Promoting Mental Health Literacy at Schools in South Korea

  • The onset of many lifelong mental illnesses is during childhood and adolescence. There has been an increase in these conditions among children and adolescents especially, during the COVID-19 pandemic. It is essential to promote mental health literacy (MHL) as a preventive and universal intervention for children and adolescents. Positive mental health status in adolescence is related to an increased level of MHL, and various MHL programs at schools have been reported to be effective for adolescent students worldwide. Recently, MHL programs have been developed in South Korea to be used by schoolteachers. There is a need for active dissemination and development of future programs. For continuous and effective education, it is desirable to include MHL education in regular school curricula.

  • In this review, we provide information about the etiology, risk factors, and clinical presentations of maltreatment to help clinicians better understand child abuse and neglect. Child maltreatment is a major global health challenge that can result in severe consequences. Abused and neglected children are likely to develop psychiatric disorders, such as major depressive disorder, anxiety disorder, and posttraumatic stress disorder. Understanding child maltreatment is expected to prevent and reduce victimization in children, adolescents, and their families.

  • Digital therapeutics based on software, such as artificial intelligence, virtual reality, games, and smartphone applications, are in the spotlight as new therapeutic alternatives in child and adolescent psychiatry. It draws attention to overcoming conventional therapeutics' limitations, such as toxicity, cost, and accessibility, and encourages patients to participate in the treatment attractively. The growth potential of the digital therapeutics market for psychiatric disorders in children and adolescents in Korea and abroad has been highlighted. Clinical studies and Food and Drug Administration approvals for digital therapeutics have increased, and cases approved by the Ministry of Food and Drug Safety have emerged in Korea. As seen above, digital transformation in child and adolescent psychiatry will change treatment paradigms significantly. Therefore, as this new field has just begun to emerge, it is necessary to verify the effectiveness and scope of the application of digital therapeutics and consider preparing a compensation system and institutional arrangements. Accordingly, this study analyzed the development trends and application status of digital therapeutics in children and adolescents and presented limitations and development directions from the perspective of application in healthcare. Further, the study is expected to identify the utility and limitations of digital therapeutics for children and adolescents and establish effective application measures.

초등학교 특수학급아동의 임상적 진단 및 감정 행동특성 연구 (Clinical Diagnosis and Emotional Behavioral Characteristics Study of Children in a Special Education Class in Korean Elementary School)

  • Objectives : The special class has been made, bringing rapid increase quantitatively. The authors carried out the child psychiatric interview and evaluation for 9 special-classed children in Asan city to find out clinical diagnosis and emotional/behavioral characteristics. Methods : The child psychiatrists evaluated special class children by DSM-IV and K-SADS-PL. Tools for the evaluation were Child Behavior Checklist- Korean version, Korean Personality Inventory for Children, Children's Depression Inventory, Abbreviated Conners Parent-Teacher Rating Scale-Revised, State-Trait Anxiety Inventory for Children, Vineland Social Maturity Scale, Wechsler Intelligence Scale for Children-III, and Childhood Autism Rating Scale. Results : Ultimately 53 children, consisting of 35 boys(67.9%) and 18 girls(32.1%), participated, and the average age was $10.5{\pm}1.3$ years old. Their measure of Vineland Social Maturity Scale was $78.7{\pm}20.0$, Childhood Autism Rating Scales was $25.4{\pm}9.0$, Child Depression Inventory was $22.2{\pm}5.2$, State-Trait Anxiety Inventory for Children was $35.2{\pm}8.2/36.5{\pm}6.2$, and Abbreviated Conners Parent-Teacher Rating Scale was $11.0{\pm}4.6$. In the clinical diagnosis evaluation, the prevalence rate of learning disorder was decreased compared to early research, ADHD had been newly appeared and depression disorder and anxiety disorder had been increased. Conclusion : This result suggests that a lot of children in a special class have complex emotional and behavioral problems in addition to educational problems.

A Study of the Reliability and Validity of the Korean Version of DSM-5 Symptom Measure-Inattention and Anger for Parent and Guardian of Child Age 6 to 17

  • Objectives: This study was conducted to investigate the reliability and validity of the Korean version of the DSM-5 Level 2 Cross-Cutting Symptom Measure-inattention [Swanson, Nolan and Pelham, version IV (SNAP-IV)] and anger [Patient-Reported Outcome Measurement Information System (PROMIS) Anger] for parents and guardians of children aged 6-17 years. Methods: We included 104 children and adolescents diagnosed with attention-deficit/hyperactivity disorder (ADHD), ADHD with anxiety and depression, depressive disorder, anxiety disorder, and tic disorder with somatic symptoms (ADHD=41, depression=9, anxiety=14, ADHD+anxious depression=11, tic+somatic symptoms=29). Their ages ranged from 8 years to 15 years. The participants' mothers completed the SNAP-IV, PROMIS Anger scale, Korean version of the IOWA Conners Rating Scale (K-IOWA), and Korean ADHD Rating Scale (K-ARS) so that the reliability and validity of the SNAP-IV and PROMIS Anger scales, which are DSM-5 scales for assessing inattention and anger of children and adolescents, could be examined. Results: The reliability coefficient of SNAP-IV (Cronbach's α) was 0.94. The correlation coefficients between SNAP-IV, K-IOWA inattention, and K-ARS inattention scores ranged from 0.73 to 0.86. The mean SNAP-IV scores of the ADHD and the ADHD+anxious depression groups were significantly higher than those of the anxiety and the tic+somatic symptoms groups. The reliability coefficient of the PROMIS Anger was 0.91. The correlation coefficient between PROMIS Anger and K-IOWA oppositional/defiant scores was 0.75. The PROMIS Anger mean score of the ADHD+anxious depression group tended to be higher than that of the other groups. Conclusion: These results suggest that the Korean version of the DSM-5 Level 2 Cross-Cutting Symptom Measure-inattention and anger for parent and guardian of child age 6-17 might be a reliable and valid test and may be useful for screening children and adolescents with ADHD.

소아기 붕괴성 장애 1례 (A CASE OF THE CHILDHOOD DISINTEGRATIVE DISORDER)

  • 1908년에 Heller는 일정한 기간 동안 정상적인 발달 과정을 밟다가 발달적 퇴행을 하는 소아를 관찰한 후 '유아기 치매'라고 명명하여 보고하였다. 그러나 후속 연구가 부족하여 정확한 개념 정립이 안된 상태에서 그 명칭도 유아기 치매, Heller 증후군, 붕괴성 정신병, 붕괴성 장애, 소아기 붕괴성 장애 등 여러가지로 사용되어 왔다. 최근에 DSM-IV및 ICD-10에 의해 소아기 붕괴성 장애라는 독립된 진단명으로 자리잡게 되면서 이에 대한 관심이 증가하고 있지만, 지금까지 이에 대한 연구가 부족하여 그 개념, 특성, 자폐증 및 유사한 다른 병들과의 관련성, 그리고 병의 치료 및 예후 등에 대한 지식이 충분하지 못한 상태다. 따라서 국내에서는 아직 이 병에 대한공식적인 보고가 없는 실정이다. 본 저자들은 소아기 붕괴성 장애로 생각되는 1례를 경험하였기에 이것을 문헌 고찰과 함께 보고하고자 한다.

주의력결핍 과잉행동장애 부모에서 MMPI 특성 (MMPI Characteristics of Parents of Children with ADHD)

  • Objectives: The current study investigated the personality characteristics of parents of children with attention-deficit hyperactivity disorder (ADHD) using the Minnesota Multiphasic Personality Inventory (MMPI), which is commonly used in clinical medicine. Methods: Participants were 117 parents of children with ADHD (96 boys and 21 girls) and 77 parents of comparison children (50 boys and 27 girls), who completed the Korean version of the MMPI. Results: The MMPI scores of the fathers of ADHD children were significantly higher on the Psychopathic Deviate (Pd), Masculinity-Femininity (Mf), Paranoia (Pa), and Psychasthenia (Pt) scales than the comparison group's were. In addition, the mothers of ADHD children had higher MMPI scores on the traits of Hypochondriasis (Hs), Psychopathic Deviate (Pd), and Schizophrenia (Sc) than the comparisong roup had, but were not significantly higher. Conclusion: The fathers of ADHD children might be antisocial, irresolute, passive, paranoid, and anxious. In addition, mothers of ADHD children might have hypochondriacal, antisocial, and/or psychological confusional traits, but these were not be significantly high. These results suggest that the psychopathology of parents of ADHD children might correlate with their children's ADHD.

아동용 외상관련 증상 평가척도의 효용성 - 성폭력 피해 아동과 일반 아동의 비교 - (The Effectiveness of Traumatic Symptom Checklist for Children(TSCC) - Comparisons of Sexually Abused Children and Nonabused Normal Children -)

  • Objectives : The research is designed in order to know how much Traumatic Symptom Checklist fur Children(TSCC) is effective in the case of child abuses, especially in the estimation of the psychopathology of child sexual abuse. Methods : A sample includes a group of thirty children of 4th, 5th and 6th grade whose sexual abuse have confirmed and also a group of thirty four normal kids who have no abuse. Along with TSCC, Children's Depression Inventory (CDI) and Revised Children's Manifest Anxiety Scale (RCMAS) were used to children and child behavior checklist (K-CBCL), sexual behaviors (CSBI), and Kiddie-Schedule for Affective Disorder and Schizophrenia-Present and Lifetime version (K-SADS-PL) record sheets were used to kids' parents. Results : In case of the sexual abuse group, the most meaningful difference from the analyzed result is shown at sexual concerns scale which is one of the clinical scale in TSCC (p<.01). Also, the sexual abuse group has the meaningful higher score than the general group's one in the stress scale after the other anxiety, depression, anger and dissociation (p<.01). Conclusion : The scale of TSCC has relatively shown the characteristic symptom and severity which were gained from the children who have experienced trauma, especially sexual abuse.

뇌전증 아동·청소년 부모의 치료이행 개념분석 (Concept Analysis of Parents' Treatment Adherence for an Epileptic Child or Adolescent)

  • Purpose: This concept analysis was conducted to clarify 'parents' treatment adherence for an epileptic child or adolescent'. Methods: The analysis used a hybrid model comprising three phases: theoretical phase, fieldwork phase, and integration phase. In the theoretical phase, fifty studies were reviewed. Interviews with four parents of epileptic children or adolescents were conducted during the fieldwork phase. In the integration phase, the results derived from prior phases were synthesized and clarified. All phases were performed cyclically. Results: The concept, 'parents' treatment adherence for an epileptic child or adolescent' was defined as parents' voluntary and goal-directed behavior towards the epilepsy treatment for their children: a collaborative decision-making process with health-care providers, establishing a support system, adaptability to the treatment plans, and appraisals of the child's health condition. Conclusion: This achievement is thought to contribute to improving the accuracy and validity of the concept measurement. It has implications for additional research on how the concept 'treatment adherence' differs in diverse health problems and other population groups than parents of children and adolescents with epilepsy.
