특수병과의 과거 군 소음 노출이 소음 노출 작업자의 청력에 미치는 영향 (The Effect of working Noise Exposure and Military Background on the Hearing Threshold)

  • 과거 군 복무 기간 동안 청력에 영향을 미치는 사격 및 포격 등 소음에 노출된 군 경력이 현재 소음 노출 근로자에게 어떤 청각학적 영향을 미치며, 소음성 난청의 발생과 관련이 있는지를 평가하고자 하였다. 중소규모 선박수리 및 건조업체 15개 사업체 440명을 대상으로 근로자의 일반 사항, 임상증상, 현재의 청력과 관련한 과거 이과적 병력, 군 복무시 충격소음의 상시적 노출 등의 군 경력, 이명 등의 자각증상에 관한 설문조사와 순음청력검사 및 중이검사를 시행하였다. 현 직종에서의 소음 노출군 중 과거 군에서 소음 노출군이 각 주파수별 청력역치와 평균 청력역치가 가장 컸으며, 특히 2-8kHz의 주파수 역치에서 차이가 크게 나타났다. 청력손실자의 비율 및 중증도의 정도가 현 직종에서의 소음 노출군 중 과거 군에서 소음 노출군이 가장 높은 비율을 보였으며, 특히 감각신경성 난청으로서 소음성 난청자가 통계적으로 유의하게 더 많았다. 이명은 현재 소음 노출 직종에 있는 군에서 높았으며, 그중 군 소음 노출군이 46.2%로 유의하게 더 높았다. 소음성 난청 여부를 종속변수로 한 로지스틱 회귀분석 결과, 연령, 현 직종 근무기간, 군 소음 노출력, 현 직종에서의 소음 노출 여부가 유의한 독립변수로 작용하였다. 우측 귀의 경우, 소음성 난청에 대한 군 소음 노출력의 비차비는 4.5이었고, 현 직종의 소음 노출의 비차비는 7.9이었다. 군 소음 노출 및 현 작업 소음 노출 여부에 따른 각 군별 좌우측 귀의 4 kHz 청력역치와 평균 청력역치는 I군에서 좌우측 귀 모두 나이 및 근무기간이 증가할수록 가장 크게 영향을 미치며 유의하게 증가하였으며 II군, III군, IV군 순으로 청력 역치가 증가하였다. 군에서 노출되는 충격소음은 청력손실과 이명 등 청각학적 영향을 미치며 사업장에서 소음 노출에 따른 소음성 난청의 발생에도 부가적인 영향을 미친다. 따라서 군에서의 소음 노출에 따른 청각학적 영향을 예방하기 위해서뿐만 아니라 소음 노출 근로자에 대한 청력 관리 측면에서도 이의 조기 진단, 치료, 보상 및 재활을 위해서는 군에서의 소음 노출 문제까지 함께 다루어야 함을 시사한다.

청력 장애 노인들의 고독감, 자아 존중감 및 삶의 질과의 관계연구 (A Study on the Relationship of the Loneliness, Self-esteem, and Quality of Life on the Hearing Disturbance Aged)

  • This study as a cross survey study was conducted to provide basic data for more practical approach to nursing the aged, by analyzing the relationship of the loneliness, self-esteem and quality of life on the hearing disturbance aged. This study was based on the survey data from the 90 aged, who are over sity years old and resident in Seoul. In order to measure the general characteristics and subjective audition, Cantril's Ladder Scale Tool was used. For the loneliness, UCLA Loneliness Scale Tool (translated into Korean to suit the Korean environment by Kim's). In order to measure the self-esteem, Rosenberg's Self-esteem Measurement Tool (translated into Korean by Chun) was used. No's Quality of Life Measurement Tool was used to the measure the quality of life. The Data were collected by direct interviewing on the subjects for this study from August 10, 1999 to September 15, 1999. In accordance with each purpose of this study, appropriate analyzing methods such as Descriptive Statistics, T-test, and ANOVA were used in analyzing the collected data. Pearson correlation coefficient was used to test the relationship of the loneliness, self-esteem and quality of life on the hearing disturbance aged. The result of this study is as follows: 1) The average of the degrees of subjective audition was 7.22, The factors that cause the result were sex, religion, the perception of health, the married state. 2) The average of the degrees of loneliness was 55.18, The factor that cause the result was the perception of health. 3) The average of the degrees of self-esteem was 28.42, The factors that cause the result were the number of family, the number of children, the perception of health. 4) The average of the degrees of quality of life was 71.87 the factors that cause the result were age, education, the number of children, the perception of health. 5) Correlation of Loneliness, self-esteem and quality of life is the perception of loneliness showed significant negative correlation with self-esteem (r=-.466, p=.0001) and quality of life (r=-.450, p=.0001). As a result of analysis above: Because of the recognition change of the aged, it seems to them that the subjective audition scale on the hearing disturbance aged is measured by only 10-point ladder scale is inappropriate. Therefore, it is necessary that the physiologic-index can confirm more exactly should be applied to them. It is very important that the social-psychological factor that has influence on the aged should be the self-perception of health. Thus, proper development of nursing intervention should be required to process in the self-perception of health on the aged.

이식형 마이크로폰과 진동체를 갖는 인공중이의 이득 보상을 위한 주파수 특성 고찰 (Study on frequency response of implantable microphone and vibrating transducer for the gain compensation of implantable middle ear hearing aid)

  • ACROSS device, which is composed of an implantable microphone, a signal processor, and a vibrating transducer, is a fullyimplantable middle ear hearing device(F-IMEHD) for the recovery of patients with hearing loss. And since a microphone is implanted under skin and tissue at the temporal bones, the amplitude of the sound wave is attenuated by absorption and scattering. And the vibrating transducer attached to the ossicular chain caused also the different displacement from characteristic of the stapes. For the gain control of auditory signals, most of implantable hearing devices with the digital audio signal processor still apply to fitting rules of conventional hearing aid without regard to the effect of the implanted microphone and the vibrating transducer. So it should be taken into account the effect of the implantable microphone and the vibrating transducer to use the conventional audio fitting rule. The aim of this study was to measure gain characteristics caused by the implanted microphone and the vibrating transducer attached to the ossicle chains for the gain compensation of ACROSS device. Differential floating mass transducers (DFMT) of ACROSS device were clipped on four cadaver temporal bones. And after placing the DFMT on them, displacements of the ossicle chain with the DFMT operated by 1 $mA_{peak}$ current was measured using laser Doppler vibrometer. And the sensitivity of microphones under the sampled pig skin and the skin of 3 rat back were measured by stimulus of pure tones in frequency from 0.1 to 8.9 kHz. And we confirmed that the microphone implanted under skin showed poorer frequency response in the acoustic high-frequency band than it in the low- to mid- frequency band, and the resonant frequency of the stapes vibration was changed by attaching the DFMT on the incus, the displacement of the DFMT driven with 1 $mA_{rms}$ was higher by the amount of about 20 dB than that of cadaver's stapes driven by the sound presssure of 94 dB SPL in resonance frequency range.

코클리어 기저막 운동의 속도특성 해석 (Analysis on the Velocity Characteristics of the Basilliar Membrane Motion in Cochlea)

  • In this study, the general characteristics, and theories of cochlear concerning with hearing are examined. Also the digital method is studied in order to analyze with microcomputer on the model equation of basiliar membrane in the cochlear derived from 3-dimensional rectangular block model which is studied by Boer. The method is illustrated for the amplitude characteristics of basiliar momtrane wave velocity. The results obtained are as follows; 1. In the magnitude characteristics, the velocity gradually increases from the stapes, shows the maximum magnitude, and then rapidly decreases to the Helicotrema. 2. The characteristics of 3-dimensional model is located between 1-and 2- dimensional models in the velocity characteristics coefficients, magnitude characteristics, and the pattern of 2-dimensional model shoves the different features from the 1-dimensional and 3-dimensional rectangular block model. 3. In the 3-dimensional rectangular block model, the characteristics of the waveform and the maximum resonant point are same whether Z(X) is linear or nonlinear.

이식형 인공중이를 위한 압전 플로팅 매스 트랜스듀서의 제안 (Proposal of a piezoelectric floating mass transducer for implantable middle ear hearing devices)

  • A new type of transducer, piezoelectric floating mass transducer (PFMT) which has advantages of piezoelectric and electromagnetic transducer has been proposed and implemented for the implantable middle ear hearing devices. By the uneven bonding of piezoelectric material to the inner bottom of transducer case, the PFMT can vibrate back-and-forth along the longitudinal axis of the transducer even though the piezoelectric material within the cylindrical case produces only the bilateral expansion and contraction according to the applied electrical signal. To improve efficiency of the PFMT, the multi-layered piezoelectric material has been adapted. The small number of components in the PFMT enables the simple manufacturing and the easy implanting into the middle ear. In order to examine the characteristics of vibration, mechanical modeling and finite element analyses of the proposed transducer have been performed. From the result of theoretical analyses and the measured data from the experiment, it is verified that the implemented PFMT can be used in implantable middle ear hearing devices.

  • An FIR filter for digital voice signal processing has been designed and implemented using a microcontroller in implantable middle ear hearing devices (IMEHDs). The designed digital voice signal processing filter which has fast and accurate filtering operation and controllable filter characteristics has been implemented using a hardware multiplier and a direct memory access (DMA) in the low power microcontroller, MSP430F169. It has been confirmed that each of the implemented 6-orders Remez FIR filters with 1 channel and 2 channels can be applied to the voice signal processing module of IMEHDs based on the evaluation results of the filtering performance experiment.

도로 교통소음의 전파특성 연구 - 인체에 미치는 영향에 대한 고찰 - (The propagation characteristics of road traffic noise - with respect to human response -)

  • 본 연구에서는 평탄한 지면 위의 도로 교통소음의 전파특성을 해석하고자 점음원과 쌍극자음원을 모델로 사용하였으며, 연속 지표면 음향임피던스에 대한 초과감쇠를 실험으로 조사하였다 연구결과 수음점이 지표면 가까이 있을 때, 도로 교통소음의 전파특성은 소음 전파경로 상의 지표면 음향임피던스에 크게 영향을 받는 것을 확인하였다. 또한 진행 차량으로부터 측정한 등가에너지레벨의 거리별 감쇠값을 이용하여 등음압곡선을 해석하였다 그리고 도로 교통소음이 인체에 미치는 영향을 조사하고자 초저주파 소음에 대한 응답특성, 깊은 수면율과 잠을 깨는 비율에 대한 연속적인 교통소음의 .효과, 연령과 소음폭로등 다양한 변수에 대하여 청력손실과 청감민감도 변화 등을 고찰하였다 해석결과 과도한 소음은 일시적 청력손실의 원인이며, 폭로가 길어지거나 강력하면 영구적 청력손실의 원인이 될 수 있음을 확인하였다.

자음지각검사 (KCPT)의 개발 (Development of Korean Consonant Perception Test)

  • 본 연구의 목적은 말지각평가의 기초 자료가 되고 청각장애 자음지각능력의 정량적 정성적 평가가 가능한 음소단위의 한국어자음지각검사 (Korean Consonant Perception Test, 이하 KCPT)도구를 개발하는 것이다. KCPT는 조합 가능한 모든 한국어 단음절어 중 유의미 단음절어만 분류하여, 7종성발음규칙을 적용하고 목표단어의 친숙도를 조절한 사지선다형 초 종성검사문항 시안을 작성한 후, 각 단어들의 조음 특성, 난이도, 음소 출현빈도수를 고려하여 완성되었다. 이를 정상인 20명을 대상으로 평가하여 난이도가 높은 문항을 제거하고 화자 간 변수가 없는 것을 통계적으로 확인한 후 회화체 빈도수에 맞도록 최종문항을 작성하였다. 세부적 수정을 거쳐 초 종성검사문항 각 200개와 100개로 구성된 총 300개 문항을 개발하였다. 30명의 청각장애인을 대상으로 검증하여 A와 B로 구분한 절반목록간 차이도 없음을 확인하고 초성검사문항은 초성자음지각평가에 종성 검사문항은 종성자음 지각평가에 적절한 도구임을 검증하였다.

Clinical Characteristics and Long-Term Prognosis of Alport Syndrome: A Retrospective Single-Center Study

  • Purpose: Alport syndrome (AS) is one of the most common inherited renal diseases caused due to mutations of genes encoding specific proteins of the type IV collagen family, and its major clinical manifestations include progressive renal failure, sensorineural deafness, and ocular abnormalities. We investigated the clinical characteristics and long-term prognosis of AS in Korean pediatric and adult populations. Methods: We conducted a retrospective review of medical records of 33 children and adults who had been diagnosed or treated with AS from 1985 to 2019. Results: The mean age of the 33 patients diagnosed with AS was 16.2±13.6 years, and the male-to-female ratio was 2:1. At the first visit, recurrent gross hematuria was the most common initial symptom. In 10 of 33 patients (30.3%), sensorineural hearing loss (SNHL) was diagnosed, but none had ophthalmic problems. Moreover, 11 of 33 patients (33.3%) had advanced to end-stage renal disease (ESRD), and a significant difference was observed in the age of the patients who progressed to ESRD based on the presence or absence of SNHL (P=0.035). Conclusion: SNHL in AS can be an important prognostic factor for long-term deterioration of renal function. Further investigation is required to confirm the clinical course and the genetic characteristics of AS in Korea through prospective national cohort studies.

아동 미각교육을 위한 쌀 Kit 개발 및 이를 활용한 미각 특성조사 (The Development of a Taste Kit for Education and Research into Sensory Characteristics)

  • This study was designed to measure taste sensitivity and the five basic senses by an educational classification instrument. The instrument was a rice kit that could use samples in a dry powder form or oil extract after long-term storage To test for taste, sucrose, salt, citric acid, and quinine sulfate were made at different concentrations and taste sensitivity was measured on a scale from level 1 to level 5. To obtain baseline data, an inspection tool for the five senses was used and randomly applied on 101 schoolchildren in the third and fourth grade in the city of Cheonan in Korea. The inspection tool was composed of 17 questions; 5 questions regarding visual characteristics and three questions each for characteristics regarding taste, hearing, smell, and touch. The average age of the schoolchildren was 9.5 years old and there were 49 third grade students (9 years of age), and 52 fourth grade students (ten years of age). There were slightly more male students than female students, 56 (55.4%) compared to 45 (44.6%), respectively. The average height of female students was higher than that of males, but the average BMI (body mass index) of the male students was slightly higher than that of female students (18.28 compared to 18.09, respectively). Female students were slightly more sensitive to salty tastes than male students (2.8 compared to 2.5, respectively). In the score distribution for each sense, touch sense was the highest at 7.59, sight sense was 7.49, hearing sense was 5.43, smell sense was 5.24, and taste sense was lowest at 3.69. Therefore, schoolchildren first tend to recognize and deem important the touch and sight of food before its taste.
