Purpose: The purpose of this study is to exam the effects of a short-term pulmonary program on lung function, exercise tolerance, and quality of life in chronic lung patients. Method: Randomized controlled pre-post test design was used. The outcome measures were forced expiratory volume in one second (FEV1, % predicted), 6 min walking distance (6MWD), Borg score after 6MWD, and Chronic Respiratory Disease Questionnaire (CRDQ). Experimental group performed the 4-week home-based pulmonary rehabilitation program composed of inspiratory muscle training, upper and lower extremity exercise, relaxation, and telephone visit. Patients in control group were only given education about self-management strategies. Thirty four patients with moderate-to-severe respiratory impairment were recruited, and 28 patients (19 in experiments, 15 in control) completed the study. Result: Significant improvements in lung function, exercise tolerance, and health related quality of life were found only in the experiment group. Conclusion: This study yielded evidence for the potential and beneficial effects of home-based pulmonary rehabilitation program in patients with moderate to severe chronic lung disease. The program could be adequately utilized for improvement of health related quality of life in chronic lung patients.
This study aimed to develop a valid and reliable self-management instrument for pre-dialysis patients with chronic kidney disease (CKD). A total of 143 patients with CKD recruited from a medical center completed the questionnaire. The dimension and items of self-management were composed by literature review. The items of self-management was evaluated using exploratory factor analysis and measures of reliability. Five factors were extracted and labelled adherence to treatment regimen and partnership, diet adherence, problem solving, health behavior, and pursuit of psycho-social health. The five factors accounted for 51.1% of total variance. Each factors showed acceptable internal reliability with Cronbach's alpha from 0.64-0.79. The developed self-management instrument can be useful in self control of their disease for patients with CKD, and both evaluating patients' self-management and developing intervention program for health care professionals.

Methods: The program evaluation methodology was applied which is an interactive program development process based on needs assessment, formative evaluation, process evaluation and outcome evaluation. The program was developed and revised while the program was implementing to 22 hypertension (HT) and 32 diabetic (DM) participants. Results: The final program has two sub-programs for HT and DM participants utilizing self-efficacy resources. They share four common components; 1) health screening of exercise risks, 2) weekly 1-hour group self-management education classes, 3) biweekly 1-hour group exercise training and 4) a mid-term individual counseling. Both sub-programs were 12-weeks long but have different education and exercise contents. Participants-rated mean satisfaction scores were 3.47/4 and 3.61/4 for HT and DM program respectively. Attendance rate were 83.1% ~ 92.3% for the classes. Conclusion: The HAHA program developed by multidisciplinary team which reflected participants needs was accepted well by participants evidenced by high attendance rate and perceived satisfaction level.
Purpose: The purpose of this study was to identify effects of a self-management program on self-efficacy and compliance in patients with CHF. Hypothesis: 1) Patients with CHF who are provided with a self-management program will show higher self-efficacy scores than a control group. 2) Patients who are provided with a self-management program will show higher compliance scores than a control group. Method: This study was designed as a nonequivalent non-synchronized pre-posttest control group. There were eight patients in the experimental group, and twelve in the control group. According to NYHA classification, all patients belonged under the classesII to IV. Data were collected using the instruments developed by the researchers. Data were analyzed using descriptive statistics and Mann Whitney U test. Result: There were significant differences in self-efficacy scores and compliance scores between the experimental and control group. Conclusion: By utilizing the program, patients were able to monitor their symptoms routinely, comply with therapeutic regimen, and feel better able to positively influence their disease. Therefore, better compliance means fewer readmissions of patients with CHF.
Objectives: The self-management of chronic diseases is currently receiving much attention. This study applied an extended technology acceptance model (ETAM) to analyze the factors influencing acceptance of a healthcare smartphone application. Methods: Three hundred people living in Seoul and Gyeonggi who used smartphones were quota sampled. A telephone survey was conducted using a structured questionnaire based on ETAM. A path analysis was carried out using the AMOS 17.0 program, and the model was verified. Results: The analysis revealed significant factors of perceived usefulness (.374, p < .001), enjoyment (.210, p < .001), subjective norms (.168, p < .001), perceived costs (.146, p < .001), and innovativeness (.138, p < .001). Cost directly influenced intention to use health applications; self-efficacy and perceived ease of use indirectly affected intention through innovation and perceived usefulness. Conclusions: This study helped to identify the main factors that influence usage intention of smartphone applications. These findings could contribute to promoting the self-management of chronic disease through future health applications using smartphones.
Purpose: This study is conducted to examine the relationship among self management, HbA1c, and perceived health status of type II diabetes patients in community health department. Methods: Data were collected from 93 type II diabetes patients who are registered at five community health center using comprehensive survey, modified self care behavior, blood glucose and HbA1c were assessed by nurses. The data was analyzed by the SPSS (14.0) computer program, and it included descriptive statistics, t-test, ANOVA, and pearson's correlation coefficient. Results: There was significant relationship between perceived health status and HbA1c (r=-.252, p=.015). There was significant relationship between self management and HbA1c (r=-.279, p=.007). Conclusion: In order to control blood glucose in the type II diabetes patients, it is necessary to develop supportive self management programs by considering perceived health status.
Chronic mental-handicapped people are lacking in non-verbal expression such as eye contact, intonation, voice volume, facial expression, and gesture as well as the contents of speech, speak with a monotonous voice, fail to be vivid and clear in voice, and have absence of expression, thereby bringing about difficulty even for social adjustment and about low self-esteem. Accordingly, the purpose of this study was to examine effectiveness for enhancing self-expression and self-esteem by applying music therapy to the chronic mental-handicapped. The subjects were the chronic mental-handicapped who receive rehabilitation service at the community rehabilitation center, and who have over 10 years in the duration of disease. 1he music therapy activity was progressed with totally 14 sessions during 7 weeks with twice a week.
