be used as a good evaluation tool for tracking improvements in knee pain.

  • Objectives : The purpose of this study was to report five patients with a sternum fracture after a traffic accident who improved with a combination of Korean medicine treatments. Methods : We collected data of traffic accident patients with a sternum fracture who were admitted to Dae-Jeon Jaseng Korean Medicine Hospital from March 2019 to September 2019. We observed five cases of patients treated with acupuncture, pharmacoacupuncture, herbal medicine, and Korean physical therapy. We measured the validity of the treatments using the Numeric Rating Scale (NRS) and European Quality of Life-5 Dimension (EQ-5D) at admission, at two weeks, and on the discharge date of hospitalization. Results : Following treatment using a combination of the above mentioned methods, the general overall condition of the patients had significantly improved. Furthermore, the NRS and EQ-5D scores were numerically better. Conclusions : After the combination of Korean medicine treatments, results from five patients with a sternum fracture after a traffic accident suggested that the treatments were effective. However, the number of subjects was insufficient and individual efficacy was not measured in this study. Therefore, further studies are needed on this topic.

  • Objectives This review was conducted to study use of Aconitum herbal medicine for pain control in musculoskeletal disease. Methods Musculoskeletal disease is a major factor of social cost increase and physical disability. Various drugs, surgery and imaging are being overused regardlessly. Aconitum herbal plants are one of the most toxic Korean traditional herbs. However, they could be utilized effectively on patients with appropriate processing and decocting time. We searched Korean medicine literature to see various features of aconitum herbal plants and tried to find the utilization of the plants in effective way for pain control in musculoskeletal disease. Results Aconitum herbal plants needs to be used carefully because of intoxication which could lead to severe damage to human body. Processing of these toxic plants could minimize the harm and raise the benefits, such as relieving various types of pain and positive inotropic action. Further studies with clearer evidence and discovering aftereffects of processing in more details are needed. Conclusions Aconitum herbal plants could dedicate to controlling pain in musculoskeletal disease with various forms and appropriate processing.

Methods : We investigated the studies on traditional Traditional Korean Medicine (TKM) treatments for degenerative knee arthritis via searching 5 Korean web databases. Results : 31 research papers were found to be analyzed according to their published year, the titles of journals, published institution, the types of study, the number of cases, the types of treatments. 31 papers were published since 1992. The studies on TKM treatments about degenerative knee arthritis were mainly published in The journal of korean acupuncture & moxibustion medicine. 13 Randomized Controlled Trials, 10 Case series, 4 Reviews, 3 Non RCTs, and 1 Case Report had been under research. In 22 cases of the studies, one method was used to treat the symptoms. Whereas in 5 cases of the studies, various TKM treatments were used to treat the symptoms. Visual analogue score(VAS) and patient's subjective complaints were used as primary assessments. Conclusion : In this study, we analyzed the trends of TKM treatments on degenerative knee arthritis. Reviewing the domestic trends of studies on Traditional Korean Medicine treatments on degenerative knee arthritis and examining the strong and weak points of those treatments are essential for the further studies.

Daoyin Exercise Therapy for Stroke : A Systematic Review (뇌졸중에 대한 도인 운동요법의 효과: 체계적 문헌 고찰)

  • Objectives : This study aimed to review the effects of Daoyin exercise on stroke patients in clinical studies. Methods : In this study, we reviewed 11 electronic databases (CAJ, EMBASE, Cochrane Library, Web of Science, Pubmed, RISS, Dbpia, NDSL, KISTI, KISS, KMBASE) on October 27, 2019. We included only randomized controlled trials (RCTs) which evaluated the effect of Daoyin exercise on stroke patients. The methodological quality of the included studies was checked using the Cochrane risk of bias tool. Results : After screening the papers, eleven RCTs fulfilled the inclusion criteria. The results of the meta-analysis showed that Daoyin exercise improves the measurement value on the Fugl-Meyer Assessment, Modified Barthel Index and National Institutes of Health Stroke Scale in stroke patients. Conclusions : We concluded that Daoyin exercise has therapeutic effects in functional recovery and in enhancing the independence of daily living activities for stroke patients. However, the quality of the original articles was low and the number of papers included were few. Thus, to confirm these results, further well-designed RCTs should be conducted.
