Descriptive Study on Selected Risk Factors and Histopathology of Breast Carcinoma in a Tertiary Care Centre in Kerala, India with Special Reference to Women Under 40 Years Old

  • Background: Breast cancer is the most common female cancer in Kerala, South India, with the incidence increasing in the past two decades, also in young women. However, there are limited data regarding the burden of disease, its epidemiology and histopathological characteristics in the state. Materials and Methods: This desciptive study covered 303 breast cancers evaluated during the period of December 2011 to August 2013 in the Department of Pathology, Government Medical College, Kottayam.The patients were also interviewed regarding selected risk factors. Results: The majority of the cases were 41-60 years of age with a mean at presentation of 53 years. Infiltrating ductal carcinoma was the most common subtype, followed by pure mucinous carcinoma and then lobular carcinoma. Of the cases, 6.6% were nullipara and 52.8% had fewer than or equal to 2 children. Median age at first child birth was 23 years (national value-19.8 years). A significant proportion (15%) had family history of breast cancer. Some 13.5%(41 cases) comprised the young breast cancer group (${\leq}40$years) with a mean age at first child birth in them was 27.4 years, 5 being nullipara and 6 having a positive family history. Conclusions: Breast cancer awareness, better availability of screening techniques and identification and targeting high risk groups all help to tackle the increasing load of breast carcinoma. A good proportion of cases comprised the young breast cancer group (under 40). Younger women should thus also be educated about breast carcinoma-risk factors, symptoms and diagnostic techniques to help in early detection and effective approach esto treatment.

The Effect of Logotherapy on the Suffering, Finding Meaning, and Suffering, Well-being of Adolescents with Terminal Cancer

  • Purpose: To evaluate the effects of a logotherapy education program. Methods: A nonequivalent control group, non-synchronized design was conducted with a convenience sample of 29 children with cancer. The experimental group (n=17) participated in the logotherapy education program which consisted of 5 daily sessions for one week. The control group (n=12) received the usual nursing care. The effects were measured using suffering, adolescent meaning in life (AMIL), and spiritual well-being (SWBS) scales. Results: There were significant differences in suffering (W=153.00, p<.05) and meaning in life (W=78.00, p<.05) between the experimental and control groups. However, there was no significant difference in spiritual well-being (W=136.50, p>.05). Conclusions: Logotherapy was effective in reducing suffering and improving the meaning in life. Logotherapy can be utilized for adolescents with terminal cancer to prevent existential distress and improve their quality of life.

Psychometric validation of the Child Healthy Lifestyle Profile in South Korea: a cross-sectional study

  • Purpose: This study aimed to validate the Child Healthy Lifestyle Profile as an instrument for screening healthy behaviors in school-aged children in South Korea. Methods: Self-reported questionnaires were administered to 454 students, comprising elementary-school students (n=221) and child cancer survivors (n=233). Reliability and validity were assessed using Cronbach's α, exploratory factor analysis (EFA), and confirmatory factor analysis (CFA). Results: Cronbach's α, as a reliability test, was 0.87. Varimax rotation yielded nine factors with eigenvalues greater than 1 in the EFA, which explained 61.0% of the total variance. In the CFA, both convergent and discriminant validities were acceptable. Therefore, the Child Healthy Lifestyle Profile was validated as an assessment tool for Korean school-aged children. Conclusion: Based on the results of this study, the Child Healthy Lifestyle Profile was identified as a reliable and valid instrument for assessing healthy lifestyles in elementary-school children in South Korea.

소아암 환자의 형제자매 중재에 관한 연구논문의 체계적 문헌고찰 (A Systematic Review of Interventions with Siblings of Pediatric Cancer Patients)

  • Purpose: The purpose of this study was to review interventions available to nurses caring for siblings of children with cancer. Methods: Searches of CINAHL, MEDLINE, PubMed, EMBASE, and RISS identified ten intervention studies published from January, 2000 to November, 2016. Results: Ten studies on interventions for siblings of pediatric cancer patients were identified as follows: 4 included camps, 4 included support groups, and 2 provided individual interventions. Theoretical frameworks were mainly cognitive behavioral theory and most studies were led by psychologists and multidisciplinary teams. The purpose of the interventions was primarily enhancing psychosocial adjustment. Siblings' fear of disease, self-esteem, and social support were improved significantly after the interventions. Findings were inconsistent with regard to depression, anxiety, behavioral problems, post-traumatic stress, health-related quality of life, and siblings' perceptions of the illness. Conclusion: Study findings showed the potential for enhancing emotional and behavioral outcomes in siblings of children with cancer. However, the number of studies was very small, and several methodological limitations were identified. In the future, more randomized controlled trials with larger samples are needed to extend the evidence base. Moreover, future research should identify sibling's characteristics and circumstances most likely to bring benefits to the siblings.

암 치료가 종료된 청소년의 사회적 지지와 자아존중감이 우울에 미치는 영향 (Relationship of Self-esteem and Social Support to Depression in Child Cancer Survivors)

  • Purpose: This study was done to evaluate depression in child cancer survivors. Methods: The participants in this descriptive research were 103 adolescents who agreed to participate. All of the adolescents were diagnosed as completely cured but remained under follow-up observation as outpatients. A structured questionnaire including the depression scale developed by Shin et al. (1991), a self-esteem scale developed by Rosenberg (1965) and a social support scale developed by Kim & Park (1999), Lee (1997) were used for data collection. The data were analyzed using SPSS. Results: The mean scores were, for self-esteem, 36.01, for paternal support, 57.21, for maternal support, 59.98, for peer support, 22.19, for teachers support, 21.07 and for depression, 27.95. Scores for depression were significantly different according to academic achievement, satisfaction with appearances, diagnosis and prognosis. Depression was negatively correlated with self-esteem, paternal support, maternal support, peer support, and teachers support. Variables affecting depression were peer support and self-esteem, accounting for 47.0% of the variance. Conclusion: The findings indicate that peer support and self-esteem, follow by maternal, paternal and teachers support, are important variables in the occurrence of depression in these adolescents. Further study is needed to develop strategies to increase this support and self-esteem.

암 환아 어머니의 희망 분석 (Analysis of the Hope for the Mothers with Cancer Children)

  • The goal of this study was to develop a method to measure the degree of hope of mothers whom their children have cancer. Data collection was held on 144 mothers who had their children diagnosed as cancer, and were treated in C hospital and S hospital located in Seoul. The data was collected from January to August of the year 1999, and collected data was then computerized for T-test, factor analysis and ANOVA. The results are as follows: 1) The measurement method used to measure the degree of hope was divided into 7 categories and composed of 35 questions in total. The level of confidence was 0.90. The 7 categories were named 'gaining insight into life', 'realizing oneself's own role in life', 'attempting a change in life', 'taking in destiny', 'having trust in the treatment being held out by the medical faculty', 'recognizing or escaping from reality' 'obtaining support from religion' respectively. 2) The average score of mothers with cancer children was 3.22. The 6th category 'recognizing or escaping from reality', had the highest score and the 1st category of 'gaining insight into life' the lowest. 3) The degree of hope did not have any differences according to the general characteristics of the children with cancer and the characteristics of the disease. However there was a difference following the characteristics of the mothers with cancer children. Mothers with jobs tend to have scored higher compared with mothers without any occupation.

암 환아 어머니의 희망, 부담감과 가족기능 (The Hope, Burden, and Family Function in Mothers of Children with Cancer)

  • The purpose of the descriptive correlation study was to examine relations among the hope, the burden and the family function in mothers caring for children with cancer. 145 mothers completed the three questionaires of the study divided into tree sections: a) The Hope Scale, b) The Burden Scale, c) FACES-Ⅲ. The collected data was analysed with t-test, ANOVA, Scheffe test and Pearson correlation coefficients. The results were as follows: 1) The average age of mothers of children with cancer was 35.6 years old and the ages between 30 to 39 were the most abundant. 57.3% of the mothers had an education level of below high school education and 66.0% had religion. The average age of the children was 8.6 years old. Ages between 1 to 7 were the most and 60.8% were diagnosed as leukemia. 2) Regarding the section of degree of burden, mothers of children with cancer marked 2.6 out of 5, and the degree of hope 3.2 out of 4. The result for family function came out to be 3.5 out of 5, an average of family cohesion of 3.9 and family adaptation of 3.1. 3) There were significantly less burden to the mothers who were living together with a spouse compared to the mothers who were not. Also mothers who replied that they preserved good health came out to be exposed to less burden compared to the mothers who did not. In analysing hope according to the general characteristics of mothers of children with cancer, mothers who were employed marked high in the degree of hope compared to unemployed mothers. Furthermore, the degree of family cohesion marked higher with mothers who had higher education of college graduate, mothers with religion and mothers with a monthly family income of over ₩3,000,000, compared to the group of mothers with lower education of high school graduate, non-religious or with a monthly family income of less than ₩1,000,000. 4) Excluding the fact that the group sorted with children diagnosed as leukemia marked a perceivably high score regarding family cohesion, compared to groups with other cancers, the degree of burden, hope and family cohesion did not show any noticeable difference according to characteristics of children with different cancers. 5) In the correlation of the hope, the burden and the family function regarding the mothers of children with cancer, the burden did not have any manifest relationship with hope or family function. However, the degree of hope and family function cohesion had a direct proportional relationship, as family cohesion marked higher when the degree of hope were high.

항암치료를 받는 소아암 환아 어머니의 경험 (Mother's experience of caring for children with cancer receiving chemotherapy)

  • 본 연구의 목적은 항암치료를 받고 있는 소아암 환아를 돌보는 어머니의 경험을 설명하는 것이다. 연구 참여자는 소아암 진단을 받은 후 3개월 이상에서 3년 이하로 항암치료를 받고 있는 소아암 환아를 돌보는 어머니 7명으로 하였다. 자료수집 방법은 심층면담을 통해 자료를 수집하였다. 수집된 자료는 Colaizzi의 현상학적 분석방법을 적용하여 분석하였다. 연구 결과 216개의 의미 진술, 62개의 구성 진술, 16개의 주제 도출, 5개의 주제 모음이 확인되었다. 5개의 주제 모음은 '심장이 녹아내림', '투병의 힘든 여정', '내 아이 지키기', '고마움과 섭섭함의 교차', '가족과 함께 하는 고통과 기대'이었다. 항암치료를 받는 소아암 환아 어머니는 아픈 아이를 돌보는 동안 심한 정서적 변화를 경험하고 있었다. 동시에 다른 한편으로는 가족과 함께 미래를 계획하는 모습을 보여 주었다. 따라서 앞으로 소아암 환아 어머니를 위한 간호에는 환아 어머니의 심리적 소용돌이를 고려한 간호계획이 이루어져야 할 것으로 생각한다.

Effect of Self-Efficacy on Turkish Children's Perceptions of the Advantages/Disadvantages of Smoking

  • Objective: This descriptive, cross sectional study was conducted to examine the effect of levels of self-efficacy on children's perceptions of pros and cons of smoking. Methodology: The sample was 233 fifth-grade students. Data were collected in September 2010 using the Socio-Demographic Data Collection Form, Self Efficacy Scale Child Form and Child Decision Balance Scale and analyzed via percentage calculations and t test with the SPSS 11.00 statistical package program. Results: Average age of the students participating in the research was $11.1{\pm}0.41$, 49.8 % (n: 116) being female and 50.2 % (n:117) male. The difference between the score averages of the pros of smoking perceptions of children with a high self-efficacy level and of those with a low self efficacy level (t=2.117, p=.042) and the difference between the score averages of the cons of smoking perceptions of children with a high self-efficacy level and of those with a low self-efficacy level (t=2.206, p=.035) were found to be statistically significant. Conclusions: Children's positive perceptions of smoking were low and negative perceptions high when their self efficacy levels were high. Conversely, children's positive perceptions of smoking were high when their self efficacy levels were low.

Current Evidence on the Relationship Between Two Polymorphisms in the NBS1 Gene and Breast Cancer Risk: a Meta-analysis

  • Introduction: Published studies on the association between Nijmegen breakage syndrome 1(NBS1) gene polymorphisms and breast cancer risk have been inconclusive, and a meta-analysis was therefore performed for clarification. Methods: Eligible articles were identified by a search of MEDLINE and EMBASE bibliographic databases for the period up to March 2012. The presence of between-study heterogeneity was investigated using the chi-square-based Cochran's Q statistic test. When there was statistical heterogeneity, the random effects model was chosen; otherwise, fixed effects estimates were reported as an alternative approach. Results: A total of 11 eligible articles (14 case-control studies) were identified, nine case-control studies were for the 657del5 mutation (7,534 breast cancer cases, 14,034 controls) and five case-control studies were for the I171V mutation (3,273 breast cancer cases, 4,004 controls). Our analysis results indicated that the 657del5 mutation was associated with breast cancer risk (carriers vs. non-carriers: pooled OR =2.63, 95% CI: 1.76-3.93), whereas the I171V mutation was not (carriers vs. non-carriers: pooled OR =1.52, 95% CI: 0.70-3.28). Conclusion: The present meta-analysis suggests that the 657del5 gene mutation in the NBS1 gene plays a role in breast cancer risk, while the I171V mutation does not exert a significant influence.
