Systematic Review of the Diagnosis of Pelvic Deviation for Chuna Manual Therapy (추나치료에 적용된 골반변위 진단법에 대한 체계적 문헌고찰)

  • Objectives This systematic review aimed to analyze research about pelvic deviation diagnosis for Chuna manual therapy (CMT) and to review the diagnosis methods, indices, and results of diagnosis. Methods Ten electronic databases were systematically searched up to January 4th 2022. Clinical studies and reviews containing pelvic deviation diagnosis for CMT or using CMT as a treatment of pelvic deviation were selected and evaluated. CMT diagnosis in clinical studies and reviews were isolated and analyzed by 2 independent reviewers. Results Thirteen clinical studies and three reviews were included in the evaluation. X-ray analysis and manual testing were the two main methods used in CMT diagnosis of pelvic deviation. For manual testing in clinical studies, leg length insufficiency testing was the most frequently used measurement index and the most common diagnostic results were anterior and posterior rotation. In the X-ray analysis, Obturator foramen and femur head line were the most frequently used measurement index and the most common diagnostic results were anterior rotation and posterior rotation. Conclusions The systematic review found that manual testing and X-ray analysis were mainly used for the diagnosis of pelvic deviation in CMT among clincial and review articles. As there was little research about diagnosing pelvic deviation in CMT and any existing research presented only low standards of evidence, further research should be updated with using a more standardized approach.

A Case Report of Combining Korean Medicine Treatment with Chuna Manual Therapy for Cervical and Lumbar Pain and Hypoesthesia of the Limbs After Total Spondylectomy for Giant Cell Tumor of the Cervical Spine (경추거대세포종에 대한 전척추제거술 후 발생한 경요추부 통증 및 사지부 감각저하에 대한 추나요법을 포함한 한방치료 증례보고 1례)

  • Objectives This study aimed to report the effects of giant cell tumor treatment to a patient who had cervical and lumbar pain and hypoesthesia of the limbs after total spondylectomy with Korean medicine. Methods A 67-year-old female patient with cervical and lumbar pain and hypoesthesia of the limbs after total spondylectomy was treated with herbal medicine (Cheongpa-jeon, Yukkongbaro-hwa), acupuncture, pharmacopuncture, and physical therapy for 36 days. The effects were evaluated using a numerical rating scale (NRS), Neck Disability Index (NDI), Oswestry Disability Index (ODI), and EuroQoL-5D (EQ-5D). Results After treatment, changes were observed in the patient's pain and hypoesthesia. NRS, NDI, ODI, and EQ-5D scores decreased. Conclusions This case study suggests that Korean medicine treatment may be effective for symptomatic treatment in patients with giant cell tumor following total spondylectomy.

  • 통증감소를 위주로 하는 기존의 슬 골관절염 치료는 슬관절의 퇴행을 가속화시킬 수 있기 때문에 한의학적 각종 침구요법 및 물리요법, 약물요법이나 의학적 주사요법 등을 '슬관절에 대한 부하 힘을 감소시킨 상태'에서 시행하는 진료형식을 제안한다. 그리고 이러한 슬관절질환 치료의 추나요법을 채택하여 임상적용이 가능하도록 이를 기계화 도구화하고, 진단과 치료를 동시에 시행할 수 있는 방식이 되도록 연구할 필요가 있다.

  • To clarify the effect of chuna treatment reached at neck pain, the faculties of Oriental Rehabilitation Medicine in Hospital of Oriental Medicine, Kyung Hee Medical Center had carried out chuna treatment for 72 patients who had neither structural defectiveness of cervical spine nor neural injury but simple soft tissue damage among people who visited the hospital with the neck pain since Jun. 11, 1997 to Dec. 31 in the year. The way of study: 32 out of 72 patients were treated by acupuncture treatment going with chuna treatment (calling 'Group A') and the other 40 patients were cured by only acupuncture (calling 'Group B'). The treatment in both Group A and B were performed 6 times totally at intervals of a time for 2 days. To judge the effect of treatment, both method-visual analog scale(VAS) and ,check of ROM-were performed each time. The results of study 1. Each one in Group A itself, according to the number of the treatment in progress, showed the note worthy decrease of pain and improvement of range of motion(ROM) in the cervical(P <0.001). 2. Each one in Group B itself, according to the number of the treatment in progress, showed the note worthy decrease of pain and improvement of range of motion(ROM) in the cervical(P <0.001). 3. In the degree of improvement in pain, the Group A who had been treated by both acupuncture and chuna treatment at the same lime showed some meaningful decrease in pain compared to the Group B with acupuncture treatment alone(P <0.001). 4. In the degree of improvement in ROM, the Group A who were treated by both chuna and acupuncture treatment had meaningful improvement compared to Group B who got the only acupuncture treatment(P <0.001).

  • Objectives The purpose of this study was to analyze the current status of utilization of Korean medical institutions. Methods This study analyzed the Health Insurance Review and Assessment Service data (2016-2020). We included patients with one of the five most frequent diseases in Korea and examined the variation in healthcare utilization. Results This study analyzed the Health Insurance Review and Assessment Service data (2016-2020). We included patients with one of the five most frequent diseases in Korea and examined the variation in healthcare utilization. Conclusions As a result of this study, It was possible to understand the impact of Chuna treatment and COVID-19 on the use of Korean medical institutions. Further studies are needed to establish a definite conclusion regarding the relevance of COVID-19.

  • Objectives: This report of three cases illustrates the potential effect of chuna therapy and myofascial release technique on back pain and curve regression in the at-risk, skeletally immature patient with adolescent idiopathic scoliosis. Methods: The correction program for scoliosis takes 3 month per case. Chuna Therapy and MRT were given once a week and other oriental medicine care was given twice a week to care the complications of scoliosis. The Cobb's angle(by X-ray, Full spine view) and Visual analogue scale(VAS) were evaluated before and after the treatments. Results: After the program, both of Cobb's angle and VAS decreased. Conclusion: Though it is a case report, we found Chuna Therapy and MRT might have valid effect on adolescent idiopathic scoliosis patients. Further rigorous case series and controlled trials are warranted.

A Review of Case Reports on the Application of Acupuncture as a Treatment for Fracture since 2013 (골절에 침 치료를 적용한 2013년 이후 증례보고 문헌고찰)

  • Objectives This study aimed to investigate the trend in treating fracture with acupuncture and to evaluate the quality of case reports. Methods All case reports of fractures treated with acupuncture were extracted from four Korean web databases. We classified these studies by five fracture sites and investigated frequently used meridian and acupoint, outcome measurements, treatment period. And we assessed the quality of the case reports by the STandards for Reporting Interventions in Clinical Trials of Acupuncture (STRICTA) guidelines. Results A total of 33 case reports were included. The outcome measurements were divided into six categories, and the outcome measurements used in more than three studies showed improvement in patient symptoms. The mainly used meridians for each fracture site were as follows: chest and abdomen (GB, LR, CV), back (BL, GB), upper limb (LI, TE), lower Limb (GB, ST, BL, SP). The most commonly used acupoints for each fracture site were as follows: chest and abdomen (Ashi points, GB24, GB25, GB26, LR13, LR14, CV16, CV17, CV18, CV19), back (BL23, BL24, BL25, BL26, BL40, BL51, BL52, BL60, GB34), upper limb (LI4, LI10, LI11, TE3), lower limb (GB34, GB40). According to the STRICTA guidelines, more than 54.54% of the reports were found to be 'not reported' or 'not sufficient' in the following categories: 'response sought', 'description of participating acupuncturist', and 'number of needle insertions per subject per session'. Conclusions The meridians and acupuncture points frequently used for acupuncture treatment of fractures were near the fracture site. Future clinical studies involving acupuncture must be reported in accordance with the STRICTA guidelines to improve transparency and uniformity.

  • Objectives : This study was carried out to investigate the effects of "Yangmuji Anchu Traction Technique" on Temporomandibular Joint Disorder Patient. Methods : The therapies were Chuna("Yangmuji Anchu Traction Technique"), acupuncture treatment, Carthami-Semen Herbal Acupuncture treatment and herbal medicine. We estimated the effects of treatment by measuring from a lower front teeth to upper front teeth in open mouth, measuring a gap of center line on front teeth in open mouth. And by measuring Visual Analog Scale(VAS) and Patient Global Assessment(PGA) before and after treatment. Results : After treatment, we confirmed these improvements : the length measuring from a lower front teeth to upper front teeth in open mouth and the gap of center line on front teeth in open mouth was improved in all cases. Also Visual Analog Scale(VAS) was improved in all cases. Conclusions : These results suggested that "Yangmuji Anchu Traction Technique" effected for Temporomandibular Joint Disorder Patient.

A Systematic Review and Meta-Analysis of Randomized Controlled Trials on Chuna Manual Therapy for Cervicogenic Headache

  • Objectives: We conducted this study to evaluate the efficacy of Chuna Manual Therapy (CMT) for treatment of cervicogenic headache (CeH) through systematic review and Meta-analysis of randomized controlled trials (RCTs) as a preceding research to further research the effective of Chuna Manual Therapy for patients who suffered from CeH. Methods: We conducted a systematic review and meta-analysis by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. We searched the studies from MEDLINE, Elsevier-EMBASE, The Cochrane Library, CAJD, KISS, KMBase, Korean Traditional Knowledge Portal, NDSL, and OASIS. The studies selected only in randomized controlled trials. We selected the chosen studies by the selection and the exclusion criteria, and evaluated the quality of the selected studies using the Jadad score and the Cochran ROB tool. We used the Visual Analogue Scale score (VAS) and Clinical total Effective Rate (CER) for the results and analyzed the results of the included studies using RevMan 5.3 software provided by the Cochran library. Results: We included 20 RCTs, including 1,673 subjects, in the systematic review and meta-analysis. After the intervention, the CMT group showed better results than the pharmacotherapy group, the physiotherapy group, and the combined treatment group. The CMT group showed a good effect on the CER and the VAS but showed a significant heterogeneity compared to the pharmacotherapy group. Conclusions: The CMT as monotherapy might have benefits on Cervicogenic Headache patient. Further well-designed studies need to be conducted.

The Clinical Case Report on a Patient with HIVD(Herniated Intervertebral Disc) and Schwannoma, Treated by Conservative Oriental Medical Treatment (요추 추간판 탈출증과 신경초종을 동반한 요각통 환자를 대상으로 한 한방 보존적 치료 치험 1례)

  • Objectives: The purpose of this study is to report clinical effects of oriental medicine w~h conservative treatments including acupuncture, Chuna treatment and herbal medicine on a patient with low back pain and lower limb numbness caused by HIVD and schwannoma. Methods: The patient was diagnosed with HIVD and schwannoma through the MRI scan. This case was treated with acupuncture, Chuna treatment and herbal medicine during the whole admission period. We evaluated the progress of the symptoms with objective criteria such as NRS(Numerical Rating Scale), SLRT(Straight Leg Raising Test), ODI(Oswestry Disability Index) score and MMT(Manual Muscle Testing). Results and Conclusions: After weeks of conducting conservative treatment on patient with low back pain and lower limb numbness caused by HIVD and schwannoma occurred at the lumbar spine, significant improvements of the symptoms were seen. These results suggest that in the case of low back pain and lower limb numbness caused by HIVD and schwannoma, conservative treatments can be considered as one of the options of treating the symptoms beside surgical way.
