성인에서 횡격막마비로 오인한 우엽간 횡격막탈장 1예 (Diaphragmatic Hernia of the Right Hepatic Lobe Mistaken for Diaphragmatic Paralysis in Adult)

  • Diaphragmatic paralysis can be demonstrated through diaphragmatic elevation on chest X-ray after thoracic lung surgery or the placement of chest tubing. Additional causes of diaphragmatic paralysis are iatrogenic, mass, atelectasis, etc. For the diagnosis of diaphragmatic paralysis, it required some studies (fluoroscopy, computed tomography [CT], magnetic resonance imaging). Diaphragmatic hernia of the liver is a rare clinical entity, usually found after trauma in adults. Congenital diaphragmatic hernia in neonates requires surgery. Non-traumatic diaphragmatic hernia of the liver in an adult is a rare right-sided diaphragmatic hernia. On developing any symptoms, surgery must be performed. When diaphragmatic hernia is incidentally found in adults without trauma, it is placed under observation for a time period. We diagnosed the diaphragmatic herniation of a right hepatic lobe by 16-slice CT scan without surgery.

The Importance of Esophageal and Gastric Diseases as Causes of Chest Pain

  • Purpose: Pediatric chest pain is considered to be idiopathic or caused by benign diseases. This study was to find out how much upper gastrointestinal (UGI) diseases are major causes of chest pain in pediatric patients. Methods: The records of 75 children (42 boys and 33 girls, aged 3-17 years old) who have presented with mainly chest pain from January 1995 to March 2015 were retrospectively reviewed. Chest X-ray and electrocardiography (ECG) were performed in all aptients. Further cardiologic and gastrointestinal (GI) evaluations were performed in indicated patients. Results: Chest pain was most common in the children of 6 and 9 to 14 years old. Esopha-gogastric diseases were unexpectedly the most common direct causes of the chest pain, the next are idiopathic, cardiac diseases, chest trauma, respiratory disease, and psychosomatic disease. Even though 21 showed abnormal ECG findings and 7 showed abnormalities on echocardiography, cardiac diseases were determined to be the direct causes only in 9. UGI endoscopy was performed in 57 cases, and esophago-gastric diseases which thereafter were thought to be causative diseases were 48 cases. The mean age of the children with esophago-gastric diseases were different with marginal significance from that of the other children with chest pain not related with esophago-gastric diseases. All the 48 children diagnosed with treated with GI medicines based on the diagnosis, and 37 cases (77.1%) subsequently showed clinical improvement. Conclusion: Diagnostic approaches to find out esophageal and gastric diseases in children with chest pain are important as well as cardiac and respiratory investigations.

둔상에 의한 기관 완전 절단 - 1예 보고 - (Complete Transsection of the Trachea by Blunt Trauma)

  • 둔상에 의한 기관-기관지 파열은 발생빈도가 낮은 질환이나, 점차 그 빈도가 증가하는 추세이다. 일단 기관-기관지 파열이 발생하면 매우 심각한 증상을 유발하여 생명의 위협을 초래하고, 많은 합병증을 야기하기 때문에 조기에 진단하여 외과적으로 치료하는 것이 바람직하다 본 저자들은 35세의 남자 환자에서 교통사고 후 발생한 기관 완전 절단의 한 예를 보고하는 바이다. 환자는 수상 후 심한 호흡곤란과 경부, 전흉부에 생긴 피하기종을 주소로 내원하여 양측 긴장성 기흉 진단하에 응급 흉관 삽관술 시행 후에도 대량의 공기 유출이 지속되었고, 피하기종은 복부와 음낭까지 진행되었으며, 호흡부전은 더욱 심화되어 기관-기관지 파열이 강력히 의심되어 응급 수술을 시행하였다. 수술실에서 시행한 술 전 기관지내시경상 성대 하에서 기관의 횡단 절단을 확인하였으며, 수술은 목에 칼라 절개를 가한 후 절단된 기관을 확인하여 4-0 바이크릴(Vicryl) 봉합사를 이용하여 단속봉합에 의한 단단 문합술을 시행하였다. 술 후 흉관을 통한 공기유출은 없었고 기관지내시경 추적 검사상 양호한 소견을 보였으며 특별한 합병증 발병 없이 퇴원하였다.

Chest wall injury fracture patterns are associated with different mechanisms of injury: a retrospective review study in the United States

  • Purpose: Research on rib fracture management has exponentially increased. Predicting fracture patterns based on the mechanism of injury (MOI) and other possible correlations may improve resource allocation and injury prevention strategies. The Chest Injury International Database (CIID) is the largest prospective repository of the operative and nonoperative management of patients with severe chest wall trauma. The purpose of this study was to determine whether the MOI is associated with the resulting rib fracture patterns. We hypothesized that specific MOIs would be associated with distinct rib fracture patterns. Methods: The CIID was queried to analyze fracture patterns based on the MOI. Patients were stratified by MOI: falls, motor vehicle collisions (MVCs), motorcycle collisions (MCCs), automobile-pedestrian collisions, and bicycle collisions. Fracture locations, associated injuries, and patient-specific variables were recorded. Heat maps were created to display the fracture incidence by rib location. Results: The study cohort consisted of 1,121 patients with a median RibScore of 2 (range, 0-3) and 9,353 fractures. The average age was 57±20 years, and 64% of patients were male. By MOI, the number of patients and fractures were as follows: falls (474 patients, 3,360 fractures), MVCs (353 patients, 3,268 fractures), MCCs (165 patients, 1,505 fractures), automobile-pedestrian collisions (70 patients, 713 fractures), and bicycle collisions (59 patients, 507 fractures). The most commonly injured rib was the sixth rib, and the most common fracture location was lateral. Statistically significant differences in the location and patterns of fractures were identified comparing each MOI, except for MCCs versus bicycle collisions. Conclusions: Different mechanisms of injury result in distinct rib fracture patterns. These different patterns should be considered in the workup and management of patients with thoracic injuries. Given these significant differences, future studies should account for both fracture location and the MOI to better define what populations benefit from surgical versus nonoperative management.

응급실에 내원한 둔상환자의 수혈 필요성 예측인자 (Predictive Factors of Blood Transfusion Requirement in Blunt Trauma Patients Admitted to the Emergency Room)

  • Purpose: In multiple blunt trauma patients, transfusion may be a significant therapeutic adjunct to non-operative management. The blood products must be expedited and efficiently to patients in impending shock caused by hemorrhage or traumatic coagulopathy, but the decision to perform blood transfusion has been made empirically, based on the clinician' and has not been guided by objective parameters, but own opinion, that may result in an underestimate of or a failure to detect bleeding, in delayed transfusion, and in a reduced outcome. This article presents quickly assessable predictive factors for determining if a blood transfusion is required to improve outcomes in multiple blunt trauma patients admitted to the emergency room. Methods: In a retrospective review of 282 multiple blunt trauma patients who visited our emergency center by emergency rescuer during a 1-year period, possible factors predictive of the need for a blood transfusion were subjected to univariate and multivariate logistic regression analysis. Results: Of blunt trauma patients, 9.2% (26/282), received red blood cells in the first 24 hours of care. Univariate analysis revealed significant associations between blood transfused and heart rate (HR) > 100 beats/min, respiratory rate (RR) > 20 breaths/min, Glasgow Coma Scale (GCS) < 14, Revised Trauma Score (RTS) < 11, white blood cell count (WBC) < 4000 or > 10000, and initial abnormal portable trauma series (Cspine lateral, chest AP, pelvis AP). A multiple regression analysis, with a correction for diagnosis, identified HR > 100 beats/min (EXP 3.2), GCS < 14 (EXP 4.1), and abnormal trauma series (EXP 2.9), as independent predictors. Conclusion: In our study, systolic blood pressure (SBP) < 90 mmHg, old age > 65 years, hemoglobin < 13g/dL, mechanism of injury were poor predictors of early blood transfusion. Initial abnormal portable trauma series, HR > 100 beats/min, and GCS < 14 were quickly assessable useful factors for predicting a need for early blood transfusion in blunt trauma patients visiting the emergency room.

외상성 흉부대동맥류치험 1례 보 (Traumatic aneurysm of descending thoracic aorta: report of one case)

  • A 25 years old male patient with traumatic aneurysm of descending thoracic aorta was treated successfully. He was admitted due to blunt trauma to the chest wall as falling down into water. He has been treated with closed thoracotomy and close observation. After a month simple chest film showed abnormal mass shadow around the aortic knob. Aortogram revealed aneurysm of descending thoracic aorta. Aneurysm was resected and replaced with woven Dacron graft with aid of Gotts shunt. His postoperative course was uneventful.

흉부외상 717례의 임상적 고찰 (A Clinical Evaluation of 717 Chest Injuries)

  • A Clinical analysis was performed on 717 cases of the chest trauma experienced at department of thoracic and cardiovascular surgery, Soonchunhyang Chunan Hospital from Mar. 1985 to Jun. 1989. 1. The sex ratio was 2.89:1 with male predominance. The patients in 75 % of total cases was between 3rd to 5th decade. 2. The most common causes were traffic accident in non-penetrating thoracic injury and stab wound by knife in penetrating injury. 3. The hemothorax, pneumothorax and hemopneumothorax were observed in 306 cases[42.7 %]. 4. The left thorax was the preferred site of penetrating and non-penetrating thoracic injury. The rib fracture was prevalent between 4th to 8th rib. 5. The open thoracotomy was performed in 55 cases[7.7 %] 6. The overall mortality was 3.07 %[22 cases] and causes of death were hypovolemic shock, adult respiratory distress syndrome, sepsis, disseminated intravascular coagulation, renal failure and pneumonia.

흉강경 수술을 이용한 횡격막 파얼의 치유 -1예 보고- (Thoracoscopic Surgery for Diaphragmatic Rupture -One case report -)

  • 외상성 횡격막 파열은 방사선 검사로 진단하기가 어려운 경우가 빈번하다. 다발성 손상을 동반한 37세 남자 환자에서 횡격막 파열이 의심되었으나, 흉부 CT 검사에서는 횡격막 파열을 확인할 수는 없었고 진단을 위해 흉강경을 이용한 수술을 결정하였다. 횡격막 신경이 횡격막으로 들어가는 부위로부터 8 cm정도의 횡격막 파열이 있었고, 횡격막 마비를 동반하였다. 작업창을 5 cm크기로 추가하여 만든 후, 파열된 횡격막을 연속 봉합하였으며, 동시에 횡격막 주름성형술을 시행하였다. 수술 3개월후 시행한 흉부 방사선 사진에서 만족할 만한 결과를 보였다.

Congenital Intercostal Lung Herniation Combined with an Unusual Morgagni's Hernia

  • A 70-year-old male visited urgent care due to coughing for 1 month and left chest pain. He had no history of trauma. The initial chest computed tomography (CT) showed the 7th left intercostal lung herniation. A follow-up CT showed an intercostal lung herniation combined with a bowl herniation, which had developed due to a Morgagni's hernia. An emergency operation was performed due to the incarceration of the bowl and lung. The primary repair of the diaphragm was performed and the direct approximation of the 7th intercostal space was determined. We concluded that the defect of the diaphragm and the intercostal muscle was a congenital lesion, and the recurrent coughing was the aggravating factor of herniation.

  • Spinal epidural emphysema is rare and has been described secondary to following medical intervention, such as lumbar puncture and epidural analgesia, pneumothorax or pneumomediastinum, degenerative disk disease, epidural abscess, and trauma. Rarely, it occurs after chest tube placement. We report a case of spinal epidural emphysema incidentally noted on HRCT after chest tube placement.
