Effect of Chuna on circulatory system - Based on Experimental Chuna Science - (추나의 순환계에 미치는 영향에 대한 고찰 - 실험추나학을 중심으로-)

  • Objectives : Many studies are being done in the method of experimental chuna science in China. The aim of this review is to understand and study the methods of experimental chuna science, focusing on the effects of chuna on circulatory system. Methods : We reviewed "實驗推拿學" and Chinese journals were searched using China National Knowledge Infrastructure(CNKI). Korean journals were searched using 3 Korean web databases(OASIS, NDSL, RISS). Results : By experimental chuna science, effect of chuna on blood pressure, cardiovascular system, hemorheologic system, blood flow velocity, circulation and lymphatic system were proved. Conclusions : Experimental chuna science is necessary for improvement of chuna to a higher level. In Korea, not many experimental studies are being done, especially in chuna and it has very narrow target diseases. For this reason we need to learn and adapt the methods of experimental chuna science of China.

The Domestic Trend of Chuna Treatments on Lumbar Spinal Disorder (요추부 질환에 적용하는 추나 치료에 대한 국내 연구 동향)

  • Objectives : The present study examines the domestic trend of Chuna treatments on lumbar spinal disorder in Korea. Methods : We investigated the studies on Chuna treatments for lumbar spinal disorder via searching 10 Korean web databases. As a result, 63 research papers were found to be analyzed according to their published year, the titles of journals, the types of study, the techniques of Chuna, the instruments for assessment, the Chuna technique and the number of the treatment trials by the cases of lumbar spinal disorder and ethical approvals. Results : The number of the research papers published tends to increase every year. The studies on Chuna treatments were mainly published in The Journal of Korea CHUNA Manual Medicine for Spine & Nerves. The most frequently adopted technique of Chuna in the examined studies was Cox flexion & distraction technique. Visual analogue scale(VAS), oswestry disability questionnaire(ODI) were used as primary means of assessments. The ethical problems of the examined studies needed to be improved. Conclusion : Reviewing the domestic trend of studies on Chuna treatments for lumbar spinal disorder and examining the strong and weak points of those treatments are essential for the future studies. It is anticipated that this review benefits the future in-depth study on the treatments for Chuna in Korean medicine.

Comparative Study of General Oriental Medical Treatment with or without Ankle Joint Chuna Therapy on Ankle Sprain (족관절 추나요법을 병행한 족관절 염좌 환자의 호전 비교)

  • Objectives : The purpose of this study is to compare with the effect of general oriental medical treatment with or without ankle joint Chuna Manual Therapy for acute ankle sprain. Methods : We investigated 36cases of ankle sprain patients, and devided patients into two groups: One was treated general oriental medical treatment(Control Group) and the other was treated ankle joint Chuna Manual Therapy with general oriental medical treatment(Experimental Group). Results : 1. Each Group had significantly decrease in Visual Analogue Scale(VAS) and significantly increase in Ankle-Hind-foot Scale(AHS) after treatment. 2. Experimental Group was significant differences in Visual Analogue Scale(VAS) and Ankle-Hind-foot Scale(AHS) compare to Control Group. Conclusions : In this clinical study, general oriental medical treatment with ankle joint Chuna Manual Therapy was more effective in reduce ankle sprain pain and improve ankle joint function.

Chuna Manual Therapy For Simple Obesity: A Systematic Review and Meta-analysis (단순성 비만에 대한 추나요법의 효과: 체계적 문헌고찰과 메타분석)

  • Objectives The purpose of this study was to investigate the effectiveness of Chuna manual therapy (CMT) in the treatment of simple obesity. Methods We used seven databases to perform a literature search using the keywords "Chuna OR Tuina" AND "Obesity." We selected randomized clinical trials in which Chuna was used as the main method for treating simple obesity. The risk of bias was assessed by three independent researchers using the Cochrane risk of bias. Results A total of 14 RCTs were selected and analyzed. The results of the meta-analysis showed a significantly higher efficacy rate in the patient group treated with CMT as the main treatment than that in the control group. Conclusions The results of this systematic review suggest that CMT is effective in the treatment of simple obesity. However, given the limitations such as the small number of studies and the high risk of bias in the studies, additional research is needed to lend further support to this claim.

  • Objectives : To report cases of HIVD of L-Spine successfully treated with Korean medical treatment including Samul-tang(Siwu-tang) Gagam. and Chuna manual treatment. Methods : Three patients diagnosed with HIVD of L-Spine were treated with Korean medical treatment including Samul-tang(Siwu-tang) Gagam and Chuna manual treatment. Numeric Rating Scale(NRS), Oswestry low back pain Disablility Index(ODI) scores and EQ-5D were collected before and after treatment for comparison. Results : There was a significant decrease in NRS, ODI scores.And, there was a significant increase EQ-5D. The mean NRS score decresed from 10 to 1.33, ODI from 66.66 to 8.88. Tne mean EQ-5D score incresed from 0.225 to 0.856. Conclusion : The result suggests Korean medical treatment including Samul-tang(Siwu-tang) Gagam and Chuna manual treatment can be an effective treatment on patients with HIVD of L-Spine.

Orally Administered Korean Herbal Medicine Medications of Randomized Controlled Trials Published in the Journal of Korean Medicine and Related Journals: A Narrative Analysis using CONSORT-CHM 2017 (CONSORT-CHM 2017 지침에 근거한 대한한의학회지 및 회원학회지에 투고된 경구투여 한약 중재를 활용한 무작위 배정 비교 임상 연구(RCT)의 양적, 질적 평가 연구)

  • Objectives: This study aims to explore the current usage status of orally administered Korean herbal medicine in randomized controlled trials (RCTs) published in the Journal of Korean Medicine and member journals using the CONSORT-Chinese Herbal Medicine Formulas 2017 (CONSORT-CHM 2017) checklist. Methods: We searched the OASIS, RISS, and KMBASE archives as well as the websites of the Journal of Korean Medicine and 45 member journals to identify RCTs that used herbal interventions. Two independent researchers searched and categorized the RCTs and performed a quantitative evaluation by journal, study design, and target disease, as well as qualitative evaluation of the literature using CONSORT-CHM 2017. Results: After the search, 66 articles were selected. The quantitative evaluation resulted in 13 articles (19.6%) that were published in the Journal of Korean Medicine and 12 articles (18.1%) in the Journal of Internal Korean Medicine. In terms of study design, 62 articles (93.9%) were parallel, 4 articles (6%) were crossover, and 2-arm parallel study designs were the most common in 45 articles (68.2%). In terms of the study participants, physiological characteristics and mechanisms in healthy individuals were the most common in 21 studies (31.8%) and obesity in 9 studies (13.6%). In terms of assessing completeness in the CONSORT-CHM 2017 items, 29 articles were rated high, 31 were rated moderate, and 6 were rated low. Items 4a, 6a, and 7a had low reporting rates (≤ 30%), while items 2a, 2b, and 12a were completely reported in all studies. Conclusion: Future RCTs using orally administered Korean herbal medicine need to be reported completely, and the CONSORT-CHM 2017 checklist can be a helpful tool for this purpose.

Application of Joint Mobilizing Chuna Following Tibial Plateau Fracture Surgery: A Study of Two Cases (경골 고평부 골절 수술 후 관절가동추나의 적용: 증례보고 2례)

  • Objectives The purpose of this study was to report the effect of Korean medicine treatment, including the application of joint mobilizing chuna, by reporting two cases after tibial plateau fracture surgery. Methods Two patients with tibial plateau fractures were treated using joint mobilizing chuna, myofascial chuna, acupuncture, and herbal medication. The effect of the treatments was evaluated using the range of motion, manual muscle test, numeric rating scale, and Korean Knee Injury and Osteoarthritis Outcome Score. Results In both cases, the range of motion, muscle strength, and pain were significantly improved. In particular, the range of motion for knee joint flexion increased by 47° in case 1 and 30° (right) and 42° (left) in case 2. Conclusions Korean medicine treatment, especially joint mobilizing chuna, may be an effective intervention for rehabilitation after tibial plateau fracture surgery.

Research Trends in Korean Medicine Treatment for Sarcopenia: Scoping Review (근감소증에 대한 한의 치료 연구 동향: 스코핑 리뷰)

  • Objectives This study examined the trends in domestic and international research on the treatment of sarcopenia and analyzed the effectiveness of the treatment. Based on the results, we suggest directions for future Korean medicine research on sarcopenia. Methods The study was conducted using Arksey and O'Malley's scoping review methodology. It included three international databases (PubMed, EMBASE, Cochrane Library) and five domestic databases (Korean studies Information Service System, Korean Medical database, Oriental Medicine Advanced Search Integrated System, Research Information Sharing Service, ScienceON) were used. Literatures published until April 30, 2023, including the contents of 'sarcopenia' and Korean Medicine treatment', were searched. Results A total of 45 studies were included in the analysis. The number of studies on Korean medicine treatments for sarcopenia is steadily increasing, with the majority of studies coming from Asian countries. Of the 45 studies, 30 were conducted in the field of medicine, and the study design was analyzed into 10 clinical studies and 35 non-clinical studies. Among the 10 clinical studies, the most common treatment interventions were herbs (n=4), followed by Daoist exercises (n=3). The most common diagnostic criteria used in clinical studies were those published by the Asian working group for sarcopenia, and various physical examinations and laboratory tests were used as outcome measures. Conclusions The future direction of Korean medicine research on sarcopenia should be to expand the number of clinical studies applying Korean medicine treatment interventions.

Manual Therapy for Failed Back Surgery Syndrome: A Systematic Review (척추 수술 실패 증후군에 대한 수기요법의 효과: 체계적 문헌 고찰)

  • Objectives This study aimed to assess the efficacy and safety of manual therapy in patients with failed back surgery syndrome (FBSS). Methods We searched eight electronic databases from October 2022 and only selected randomized controlled trials (RCTs) reporting outcomes for manual therapy in FBSS patients. Included studies were analyzed qualitatively. Results A total of twelve RCTs were included and three studies were meta-analyzed. Manual therapy combined with exercise demonstrated more favorable outcomes in terms of pain reduction compared to exercise therapy only (P<0.01). Some studies reported that manual therapy resulted in low efficacy, while some studies reported that specific manual techniques have a significant effect in pain relief. We, therefore, conclude that several studies have a high risk of bias. Conclusions Findings suggest that manual therapy is a safe and non-invasive, promising option for managing FBSS. However, manual therapy is not the best option in all circumstances; thus, this should be selectively applied by a well-trained practitioner. Further studies such as well-designed, risk-controlled RCTs are required to gain stronger evidence.
