A Clinical Report of Chronic Diarrhea Treated with GwakHyang-JungGiSan (만성설사환자(慢性泄瀉患者)에 대한 곽향정기산(藿香正氣散) 치험2례 보고)

  • Objective : Diarrhea is defined as an increased frequency of stools(more than three per day), increased liquidity(loose or watery) and decreased consistency or stools. This study was done to examine the efficacy of Gwakhyang-junggisan for two patients who had suffered from chronic diarrhea of unknown origin on examination. At the time of admission, patients complained of diarrhea 4 or 5 times per day accompanied with epigastric discomfort, quadri pain. As of the fourth day of Gwakhyang-junggisan administration, the frequency and liquidity of diarrhea decreased and accompanying symptoms also improved. Method : Gwakhyang-junggisan was used for these patients suffering from chronic diarrhea of origin unknown to western medicine. patients were diagnosed as daejang-sul as bihu disease. Result : Results were taken as two cases of effective treatment of chronic diarrhea with Gwakhyang-junggisan. Conclusion : Result of this study suggest that Gwakhyang-junggisan is all effective treatment for such symptoms of chronic diarrhea as epigastric discomfort, quadri pain.

  • Chronic diarrhea in children is a common problem with numerous causes. Although most of these causes are benign, critical illness may present as chronic diarrhea. In a patient of chronic diarrhea, gastrointestinal infections are the most common causes in children of all ages and antibiotics may cause chronic diarrhea by altering intestinal microflora, which can result in the emergence of bacterial overgrowth. Overgrowth of Clostridium difficile may cause pseudomembranous colitis. We experienced 25-month-old boy who suffered from chronic diarrhea and partially treated with antibiotics irregularly. Colonoscopic findings of this child showed multiple plaques with white to yellowish exudate which adhere to the mucosal surface of a variable length of rectum. Histollogically, each plaque comprised a pseudomembrane of mucous debris, inflammatory cells, and exudate overlying groups of partially disrupted glands. A latex agglutination test on patient's stool was positive to toxin A of Clostridium difficile. He was recovered after stopping the antibiotics he has been prescribed, and being given vancomycin for 2 weeks. We report this case with brief review of literature.

Case Report about Korean and Western Medicine Treatments of a Stroke Patient with Chronic Diarrhea and Clostridium Difficile Associated Diarrhea (뇌졸중 환자에게 발생한 만성 설사 및 Clostridium difficile 연관 설사 한‧양방 병행 치료 치험 1례)

  • The objective of this study is to report the clinical application of Korean and Western medicine on chronic diarrhea and Clostridium difficile associated diarrhea. Clostridium difficile associated diarrhea is usually triggered by using antibiotics. This patient also took some antibiotics and she started to suffer diarrhea diagnosed as chronic and Clostridium difficile associated diarrhea. She had suffered from diarrhea over 10 times per day. We checked changes of numbers, total volume of defecations and stool form. We treated the patient with acupuncture, moxibustion, herbal medicine and metronidazole. After treatment, diarrhea was stopped and the patient defecated normal form stool. According to this study, acupuncture, moxibustion, herbal medicine and metronidazole was effective for 1 case of stroke patient with chronic and Clostridium difficile associated diarrhea.

Antibiotic Sensitivity of Clostridium perfringens Isolated from feces of Siberian Tiger with Chronic Diarrhea (시베리안 호랑이의 만성설사 분변에서 분리한 Clostriduium perfringens의 항생제 감수성)

  • The purpose of this study was to investigate the cause of chronic diarrhea from anaerobic bacteria isolated from Siberian tiger with chronic diarrhea. Major anaerobic bacteria isolated from faces were Clostridium perfringens and their population was $6 {\times} 10^3$ cfu/g feces. Antibiotic sensi- tivity test against Clostridium perfringens was performed using 6 antibiotic drugs including colistin gentamicin, trimethoprim/sulfamethoxazole, amikacin, enofloxacin and penicillin. Clostridium per- fringens were sensitive to amikacin, enrofloxacin and penicillin while they were resistant to colistin. gentamicin and trmetboprim/sulfamethoxazole. And we found that chronic diarrhea of Siberian tiger was treated successfully with penicillin. These results suggested that Clostridium perfringens may bee a cause of chronic diarrhea in Siberian tiger.

  • A neurolytic celiac plexus block produces long-lasting pain relief in upper abdominal cancer patients. Unwanted side effects such as local pain, hypotension, and diarrhea are common but the durations of these side effects are usually transient. Chronic diarrhea induced by a neurolytic celiac plexus block is rarely reported and is considered to be an autonomic neuropathy due to sympathetic denervation. A 73-year-old Klatskin tumor patient developed chronic diarrhea after a neurolytic celiac plexus block and the diarrhea was sustained for 3 months despite the use of conventional antidiarrheal treatments. We report a case of chronic diarrhea that was induced by a neurolytic celiac plexus block.

The effect of Dochetang for the treatment of chronic diarrhea in a 4 months-old-female Korean native calf (만성설사를 보이는 생후 4개월령의 한우 암컷 송아지에서 도체탕의 치료효과)

  • This study was carried out to evaluate the effects of Dochetang for the treatment of chronic diarrhea in a 4 months-old-female Korean native calf. The calf was presented to the Wow Animal Clinic, Iksan with the history of chronic diarrhea for several weeks. The feces test did not reveal the presence of the parasites or microbes causing diarrhea in calf. The blood test was also negative to the virus that causes of diarrhea in calf. Adminstration of parenteral antibiotics resulted in improvement of the condition temporarily but diarrhea was recurred again after 2-3 weeks. Then the calf was treated with Dochetang administered orally once a day in an empty stomach for 15 days. Feces was significantly reduced in moisture on 7 days after initial treatment. On 9 days after initial treatment, the calf had normal appetite and defecation in physiological conditions. Blood samples were collected before administration and on 1, 2 and 3 weeks after initial administration of Dochetang for hematology and biochemistry. A significantly differences were observed in the white blood cell (WBC), mean corpuscular volume (MCV), mean corpuscular hemoglobin (MCH), mean corpuscular hemoglobin concentration (MCHC), albumin (ALB), glutamic pyrubic transaminase (GPT), inorganic phosphorus (IP) and magnesium (Mg), while no significant differences were seen in the red blood cell (RBC), hemoglobin (Hb), hematocrit (HCT), platelet (PLT), glucose (Glu), total protein (T-pro), glutamic oxaloacetic transaminase (GOT), blood urea nitrogen (BUN) and creatine (CRE). This study suggests that Dochetang administration can be a successful alternative therapeutic agent in instead of antibiotics for the treatment of chronic diarrhea in calves.

  • Acrodermatitis enteropathica, an autosomal recessive disease, usually presents with severe acral and circumorificial dermatitis, diarrhea, alopecia, intercurrent bacterial infection during early infancy, and is eventually fatal if left untreated. We report a case of acrodermatitis enteropathica in a 2-month-old male infant who presented with chronic diarrhea not responsive to conventional therapy and developed disseminated intravascular coagulation (DIC). He showed the characteristic eczematoid skin lesions, chronic diarrhea, failure to thrive, and low serum zinc concenturation. $Zn^{2+}$ was administered with dramatic improvement of skin lesions, DIC and diarrhea. He rapidly catched up normal growth and development on continuing zinc supplementation.

A Case of Chronic Intractable Diarrhea with IgA, $IgG_2$ and $IgG_4$ Deficiency (면역글로불린 A 및 $G_2$, $G_4$ 결핍에 동반된 만성 난치성 설사 1례)

  • In most cases, acute diarrhea in childhood heals spontaneously, but it may become the form of chronic diarrhea in immunodeficient children and then cause weight loss, dehydration, malabsorption and malnutrition. The immunodeficient diseases associated with chronic diarrhea include severe combined immunodeficiency syndrome, common variable immunodeficiency, acquired immunodeficiency syndrome, agammaglobulinemia or selective IgA deficiency. IgA deficiency is the most common primary immunodeficiency. Because many IgA deficient individuals seem to have compensated for their deficiency with increased IgM production and various nonimmunologic factors, the incidence of gastrointestinal involvement is not prominent. Some of those with IgA deficiency and recurrent infections have been found to also have IgG subclass deficiency. IgA deficiency with $IgG_2$ and $IgG_4$ subclass deficiency have high susceptability to infection and chronic diarrhea. IgG subclass deficiency, when present, is more likely to be found in association with a partial IgA deficiency rather than complete IgA deficiency. We report a 3-month-old male with intractable diarrhea accompanied by IgA, $IgG_2$, and $IgG_4$ deficiency.

The effect of Sagunjatanghapchangchuljiyutang on chronic diarrhea in rats (사군자탕합창출지유탕(四君子湯合蒼朮地楡湯)이 흰쥐의 만성설사(慢性泄瀉)에 미치는 영향)

  • Objective : To observe Sagunjatanghapchangchuljiyutang in rectifying the digestive dysfunctional spleen deficiency syndrome caused by chronic diarrhea. Methods : Sagunjatanghapchangchuljiyutang was chosen to treat the digestive dysfunctional spleen deficiency syndrome caused by chronic diarrhea. The model was induced by Rhei Radix and castor oil. The effects on the gastric secretion ability(gastrin, secretin, volume of gastric juice, free acidity, total acidity, pepsin activity) and nutritional condition(protein, albumin, total cholesterol, triglyceride, total lipid, BUN, creatinine, RBC, WBC, hemoglobin, hematocrit) were observed in vivo. Results : The group induced chronic diarrhea by Rhei Radix and castor oil had lower gastric secretion ability and nutritional condition was lower than that of the normal group. The figures for group given Sagunjatanghapchangchuljiyutang showed improvement, especially gastrin, total lipid and hematocrit. Conclusion : Sagunjatanghapchangchuljiyutang have an effect on the digestive dysfunctional spleen deficiency syndrome caused by chronic diarrhea. And experimental study syndrome could be proceed.

Case Report of Chronic Diarrhea Treated with Omae-hwan (오매환으로 호전된 만성 설사 환자의 한방 치험 1례)

  • Objectives: This study reported a clinical case of a chronic diarrhea patient who showed improvement with Korean medicine treatment. Methods: A patient with chronic diarrhea was treated with herbal medicine (Omae-hwan and Omae-hwan-ryo), acupuncture, cupping, and moxibustion. Changes in stool form were measured using the Bristol Stool Scale (BSS). The frequency of defecation was checked every day. The Short Form 36-Item (SF-36) was used to compare patients' quality of life before and after treatment. Results: Korean medicine treatment improved the stool form, frequency of defecation, and quality of life. Conclusion: Korean medicine treatment containing Omae-hwan and Omae-hwan-ryo improves clinical symptoms and quality of life.
