A Clinical Study on One Case of a Patient with Fibromyalgia by Chuna Therapy and Traditional Korean Medicine Treatment (섬유근통 환자의 추나요법 및 한의약치료 증례보고)

  • Objectives : The purpose of this study is to report a clinical progress of treatment of fibromyalgia through Traditional Korean medicine treatment. A patient was diagnosed with fibromyalgia by clinical correlation on 2009 in Jeonnam university hospital, he had multi sited pain and severe muscle spasm. Methods : Between 27th march 2014 and 15th April 2014, he was treated with acupuncture, cupping, electro-acupuncture and chuna therapy every day and observed by visual analogue scale and assessment about number of pain site and square of pain. Results : After treated by using above treatment, VAS and assessment about number of pain site and square of pain were improved. Conclusions : Chuna therapy and other Traditional Korean medicine treatment are estimated to be good for fibromyalgia. But more cases are required to prove the effectiveness of chuna therapy on fibromyalgia.

  • Objectives : The object of this study is to report a clinical effect of oriental medical treatments lot chronic muscle weakness caused by herniated intervertbral lumbar disc. Methods : The patient was diagnosed as lumbar disc herniation, and was treated by conservative treatments including acupunture, herbal mixture, pharmacopuncture. And we measured Visual Analog Score(VAS), Walking time and Manual Muscle testing(MMT). Results : After treatments, Visual Analog Score, Walking time and Manual Muscle testing(MMT) were improved in case. Conclusions : Chronic Muscle weakness caused by lumbar disc herniation can be improved by conservative maneuver as to oriental medical method.

Research about Adverse Effect of Spinal Manipulation Therapy : Systemic Review of Literature in Korea and Pubmed (척추수기치료의 부작용에 대한 국내 및 국외 현황 비교)

  • Objectives : The aim of this systemic review was to summarize and compare adverse effect of spinal manipulation at domestic and abroad. Methods : We searched articles in Korean databases and Pubmed. We selected and analysed the articles met inclusion criteria. Results : 51 cases were included. 32 cases in Pubmed show that adverse effect related to cervical rotation and high velocity low amplitude(HVLA) thrust manipulation. It leads to severe adverse effect and sequela about vascular. While, 19 cases in Korean articles shows that domestic spinal manipulation is more safe becase of lower use of high velocity low amplitude(HVLA) thrust. It leads to low incidence of vascular adverse effect and no sequela. Conclusions : We concluded that Chuna is difficult manipulation because of possibility of adverse effect. Chuna is recommended to be taken by licensed medical practitioners who are skilled and complete the regular curriculum. Further studies like meta analysis about spinal manipulation are needed to estimate incidence of adverse effect. And adverse effects are going to be collected continually through randomized controlled trials(RCT) related to spinal manipulation.

An Analysis of the Study Tendency on Meridian Muscle (국내의 경근(經筋) 연구동향에 대한 고찰)

  • Objectives : The aim of this research is to analyze the current trend of the studies about eridian muscle and to provide background for further studies. Methods : Reviewing 33 domestic oriental medical studies about meridian muscle, and comparative analysis was made. These studies were classified by method, theme and subtitle. Results : 1. According to the classification by study method, number of literary studies are 22(67%), which is more than half, number of experimental studies are 5(15%) and clinical studies are 6(18%). 2. According to the classification by study theme in literary study, percentage of 'Structure amp; Movement of Meridian Muscle' took 64%, Theory study of Meridian Muscle' took 14%, 'Application of Concept of Meridian Muscle' took 14%, 'Treatment of Meridian Muscle disorder' took 9% arranged in order. 3. In 'Theory study of Meridian Muscle', there were not only literary approaches but also Deficiency-Excessiveness(虛實) and historical approaches. Study about 'Structure & Movement of Meridian Muscle' includes analysis of muscle and Myofascial pain syndrome. On this background, it is necessary to recognize the linkage and motion analysis of Meridian Muscle. Therefore, studies were changed into interpretation about Anatomy trains, analysis of motion. The study about 'Treatment of Meridian Muscle disorder' provided the various treatment method-Acupuncture, Manual therapy, Ashi(阿是)-point therapy, CHUNA therapy etc.- in literary study. The study about the 'Application of Concept of Meridian Muscle' has been performed in relation to Embedding Therapy, Kyungkuen chuna, Ki-gong therapy. 4. Experimental Studies were all Anatomical Studies. Studies were done in trial of discovering the actual existence, but revealed problem in interpretating the meaning of Meridian Muscle. 5. Clinical Studies based on Ashi(阿是)-point therapy CHUNA Muscles Along Meridians Release Therapy etc, were performed. Experimental studies about Meridian Muscle were assessed as low grade according to Jadad Scale. There were no studies which were based on well-organized Meridian Muscle theory. Conclusions : There needs to be more discussion about concept of Meridian Muscle and proceed more reliable experimental studies with organized Meridian Muscle theory. Further objective studies about treatment of Meridian Muscle should be done.

A Systematic Review and Meta-analysis of Manual Therapy for Temporomandibular Disorder (턱관절장애에 대한 수기요법의 체계적 문헌 고찰과 메타분석)

  • Objectives: To evaluate and summarize the efficacy and safety of manual therapy in temporomandibular disorder(TMD). Methods: Ten databases (Pubmed, Cochrane, EMBASE, AMED, CINAHL, CNKI, and four Korean databases) were searched up to March 2017. Pain and range of motion(ROM) of temporomandibular joint were used as the primary outcome measure, and effective rates, dysfunction scale and adverse events were used as secondary outcome measures. The Cochrane risk of bias tool was used to evaluate the methodological quality. Results: 10 randomized controlled trials(RCTs) involving 603 participants were included. There were no significant differences between the two groups in comparison with placebo manual therapy group. In comparison with Wait-list and conventional treatment groups, manual therapy significantly alleviated pain and ROM. Five RCTs(50.0%) reported adverse events and there were no adverse cases by manual therapy. Conclusions: We found evidence that manual therapy may alleviate symptoms of patients with TMD. A positive effect was observed in comparison with conventional treatment and Wait-list, but no conclusion for the comparisons with placebo manual therapy. There is no evidence of worsening symptoms or causing adverse events.

A Clinical Analysis to Study Effectiveness of Korean Medicine for Medial Collateral Ligament Injury of the Knee (무릎 내측측부인대 손상의 한의학적 치료에 대한 국내외 임상연구 동향 분석)

  • Objectives This study aimed to review clinical studies on traditional Korean medicine treatment for medial collateral ligament injury of the knee. Methods Clinical studies on Korean traditional medicine treatment of medial collateral ligament injury were conducted. We used five Korean online databases (OASIS, KISS, RISS, DBPia, and ScienceOn) and three foreign databases (PubMed, Cochrane Library, and CNKI). Out of 99 studies that were found, we excluded repeated articles, studies that were not related to Korean medicine, and those not relevant to the topic of the study. Results Ten randomized controlled trials and 20 case studies were selected. Eight traditional Korean medicine treatments, including acupuncture, herbal medicine, chuna, and herbal ointment, were used in these studies. The most commonly used treatment was found to be acupuncture. Conclusions Our study showed that traditional Korean medicine for medial collateral ligament injuries was effective. However, there were some limitations. Further clinical studies and randomized controlled clinical trials are needed for more evidence on Korean traditional medicine.

  • Objectives The purpose of this report was to study the circulation of meridian WiGi, YoungGi, from the viewpoint of manual medicine. Methods First, the Korean Medical approach analyzes documents about the circulation of meridian WiGi, YoungGi, and the biomechanical approach is to analyze documents about kinetic force and kinematic movement. The third inherent energy approach is to analyze documents about craniosacral rhythm and visceral motility. Finally, it is to study the correlation between the circulation of meridian WiGi, YoungGi, and the viewpoint of biomechanics force and movement, the inherent energy of manual medicine. Results Meridian WiGi is fast, powerful, and changeful. It circulates through the head and extremities in the daytime and visceral organs at night. The deviation pelvis and distorted thoracic cage create kinetic force and kinematic movement. Meridian YoungGi is very small and soft energy and circulates meridians and visceral organs permanently. Craniosacral rhythm and visceral motility radiate continuously from cranial and visceral organs to the whole body. Conclusions Circulation of meridian WiGi is closely related to the biomechanical approach. In addition, circulation of meridian YoungGi is closely related to the inherent energy approach.

A Study of the Role and Treatment of Scalene Muscle in Breathing Pattern Disorder (호흡 양상 장애에 대한 사각근의 역할과 치료법에 대한 소고)

  • There are studies on breathing pattern disorder (BPD), but the causes of BPD are still complex, and various studies are ongoing. This study reviewed several studies to investigate the possibility that pathological changes in the scalene muscles may be one of the causes of dyspnea, and that treatment of them may improve respiratory disorders. Anatomically, the scalene muscles are located between the cervical vertebrae and the transverse process of the ribs and act as a respiratory muscle. If there is a problem or excessive in its role, it can cause chest breathing or oral breathing. These problems may further affect respiratory diseases such as hyperventilation syndrome, obstructive disease, restrictive disease, and respiratory disorders. According to the results of previous studies, it seems that manual therapy or exercise therapy for the scalene muscles can contribute to the treatment of BPD.

A Literatual Study on the Chuna of 『Naegyung』 (『내경(內經)』의 추나(推拿)에 관한 연구(硏究))

  • Objectives : Through the study of Chuna of "Naegyung", this study attempts to secure the basis of Chuna therapy. Methods : This study selected sentences such as Kyo, Gyo, Do, Ma, Mun, Bu, Ahn, Ol, In, Jeol, Jo, Chu, Tan(喬, ?, 導, 摩, ?, ?, 按, ?, 引, 切, ?, 推, 彈), etc. to try various studies related to Chuna. Results : As for Chuna in "Naegyung", the origin and role of Chuna, the instruments of Chuna, the mechanism of Chuna treatment, the application range of Chuna, and the notes of Chuna, etc. are summarized in this study. Conclusions : Based on Chuna of "Naegyung", it is necessary to study the development process of Chuna and to create various manual techniques of Chuna.
