Factors Associated with Removal of Impactted Fishbone in Children, Suspected Ingestion

  • Purpose: The management and clinical course in pediatric patients who had ingested foreign body were investigated retrospectively to evaluate the frequency and factor associated with successful removal of fishbone foreign body. Methods: Based on the medical records of patients younger than 15 years old who visited emergency room because of foreign body ingestion from January 1999 to December 2012, the authors reviewed clinical characteristics including type of ingested foreign bodies, time to visits, managements and complications. Results: Fishbone (50.1%) was the most common ingested foreign body in children. Among 416 patients with ingested fishbone, 245 (58.9%) were identified and removed using laryngoscope, rigid or flexible endoscope from pharynx or upper esophagus by otolaryngologists and pediatric gastroenterologists. The kind of ingested fish bone in children was diverse. The mean age of identified and removed fishbone group was 7.39 years old, and higher than that of unidentified fishbone group (5.81 years old, p<0.001). Identified and removed fishbone group had shorter time until hospital visit than the unidentified fishbone group (2.03 vs. 6.47 hours, p<0.001). No complication due to ingested fishbone or procedure occurred. Conclusion: Older age and shorter time from accident to hospital visit were the different factors between success and failure on removal of ingested fish bone in children.

은교산가미방(銀翹散加味方)으로 치료한 급성 세기관지염 3례 (Three Cases Report of Acute Bronchiolitis Children Treated by Eunkyosan-gamibang)

  • Objectives Acute Bronchiolitis is an acute lower respiratory disease that the most often seen in children. The purpose of this study is to report three cases of acute Bronchiolitis children treated by oriental medicine. Methods The subjects are children diagnosed as having an acute Bronchiolitis. We treated three children with herbal medicine(Eunkyosan-gamibang) Results After treatment, the symptoms(cough, dyspnea, wheezing) of Acute Bronchiolitis were relieved. Conclusion This study shows that oriental medicine can be an effective treatment for Acute Bronchiolitis. Further study is needed with more cases of treatment.

  • Immunonutrition is the provision of specific nutrients that modulate the activity of the immune system. Several nutrients including arginine, glutamine, nucleotides, omega-3 fatty acids, vitamins, minerals, and prebiotics can be provided to enhance immunity in critically ill patients. Supplying immunonutrition to critically-ill children, better prognosis and shortening of the hospital stay are expected from its immuno-modulating effects. Therefore, immune-enhancing enteral and parenteral formulas can be recommended in children with severe illness.

Anemia in children with chronic kidney disease

  • Chronic kidney disease (CKD) causes numerous changes that destabilize homeostasis, of which anemia is one of its important complications. Anemia significantly reduces the quality of life in children with CKD and plays a crucial role in the progression of cardiovascular disease such as left ventricular hypertrophy, a major cause of mortality in those with advanced CKD. The treatment of anemia is a pivotal factor in reducing morbidity and mortality rates in children with CKD, representing one of the methods for enhancing patients' quality of life.

Experiences with Endoscopic Interventions for Variceal Bleeding in Children with Portal Hypertension: A Single Center Study

  • Purpose: The aim of this study was to compare the efficacy and safety of band ligation and injection sclerotherapy in the endoscopic treatment of children with variceal bleeding. Methods: The study population included 55 children, all of whom were treated at the time of endoscopic diagnosis of esophageal varices at Asan Medical Center, Seoul, Korea, between January 1994 and January 2011. The primary outcomes included initial success rates and duration of hemostasis after endoscopic management (band ligation vs. injectionsclerotherapy). Results: The mean age was $6.7{\pm}5.2$ years and the mean follow-up time was $5.4{\pm}3.7$ years. The most common cause of esophageal varices was biliary atresia. Of 55 children with acute variceal bleeding, 39 had band ligation and 16 had injection sclerotherapy. No differences between groups were observed in terms of the size, location, and presence of red color sign. The success rates of band ligation and sclerotherapy in the control of acute bleeding episodes were 89.7% and 87.5%. The mean duration of hemostasis after endoscopic intervention was $13.2{\pm}25.1$ months. After one year, 19 of 39 patients (48.7%) treated with band ligation and 7 of 16 patients (43.8%) with injection sclerotherapy had experienced rebleeding episodes. Complications after the procedures were observed in 10.3% and 18.8% of children treated with band ligation and injection sclerotherapy. Conclusion: The results of our current study suggest that band ligation and injection sclerotherapy are equally efficient treatments for the control of acute variceal bleeding and prevention of rebleeding.

공공기관 의무기록부에서 관찰된 기아(棄兒)에 관한 고찰 (A Study on Abandoned Children through the Analysis of Medical Records in a Public Hospital)

  • It is the purpose of this study to provide the basic data to stimulate the social understanding of an abandoned child from the medical viewpoint and the viewpoint of administrative management. The data are based on the medical dossier of 305 patients which were hospitalized in 'A' hospital (of Seoul municipal hospital) between 1992 and 1996. The method of analysis is SPSS statistical package and the following results can be obtained 1. The abandoned children were frequently found in the Spring by season, in April by month, on Friday by day, and especially in the night and before an individual home and an medical institution in many cases. 2. The children found in an individual home were younger, handicapped smaller, and hospitalized for shorter period. As they were healthy relatively, parents might hope that they were accepted as foundling foster children. Contrary to above case, the children found in an medical institution were worse, handicapped more, and hospitalized for longer period. Accordingly, parents might hope that they were protected in an public institution. 3. The $69.8\%$ of abandoned children had caught a disease at that time and the $53.8\%$ of them had been handicapped. The most serious disease and handicap were the heart-lung trouble and the cerebral palsy, respectively. 4. The only $60.8\%$ of them were adopted and the most of them were healthy relatively. The $10.2\%$ which have been dead had caught four diseases on the average. As the results of above, the followings can be proposed : 1) It is required that the system connected to administrative management, which can provide the abandoned children turned for the better with the family relations to voluntary servants, be developed. 2) It is required that the system of registration management and plan for support economy, which can support the handicapped birth as the congenital malformation in national aspect, be developed. 3) It is required that to protect abandoned children, the health educational program for childbirth-people be prepared in the unit of a public health center.

한국어판 아동식습관 설문지를 사용한 식욕부진 소아의 식욕 개선 평가 (Assessment of Herbal Treatment in Appetite Improvement of Anorexia Children Using Korean Children's Eating Behaviour Questionnaire (K-CEBQ))

  • Background Children's Eating Behaviour Questionnaire (CEBQ) is a parents-targeted questionnaire to measure eating behavior of children, developed in United Kingdom and verified validity and reliability in several countries. Validity of Korean version of CEBQ (K-CEBQ) was verified in 2009. Objectives The study was conducted to assess the appetite improvement after herbal treatment in children who have anorexia. Methods The parents of the 26 children(anorexia 11, non-anorexia 15; $79.54{\pm}38.17$ months) who visited one university hospital was surveyed using K-CEBQ from April 2010 to April 2011. It was re-surveyed at intervals of 3 months. Differences between the anorexia group and the non-anorexia group were analyzed by Mann-Whitney U test, and comparisons of pre-treatment and post-treatment in anorexia group and non-anorexia group were verified by Wilcoxon signed-rank test. Results Depends on the presence of anorexia in child, Slowness in eating/Satiety responsiveness (SE/SR), Enjoyment of food (EF), Emotional undereating (EUE) and total scores were significantly different ($p$<0.05). Also, there were significant difference in SE/SR and total scores before and after the herbal treatment in anorexia group (p<0.05), and all scores tended to increase a few. However there were no significant difference before and after the herbal treatment in non-anorexia group, and the scores except for Desire to drink (DD) and Emotional overeating (EOE) were decreased. Conclusions Herbal treatment in children who have anorexia has improved their appetite in terms of eating sensitivity.

QT dispersion in children with Kawasaki disease

  • Background: We analyzed the changes in QT dispersion (QTd) in children with Kawasaki disease (KD), and determined the presence of repolarization abnormality in these children even in the absence of coronary artery abnormalities. Methods: Ninety-one children with KD and 20 healthy controls were enrolled in this retrospective study. Serial echocardiographic and electrocardiographic (ECG) measurements in the beginning of treatment, 2nd month and 6th month after the diagnosis were compared. Fifty-one of 91 children had at least 2 serial ECG data. The number of patients who had 3 consecutive ECG data was 23. Results: Among the 67 KD patients with no coronary artery changes, the consecutive mean QTd values were 41.86 ms, 37.84 ms, and 25.47 ms, respectively (26 ms for controls). In the analysis of changes among KD patients without coronary artery abnormalities, QTd showed a significant decrease with time (p=0.01). Especially, the 1st month and the 6th month QTd values were significantly different (p=0.028). The mean QTd values in KD patients with coronary artery changes were significantly higher than those in KD patients with no coronary artery changes at each time (1st, 2nd, and 6th month exam). Conclusion: QTd is significantly increased in children during the early stage of KD. Repolarization abnormality may exist during the acute stage of KD, regardless of the echocardiographic changes.

Clinical spectrum and short-term outcomes of multisystem inflammatory syndrome in children in a south Indian hospital

  • Background: Multisystem inflammatory syndrome in children (MIS-C) is a new hyperinflammatory variant that evolved during the coronavirus disease 2019 pandemic. Although the precise pathophysiology of MIS-C is uncertain, it is thought to be due to immune dysregulation occurring after recovery from acute infection. Purpose: Our study aimed to analyze the clinical spectrum, laboratory parameters, imaging characteristics, treatment strategies, and short-term outcomes of children with a diagnosis of MIS-C. Methods: This retrospective and prospective observational study included children less than 16 years of age who were admitted to the pediatric unit of a tertiary care teaching hospital in south India between August 2020 to January 2021 with a diagnosis of MIS-C according to World Health Organization criteria. Results: Twenty-one children were included in the analysis; all had fever with variable combinations of other symptoms. The mean age was 6.9 years; 71.4% were male. Gastrointestinal (80.9%) and cardiovascular (80.9%) systems were the most commonly affected. The majority of children had elevated inflammatory markers, and 16 (76.2%) had echocardiographic abnormalities mimicking Kawasaki disease. Eleven children (52.4%) required intensive care admission, 3 (14.3%) required supplemental oxygen, and 4 (19%) required inotropes. Nine (42.9%) were treated with intravenous immunoglobulin alone, 6 (28.6%) with steroids alone, and 3 (14.3%) with steroids and immunoglobulin. The median hospital stay was 6 days; there were no fatalities. Overweight/obesity, elevated ferritin, and mucocutaneous involvement were significantly associated with a prolonged hospital stay (≥7 days). Sixteen children (76.2%) were followed up till now and all of them had no clinical concerns. Conclusion: MIS-C is an emerging disease with variable presentation. A high index of suspicion is necessary for its early identification and appropriate management. Further research is essential for developing optimal treatment strategies.

Experience of Campylobacter gastroenteritis in Korean children: Single-center study

  • Objectives: Although Campylobacter is the main cause for bacterial acute gastroenteritis (AGE), there has been no notable clinical research into it, especially for Korean children. In this study, we share our experience of clinical, laboratory and image findings with Campylobacter AGE. Methods: Between May 2013 and June 2016, children diagnosed as having Campylobacter AGE were retrospectively enrolled in our study. Campylobacter AGE was considered diagnosed when a patient had symptoms of bacterial AGE and a positive Campylobacter result in stool using multiplex PCR. Results: Among 539 patients with suspected bacterial AGE, 31 (5.8%) patients had a positive result for Campylobacter. The average age of the 31 patients was $10.2{\pm}5.0$ years with a range between 1.1 and 16.9 years. Eighteen (58%) of the total patients were hospitalized between June and August. Diarrhea (93.5%), abdominal pain (83%) and fever (83%) were common symptoms. For 20 patients (65%), diarrhea lasted for less than three days, and fever lasted for 2.1 days on average. Among the 20 patients subjected to imaging studies, 12 patients (60%) showed bowel wall thickening on the right side of colon. In blood tests of 30 patients, 22 (73%) and 29 (97%) patients exhibited leukocytosis and elevated C-reactive protein, respectively. During treatment for Campylobacter AGE, prediagnostic empirical antibiotics were used for 6 (19%) patients. All patients recovered without complications. Conclusions: Among the children with suspected bacterial AGE, 5.8% had a positive result on Campylobacter in stool using multiplex PCR. Therefore, we observe that Campylobacter AGE should be considered in school-age children who have diarrhea, fever, and abdominal pain.
