한국 소아 ${\cdot}$ 청소년정신의학 - 과거 ${\cdot}$ 현재 ${\cdot}$ 미래 - (KOREAN CHILD AND ADOLESCENT PSYCHIATRY - A REVIEW OF THE PAST, THE PRESENT AND THE FUTURE -)

  • 연구목적:대한소아청소년정신의학회 창립 20주년을 맞아 한국 소아청소년정신의학의 발전 과정(과거)을 돌아보고, 현재의 상황과 문제점을 짚어보고, 앞으로 나아가야할 방향과 주요과제를 살펴 보고자함. 연구방법:학회기록, 관련논문, 원로들과의 개인통신, 저자개인기록과 참여경험을 토대로 함. 결 과:한국소아정신과의 공식출범은 1979년 서울대학교병원외래에 소아정신과클리닉의 개설과, 다음해 동대학병원에 소아정신분과 설립으로 볼 수 있음. 대한소아, 청소년정신의학회는 1983년 한국소아정신의학연구회로 시작하여 1986년 동학회가 공식 창립되었음. 창립후 지난 20년간 학회는 괄목할만한 성장과 발전을 하였는데 그 원동력은 소아청소년정신과 전임의 수련프로그람이 주요대학병원(현재 총 7개)에 생겼고, 1994년부터 한국소아청소년정신의학회지 발간, 1996년 대한소아청소년정신과 수련심사위원회의 발족과 역할수행에서 찾을 수 있음. 현재 한국에 77명의 소아청소년정신의학 분과전문의와 48명의 부가수련중인 전문의, 18명의 전임의가 수련 중에 있음. 현재 한국소아청소년정신과는 국내적으로는 중흥기를 맞이하고 있고 인접전문분야의 발전에도 크게 기여하고 있음, 국제적으로도 아시아소아청소년학회의 창립과 학술활동에 중추적 역할을 하고 있을 뿐 아니라 미국 및 국제 소아청소년정신의학회등 국제무대로 역할을 확대 중임. 결 론:한국소아청소년정신의학은 지난 20년간 성공적인 발전을 해왔고, 의학계와 일반 사회에 크게 기여하고 있음. 그러나 청소년문제에 대한 적극적 개입, 병원 및 정신질환 관련기관에서의 자문, 연결, 학교자문서비스에 좀더 적극적인 참여가 필요함. 우리나라는 지난 30여년간 급격한 정치적, 경제적, 사회문화적 변화를 겪었고, 특히 가족체계의 변화와 붕괴현상은 날로 심각해져, 소아청소년의 정신건강문제는 증가일로에 있는 것이 분명함으로 앞으로 소아청소년정신과 전문의의 역할은 중차대함. 뿐만 아니라, 우리는 아시아지역 국가들의 낙후된 소아청소년정신과서비스 개발과 발전에도 기여하여야할 사명이 있는 것으로 제안함.

소아 청소년 양극성장애 환아군에서의 첫 기분 삽화의 극성에 따른 단기 예후 비교 : 단일 기관 연구 (The Comparison of the Short-Term Prognosis According to the Polarity of First Episode in Children and Adolescent with Bipolar Disorder : A Single Center Study)

  • Objectives : This study was conducted to evaluate the association between first episode polarity of pediatric bipolar disorder and prognosis. Methods : We analyzed the clinical records of 66 inpatient subjects with DSM-IV defined pediatric bipolar disorder. The patients were split into 2 groups according to the polarity of the illness onset [depressive onset (DO) vs. manic/hypomanic/mixed onset (MO)]. Clinical feature and prognosis were compared between the two groups of patients. Results : In our sample, 68% of patients experienced a depressive onset. In DO patients, rates of suicidal attempt, episodic illness course and comorbid disruptive behavior disorder were higher than rates in MO patients. Conclusion : Findings from this study suggest that polarity of illness onset may be useful in predicting the prognosis of pediatric bipolar disorder.

Experiences of the Development of Parent-Adolescent Relationships among Korean Mothers

  • Purpose: This study was conducted to explore Korean mothers' experiences of the development of parent-adolescent relationships. Methods: The participants were 18 Korean parents of adolescents. Data were collected through in-depth interviews. The main question was, "Could you tell me about your experiences of developing a relationship with your adolescent child?" The data were analyzed using Strauss and Corbin's grounded theory methodology. Results: The central phenomena of the experiences of parent-adolescent relationships among Korean mothers were "trying to reduce my expectations, but not being able to" and "having no idea where to go". The major action/interaction strategies were "putting aside my desires and adapting to my child's needs" and "waiting and seeing at a distance". The consequences included "appreciating myself and my child at the same time". Conclusion: These findings indicate that mothers tried to adapt to their children's needs and to keep their children at a distance. Their efforts were influenced by advice from friends and family members. The findings of this study emphasize specific aspects of how Korean mothers experienced the process of developing respectful relationships with their children.

스마트 토이를 활용한 아동용 인지능력 증진 프로그램의 예비 효과 연구 (A Preliminary Study of Computerized Cognitive Ability Enhancement Program Using Smart-Toy for Children)

  • Objectives: This study was to examine the effectiveness of computerized cognitive ability enhancement program (CCAEP) using Smarttoy. The CCAEP using Smart-toy which can interact with children via bluetooth is a kids-friendly and convenient method for improving children's cognitive abilities by increasing their motivation for performing the program. We developed the CCAEP which designed to train auditory-verbal memory, visual-spatial memory, auditory-verbal working memory, and visual-spatial working memory. Methods: Eighteen children aged 8 to 10 participated in CCAEP individual training composed of 8 sessions of 40 minutes each for 4 weeks. The effect of the training was measured with Smart Toyweb's cognitive assessment tasks (smart device based assessment) as well as traditional neuropsychological tests before and after the training. Results: Children showed significant improvement in auditory-verbal memory, visual-spatial memory, auditory-verbal working memory and visual-spatial working memory abilities after the training. Conclusion: This study demonstrated promising results suggesting the effectiveness of CCAEP using Smart-Toy in clinical settings as well as school and home situations. Further controlled study with larger sample size including various clinical groups is needed to confirm the present results.

  • Antidepressants, in particular selective serotonin reuptake inhibitors (SSRIs), are one of the most commonly used classes of psychotropic drugs for treating children and adolescents. The US Food and Drug Administration has issued a black box warning concerning the increased risks of suicidal ideation and behavior associated with antidepressant treatment in children and adolescents. The aim of this review is to assess the risks and benefits of antidepressants in the treatment of child and adolescent psychiatric disorders.

  • Adolescents diagnosed with autism spectrum disorder (ASD) often encounter significant challenges, particularly aggressive behaviors that persist into adulthood and profoundly affect their daily functioning and quality of life. These behaviors not only pose hurdles for affected individuals but also present considerable challenges for caregivers and families. Managing aggression in adolescents with ASD requires comprehensive treatment approaches encompassing both non-pharmacological and pharmacological interventions. This paper reviews current interventions that have proven to be effective through empirical studies in managing aggression among adolescents with ASD. By synthesizing evidence-based practices, this study underscores the importance of a multidisciplinary approach involving medical, psychological, and educational interventions to effectively manage aggression among adolescents with ASD. It aimed to inform clinicians, educators, and caregivers of practical strategies and evidence-based interventions to address aggression in this population.

정신건강서비스 영역에서 아동청소년 기능평가척도의 활용에 관한 연구 (Study on the Utilization of the Child-Adolescent Functional Assessment Scale in Mental Health Service)

  • Objectives: This study aims to supplement the limitations of the preliminary study and expand the use of the Child-Adolescent Functional Assessment Scale by providing standardized T-scores for measuring function in different areas, as well as guidelines for their interpretation. Methods: The Child-Adolescent Functional Assessment Scale and Korean-Child Behavior Checklist were administered to a total of 623 individuals, including 467 parents of emotionally and behaviorally disturbed elementary school students, 45 parents of emotionally and behaviorally disturbed middle school adolescents and 111 parents of elementary school students with developmental disorders, as well as to a control group consisting of 186 parents of mainstream elementary school children in the Seoul metropolitan area. Results: In contrast to the preliminary study, the emotional domain was disassociated into two factors, negative affect and emotion regulation, and the coefficient of determination for misconduct was increased significantly. Also, the overall reliability and validity of the Child-Adolescent Functional Assessment Scale were similar to those in the preliminary study, and the item structure was consistent. This study provides standardized T-scores of function in different areas based on the revised scale, and provides guidelines for their interpretation. Conclusion: The Child-Adolescent Functional Assessment Scale can assist the Child Global Assessment in Functioning in providing the information necessary for case management and treatment planning by comparing the impairment severity in each area of functioning, and also determining changes in behavior and function after treatment interventions. On the other hand, this scale has yet to clearly distinguish between rebellious behavior and misconduct. Further research is necessary to provide standards for more diverse age groups and for its utilization.
