Spontaneous Chronic Subdural Hematoma in an Adolescent Girl

  • In most cases, subdural hematoma (SDH) is regarded as a complication of head injury and nontraumatic causes are rare. Moreover, spontaneous chronic SDH in child or adolescent is very unusual. Here, we present the case of a healthy 14-year-old girl who was diagnosed as a spontaneous chronic SDH. The patient presented with severe headache following blurring of vision two weeks ago without any history of trivial head injury. Computed tomography and magnetic resonance imaging depicted a chronic SDH. The cause of the hematoma was not established. After performing burr hole drainage of the hematoma, the patient made an uneventful recovery. We explore the potential risk factors and pathophysiology implicated in this condition. Possible pathogenic mechanisms of this unique case are discussed and a review of the pertinent literature is included.

A Case Report of Chronic cough treated by Soshiho-tang based on Shanghanlun Provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 소시호탕(小柴胡湯)투여 후 호전된 만성기침 1례)

  • Objective : The purpose of this paper is to report the improvement of patient with chronic cough treated by herb medication based on Shanghanlun disease pattern identification diagnostic system. Methods : According to 'Disease Pattern Identification Diagnostic System based on Shanghanlun Provisions', the patient diagnosed with Taeyang-byung, number 96 provision, and took Soshiho-tang herb medication 50days. The changing symptom of chronic cough was estimated by LCQ(Leicester Cough Questionnaire) and VAS(Visual Analogue Scale). Results : The LCQ score changed 57 to 111 and VAS changed 10 to 0. Conclusions : On the number 96 provision of Shanghanlun, '或欬' means 'H or K' sound cough. This is the first case report treating the chronic cough with herb medication of shanghanlun in Korea.

Effect of Myofacial Releasing of Abdominal Muscles on Chronic Cough by Oriental Medical Treatment : 2 Cases Report (한방치료를 통한 복근군의 근막이완으로 호전된 만성기침환자 치험 2례)

  • Objectives : We supposed that abdominal muscles are related to chronic cough, because abdominal pressure and respiratory muscles are influenced by abdominal muscle function. We have evaluated the effect of myofacial releasing of abdominal muscles with oriental medical treatment by experimenting two patients suffering from chronic cough. Methods : One patient was treated with acupuncture therapy and muscle stretching exercise for myofacial releasing of abdominal muscle. The other patient was treated with hot pack therapy and abdominal respiration training for myofacial releasing of abdominal muscle. Results and Conclusions : After oriental medical treatment of abdominal muscles in two cases, We figured out that the patients were on the mend. These results suggest that myofacial releasing of abdominal muscles using oriental medical treatment was effective in improving chronic cough.

  • The purpose of this case study is to report the effect of oriental medical treatment on chronic tension-type headache. Despite the patient was treated by cervical nerve block for headache, headache was not improved. We diagnosed chronic tension-type headache according to ICHD-II(The International Classification of Headache Disorders) and Qi deficiency, dampness and phlegm by oriental differential diagnosis of symptom and signs. We applied herbal medicine, acupuncture, moxibustion and cupping therapy for hospitalization(7 days). Oriental medical treatment may have effective results in treating chronic tension-type headache that was not improved by cervical nerve block treatment. But this is a single case study, so further case-series research should be compiled.

The Clinical Observation on Cases of Chronic Paranasal sinusitis (鼻淵에 應用되는 補中益氣湯 加減方의 治驗例)

  • We know paranasal sinusitis is one of the most common disease in the field of otolaryngology. Especially in the case of chronic paranasal sinusitis, patient is sufferred from recurrence. So we prescribed Bojungikgitang(補中益氣湯) to the patients who were suffered from chronic paranasal sinusitis, and we witness some noticeable improvement. Here are the findings of my experience 1. The addition and subtraction temperament drugs of Bojungikgitang(補中益氣湯加減方) are effective to the patient of chronic and feeble paranasal sinusitis by the effect of removing wind(祛風), unnecessary metabolite(祛痰), and draining pus(排膿) and strengthening digestive organs(健碑) and enriching the lung yin(益肺陰) and alleviation of pain(止痛). 2. In the symptom of chronic paranasal siunutitis, nasal obstruction, postnasal drip, sputum, headache were quick to disappear, and dysosmia, internasal polyp, etc got better slower. 3. The most quick improvement appeared within two weeks and most symptoms improved within one month, and disappeared within two months.

Chronic Rhinitis Treated by Dangguisayeokgaosuyusaengang-tang Based on Shanghanlun Provisions (『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 당귀사역가오수유생강탕(當歸四逆加吳茱萸生薑湯) 투여 후 호전된 만성 비염 증례 1례)

  • Objective : This study aimed to report the improvement of a patient with chronic rhinitis treated with herbal medication using the disease pattern identification diagnostic system (DPIDS) based on the Shanghanlun provisions. Methods : According to the Shanghanlun DPIDS and an etymological interpretation of Chinese characters, the patient was diagnosed with Reverting yin disease (provision 352) and took Dangguisayeokgaosuyusaengang-tang herbal medication for 50 days. The change in symptoms of chronic rhinitis were estimated using the Total Nasal Symptom Score (TNSS), the Rhinitis Control Assessment Test (RCAT), and a visual analog scale (VAS). Results : The TNSS score decreased from 8 to 3, the RCAT score decreased from 14 to 24, and the VAS decreased from 10 to 2. Conclusions : The main causes of the disease were 厥 and 內. This is the first case report of describing the treatment of chronic rhinitis with Dangguisayeokgaosuyusaengang-tang.

A Case Study of Chronic Cough of Taeumin throuth the Change Process of Sweating (한출변화과정(汗出變化過程)을 통(通)한 태음인(太陰人) 만성해수(慢性咳嗽) 치험례(治驗例))

  • 1. Objectives In this study, it is experienced that the process of sweating is important in chronic cough of Taeumin. In this case report, the patient has been simproved throuth reaction of sweating improvement. In "Dongyi Soose Bowon", The process of sweating change is described in detail. "Hamkeylhae" is similar to chronic cough in "Dongyi Soose Bowon", it is cough that is hard to spit. The chronic cough of Taeumin is related to the pathology of Liver and Lung, sweating condition is also related to condition of Lung in Sasang constitutional medicine. So the study for the process of sweating is needed. 2. Methods One patient diagnosed as chronic cough was practiced with Sasang constitutional medicine. And We observed sweating change process during treatment. 3. Results As sweating condition improved, chronic cough also decreased, but not improved, not decreased. 4. Conclusions So treating of Taeumin's chronic cough is closely connected with sweating change process.

Chronic subtalar joint instability - One case report - (거골하 관절의 만성 불안정성 - 1예 보고 -)

  • Purpose: Chronic subtalar instability is not common and similar to chronic ankle instability and the incidence and cause chronic subtalar instability are not well known. Recently we have experienced chronic subtalar instability without chronic ankle instability which was treated with modified Brostrom procedures. Materials and Methods: The patient is 46 year old man who has suffered from left ankle sprain for 30 years and recently aggravated more than twice a day. On subtalar stress view, 14 degree angulation of subtalar joint was noted and on anterior drawer view, 8 mm anterior displacement of left ankle was seen. Results: In operation, there was no anterior talofibular ligament abnormility but calcaneofibular ligament loosening was found. Ligament reconstruction was performed using modified Brostrom procedure. At 12 months after operation, the patient complains no pain and no limit of motion and no instability. Conclusion: We experienced chronic subtalar instability without ankle instability treated with modified Brostrom procedures. No instability was found after treatment without complication.

Comparison of Hip Joint Strength between Young Woman Patient with Chronic Low Back Pain with Lumbar Instability and Normal Subjects

  • Objective: The purpose of this study was to compare the hip joint muscle strength of patients with chronic back pain with lumbar instability and normal subjects. Design: A case control study. Methods: Five types of lumbar instability test were conducted on forty young women with chronic low back pain for more than six months, and those who had 3 or more positive tests were selected as subjects. To select chronic back pain patients with lumbar instability group, aberrant movement patterns during lumbar flexion test (FMT), prone instability test (PIT), posterior-anterior mobility test (PAT), passive lumbar extension test (PLE), and pressure bio-feedback (PBF) were applied. In addition, a digital muscle strength meter was used to measure the hip flexor, extensor, adductor, and abductor muscles of chronic low back pain patients with lumbar instability group (n=20) and normal subject group (n=20). Results: As a result of comparing the hip joint muscle strength between the chronic back pain patient group with lumbar instability and the normal group, there were significant differences in the hip extensor, abductor, and adductor muscles (p<0.05). Conclusions: Patients of chronic back pain with lumbar instability were found to have weak hip joint muscle strength. Therefore, this study suggest that include hip joint strength exercise for functional recovery of chronic back pain patients.

Case of Restless Leg Syndrome Patient with Chronic Kidney Failure Treated with Jakyakgamcho-tang (만성 신장병 환자의 하지불안증후군에 대한 작약감초탕 치험 1례)

  • Restless leg syndrome is a nervous system disorder that causes an overpowering urge to move one's legs. Symptoms of restless leg syndrome usually worsen when one tries to fall asleep and can prevent sufficient sleep. Restless leg syndrome is common in patients with chronic kidney failure and can be caused or worsened by chronic kidney failure and hemodialysis. Various medications can treat restless leg syndrome, though the long-term use of medications can cause augmentation and adverse effects. In addition, the use of dopamine agonists is limited in patients with chronic kidney failure. This is because the dose of administration should be controlled for patients with chronic kidney failure, and the treatment effect has not been clearly proven. This study reports the case of a 56-year-old male diagnosed with chronic kidney failure complaining of uncomfortable leg sensations. The patient underwent Korean medicine treatment using Jakyakgamcho-tang. The IRLS, NRS, and AIS scores were evaluation tools during treatment. This study suggested significantly improved symptoms through the individual interventions of Jakyakgamcho-tang in a restless leg syndrome patient with chronic kidney failure.
