The purpose of this study was to assess the clinical outcomes of active periodontal therapy (APT) and supportive periodontal therapy (SPT) through loss of teeth in the Korean patients with aggressive periodontitis. The number of missing and residual teeth, probing pocket depth were examined in 33 patients diagnosed with aggressive periodontitis at the baseline and who had participated in APT and SPT for 3 years or more. A 20 and 3 teeth were lost during the APT and SPT, respectively. The mean tooth loss rate of 0.13 per patient and year. There was no significant statistical difference in clinical improvement between the smoker and non-smoker groups. Within the limitation of this study, APT combined with supportive therapy could decrease the risk of tooth loss in patients suffering from aggressive periodontitis.
Purpose: The purpose of this study is to evaluate the difference between open reduction and internal fixation (ORIF) and intermaxillary fixation (IMF) of mandibular fractures. Methods: A total of one hundred twenty-seven patients who were treated for mandibular fractures at Chosun University Dental Hospital, from January 2008 to December 2010, and analyzed their prognoses based on the use of IMF at the time of fracture reduction. The patients were divided into two groups; the manual reduction group without IMF and IMF group. Results: After reduction of the mandibular fracture, good results were obtained with majority patients. Nonetheless, seven patients (13.0%) in manual reduction method without arch bars or IMF, developed complications after surgery. Three patients underwent IMF due to occlusal instability after surgery, while one patient underwent re-operation. Thus, a significant difference was not observed between the IMF and manual reduction groups. Conclusion: Manual reduction and IMF at mandibular simple fracture could produce good results. In case of mandibular simple fracture, it was recommended with only manual reduction without IMF or IMF during a short period.
Purpose: Orthognathic surgery is required in patients with severe skeletal disharmony and facial asymmetry, which results in functional and esthetic improvement. Recently, bimaxillary surgery has become generalized. Establishment of the occlusal plane among several other factors included in the surgery plan is a major consideration for the diagnosis and treatment plan and it is also an important factor for postoperative stability. Methods: In this study, we assessed postoperative stability of occlusal plane, B-point, and pogonion point on 20 patients who underwent two-jaw surgery in the Chosun Dental Hospital from 2000 to 2007. Preoperative and postoperative states and at least a one year postoperative follow-up were compared. Results: The postsurgical relapse volume of the occlusal plane to the SN plane and the FH plane was $-0.26{\pm}2.8^{\circ}$ and $-0.44{\pm}3.29^{\circ}$, respectively and after two-jaw surgery, the stability of occlusal plane was maintained. The horizontal relapse degree was $0.85{\pm}0.46$ mm and $0.76{\pm}0.48$ mm, respectively, and the vertical relapse degree was $1.16{\pm}0.36$ mm and $1.13{\pm}0.71$ mm of the B point and the Pogonion point at the time after minimal 1 year. Conclusion: The vertical relapse amount was shown to be slightly larger than the horizontal relapse amount.
Purpose: The purpose of this study was to evaluate the effect of collagen matrix with apically positioned flap (APF) on the width of keratinized gingiva, comparing to the results of APF only and APF combined with free gingival graft (FGG) at the second implant surgery. Methods: Nine patients were selected from those who had received treatments at the Department of Periodontics, Chosun University Dental Hospital, Gwangju, Korea. We performed APF, APF combined with FGG, and APF combined with collagen matrix coverage respectively. Clinical evaluation of keratinized gingival was performed by measuring the distance from the gingival crest to the mucogingival junction at the mid-buccal point, using a periodontal probe before and after the surgery. Results: The ratio of an increase was 0.3, 0.6, and 0.6 for the three subjects in the APF cases, 3, 5, and 7 for the three in the APF combined with FGG case, and 1.5, 0.5, and 3 for the three in the APF combined with collagen matrix coverage case. Conclusions: This study suggests that the collagen matrix when used as a soft tissue substitute with the aim of increasing the width of keratinized tissue or mucosa, was as effective and predictable as the FGG.
Objectives : Currently, it is being reported that new health technology (HT) is introduced and spread throughout Korea at a very fast rate. However, the current status of new HT related to dentistry has not been known or studied. Therefore, this study aims to understand the present state and awareness on dentistry-related domestic new HT via surveys and research, and thus apply this as a reference to construct an effective new HT-related system in the field of dentistry. Methods : The assessment status of New Health Technology Assessment (nHTA) committee was searched on the website for any updates on new HT5). The cases were searched in the range between Jan 1 2012 and Jul 31 2016 by submission date and were analyzed by year, division of technology, progress and major operating practitioner. Among those, technologies utilized by dentists were additionally analyzed. The survey result was obtained by combining responses of 25 participants from the workshop for new HT application and 11 members of Korean Association of Oral and Maxillofacial Surgeons (KAOMS). The workshop was co-hosted by KAOMS and Clinical Translational Research Center for Dental Science (CTRC) under Seoul National University Dental Hospital (SNUDH). Results : Most of the respondents to the survey have heard of new health technology but were unaware of the application procedure. 'Safety and effectiveness' was selected the most for things to consider when introducing new HT, followed by 'scientific evidence of the technology.' For new dental HT status, submissions between 2012 and Jul 31 2016 were analyzed by major operating practitioner, including cases where there are multiple practitioners. Yet, only 19 cases (17 %) of dentists were reported. Among 385 cases approved by nHTA, only 2 cases included dentists. Conclusions : Active publicity by the government on new HT and its application procedure, and continued interest and research in the dental and medical community is likely to be required for the development of dentistry.
Because of the prominence of the nose and its central location, it is the most frequently encountered fractures in the face. Yet reports about the nasal bone fractures are virtually rare in the oral and maxillofacial surgical literatures. This is a retrospective study on 19 nasal bone fractures treated in Chosun university hospital Department of Oral & Maxillofacial Surgery from Jan. 1991 to Sep. 1993, under admission to our Dept. and the obtained results were as follows. 1. Of the 240 patients with facial bone fractures, 28 patients suffered nasal fractures(12%) and male to female ratio was 5.3:1. 2. The most frequent cause was traffic accidents(39%)m, the next fall down(36%), first blow(4%). 3. The age frequency was the highest in the fifth decade (32%). 4. Clinical classification of nasal fractures was simple fractures(74%), combined fractures(26%), and single fractures(37%), combined fractures(63%). 5. The most frequently combined site was maxilla(50%). 6. Treatments of nasal fractures were closed reduction(63%), open reduction(5%), and secondary rhinoplasty(32%). 7. The initial treatment time from accident was 1.7 days in single fractures, and 3.5 days in combined fractures, and the period of splint retained was about 8.2 days in single fracture, about 8.7 in combined fracture. 8. It was necessary to treat secondarily in delayed treatment, and all treatment methods showed relatively good prognosis. 9. Closed reduction was treated under local anesthesia, but open reduction & secondary rhinoplasty was treated under general anesthesia except 1 case. 10. The complications were disturbance of swellings 5 cases, ethetic problem 5 cases, epiphora 3 cases, abnormal sensation 6 cases in relation with other fractures.

Osteomyelitis in the oral and maxillofacial area is a relatively uncommon inflammatory disease that occurs due to odontogenic causes such as endodontic infection, facial trauma, insufficient blood supply caused by some medical conditions, and iatrogenic postoperative infections. Among them, the incidence rate of candida osteomyelitis in this area is minimal; therefore, no consensus on the diagnosis, treatment, and prognosis has not been established yet. With the increasing number of immunocompromised elderly patients, candida osteomyelitis of the jaw is expected to become more prevalent. In this case report, we present an 81-year-old male patient with candida osteomyelitis of the jaw, including the maxillary and ethmoid sinuses.

Objective : This prospective study sought to verify the stability of three types of short implants measuring 7mm or less. Materials and methods : Implants measuring 7mm or less were placed in patients at multicenter dental clinics in Korea, China, Taiwan, and Singapore. Initial stability, intraoperative and postoperative complications, crestal bone loss, and survival rate of the implant were prospectively evaluated. Results : The primary stability of a 6-mm implant was lower than that of a 7-mm implant. The marginal bone loss of short implants measuring less than 7mm was minimal. Complications such as wound dehiscence, implant mobility, and peri-implant mucositis developed, and these were associated with initial implant failure. The short-term survival rate of 6-mm implant was 93.7%, and that of 7-mm implant, 96.6%. Conclusion : Short implant for the mandible with insufficient height for the residual ridge can be selectively used. Poor primary stability and wound dehiscence can cause osseointegration failure and alveolar bone loss.
Objectives: The present study is aimed at comprehending the organizational setup of clinical dental hygienists and to establish an organizational setup that fits their roles. Methods: The survey data of 776 clinical dental hygienists who understood the purpose of the study and agreed to write the questionnaire was utilized. Statistical analysis was performed using SPSS Ver. 20.0 program and the significance level was 0.05. Results: The presence of an organizational setup in dental hygienists was surveyed as being higher, with university dental hospital (89.86%) showing the highest. However, the presence of position terms was highest in dental hospitals with 76 subjects (38.78%). Standards for positions most commonly followed hospital regulations in the case of general hospitals (48.42%), whereas they were often based on service period in the case of dental hospitals (48.90%). Salary standards were most commonly determined by service period in all institutions. The job satisfaction of dental hygienists was significantly different according to social status and financial satisfaction depended on having organizational hierarchy for those who work in university dental hospitals. Conclusions: The results of the present study show that dental hygienists who work in organizational setups mostly enjoy high satisfaction in terms of job satisfaction. In other words, the study shows that systemizing the organizational setup of dental hygienists within dental institutions is required, and additional research to achieve efficient human resource management within the organization is necessary as well as institutional utilization of the results of the present study.
Purpose: Temporomandibular disorder (TMD) is one of the most common diseases causing chronic orofacial pain. Prolotherapy is called 'regenerative injection therapy' or 'growth factor stimulation injection', and it induces the functional reactivation of tissues such as ligaments and tendons. The aim of this study is to evaluate the efficacy of ultrasound-guided prolotherapy with platelet-rich plasma (PRP) for the patients who had the TMD symptoms, especially in temporomandibular joint (TMJ) pain, restricted mouth opening, and TMJ sound. Methods: Twenty-seven patients visited Chosun University Dental Hospital with the symptoms of pain, restricted mouth opening, and TMJ sound were included in this study. When the patients visited the hospital, we measured; the degree of pain, range of mouth opening (ROM), and TMJ sound, and grouped them according to their chief complaints. TMJ pain and ROM were measured both at the first visit and the fourth week after the PRP injection, and also evaluated the impact of the treatment on their daily activities. Results: After the treatment, the patients in the TMJ pain group showed some improvement (visual analogue scale [VAS] 5.6 to 3.6), and the patients in the restricted mouth opening group exhibited increased ROM (26 mm to 32 mm; p<0.05). On the other hand, the patients in the TMJ sound group had no improvement. Conclusions: PRP prolotherapy could be effective for the treatment of TMJ pain and restricted mouth opening. However, further studies are still necessary in terms of TMJ sound and longterm effect of PRP prolotherapy.
