자기공명영상(MRI)을 통한 요통환자의 다열근 위축에 대한 정량적 분석 (Quantitative analysis of lumbar multifidus atrophy with low back pain patients using Magnetic Resonance Image)

  • Purpose : The purpose of this study was to compare chronic LBP patients and asymptomatic subjects on measures of multifidus size (cross-sectional area;CSA, thickness) and symmetry (proportional difference of relatively larger side to smaller side). Methods : Data were obtained from 12 asymptomatic subjects without a prior history of LBP (8 females, 4 males), and a retrospective audit was undertaken of records from 12 chronic low back pain patients (8 females, 4 males). CSA and Thickness of the lumbar multifidus muscles was measured from axial T1-weighted magnetic resonance images(MRI). Results : The results of the analysis showed that chronic LBP patients had significantly smaller multifidus CSA and thickness than asymptomatic subjects at L4-5 vertebral levels(p<.05). The asymmetry between sides was seen at L4- L5 vertebral level in patients with chronic low back pain presentations(p<.05). Conclusions : MRI provided a quantitative measure of change between asymptomatic subjects and chronic low back pain patients of multifidus muscle. MRI identified significant differences in cross-sectional area and thickness and helps to evaluate clinically and plan the treatment modalities of LBP.

승마 운동이 만성요통환자의 통증 및 체간 유연성에 미치는 영향 (The Effect of Horse-Riding Exercise on Pain and Body Flexibility for the Patient with Chronic Low Back Pain)

  • Purpose : The purpose of this study was to investigate the effect of horse riding exercise for pain and body flexibility in the patient with chronic low back pain. Method : 26 subjects in H-equestrian were randomly divided two group, ball exercise(BE) group and horse-riding exercise(HE) group. Each group carried out 40 minutes exercise three times a week for 8 weeks. VAS were measured for pain, and evaluation of body flexibility in position with forward and backward. Result : The results were as follows, the pain scales of VAS(visual analog scale) between ball exercise and horse-riding exercise groups in post-test, were significantly different in measures(p<.05). And there were significant in two group after exercise(p<.05). The flexibility scales of body between ball exercise and horse-riding exercise groups in post-test, were significantly different in measures(p<.05). And there were significant in two group after exercise(p<.05). Conclusion : These finding revealed that horse-riding exercise was effective on pain and body flexibility of patient with chronic low back pain so that these exercise can be new altematives exercise for pain and body flexibility in the patient with chronic low back pain.

만성(慢性) 신허요통(腎虛腰痛) 환자의 침치료가 양도락 점수에 미치는 영향 (Effect of Acupuncture Treatment on Ryodoraku Score of the Patients with Chronic Low Back Pain Due to the Kidney Deficiency)

  • Objectives : This study was done for reporting the effect of acupuncture treatment on Ryodoraku score of the patients with chronic low back pain due to the kidney deficiency Methods : We investigated 37 cases of patients with chronic low back pain due to the kidney deficiency, and devided patients into two groups : We specially treated one group by acupuncture treatment, which was not applied to the other group we analyzed of each group the Ryodoraku score(F3) of each group before and after acupuncture treatment and compared it. Results : 1. In acupuncture treatment group compared with baseline, at final, Ryodoraku score(F3) was significantly increased. 2. At final, acupuncture treatment group showed significant increase on Ryodoraku score(F3) score compared with non acupuncture treatment group. Conclusions : It is suggested that Ryodoraku score(F3) should be available for diagnosing kidney deficiency-induced chronic low back pain as a promising diagnostic index and a outcome measurement.

Comparison of the Effects of Joint Mobilization, Gym Ball Exercises, and Breathing Exercises on Flexion Relaxation Phenomenon and Pain in Patients with Chronic Low Back Pain

  • Background: Although various exercises have been performed for patients with chronic low back pain (CLBP), the effects of these exercises including joint mobilization, gym ball exercises, and breathing exercises on flexion relaxation ratio (FRR) have not been compared. Objective: To compare the effects of joint mobilization, gym ball exercises, and breathing exercises on the flexion relaxation phenomenon (FRP) and pain in patients with chronic low back pain. Design: Randomized pretest-posttest control group design. Methods: Thirty-six patients with chronic low back pain who were undergoing rehabilitation at a rehabilitation center were included. The patients were randomly divided into three groups: joint mobilization group (JMG; n=12), gym ball exercise group (GBG; n=12), and breathing exercise group (BEG; n=12). The exercises were performed for 40 minutes a day, twice a week, for a total of 12 weeks. Results: There were no significant differences in FRR between the three groups (P>.05). Significant decreases in the modified visual analog scale (MVAS) scores after intervention between the groups were found (P<.05). The GBG was significantly decreases from the JMG in the MVAS (P<.05). However, there were significant improvements between the pre- and post-interventional findings on FRR and MVAS in the three groups (P<.05). Conclusion: We demonstrated that intervention using joint mobilization, gym ball exercises, and breathing exercises improve FRP and pain in patients with CLBP.

Effect of Proprioceptive Neuromuscular Facilitation Stretching on Pain, Hip Joint Range of Motion, and Functional Disability in Patients with Chronic Low Back Pain

  • Objective: We aimed to identify the effects of proprioceptive neuromuscular facilitation (PNF) stretching on pain, hip range of motion, and functional disability in patients with chronic low back pain. Design: Randomized controlled trial Methods: In total, 45 patients with chronic low back pain were randomly divided into a conventional stretching group (n=22) and a PNF stretching group (n=23). Both interventions were performed three times per week for 6 weeks. Assessments were made using the visual analog scale, Flexion-Abduction-External Rotation test, modified Thomas test, prone hip extension test, and Oswestry disability index before and after the 6-week intervention period. We conducted a paired t-test to compare the within-group findings before and after the intervention. An independent t-test was used to compare the between-group differences. The statistical significance level was set at α=0.05, for all variables. Results: Both groups showed significant improvements in pain, hip range of motion, and functional disability after the intervention (p<0.05). A significant difference was observed in pain, hip range of motion, and functional disability in patients belonging to the PNF stretching group (p<0.05). Conclusions: This study provides evidence that the application of PNF stretching can effectively reduce pain and improve hip range of motion and functional disability in patients with chronic low back pain.

만성요통 환자의 질환명에 따른 흉요추부의 관절가동범위 비교 (Comparison of Ranges of Motion in The Thoracolumbar Region for Clinical Diagnoses of Patients with Chronic Low Back Pain)

  • Purpose : The purpose of this study was to compare differences in thoracolumbar ranges of motions by comparing ratios among 4 type diagnosis for patient with chronic low back pain. Methods : The subjects were 58 chronic low back pain patients. A motion analysis program (Global Postural System) was used after photography for posture measurement. To analyze differences in mobility percentages and ratios of thoracolumbar ranges of motion, one-way ANOVA was used. Results : Regional difference spinal posture and movement were found to diagnosis patients with chronic low back pain. Comparison of thoracolumbar ranges of motion revealed significant differences in the thoracic region during forward-bending of the trunk (p<.05). In the upper thoracic region, the herniated intervertebral lumbar disc (HILD) group was significantly larger than the spinal stenosis/herniated intervertebral lumbar disc (SS/HILD) group (p<.05). In the lower thoracic region, chronic sprains (CS) were significantly greater than in the spinal stenosis (SS) group and in the (SS/HJLD) group (p<.05). Comparative analysis of thoracic/lumbar mobility ratio showed the CS group's ratio during forward bending was largest: 1.66, while the HILD group's ratio was smallest: 84 a significant difference between the two groups (p<.01). Diagnosis was not associated with difference in thoracolumbar backward-bending range of motion (p>.05). Conclusion : Theses results indicate the clinical efficacy of diagnosing for chronic low back pain by evaluating spinal mobility.

만성요통환자의 한발서기 시 체간 안정화 근육의 활성화 변화 (Trunk Stabilization Muscle Activity in Chronic Low Back Pain Patients during One Leg Stance)

  • Purpose : The purpose of this study was to analyze and compare the surface electromyography(EMG) activity of trunk region muscle between normal subjects and chronic low back pain(CLBP) patients during one leg stance. Methods : The subjects were 27 people, were consisted of 12 subjects who don't have low back pain and 15 subjects who have low back pain from 19 to 28 year of age(mean age 22.22). We used surface EMG to evaluate the activity of the Rectus abdominis, External abdominal oblique, Quadratus lumborum, Gluteus medius muscle. We used independent two samples t-test for statistical data. Results : The result of this study showed that the maximal voluntary isometric contraction(%MVIC) ratio of the trunk muscles was higher in patients with CLBP than in normal subject. But there were no statistically significant differences. There were statistically significant differences of the activity of the Quadratus lumborum, Gluteus medius muscle(p<0.05).

The Effects of Spinal Decompression Therapy on Pain and Disability in Patients with Chronic Low Back Pain

  • Purpose: The purpose of this study was to examine the effects of spinal decompression therapy on pain and disability in patients with chronic low back pain. Methods: Twenty patients with chronic low back pain were divided into an experimental group (spinal decompression therapy, n=10) and a control group (conservative physical therapy, n=10). Both groups were treated three times a week over a four-week period. Results: The comparison of between-group changes post-treatment revealed statistically significant lower levels of pain and disability in the experimental group than the control group. The comparison of within each group changes before and after the treatment showed statistically significant declines in pain and disability indexes of both groups. Conclusion: Spinal decompression therapy may be an effective intervention for improving pain and disability in patients with chronic low back pain.

  • Background: The purposed of this study is to examine the static and dynamic plantar foot pressure in chronic low back pain patients and normal adults. Methods: The subjects were divided into a group of 30 patients with chronic low back pain and a control group of 30 healthy persons. While static posture and dynamic posture at comfortable walking speeds, the low back pain group and the control group measured their plantar foot pressure and the trajectory of their center of pressure (COP) using the Matscan(R) system. Independent t-tests were measured to compare differences in plantar foot pressure characteristics between the left side and right side of the low back pain group and the control group. Results: In the comparison of differences in plantar foot pressure characteristics between the left side and right side of the low back pain group and the control group, the anteroposterior (AP) displacement of COP showed significant differences (p<.05). Although the low back pain group and the control group did not show any significant differences in leg length, weight distribution, mediolateral (ML) displacement of COP, static contract area, dynamic contract areas (p>.05), increases in the contract area values were shown in the hind foot in general. Conclusion: In this study, it was shown that patients with chronic low back pain were walking with short AP displacement of the COP as a compensatory action to avoid pain.

만성요통환자의 능동 하지직거상 동작 시 골반조절 방법에 따른 복부 근활성도와 요골반부 회전각도에 미치는 영향 (Effects of the Pelvic Control Method on Abdominal Muscle Activity and Lumbopelvic Rotation Angle during Active Straight Leg Raising in Patients with Chronic Low Back Pain)

  • Purpose: The purpose of the study was to compare the effects of different methods of pelvic control on abdominal muscle activity and lumbopelvic rotation angle during active straight leg raising (ASLR) in patients with chronic back pain. Methods: The study participants were patients with low back pain (n = 30). They were instructed to perform ASLR with pelvic control, ASLR with pelvic belt, and ASLR only. Surface electromyography data were collected from the ipsilateral rectus femoris (IRF), ipsilateral internal oblique (IIO), contralateral external oblique (CEO), and ipsilateral rectus abdominal (IRA) muscles, and lumbopelvic rotation angle was measured using a motion analysis device. Results: Activation of all abdominal muscles was greater in the ASLR with pelvic control group than in the ASLR with pelvic belt and ASLR groups. The lumbopelvic rotation angle was lower in the ASLR with pelvic control group than in the other two groups (p < 0.05). Conclusion: These results suggest that ALSR with pelvic control is an effective means of increasing abdominal muscle activity and reducing unwanted lumbopelvic rotation in patients with chronic low back pain. Controlling the pelvis using the opposite leg is an effective form of ASLR exercise for patients with chronic low back pain.
