A Large Epiphrenic Esophageal Diverticulum Communicating with the Left Lower Lobe

  • Epiphrenic diverticula are known to cause a series of complications. We report the case of a 54-year-old woman who was diagnosed with an epiphrenic diverticulum at a regular checkup in November 2006. Ten years later, she presented with massive hematemesis. Imaging studies revealed an epiphrenic diverticulum measuring 7.8 cm in diameter and a large amount of bleeding inside the diverticulum. Computed tomography showed fistula formation between the diverticulum and the left lower lobe of the lung, leading to the development of a pulmonary abscess. Diverticulectomy and $180^{\circ}$ posterior partial fundoplication were performed transabdominally. The pulmonary abscess was treated with antibiotics alone. She was discharged 16 days after the operation without any complications over 7 months of follow-up.

  • 현재 자동차 교통사고 발생원인 중 타이어의 불량으로 인한 교통사고는 매년 급증하고 있다. 자동차 타이어의 상태를 점검하여 미연에 사고를 예방하는 캠페인이 방송매체를 통해 진행되고 있으며 이와 관련된 행사도 실시하고 있다. 기존의 타이어 측정방법은 객관적이지 못하며 전문적인 기관에 의뢰해도 사정은 마찬가지다. 운전자에게 편리하며 객관적인 타이어 상태를 측정하려는 방법은 연구되지 않는다. 본 논문에서는 운전자에게 편리하며 객관적인 타이어 상태 정보를 제공하고, 계속적으로 누적된 타이어 상태 측정데이터를 분석하여 운전자에게 유용한 타이어 상태정보를 제공하는 시스템을 설계하기 위한 연구내용을 다룬다.

Case Report of Hypertension Patient by Community Health Practitioner (보건진료소의 고혈압 환자 간호사례)

  • Purpose: This study was done to describe a community health practitioner's nursing case study for a hypertension patient. Methods: For this purpose, data were collected through interviews and nursing records. The nursing process was carried out from January to March in 2019. Results: For this patient, blood pressure was checked 5 times and total cholesterol test was performed 5 times over 41 consultations. Even though the patient was 84 years old, he was very interested in health. When the blood pressure and cholesterol level measured by community health practitioner were high, he was embarrassed by the recommendation to get a hospital checkup. After referral to a medical clinic and hospital, he was satisfied with the results of the tests and medical treatment, and constantly practiced daily walking exercise. Conclusion: It is very important for community health practitioners to assess patient symptoms correctly and refer the patient timely to manage complications.

Arachnoid Granulations Mimicking Multiple Osteolytic Bone Lesions in the Occipital Bone

  • We report a rare case of arachnoid granulations mimicking multiple osteolytic bone lesions. A 66-year-old woman was admitted to a local clinic for a regular checkup. Upon admission, brain CT showed multiple osteolytic lesions in the occipital bone. These needed to be differentiated from multiple osteolytic bone tumor. Subsequent brain MRI revealed that the osteolytic lesions were isointense to cerebrospinal fluid, hyperintense on T2-weighted image, hypointense on T1-weighted image, and with subtle capsules around the osteolytic lesions that were visible after gadolinium injection. A bone scan revealed no radiotracer uptake. The lesions were in both the transverse sinuses and the torcular herophili. With typical radiological appearances of the lesions, the osteolytic lesions were diagnosed as multiple arachnoid granulations. No further treatment was planned. A 1-year follow-up brain CT scan revealed no change. We should consider the possibility of arachnoid granulations when multiple osteolytic lesions are observed in the occipital bone.

Delamination analysis of inhomogeneous viscoelastic beam of rectangular section subjected to torsion

  • This paper considers a delamination analysis of a statically undetermined inhomogeneous beam structure of rectangular section with viscoelastic behavior under torsion. The beam is built in at its two ends. The beam has two longitudinal inhomogeneous layers with a delamination crack between them. A notch is made in the upper crack arm. The external torsion moment applied on the beam is a function of time. Under these conditions, the beam has one degree of indeterminacy. In order to derive the strain energy release rate, first, the static indeterminacy is resolved. Then the strain energy release rate is obtained by analyzing the balance of the energy with considering the viscoelastic behavior. The strain energy release rate is found also by analyzing the compliance of the beam for checkup. Solution of the strain energy release rate in a beam without a notch in the upper crack arm is derived too. In this case, the beam has two degrees of static indeterminacy (the torsion moment in the upper crack arm is treated as an additional internal redundant unknown). A parametric investigation of the strain energy release rate is carried-out.

A Study on Oral Symptoms and Oral Health Behavior among Secondary Students (S중, 고등학생의 구강검진에 따른 구강 증상 및 구강건강행태 조사)

  • This study carried out dental checkup targeting 251 students in the 1st grade of S middle school and 220 students in the 1st grade of S high school in Seoul Metropolis. The results are as follows. As for the experience of dental hypersensitivity to oral symptoms in middle-and-high school students, 23.2% of high school students had experience of painful tooth when drinking or eating cold and hot beverage or food, compared to 14.3% of middle school students, and showed significant difference($\chi^2=6.08$, p<.05). As for the experience of painful teeth, 9.1% of high school students had experience of being sore, aching, and painful in the teeth, compared to 4.4% of middle school students, and showed significant difference($\chi^2=4.23$, p<.05). As a result of the oral health behavior in middle-and-high school students, 48.6% of middle school students were indicated to be higher than 26.4% of high school students in the experience of having ever gone to dentist for the past 1 year, and showed statistically significant difference($\chi^2=24.56$, p<.001). As for the period of toothbrushing during one day, 68.5% of middle school students brushed teeth after having breakfast($\chi^2=6.39$, p<.05) more than 57.3% of high school students. After having lunch, 16.8% of high school students brushed teeth more than 7.2% of middle school students, and showed statistically significant difference($\chi^2=10.58$, p<.01). As a result of judging dental checkup in middle-and-high school students, 44.1% of high school students had dental caries($\chi^2=25.36$, p<.001) more than 22.3% of middle school students. 9.6% of middle school students had tooth loss more than 4.1% of high school students, and showed statistically significant difference($\chi^2=5.39$, p<.05).

Policy suggestions for active reporting of medical professionals for early detection of child abuse (아동학대의 조기발견을 위한 제도적 개선 방안: 「아동학대범죄의 처벌 등에 대한 특례법」이 정한 의료인에 의한 신고를 중심으로)

  • The Act on Special Cases Concerning the Punishment, etc. of Child Abuse Crimes intends to encourage reporting and punishment of child abuse by using the concept of 'crime' in child abuse cases. Article 10 of the Act imposes duty to report child abuse on a number of different professions, including medical professionals. Currently, more than 80% of child abuse cases occur among family members and the detection rate of child abuse is as low as 0.5% in Korea. On the other hand, medical professionals can identify child abuse relatively clearly with specific medical opinions. Therefore, it is necessary that medical professions are informed of this duty and does not bear disincentive from reporting. This paper makes policy suggestions in this regard. First, it is necessary that medical students and medical professionals receive regular education about the obligation to report child abuse. Education should include details of the reporting duty, as well as the fact that there is legal obligation to report even if the child abuse is "suspicious", not certain. Second, it is imperative to establish and implement protective programs for medical professionals who report child abuse. The current law provides a rough framework for protection of people who report child abuse, but it is necessary to produce detailed guidelines that are applicable in the context of medical setting. Education for medical students and medical professionals should include the contents of these guidelines, so that they do not hesitate reporting because they fear the aftermath of reporting. Third, it is highly recommended that physicians use the national Baby/Infant Health Checkup Program as an opportunity to detect child abuse. In Korea, the Baby/Infant Health Checkup Program provides physicians to periodically monitor health condition of all babies and children until the age of 71 months. In order to utilize this program for early detection of child abuse, it is imperative that the bBaby/Infant Health Checkup Program is modified to involve child abuse experts and medical professionals who participate in the program are educated about child abuse.

Association between oral health status and oral health impact profile(OHIP-14) among the community elderlies (노인의 객관적 구강건강상태와 주관적 구강건강수준간의 관련성)

  • Objectives : This study was attempted in order to grasp oral health level according to socio-demographic characteristics in elders in some communities, and to evaluate oral health status and its association. Methods : The subjects in this study were performed with 235 people, who were over 65 years and resided in Daejeon Province, from June 20 to July 10, 2011. An individual interview was held, and they got a dental checkup. As for data analysis, chi-square test, t-test, one-way ANOVA, pearson correlation were utilized. Results : The older age in the whole research subjects and the lower educational level led to the less remaining teeth and the larger missing teeth index. The decayed missing filled teeth index and the decayed missing filled teeth rate were higher in more women and older age and in the lower educational level. Tooth mortality rate was higher in the older age, the lower educational level, and the group of living together with spouse. The maxillary-mandibular fixed-bridge status in the mouth was indicated to be the highest in the full-denture mounting ratio as for elders in over 80 years old. Oral Health Impact Profile(OHIP-14) average score was $56.05{\pm}11.64$ in the whole research subjects The decayed missing filled teeth index and the decayed missing filled teeth rate showed significantly positive correlation with the decayed missing filled teeth rate, tooth mortality rate and showed significantly negative correlation with OHIP-14. Tooth mortality rate showed significantly negative correlation with OHIP-14 Oral Health Impact Profile(OHIP-14) showed significantly positive correlation with its factors. Conclusions : Accordingly, the policy effort is considered to be necessary that implements in elders in order to spend active senescence, and that elders' health and oral-health behavior can be implemented continuously and preventively through classification according to elders' physical function.

Home Health Care Service Using Routine Vital Sign Checkup and Electronic Health Questionnaires (주기적인 생리변수 측정과 전자건강설문을 이용한 재택건강관리서비스)

  • In this Paper. we describe a home health care service using electronic health questionnaires and routine checkup of vital signs Including ECG (Electrocardiography) , blood pressure. and SpO$_2$ (Oxygen Saturation) . This system is for patients at home with chronic diseases, discharged Patients, or any normal people for the Prevention of disease The service requires a home health care terminal and a PC with Interned connection installed at Patient home. The distance health care management center is equipped with a vital-sign and questionnaire interpreter as well as database, Web, and notification servers with UMS (Unified Messaging System). Participating Physician can access the servers at the center using a Web browser running on a PC available to them at any time. These components are linked together through various kinds of data and voice communication channels including PSTN (Public Switched Telephone Network) . CATV(Community Antenna TV) . Interned. and mobile communication network. Following the Physician's direction given to a Patient. he or she uses the home health care terminal to collect vital signs and fill out the questionnaire. When the terminal automatically transmits these data to the management center. the data interpreter and servers at the center process the information fo1lowing the Protocol implemented on the system. Physicians can retrieve and review data corresponding to their Patients and send back their diagnostic reports to the center. UMS at the center delivers the physician 's recommendation to the corresponding patient through the notification server. Patients can also reprieve and review their own records as well as diagnostic reports from physicians. The system Provides a new way of collecting diagnostic information and delivering doctor's recommendation to patients at home for their health management. Future works are needed in the development of new technology for measurements and interpretations of various vital signs .

The Prevalence of High Myopia in 19 Year-Old Men in Busan, Ulsan and Gyeongsangnam-Do (부산, 울산 및 경상남도 만 19세 남자의 고도근시 유병률)

  • Objectives: The purpose of this study was to estimate the prevalence and correlated factors of high myopia in 19 year-old men in Southeast Korea. Methods: This retrospective study was based on the medical checkup data of conscription during 2005. The study subjects were 19 years old men in Busan, Ulsan and Gyeongsangnam-do. The health checkup data of the conscripts consisted of noncycloplegic autorefraction test, the biometric data and social factors. To analyze the social and biometric effects, we classified the biometric factors into 4 or 5 groups and the social factors into 3 groups. High myopia was defined as a spherical equivalent of under -6.0 diopter. Data analysis was performed using the chi square test for trends and multiple logistic regression analysis. The SAS(version 9.1) program was used for all the analyses. Results: The prevalence of high myopia was 12.39% (6256 / 50 508). The factors correlated with high myopia were the residence area (OR, 2.07; 95% CI, 1.77 to 2.4 for small city; OR, 2.01; 95% CI, 1.72 to 2.34 for metropolis; the reference group was rural area), academic achievement (OR, 1.43; 95% CI, 1.34 to 1.53 for students of 4-and 6-year-course university; the reference group was high school graduates & under) and blood pressure (OR, 1.54; 95% CI, 1.10 to 2.16 for hypertension; OR, 1.09; 95% CI, 1.02 to 1.17 for prehypertension; OR, 1.10; 95% CI, 1.01 to 1.20 for hypotension; the reference group was normal blood pressure). Conclusions: More than one tenth of the young men were high myopia as one of the risk factor for visual loss. Further studies on high myopia and its complications are needed to improve eye health in Southeast Korea.
