  • UL-syndrome(鬱證) is a syndrome of emotional depression and various physical symptoms generally due to chronic psychological stress, without constitutional abnormality. These are cases of female patients suffering from headache, chest discomfort, heart burning, insomnia, dyspepsia, anorexia, fatigue and some other minor symptoms due to chronic stress. We diagnosed these cases as UL-syndrome and treated them with oriental medicine, specifically Gamisoyosan(加味逍遙散). This paper describes the symptoms that the patients suffered, the treatment used, and identifies stages in the development of UL-syndrome, and suggests a stage in which Gamisoyosan is efficacious.

Influence of Shiftwork on Greek Nursing Personnel

  • Background: The aim of this study was to investigate the burden experienced by nursing personnel working irregular shifts in Greece and to conduct the first test of a Greek version of the Standard Shiftwork Index (SSI). Methods: A cross-sectional survey was carried out. The SSI was completed by 365 nurses and nursing assistants working shifts, including nights. Results: Female nursing personnel and those suffering from a chronic disease were most affected by working rotating shifts as they had elevated scores on the majority of the SSI scales, such as sleep, chronic fatigue, digestive and cardiovascular problems, general health questionnaire, cognitive and somatic anxiety, shift time satisfaction, engagement and disengagement strategies, languidity, flexibility, and neurotisicm. Nurses with longer working experience and those with family responsibilities also scored higher on some of the SSI scales, such as the sleep, shift time satisfaction, social and domestic disruption, disengagement strategies, morningness, and languidity scales. Conclusion: Shiftwork affects female nurses, those with chronic disease, older age, and domestic responsibilities more severely. Therefore management should take these factors into account when designing work schedules to alleviate the burden caused by shiftwork.

A National-wide Survey on Utilization of Pattern Identification for Chronic Diseases among Korean Medicine Doctors (전국 한방의료기관 한의사 대상 만성질환의 변증활용 현황 조사)

  • Objectives : The aim of this study was to survey the present utilization of pattern identification(PI) by chronic disease and using PI system in the clinical field among Korean medicine doctors. Methods : This survey was conducted from Oct. 1 to Oct. 31, 2014 by face-to-face interview using structured questionnaire. The subjects were 400 Korean medicine doctors who worked in hospitals or clinics. The questionnaire consisted of whether use clinical experience by chronic disease and the utilization of PI, the utilization of PI systems, the PI utilization rate, and the correlation between utilization PI and so on. General elements consisted of sex, age, clinical experience, place of work, district and so on. Results and Conclusions : This study revealed that clinical experience by chronic disease used 24.0-90.8%. The most chronic diseases were chronic fatigue and chronic pain, and least disease was cancer. Experience of the utilization of PI among Korean medicine doctors who have clinical experience was 87.8-97.1%. Also, the most utilized disease was chronic gastritis & gastric ulcer, and least disease was dyslipidemia. In this case, the most common cases were using the one PI between the utilization of PI systems, and the utilization ratio of Visceral PI was the highest. Phi coefficient between the PI systems in chronic diseases divided into positive correlation and negative correlation. The correlation of dementia between Triple energizer PI and Six excesses PI was the highest at positive correlation, and the frequency of occurrence between Defense, Qi, Nutrient and Blood PI was the highest at positive correlation. The correlation of osteoarthropathy between Meridian and Collateral PI and Sasang Constitutional Medicine was the highest at negative correlation. Also the frequency of occurrence between Qi, Blood, Fluid and Humor PI and Sasang Constitutional Medicine was the highest at negative correlation. We hope that additional studies on systematic PI research of chronic disease which needed to be in korean oriental medicine and meet the needs of clinical consumer continue based on this study.

  • Fibromyalgia is a syndrome characterized by chronic pain in the skeletal system accompanied by stiffness, sleep disturbance, fatigue, and psychiatric problems, such as anxiety and depression. Fibromyalgia commonly affects orofacial health, presenting with a variety of oral manifestations, including temporomandibular disorder, xerostomia, glossodynia, and dysgeusia. Therefore, oral healthcare providers need to be aware of this clinical entity to effectively manage oral symptoms and provide proper oral self-care modification and education on the nature of fibromyalgia. This review focuses on the epidemiology, pathophysiology, clinical manifestation, diagnosis, orofacial concerns, and treatment of fibromyalgia.

  • Objective: A case report on the improvement of metabolic syndrome by Ortho-Cellular Nutrition Therapy (OCNT). Methods: A 50-year-old Korean male with chronic fatigue and overlapping risk factors impaired fasting glucose, hypertension, and hypertriglyceridemia. Results: Diabetes and blood triglyceride levels improved after Ortho-Cellular Nutrition Therapy (OCNT). Conclusion: Ortho-Cellular Nutrition Therapy (OCNT) is effective in relieving the symptoms of metabolic syndrome patients.

  • Objectives and Methods : Chronic pancreatitis is an inflammative disease characterized by both persistent and irreversible progression of pancreatic lesions that accompanies complications and pain which hinders everyday life activities. As the alcohol consuming population increases, chronic pancreatitis is increasing consequently. We retrospectively analyzed the medical record of a chronic pancreatitis patient who had been treated with oriental herbal medicine, acupuncture and moxibustion for 40 days. He complained of anorexia, fatigue, indigestion, weight loss, constipation, abdominal discomfort and pain. We prescribed him Daekumeumja, Bojungikgi-tang and other oriental herbal medicines in order to cure these symptoms. Results and Conclusion : For 40 days he was treated with oriental medicine and the symptoms mostly disappeared. When it comes to the therapeutic effects, it could be suggested that oriental medicine has significant effects on improving symptoms and qualities of lives.

A Clinical Study of Chronic Urticaria (만성 두드러기에 대한 임상 연구)

  • Objective: This study was designed to investigate clinical features of chronic urticaria and assess Korean traditional medical therapy on that. Methods: The ratio of male and female, duration of symptoms, personal history and family history of allergic diseases, serum IgE level, and suspected causes and provocation factors were investigated in 41 patients with chronic urticaria. And frequency of wheals, duration of wheals, number of wheals, itch severity, and antihistamine counts of 6-point(0-5) score was assessed in 26 patients. The change of total scores between baseline and follow up were analyzed using Wilcoxon signed-rank test(p〈0.05). Results: 1. The ratio of male and female was 1:1.28 and the mean duration of symptoms was 26.0 months. 2. The frequency of personal history and family history of allergic diseases was each 53.7$\%$ and 48.8$\%$ and 30.6$\%$ had over 200 IU/mL serum IgE level. 3. The suspected causes were none(43.9$\%$), foods(29.3$\%$), drugs(9.8$\%$) and stress(7.3$\%$). 4. The suspected provocation factors were foods(39.0$\%$), none(19.5$\%$), fatigue(17.0$\%$), heat and sweating(l4.6$\%$), pressure(12.2$\%$) and stress(9.8). 5. The total scores between baseline and follow up of 26 patients was lowered from 14.85${\pm}$3.80 to 7.65${\pm}$4.31 and it was significant statistically. Conclusions: Korean traditional medical therapy was proved to be effective on the chronic urticaria and we need to consider foods as important factor in both treatment and patient education of urticaria.

Integrated Medicine Therapy for an End-stage Lung Cancer Patient with Brain Metastasis (양한방 협진을 통한 뇌전이 동반 말기 폐암환자 치험 1례)

  • Objectives : This study reports one case of a patient diagnosed with non-small cell lung cancer with brain metastasis. Methods : A 79 year-old male patient diagnosed with non-small cell lung cancer with brain metastasis was treated chemotherapy for 3 months and quitted it due to poor general condition. He transferred to Korean medicine hospital and was treated herbal medicine(Saengmaek-san plus Baekhapgogeum-tang gagam), acupuncture, moxibustion, interferential current therapy. To evaluate the patient, we measured of Visual Analogue Scale (VAS), Functional Assessment of Cancer Therapy-Lung (FACT-L), Functional Assessment of Chronic Illness Therapy-Fatigue (FACIT-Fatigue). Results : Patient's outcomes were improved during the 1st week regarding VAS (41${\rightarrow}$25), FACT-L (81.2${\rightarrow}$90), FACIT-fatigue (25${\rightarrow}$10). However, dyspnea and cough were was aggravated in the 2nd week. Conclusions : Treatment with Korean medicine therapy may have substantial benefit for patients with end-stage lung cancer. But, Integrated medicine therapy is necessary for proper management of end-stage cancer patients.

Dietary Behaviors, Consumed Time for Job Duties and Fatigue Levels of Dietitians by School Foodservice Type in Taejon and Chungnam (학교급식 영양사의 식행태, 업무소요시간 및 피로도에 과한 실태조사 -대전.지역충남의 급식방식별 비교 -)

  • The school foodservice(FS) dietitians of conventional, co-management and central kitchen systems were compared for their levels of fatigue degrees in order to examine their work specification but no to diminish their educational roles. One hundred and thirtheen primary schools in Taejon and Chungcheongnam-Do participated in the survey, of which foodservice were consisted of 99 conventional types, 7 comanagement types and central kitchen types. Information was collected about the various working hours dietary behavior and fatigue symptoms of the dietitians. The results of this study were as follows : Among 3 school foodservice types, dietitians spent 8 hours and 16 minutes daily for their for their work on the average. However, the central kitchen type dietitans in cities spent 3 hours more compared to the others. The data from the dietitians' work schedule sheets showed that the 5 parts of dietitian's duties set in the school foodservice laws were not well performed. In the central kitchen type they spent more time for food production whereas in the conventional they spent more time for menu formation and hygiene control. It was found that fatigue levels of school foodservice dietitians were chronic with no difference among foodservice systems or among school locations. Our results indicated that nutrition education in primary schools was almost neglected. It is suggested that the school foodservice should meet all the objectives of school-lunch program even though they saved dietitians' work load more or less.   • Objectives: In comparison with functional dyspepsia, food accumulation is known to have wider concept. On this study, we will make a proposal on the concept of food accumulation by a statistical comparison between pyungweesan questionnaire and health related questionnaires. Methods: We studied the similarities between six factors of pyungweesan questionnaire and four health related questionnaires(Rome II criteria of functional dyspepsia, subjective symptoms of fatigue test, beck depression inventory, state-trait anxiety inventory, etc.) using multidimensional scaling. Results and Conclusions: 1. Physical-emotional axis and acute-chronic axis were labelled in a two-dimensional scaling solution. 2. Seup-dam(濕痰) factor and sik-che(食滯) factor of food accumulation were closely related to fatigue. 3. Bi-seup(脾濕) factor of food accumulation was closely related to depression and anxiety. 4. The findings suggest that a concept of food accumulation reflects some aspects of physical fatigue and emotional depression as well as dyspepsia.
