저선량흉부 CT (Low Dose chest CT, LDCT)에서 Scout 관전압을 변화시키고 scan parameter인 자동노출제어장치(Auto Exposure Control, AEC)와 적응식 반복재구성기법(Adaptive Statistical Iterative Reconstruction, ASIR)등을 적용하여 최적의 프로토콜을 찾음으로써 방사선 피폭선량과 화질을 평가하고자 하였다. Scout 관전압을 80, 100, 120, 140 kV로 변화시키며 LDCT 프로토콜로 5회 반복 측정 후 선량을 비교하기 위해 장비에서 제공된 Dose report를 이용하여 연구 목적에 적합한 관전압을 선택하였다. 120 kV, 30 mAs의 조건으로 기본 LDCT 촬영한 후, 이 조건에 ASIR 50%를 적용하였으며 신호대잡음비와 대조도대잡음비를 평가하기 위해 배경의 노이즈를 측정하였다. 선량 비교를 위해 장비에서 제공되는 CTDIvol과 선량길이곱(Dose length product, DLP)를 식을 이용하여 비교 분석하였다. 그 결과 S140 + LDCT + ASIR 50 + AEC를 적용한 프로토콜에서 고식적인 LCDT보다 방사선 피폭선량을 감소시키고 영상의 질을 향상시켰으며 최적의 프로토콜을 얻을 수 있었으며 LDCT는 매 검사 시 필요 이상의 피폭선량이 우려되기 때문에 적절한 Parameter를 적용하는 것이 중요하며, 향후 LDCT를 이용한 건강검진에서 국민의 건강에 이바지 하는데 긍정적인 요인으로 작용될 것으로 사료된다.

Objective: To extract pulmonary and cardiovascular metrics from chest CTs of patients with coronavirus disease 2019 (COVID-19) using a fully automated deep learning-based approach and assess their potential to predict patient management. Materials and Methods: All initial chest CTs of patients who tested positive for severe acute respiratory syndrome coronavirus 2 at our emergency department between March 25 and April 25, 2020, were identified (n = 120). Three patient management groups were defined: group 1 (outpatient), group 2 (general ward), and group 3 (intensive care unit [ICU]). Multiple pulmonary and cardiovascular metrics were extracted from the chest CT images using deep learning. Additionally, six laboratory findings indicating inflammation and cellular damage were considered. Differences in CT metrics, laboratory findings, and demographics between the patient management groups were assessed. The potential of these parameters to predict patients' needs for intensive care (yes/no) was analyzed using logistic regression and receiver operating characteristic curves. Internal and external validity were assessed using 109 independent chest CT scans. Results: While demographic parameters alone (sex and age) were not sufficient to predict ICU management status, both CT metrics alone (including both pulmonary and cardiovascular metrics; area under the curve [AUC] = 0.88; 95% confidence interval [CI] = 0.79-0.97) and laboratory findings alone (C-reactive protein, lactate dehydrogenase, white blood cell count, and albumin; AUC = 0.86; 95% CI = 0.77-0.94) were good classifiers. Excellent performance was achieved by a combination of demographic parameters, CT metrics, and laboratory findings (AUC = 0.91; 95% CI = 0.85-0.98). Application of a model that combined both pulmonary CT metrics and demographic parameters on a dataset from another hospital indicated its external validity (AUC = 0.77; 95% CI = 0.66-0.88). Conclusion: Chest CT of patients with COVID-19 contains valuable information that can be accessed using automated image analysis. These metrics are useful for the prediction of patient management.
목적 Coronavirus disease 2019 (이하 COVID-19) 폐렴에서 CT를 일차 진단 검사로 사용하고자 하는 논의가 있지만, 대규모 인구에게 CT 검사를 적용했을 때의 상황을 고찰한 연구는 없었다. 본 연구에서는 COVID-19 폐렴을 다룬 연구들에서 CT 프로토콜과 방사선량을 분석하고, CT 검사가 일차 진단 검사법으로 사용될 때 필요한 CT 검사량에 대해 알아보고자 한다. 대상과 방법 본 연구는 9개의 인용도가 높은 영상의학과 저널에서 COVID-19 폐렴의 CT 기반 진단을 다룬 문헌들을 검색하였다. 먼저, 연구에서 제시된 CT 프로토콜, 방사선량을 조사하여, 이를 해당 국가의 diagnostic reference level과 비교하였다. 추가로, COVID-19에 대한 CT 민감도 94%, 특이도 37%를 적용하여, 우한시와 뉴욕, 이탈리아의 초기 COVID-19 outbreak에서 polymerase chain reaction (이하 PCR) 검사 양성률에 기반한 number needed to diagnose (이하 NND)와 number needed to predict (이하 NNP)를 계산하였다. 결과 총 86개의 연구가 검색되었고, 그중 CT 프로토콜은 81개의 연구에서(94.2%), 방사선량은 17개의 연구에서(19.8%) 보고되었다. 저선량 흉부 CT는 표준선량 흉부 CT보다 2배 많은 연구에서 활용되었다(39.5% vs. 18.6%). 방사선량을 보고한 17개의 연구들 중, 15개의 연구에서 방사선량은 해당 국가의 diagnostic reference level 수치보다 낮았다(88.2%). COVID-19에 대한 CT 민감도 94%, 특이도 37%를 적용하였을 때, NND는 3.2회 CT scans으로 나타났다. 한편, PCR 검사 양성률 50%, 25%, 10%, 5%에서의 한 명의 COVID-19 환자를 진단 위한 CT 검사량을 나타내는 NNP는 각각 2.2, 3.6, 8.0, 15.5회의 CT scans로 나타났다. 우한 시에서는 최종 17365명의 COVID-19 환자를 진단하기 위하여 약 35418명에서(PCR 검사 양성률 58%) 44840명(PCR 검사 양성률 38%)의 사람들이 CT 검사를 받은 것으로 나타났다. 뉴욕시와 이탈리아의 초기 COVID-19 유행 10주간, PCR 검사 양성률에 따라 일 CT 검사량이 최대 5.4, 10.9배까지 변화하였다. 결론 CT를 COVID-19에 대한 일차적인 진단검사로 사용할 경우, PCR 검사 양성률에 따라 CT 검사량은 변동량이 크고, 이는 추후 판데믹 상황에서 고려되어야 할 것이다.
배경 : 탈장이 장관을 포함하는 경우 단순흉부촬영과 위장관촬영 등으로 진단할 수 있으나 복잡한 경우 CT 나 MRI 가 진단에 많은 도움을 주는 것으로 알려져 있다. 그러나 CT 소견을 이용한 Morgagni 탈장과 섬막주위지방과의 감별에 대한 체계적인 연구는 없었다. 본 연구에서는 이를 위하여 도움이 될 수 있는 CT 소견을 분석하고자 하였다. 대상 및 방법 : Morgagni 탈장 8 예와 풍부한 섬막주위지방이 있던 20 예의 CT 를 후향적으로 분석하였다. CT 촬영은 흡기상태에서 횡격막 전체를 포함하도록 하였다. 분석한 내용은 1) 전횡격막의 결손 유무, 2) 폐와 지방의 경계, 3) 지방과 흉벽의 각도, 4) 지방과 흉막외지방과의 연속성, 5) 지방내 혈관의 존재 여부, 6) 지방을 둘러싸는 선상음영의 존재 여부 등이었다. 결과 : Morgagni 탈장의 경우 전예에서 전횡격막의 결손이 있었고, 폐와 지방의 경계는 분 명하며, 폐쪽으로 볼록한 양상을 보였으며, 흉벽과는 예각을 보인 반면, 흉막외지방과의 연속성은 보이지 않았다. 섬막주위지방의 경우 전횡격막의 결손은 3 예 (15%), 폐와 지방의 경계는 보통 불규칙하고 (n= 10), 평편(n= 17) 하였으며, 흉벽과의 각도는 다양하였고, 대부분의 경우 (n=16) 늘어난 흉막외지방과의 연속성을 보였다. 지방을 둘러싸는 선상음영은 4 예의 Morgagni 탈장에서 관찰된 반면, 섬막외지방에서는 관찰되지 않았다. 이상의 소견들은 통계적으로 유의한 차이를 보였으나, 지방내 혈관은 Morgagni 탈장(n=8f8)과 섬막주위지방 (n=14/20)의 감별에 유의한 차이를 보이지 않았다. 결론 : Morgagni 탈장의 진단에 유용한 CT 소견은 경계가 분명하고, 폐쪽으로 볼록하고, 흉벽과 예각이며, 주위에 않은 선상음영을 갖는 지방 종괴이다. Morgagni 탈장의 특징적 소견으로 알려진 대망혈관을 시사하는, 가지치는 양상의 선상음영은 탈장과 심막주위지방과의 감별에 도움이 되지 않는다.
본 논문은 흉부 CT 영상에서 폐 흉벽에 결절 및 폐혈관이 붙어 있는 경우에도 폐 정보의 손실 없이 폐와 기관지를 분리할 수 있는 알고리즘을 제안 하였다. 마스크 영상의 활용은 폐 및 기관지 분할에서 시간 단축 및 성능을 향상 시킬 수 있었다. 또한 폐 흉벽과 밝기값이 같은 결절을 찾아 제거 하는 방법은 좌 우측폐의 외곽 영상을 2진 영상으로 변환하고, 형태학적 정보를 활용함으로써 가능 하였다. 마지막으로 제거된 부분의 외곽선 연결은 거리가 고려된 최적 화소 추가와 3차 Spline 보간법을 적용하였다. Matlab 시뮬레이션 결과 제안된 알고리즘은 기존 문제점이 보완됨을 확인 할 수 있었다.
기술의 발전으로 CT 검사에 있어 환자가 받는 피폭선량을 줄이기 위한 노력은 새로운 재구성 기법 개발과 함께 계속 진행되고 있다. 최근에는 반복적 재구성 기법의 한계를 극복하기 위해 딥러닝 재구성 기법이 개발되었다. 본 연구는 소아 흉부 CT 영상에서 재구성 기법에 따른 영상의 유용성을 평가하였다. 환자 실험은 2021년 1월 2일부터 2022년 12월 31일까지 경상남도 P 병원에서 흉부 조영 CT 검사를 받은 소아 환자 중 85명을 대상으로 연구를 진행하였다. 팬텀 실험에 사용된 팬텀은 Pediatric Whole Body Phantom PBU-70이다. 검사 후 FBP, ASIR-V(50%), DLIR(TF-Medium,High)로 영상을 재구성했고, 동일한 크기의 ROI를 설정하여 HU값, SD값을 획득하여 SNR, CNR 값을 산출하여 영상을 평가하였다. 그 결과 DLIR의 TF-H가 모든 실험에서 ASIR-V(50%)와 TF-M에 비해 잡음 값이 가장 낮았으며, SNR과 CNR의 값이 가장 높았다. 소아 흉부 CT 검사에서 DLIR이 적용된 TF 영상이 ASiR-V 영상보다 잡음이 적었고, CNR과 SNR은 높은 것으로 나타났으며 DLIR이 적용되면 기존의 재구성법에 비해 영상의 질이 더 향상될 것으로 판단된다.
Purpose: The present study will identify risk factors for aspiration in severe trauma patients by comparing patients who showed a sign of aspiration lung disease on chest computed tomography (CT) and those who did not. Methods: We conducted a retrospective review of the Korean Trauma Data Bank between January 2014 and December 2019 in a single regional trauma center. The inclusion criteria were patients aged ≥18 years with chest CT, and who had an Injury Severity Score ≥16. Patients with Abbreviated Injury Scale (AIS)-chest score ≥1 and lack of medical records were excluded. General characteristics and patient status were analyzed. Results: 425 patients were included in the final analysis. There were 48 patients showing aspiration on CT (11.2%) and 377 patients showing no aspiration (88.7%). Aspiration group showed more endotracheal intubation in the ER (p=0.000) and a significantly higher proportion of severe Glasgow Coma Scale (GCS) (p=0.000) patients than the non-aspiration group. In AIS as well, the median AIS head score was higher in the aspiration group (p=0.046). Median oxygen saturation was significantly lower in the aspiration group (p=0.002). In a logistic regression analysis, relative to the GCS mild group, the moderate group showed an odds ratio (OR) for aspiration of 2.976 (CI, 1.024-8.647), and the severe group showed an OR of 5.073 (CI, 2.442-10.539). Conclusions: Poor mental state and head injury increase the risk of aspiration. To confirm for aspiration, it would be useful to perform chest CT for severe trauma patients with a head injury.

The Coronavirus disease 2019 (COVID-19) has rapidly spread across the world and caused a pandemic. It can be transmitted by an infected person or an asymptomatic carrier and is a highly contagious disease. Prevention and early identification of COVID-19 are important to minimize the transmission of COVID-19. Chest computed tomography (CT) has a high sensitivity for detecting COVID-19, but relatively low specificity. Therefore, chest CT may be difficult to distinguish COVID-19 findings from those of other infectious (notably viral types of pneumonia) or noninfectious disease. Pulmonary contusion has also a lot of similarities on chest CT with COVID-19 pneumonia. We present trauma patients with pulmonary contusion whose CT scans showed findings similar to those of COVID-19, and we report our experience in the management of trauma patients during the COVID-19 pandemic.
Ground-glass opacity nodules(GGNs) in chest CT images are associated with lung cancer, and have a different malignant rate depending on existence of solid component in the nodules. In this paper, we propose a method to classify pure GGNs and part-solid GGNs using multiview images and texture analysis in pulmonary GGNs with solid components of 5mm or smaller. We extracted 1521 features from the GGNs segmented from the chest CT images and classified the GGNs using a SVM classification model with selected features that classify pure GGNs and part-solid GGNs through a feature selection method. Our method showed 85% accuracy using the SVM classifier with the top 10 features selected in the multiview images.
목적: 이 연구의 목적은 흉부 전산화단층촬영(CT)에서 우연히 발견된 유방 병변의 임상적 중요성을 평가하고 유방 전문 진료 의뢰가 필요한 영상 소견을 제안하는 것이다. 대상과 방법: 2009년 5월부터 2014년 4월까지 촬영된 조영증강 흉부 CT 중에서 판독문에 '유방'이라는 단어가 포함된 검사를 후향적으로 검토하였다. 이 중 우연히 발견된 유방 병변으로 유방 초음파와 유방 촬영술을 한 뒤, 병리학적 확인이나 1년 이상의 추적 관찰을 받은 총 86명의 환자(모두 여성, 평균 연령, $48.9{\pm}12.6$세)를 대상으로 하였다. 흉부 CT에서 유방 병변의 크기, 위치, 모양, 경계 및 조영 증강을 포함한 영상 특징을 평가하였다. 다양한 CT 특징과 병리소견과의 연관성을 평가하고 여러 조합의 CT 특징의 진단 정확도를 평가하였다. 결과: 흉부 CT에서 우연히 발견된 유방 병변 중 악성 종양은 16.3%(14/86)였다. 악성 병변은 양성 병변에 비해 불규칙한 모양, 불분명한 경계, 높은 조영증강을 보였다. CT 특징의 조합 중 불분명한 경계와 높은 조영증강의 조합이 악성 병변을 예측하는데 가장 높은 정확도를 보였다(97.7%). 결론: 흉부 CT에서 우연히 발견된 유방 병변의 16.3%는 악성이었으며, 불분명한 경계와 높은 조영증강을 동시에 보일 경우 악성 가능성이 높으므로, 정확한 진단을 위해 유방 전문 진료 의뢰가 필요하다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
