중성어혈 약침과 인삼백하오관중탕으로 호전된 비심인성 흉통 증례 1례 (A Case Report of non-cardiac chest pain treated with Jungsongouhyul Pharmacopuncture and Insambaekhaokwanjung-tang)

  • ■ Objectives The purpose of this study is to report the effectiveness of Korean medicine in the treatment of non-cardiac chest pain. ■ Methods The patient with non-cardiac chest pain was treated with herbal medicine, Insambaekhaokwanjung-tang, and Jungsongouhyul pharmacoacupuncture. The severity of symptom was assessed by daily chest pain frequency, Numerical Rating Scale(NRS) of chest pain. ■ Results After the treatment, the severity of chest pain was reduced from NRS 9 to NRS 0 and the palpitation intensity was reduced from 9 to 0. The chest pain frequency was reduced from 7 to 0. In addition, other symptoms such as anorexia, sleep disturbance, and fatigue have shown improvements. ■ Conclusion The findings of this study suggests that treatment with Korean medicine can be an effective option in treating non cardiac chest pain.

The Importance of Esophageal and Gastric Diseases as Causes of Chest Pain

  • Purpose: Pediatric chest pain is considered to be idiopathic or caused by benign diseases. This study was to find out how much upper gastrointestinal (UGI) diseases are major causes of chest pain in pediatric patients. Methods: The records of 75 children (42 boys and 33 girls, aged 3-17 years old) who have presented with mainly chest pain from January 1995 to March 2015 were retrospectively reviewed. Chest X-ray and electrocardiography (ECG) were performed in all aptients. Further cardiologic and gastrointestinal (GI) evaluations were performed in indicated patients. Results: Chest pain was most common in the children of 6 and 9 to 14 years old. Esopha-gogastric diseases were unexpectedly the most common direct causes of the chest pain, the next are idiopathic, cardiac diseases, chest trauma, respiratory disease, and psychosomatic disease. Even though 21 showed abnormal ECG findings and 7 showed abnormalities on echocardiography, cardiac diseases were determined to be the direct causes only in 9. UGI endoscopy was performed in 57 cases, and esophago-gastric diseases which thereafter were thought to be causative diseases were 48 cases. The mean age of the children with esophago-gastric diseases were different with marginal significance from that of the other children with chest pain not related with esophago-gastric diseases. All the 48 children diagnosed with treated with GI medicines based on the diagnosis, and 37 cases (77.1%) subsequently showed clinical improvement. Conclusion: Diagnostic approaches to find out esophageal and gastric diseases in children with chest pain are important as well as cardiac and respiratory investigations.

협심증이 의심되는 환자에서 운동부하검사로 유발되는 흉통의 양상과 생리적 변인에 관한 연구 (A study on characteristics and physiological variables of chest pain induced by exercise test in angina suspected patients)

  • The purpose of this study was to identify the characteristics and physiological variables of chest pain induced by exercise test in angina suspected patients. The subjects of this study consisted of 28 inpatients and outpatients aged between 40 and 75 who underwent treadmill test at exercise testing laboratory of S-University from January 2000 to June 2000. Subjects were interviewed with questionnaire regarding sociodemography, the past health history and history related to chest pain before the exercise test. Subjects were interviewed with questionnaire concerning quality, intensity, duration of chest pain induced by walking on the treadmill(Marquette, U.S.A. 1992) according to Bruce protocol following exercise test. Systolic and diastolic blood pressure were measured before, during and after the test, heart rate was determined by ECG. The results of this study were as follows ; 1) Quality of chest pain induced by exercise test were feeling stiffness 19(67.9%), heavy 10(36.0%), exploded 9(32.1%), crushing, suffocating, tight 8(28.6%), stuffy, prickly 7(25.0%), burning 6(21.4%), clasp 5(17.9%), cleaved, tensed, piercing 3(10.7%), perfectly fitting, sore 2(7.1%), tearing, tingling, ticklish, heartburn 1(3.6%). 2) Mean score of VAS(intensity of pain) following exercise test was $5.79{\pm}2.27$ and mean duration of chest pain after the test was $7.83{\pm}5.31$ minutes. 3) Sites of chest-pain induced by exercise test were middle site 11(39.3%), left-chest 10(35.6%), right-chest 6(21.5%). Radiation site of chest-pain was neck(18.0%), right flank site 1(3.6%), left shoulder & arm 2(7.1%) and back 1(3.6%). 4) Symptoms other than chest-pain induced by exercise test were dyspnea 21(75.6%), perspiration 14(50.4%), fatigue 12(43.2%), leg-pain 11(39.6%), dizziness 7(25.2%) anxiety toward chest-pain 3(10.8%), thirst 2(7.1%), and palpation, headache and tingling sensation of hand and leg 1(3.6%). 5) Mean MET(intensity of exercise) during the exercise test was $7.64{\pm}2.57$ and mean RPE(rating of perceived exertion) was $15.89{\pm}2.36$. Mean duration of exercise was $6.79{\pm}2.88$. 6) correlation coefficients between RPE and VAS was 0.500(p=0.003), those between MET and VAS was 0.287(p=0.069) and those between either depression or elevation of ST segment and VAS was 0.236(p=0.114). 7) There was a significant difference in mean systolic pressure between before and after the test as $146.29{\pm}28.18mmHg$ and $177.96{\pm}28.82mmHg$(t=-5.640, p=0.000), a significant difference in mean diastolic blood pressure between before and after the test as $84.85{\pm}15.07mmHg$ and $88.89{\pm}13.72mmHg$(t=-2.082, p=0.047), and there was a significant difference in mean heart-rate between before and after the test as $81.89{\pm}12.22/min$ and $160.68{\pm}21.77/min$(t=-21.255, p=0.000).

우울과 불안이 흉통 환자의 삶의 질에 미치는 영향 (Impact of Depression and Anxiety on Quality of Life in Patients with Chest Pain)

  • Objectives : There has been substantial evidence that patients with chest pain have depression and anxiety, and show impaired quality of life (QoL). This study aimed to campare the QoL according to types of chest pain and to examine the impact of depression and anxiety on QoL in patients with chest pain. Methods : Forty-seven patients with chest pain were divided into Cardiac-Typical Chest Pain (CTCP, n=22) and Non-Cardiac-Atypical Chest Pain groups (NCACP, n=25) according to the pain characteristics and cardiovascular disease. Patients were assessed for depression using the Beck Depression Inventory (BDI), for anxiety using the State-Trait Anxiety Inventory (STAI), and QoL was assessed using the Korean version of the SmithKlein Beecham 'Quality of Life' Scale (KvSBQOL). Results : Compared with the CTCP group, the NCACP group reported significantly higher anxiety, and lower QoL. There was no significant difference in QoL between the two groups after adjusting for anxiety. The QoL was associated with depression and trait-anxiety in the CTCP group, and with trait-anxiety in the NCACP group. Conclusion : The findings suggest that there are different effects of depression and anxiety on QoL in individuals with CTCP and NCACP. Understanding about these differences can be important in the treatment of patients with chest pain. A large prospective study is needed to confirm these results.

  • Objectives: This study aimed to describe the use of traditional Korean medicine to relieve chest pain, insomnia, and dyspnea in a patient with unstable angina pectoris Methods: To relieve the symptoms, the patient was treated with a range of traditional Korean medicine, including acupuncture, moxibustion, and an herbal medicine (Gamiondam-tang-gagam). The numerical rating scale (NRS) was used to measure the patient's status and improvements in the frequency of symptoms. Results: After the treatment, the NRS score for chest pain, insomnia, and dyspnea decreased from 10 to 0. In addition, chest pain and insomnia almost subsided. Conclusions: This report suggests that Gamiondam-tang-gagam can be effective in relieving chest pain, insomnia, and dyspnea.

  • This case study is to report the effectiveness of Korean medicine in the treatment of musculoskeletal chest pain in Parkinson's patient. The Parkinson's patient with chest pain was treated with Jungsongouhyul Pharmacopuncture for 15 days. The severity of chest pain was assessed using Face Pain Scale-Revised, Short Form McGill Pain Questionnaire(SF-MPQ) and King's Parkinson's Disease Pain Scale(KPPS). Additionally, duration of the pain and standing state was evaluated. After treatment, chest pain was removed. SF-MPQ score was decreased from 6 to 0, and KPPS score 21 to 0. As a result this case showed the effect of Jungsongouhyul Pharmacopuncture on musculoskeletal chest pain in Parkinson's disease.

Analysis of clinical characteristics and causes of chest pain in children and adolescents

  • Purpose: Chest pain is common in children and adolescents and is a reason for referral to pediatric cardiologists. Although most cases of chest pain in these age groups are benign and do not require treatment, timely diagnosis is important not to miss life-threatening diseases requiring prompt treatment. We investigated certain clinical characteristics that may be useful in the diagnosis of such critical diseases. Methods: Patient medical records between July 2006 and September 2013 were retrospectively examined. We included 517 patients who presented with chest pain to the Department of Pediatrics at Kyung Hee University Hospital in Gangdong. Results: Most cases of chest pain were idiopathic in origin (73.6%), followed by cases with respiratory (9.3%), musculoskeletal (8.8%), cardiac (3.8%), gastrointestinal (2.9%), and psychiatric (1.4%) causes. In 6 patients (1.2%) with air-leak syndrome including pneumothorax or pneumomediastinum, the pain was abrupt, continuous, and lasted for a short period of 1-2 days after onset in the older adolescents. Of the patients with cardiac pain, 13 had cardiac arrhythmias (65.0%), 6 had congenital heart diseases (30%), and 1 had coronary aneurysms caused by Kawasaki disease (5.0%). One patient with atrial flutter had only symptoms of syncope and chest pain. Conclusion: The abrupt, continuous chest pain of a short duration in the older children was characteristic of air-leak syndrome. In patients with pneumomediastinum, radiological diagnosis was difficult without careful examination. Combined syncope should not be neglected and further cardiac workup is essential in such patients.

  • Objective: Improvement of angina pectoris using OCNT Methods: OCNT was applied to a Korean woman in her 60s who was experiencing chest pain and dizziness due to angina pectoris for about six months. Results: After starting OCNT, symptoms of chest pain and dizziness due to angina pectoris gradually decreased, and after about six months, such symptoms no longer caused discomfort. Conclusion: For patients with angina pectoris who experience chest pain and dizziness, OCNT can help relieve symptoms.

이중 에너지 전산화 단층촬영 ECG Gating High Pitch Chest Pain Protocol 모드를 이용한 방사선량과 영상 품질에 관한 연구 (A Study on Radiation Dose and Image Quality using Dual Energy Computed Tomography ECG Gating High Pitch Chest Pain Protocol Mode)

  • 본 연구는 심전도 게이팅을 한 방법과 심전도 게이팅 없이 검사하는 방법으로 검사하여 대동맥 뿌리 영상을 비교하는 것이며 high pitch(flash) chest pain protocol 방법으로 검사한 영상들과 기존의 방식으로 심전도 게이팅 없이 검사한 환자의 대동맥 뿌리 영상의 질환 유무를 관찰하였다. High pitch(flash) chest pain protocol과 일반적인 chest pain protocol로 AAPM 팬텀을 스캔하였으며 이렇게 획득된 팬텀 영상을 가지고 동일한 영상 품질을 가지는 파라메타 값을 조절하고 방사선량, 즉 CTDI 값을 비교해 보았다. 심전도 게이팅을 함으로 해서 상행대동맥의 이미지 왜곡은 기존의 심전도 게이팅을 하지 않은 검사방법보다 월등하게 감소시킬 수 있었고 대동맥 뿌리의 영상 품질은 향상되었다. 영상 이미지 품질의 차이를 보이지 않는 파라메타 범위 내에서, high-pitch chest pain protocol로 검사하였을 때 CTDI 값이 더 낮게 나왔으므로 방사선량 감소에도 이점이 있음을 알 수 있었다. 심장과 관련된 분야에서 선량 감소 모드를 이용하여 대동맥 박리와 같은 진단 분야에 응용하여 적용한다면 획기적인 피폭선량 감소 효과뿐만 아니라 빠른 진단과 함께 신속한 치료가 필요한 환자들에게 매우 중요한 검사방법이 될 것으로 사료된다.

Acupuncture for chronic post-thoracotomy pain in a patient with major chest trauma: A case report

  • Objectives : Chronic post-thoracotomy pain is a refractory condition that responds poorly to existing pain treatments. The aim of this report is to describe favorable symptom changes in a patient with chronic post-thoracotomy pain after major chest trauma who received a series of manual acupuncture treatments over 3 months. Methods : Twelve sessions of manual acupuncture were provided to the patient, once or twice a week. Results : The manual acupuncture sessions were well tolerated. Minor adverse events, including temporary minor bleeding at the needled sites, were intermittently observed. The patient's symptoms, including pain, sleep disturbance, and anxiety gradually resolved and had almost completely disappeared after 12 sessions of acupuncture treatment. Conclusion : This single case report cannot confirm whether the observed positive changes in the patient's symptoms and dysfunctions are associated with the acupuncture treatments. Randomized controlled trials are necessary to explore the role of acupuncture in chronic post-thoracotomy pain in patients with major chest trauma.
