Summary: We report the first hepatic adverse effect of tosufloxacin tosylate in a muscle invasive bladder cancer patient with normal liver functions and with scheduling to undergo a surgical operation for a neobladder. Tosufloxacin tosylate 150 mg was administered to a 57-year-old man who maintained transurethral resection of bladder tumor (TUR-BT) postoperative multiple medications. His labs presented significant increases in alanine amino transferase (ALT) and aspartate amino transferase (AST) levels with 2-week compliance of 150 mg tablet three times a day. After discontinuing tosufloxacin tosylate, the levels slowly decreased and completely returned to normal ranges without any intervention in a few weeks. The Naranjo Causality Algorithm indicates a probable relationship between increased ALT and tosufloxacin. The patient was to have the second surgical operation as scheduled after getting normal range of ATL level. Therefore, tosufloxacin should be avoided in patients at risk for having liver dysfunctions or diseases if the patients have a schedule for any operation. Background: Tosufloxacin tosylate has been shown to have favorable benefits as an antibiotic. Tosufloxacin tosylate may be considered to have the adverse effects such as nauseas, vomiting, diarrhea, abdominal pain, stomatitis, tendonitis, tendon rupture, headache, dizziness, drowsiness, insomnia, weakness, agitation including hemolysis in the event of glucose-6-phosphate dehydrogenase deficiency as other fluoroquinolones. More severe adverse reactions of tosufloxacin tosylate over the above common adverse effects of fluoroquinolones were thrombocytopenia and nephritis. It also is not well known that tosufloxacin can cause hepatic problem. Here the study reports the first hepatic reaction from tosufloxacin and might arouse heath care providers' attention to appropriate drug choice for patients.
Special track systems used to divert a train to other directions or other tracks are generally called 'railway turnout'. A traditional turnout system consists of steel rails, switches, crossings, steel plates, fasteners, screw spikes, timber bearers, ballast and formation. The wheel rail contact over the crossing transfer zone has a dip-like shape and can often cause detrimental impact loads on the railway track and its components. The large impact also emits disturbing noises (either impact or ground-borne noise) to railway neighbors. In a brown-field railway track where an existing aged infrastructure requires renewal or maintenance, some physical constraints and construction complexities may dominate the choice of track forms or certain components. With the difficulty to seek for high-quality timbers with dimensional stability, a methodology to replace aged timber bearers in harsh dynamic environments is to adopt an alternative material that could mimic responses and characteristics of timber in both static and dynamic loading conditions. A critical review has suggested an application of an alternative material called fibre-reinforced foamed urethane (FFU). The full-scale capacity design makes use of its comparable engineering characteristics to timber, high-impact attenuation, high damping property, and a longer service life. A field trial to investigate in-situ behaviours of a turnout grillage system using an alternative material, 'fibre-reinforced foamed urethane (FFU)' bearers, has been carried out at a complex turnout junction under heavy mixed traffics at Hornsby, New South Wales, Australia. The turnout junction was renewed using the FFU bearers altogether with new special track components. Influences of the FFU bearers on track geometry (recorded by track inspection vehicle 'AK Car'), track settlement (based on survey data), track dynamics, and acoustic characteristics have been measured. Operational train pass-by measurements have been analysed to evaluate the effectiveness of the replacement methodology. Comparative studies show that the use of FFU bearers generates higher rail and sleeper accelerations but the damping capacity of the FFU help suppress vibration transferring onto other track components. The survey data analysis suggests a small vertical settlement and negligible lateral movement of the turnout system. The static and dynamic behaviours of FFU bearers appear to equate that of natural timber but its service life is superior.

Background: Surgical excision is the treatment of choice for feline intracranial meningioma. Objectives: To report clinical findings, complications, and outcomes following surgery for feline intracranial meningioma. Methods: Medical records (01/2000-01/2017) of cats that underwent surgical excision of an intracranial meningioma at our institution were reviewed. Patient data included signalment, clinical signs, surgical technique, complications, histopathologic diagnosis, survival time, and owners' answers to a satisfaction questionnaire. Survival was assessed using the Kaplan-Meier method and log-rank test. Results: Twenty-six cats were included in this study. The exact cause of death was known in 17 cases and was not related to meningioma in 9/17 cases. Overall median survival time was 881 days (95% confidence interval 518; 1248). The age of the cat did not influence survival (p = 0.94) or the occurrence of complications (p = 0.051). Complications occurred in 13/24 cats, including dramatic complications in 4/24 cats. Most complications appeared in the first 24 hours post-surgery (12/13). Males had more postoperative complications (p = 0.042), including more seizures (p = 0.016). Cats with cranioplasty had fewer complications (p = 0.021). Clinical recurrence was confirmed in 3 out of 17 cats. Recurrence-free survival time was 826 days. Most owners (12/14) were satisfied with the outcome. Conclusions: Surgical treatment of intracranial meningioma in cats was associated with a long median survival time but also with a high rate of minor and major postoperative complications, including early postoperative seizures. Cranioplasty may reduce complications. Age at the time of surgery had no effect on outcomes.
Background: It is widely acknowledged that single bedrooms have many potential advantages compared to multiple bedrooms. However, Korea has a reimbursement system that patients have to pay the additional fee if they will use single bedroom or pay-bed (1-3 bedroom). There is little research on patients' bed selection and relationship between patient satisfaction and bed type in the Korean setting. Methods: Using the 2017 Korea Health Panel (KHP) Survey data, we modified bed type by two dichotomous variables: single bedroom vs. multiple bedroom (2+ bedrooms) and pay-bed (1-3 bedroom) vs. reimbursed bed (4+ bedroom). Multivariate logistic regression is performed to determine the factors affecting the patient's choice of room types. Multivariate regression analysis was conducted to examine how hospital room types are affecting patient satisfaction. Results: Single room and pay-bed (1-3 bedroom) use was influenced by patient age (19- years old), male, a person at work, hospitalizing in a clinic, and birth. After controlling variables of the behavioral model of medical utilization, the association between patient satisfaction and use of single bedroom & pay-bed (1-3 bedroom). Cause of hospitalization, major treatment, and recognition of unnecessary care are statistically significant variables on patient satisfaction. Conclusion: Although the single room is about the standard for newly built hospitals in western countries, it remains unclear that single room or pay-bed (1-3 bedroom) has positive effects on patient's outcomes and satisfaction. In this empirical study, the difference in patient satisfaction by bedroom type was not noticeable. In terms of bed management by hospital staff, securing patient credibility in hospital care is an important task. Patients' perception of whether medical staffs have encouraged unnecessary treatment or tests has a significant impact on patient's satisfaction.

Accidents on offshore oil and gas platforms (OOGPs) usually cause serious fatalities and financial losses considering demanding environment platforms locate and complex topsides structure platforms own. Evacuation planning on platforms is usually challenging. The computational tool is a good choice to plan evacuation by emergency simulation. However, the complex structure of platforms and varied evacuation behaviors usually weaken the advantages of computational simulation. Therefore, this study developed a simulation model for OOGPs to evaluate different evacuation plans to improve evacuation performance by integrating building information modeling (BIM) and agent-based model (ABM). The developed model consists of four parts: evacuation model input, simulation environment modeling, agent definition, and simulation and comparison. Necessary platform information is extracted from BIM and then used to model simulation environment by integrating matrix model and network model. During agent definition, in addition to basic characteristics, environment sensing and dynamic escape path planning functions are also developed to improve simulation performance. An example OOGP BIM topsides with different emergent scenarios is used to illustrate the developed model. The results showed that the developed model can well simulate evacuation on OOGPs and improve evacuation performance. The developed model was also suggested to be applied to other industries such as the architecture, engineering, and construction industry.
The present study focused on frustration emotion investigated the variables evoking frustration and the process in which sense of frustration would be overcome, and explored the principle of aesthetics the consumers under the frustration might prefer in choosing goods. The results of this study are as follows. First, consumers usually felt frustration rather than anger when it became obvious that their objectives could not be met and that they failed to achieve their objectives due to uncontrollable cause. Second, consumers who felt frustration wanted to receive emotional support and comfort from close acquaintances rather than to cope actively with the unsuccessful situation by renewed challenges. Third, consumers, once frustrated, preferred harmony principle rather than emphasis principle in terms of esthetics in choosing design of goods. Therefore, marketing managers are advised to approach to the consumers who felt frustration on the basis of harmony principle in design of goods.
We estimate the power spectra of the cosmic microwave background radiation (CMB) temperature anisotropy in localized regions of the sky using the Wilkinson Microwave Anisotropy Probe (WMAP) 7-year data. We find that the north and south hat regions at high Galactic latitude ($|b|{\geq}30^{\circ}C$) show an anomaly in the power spectrum amplitude around the third peak, which is statistically significant up to 3. We try to explain the cause of the observed anomaly by analyzing the low Galactic latitude ($|b|$ < $30^{\circ}C$) regions where the galaxy contamination is expected to be stronger, and the regions weakly or strongly dominated byWMAP instrument noise. We also consider the possible effect of unresolved radio point sources. We find another but less statistically significant anomaly in the low Galactic latitude north and south regions whose behavior is opposite to the one at high latitude. Our analysis shows that the observed north-south anomaly at high latitude becomes weaker on regions with high number of observations (weak instrument noise), suggesting that the anomaly is significant at sky regions that are dominated by the WMAP instrument noise. We have checked that the observed north-south anomaly has weak dependences on the bin-width used in the power spectrum estimation, and on the Galactic latitude cut. We also discuss the possibility that the detected anomaly may hinge on the particular choice of the multipole bin around the third peak. We anticipate that the issue of whether or not the anomaly is intrinsic one or due to WMAP instrument noise will be resolved by the forthcoming Planck data.
During last few decades dietary guidelines for the weight management mainly have focused on a low-fat, high carbohydrate diet. Carbohydrate was supposed to be low-dense, highly satiating as well as it affects little on the lipogenesis. Although low-fat diet has significant weight-reducing effect, the loss was modest and usually regained after cessation of the diet. Furthermore, low-fat, modest-carbohydrate diet did not impact on the ever increasing rates of overweight and obesity. Alternative approaches include low-carbohydrate diet, high-carbohydrate diet and low-glycemic index diet. Although none of above mentioned diet have sufficient evidence for standard weight management diet, short-term efficacy and safety are being approved continuously. Low-carbohydrate diet contains less than 45% of carbohydrate in daily energy consumption, it is claimed to have more satiating effect and to improve metabolism. However, low compliance due to the limitation of food choice should be considered on prescribing the diet. High-carbohydrate which contains 90% of carbohydrate in total daily energy consumption, is effective in providing satiety and lowering total calorie intake and cholesterol. On the other hand, nutritional unbalance should be took into account. Low-glycemic index diet is based on the theory that contemporary diet contains significantly less fiber and unrefined carbohydrate, therefore insulin secretion is disturbed. Because low glycemic index food slowly increase blood glucose and insulin level, it induces much satiating effect and may decrease calorie ultimate intake. However, poor standardization of glycemic index is one of the main obstacle for the diet to be applied in the clinic. Meanwhile, high fructose food and beverage should be discouraged because it has little satiating effect and may cause insulin resistance. High fiber food is another recommendation for healthy, lean diet.

Recently, purchasing expensive branded apparel became prevalent among adolescents as they consume conspicuously. People in their adolescence strongly tend to follow peers and are very likely to pay attention to how they wear look to others. Already a good deal of prior research about young people's conspicuous consumption have focused on variables related to these behaviors. The purpose of this study is to examine demographic characteristics of adolescents who show a conspicuous consumption and factors affecting this consumption which is related to peer relations. And we will examine how negative emotions arisen out of relations influence teenagers' conspicuous consumption of branded apparels ultimately. Being out of favor from peers has an effect on anxiety which is negative emotion in association with peer relations. According to the survey, adolescents did conspicuous consumption following others or following trends to get rid of anxiety. Also, if appears to be acting on clothing consumption as a means not to be marginalized. All these findings are expected to be useful information to adolescents to have a better habit of consumption life, and also anticipated to be valuable information for consumer behavior education. The cause of youth's show-off behavior, which is still incomplete in identity formation and immature in choice and judgement, will have to be analyzed and countermeasures taken against it.
