The propose of this study was to identify fatigue, pain and coping of pain and to compare the variables between fibromyalgia and chronic arthritis. The sample consisted of 133 patients who visited H university hospital. Data were collected by questionnaire from May 1 to September 30, 1999. Data was analyzed by descriptive statistics, $x^2$-test, pearson correlation coefficient, and ANOVA. As a results, most of all patients felt fatigue and the mean score of the fatigue was above average. The mean score of rheumatoid arthritis and fibromyalgia patients on pain was higher than Osteoarthritis patients, and there was the statistically significant difference among three groups on pain(F=10.63, p=0.00). There was also the statistical difference among three groups on coping of pain(F=4.74, p=0.01). The mean score of rheumatoid arthritis and fibromyalgia patients on coping of pain was higher than Osteoarthritis patients. Fatigue showed positive relationship with pain(r=.262, p=.002), and pain showed positive relationship with coping of pain(r=.319, p=.000). According to this finding, fibromyalgia patients and rheumatoid arthritis patients felt high fatigue and pain, therefore the development of nursing intervention for relieving fatigue and pain would be needed.
This study was conducted to identify the characteristics of pain, self-esteem, perceived health status, and self-efficacy and its influencing factors in patients with chronic arthritis. The data were obtained from 104 patients with chronic arthritis registered in one university hospital in Seoul, from May to August, 2000. For analysing the data, SAS program was used for t-test, ANOVA, Scheffe test, Pearson correlation, and stepwise multiple regression. The results were as follows: 1. The variables which showed significant difference with pain were sex, number of painful joints. 2. The variables which showed significant difference with perceived health status were age, diagnosis. number of painful joints. 3. The variables which showed significant difference with self-efficacy were duration of disease, number of painful joints. quality of sleeping. 4. Moderate negative correlation were observed between pain and perceived health status, self-efficacy, and low positive correlation was observed between self-esteem and perceived health status. Also self-esteem revealed moderate positive correlation with self-efficacy, and moderate positive correlation was observed perceived health status and self-efficacy. 5. The predictors to explain self-efficacy were number of painful joints, self-esteem, duration of disease, duration of exercise and pain. These predictors explained $36.05\%$ of variance of self-efficacy. In conclusion, the nursing intervention to improve self-efficacy for chronic arthritis patients focused not only physiological symptoms such as pain. but also psychosocial factors such as self-esteem.
만성통증을 동반한 류마티즘 환자에서 자율신경계 이상(autonomic nervous system dysfunction)이 동반되기도 한다. 특히 만성통증 환자에서 자율신경계 이상은 국소 통증 강도 증가 및 통증 역치 감소를 발생하여 만성 근골격계 통증에 악영향을 미친다고 확인되었다. 이런 만성 근골격계 통증 환자에 온열-척추 마사지 치료를 실시한 실험에서 통증경감과 자율신경계 회복이 되었다고 보고되고 있다. 그래서 우리는 만성 통증과 자율신경계 이상이 동반된 류마티즘 환자에 온열-척추 마사지 치료를 적용하였고 자율신경기능의 회복과 통증의 감소를 경험한 사례가 있어 보고하고자 한다.
Purpose: Illness intrusiveness (illness induced lifestyle disruption) by symptoms was investigated in 439 patients with rheumatoid arthritis. Method: The cross-sectional and retrospective survey design was used. Fatigue, disability, pain, and depression were considered as symptoms of rheumatoid arthritis. Result: Patients were reported to perceive more illness intrusiveness than other chronic disease patients such as end stage renal disease, bipolar disorder, and peritoneal dialysis patients. They were especially intrusive into work, health, and active recreation domains. Stepwise regression analysis identified four variables counting for 26% of illness intrusiveness variances: fatigue, disability, marital status, and depression. All of the symptoms except pain were found to significantly predict illness intrusiveness. Fatigue explained 16% of illness intrusiveness variances. Conclusion: Fatigue and depression usually overlooked by health professionals must be carefully assessed and managed to reduce perceived illness for rheumatoid arthritis patients.

The data were obtained from 104 patients with chronic arthritis in one university hospital from May to August. 2000. For analysing data. SAS program was used for t-test. ANOVA, Schefte test. Pearson correlation. and stepwise multiple regression. The results were as follows: 1. The variables which influenced self-efficacy were duration of disease. number of painful joint, quality of sleep and alcohol drinking. 2. The variables which influenced fatigue were diagnosis and number of painful joint. 3. The variables which influenced ADL were age. duration of disease. diagnosis. number of painful joint. number of exercise and alcohol drinking. 4. ADL was positive correlation with self-efficacy and negative correlation with fatigue. And self-efficacy was negative correlation with fatigue. 5. The predictors to explain ADL were self-efficacy. number of painful joint. lupus. duration of disease and religion. These predictors explained $66.01\%$ of the activites of daily living. According to these findings. the most significant influencing factor of ADL was self-efficacy. therefore the development of nursing intervention for enhancing self-efficacy would be needed. Also. it is suggested that an exercise program should be recommended as one of useful and appropriate nursing intervention for reducing fatigue and increasing ADL.
Patients with chronic disease have various treatment patterns because it shows a progressive degenerative feature. Especially various physical and emotional problems of the rheumatoid arthritis patients leave them shopping around various types of treatment. According to previous studies, over 70% of patients with arthritis experienced the traditional oriental medicine or folk remedies simultaneously with medical treatment within one year after the onset of disease. The purposes of this study are 1) to compare the patterns of treatment-seeking behaviors between Korean arthritis patients and Americans ; and 2) test two models of treatment-seeking behaviors by path analysis, one for early treatment-seeking behavior model(ETBM) and the other is chronic treatment-seeking behavior model (CTBM) in Korean sample. The interview survey was performed to 133 RA patients with structured questionnaire at out-patient clinic or public health center. Patients characteristics such as age, duration of disease were similar in two countries except higher educational background in Americans. There were no patients using only alternative therapies or no medical treatment in the US. Most of the American patients have chosen both medical treatment and alternative therapy, while the Koreans less than American. In Korea, combined treatment group usually consists of the people who are younger, more educated and higher economic status than the characters of other groups in early or chronic stages. In early stage, they tend to have strong belief of curing from the disease, satisfy the relationship with their physicians and comply with direction of the medical professional. The paths of two models were explained by 70% in ETBM and 33% in CTBM. When the models were modified, almost all paths of the CTBM were the same as the previous one, but direct determinant factor was changed from the relationship with physicians to the lay referral system in chronic model. These two models' explanation powers became 94% and 88%, respectively. The attitude or perception of disease, lay referral system and the relationship with medical personnel are the main determinants of treatment-seeking behaviors.
Purpose: This study was to understand the level of family support, self-efficacy and compliance and to examine the relationships of the above three variables in chronic arthritis. Method: The subjects of this study were 116 patients sampled among the limited population of outpatients and inpatients at Rheumatism Center of the H university hospital in S city. The research instruments used in this study were family support, self efficacy and compliance scale. In data analysis, SPSS 8.0 program was used. Result: 1. Family support and self-efficacy level were above the medium. 2. Compliance level was medificant. Conclusion: Rheumatism patients will be able to live satisfactorily if they gium. 3. The relation among the family support, self efficacy and compliance was signot a good education about efficient training program aimed at compliance.

Asthma is traditionally regarded as a chronic airway disease, and recent literature proves its heterogeneity, based on distinctive clusters or phenotypes of asthma. In defining such asthma clusters, the nature of comorbidity among patients with asthma is poorly understood, by assuming no causal relationship between asthma and other comorbid conditions, including both communicable and noncommunicable diseases. However, emerging evidence suggests that the status of asthma significantly affects the increased susceptibility of the patient to both communicable and noncommunicable diseases. Specifically, the impact of asthma on susceptibility to noncommunicable diseases such as chronic systemic inflammatory diseases (e.g., rheumatoid arthritis), may provide an important insight into asthma as a disease with systemic inflammatory features, a conceptual understanding between asthma and asthma-related comorbidity, and the potential implications on the therapeutic and preventive interventions for patients with asthma. This review discusses the currently under-recognized clinical and immunological phenotypes of asthma; specifically, a higher risk of developing a systemic inflammatory disease such as rheumatoid arthritis and their implications, on the conceptual understanding and management of asthma. Our discussion is divided into three parts: literature summary on the relationship between asthma and the risk of rheumatoid arthritis; potential mechanisms underlying the association; and implications on asthma management and research.
This study was designed to distinguish by the characteristic difference and the degree of symptoms such as fatigue, pain, coping to pain, and efficacy on pain, and to offer descriptive data for nursing intervention for improving coping ability to pain along each characteristics of disease in chronic arthritis and systemic lupus erythematosus. The subjects were 135 outpatients in the hospital for rheumatic disease in H-university, Seoul. The data were collected by structural questionnaire, from April 29 to June 29, 1999. The results were that the fatigue score was high in the OA patients while the RA patients and SLE patients experienced middle range of fatigue score, but which was not statistically different. Although the RA patients felt higher pain than other diseases, they have well coped with their pain than the others. In efficacy on pain the SLE patients had higher score than others but all of the disease showed lower score. No statistically significant difference among the three group was recorded in efficacy on pain. Therefore, pain management in the RA patients was primary nursing intervention because they felt severe pain and have well coped with pain while they had lower pain efficacy score than the others. It is also important that fatigue management and coping strategies on pain for the OA patients and SLE patients are specially supportive in the nursing intervention.
The purpose of this study was to examine the effects of self-help program on patients with chronic arthritis. This study was performed from 13th of August to 29th of October 2002. 30 arthritis patients with an average age of 56 years were participated in the self-help program. At the completion of 6 weeks, 23 patients completed both pretest and posttest, overall dropout rate was 23%. Outcome measures were status of daily living, self-care activities, flexibility, grip strength and depression. Data were analysed by percentage, paired t-test, Pearson's correlation coefficient. There were statistically significant in self-care activities, flexibility, grip strength after the self-help program. However no significant differences were found in daily living and depression. There were significant relationships between daily living and depression. There were significant relationships between right flexibility & left flexibility and right and left grip strength. In conclusion self-help program was proved to be an effective nursing intervention to increase the self-care activities, flexibility and grip strength. These results of this study suggested that the follow-up program after the self-help program should be run to help the physical psychological wellbeing of arthritis patients.
