The Effects of Cervical Extension-Traction Exercise on Cervical Alignment, Pain, and Neck Disability in Patients with Mild Turtle Syndrome (경추 신전-견인 운동이 경증 거북목증후군 환자의 경추정렬, 통증, 기능장애에 미치는 영향)

  • Background: The purpose of this study was to determine the effects of cervical extension-traction exercise on cervical alignment, pain, and neck disability in patients with mild turtle syndrome. Methods: Thirty two outpatients with mild turtle neck syndrome were recruited and randomly divided into two groups. Participants in the experimental group was applied cervical extension-traction exercise (CETE, n=16) and in the control group applied cervical stabilization exercise (CSE, n=16) for three times a week for 4 weeks. Results: Cobb angle and Jochumsen depth were CETE showed significant difference within the group post test (p<.05). And the CETE was significantly higher than the CSE. In the pressure pain threshold, both CETE and CSE showed significant differences within post test (p<.05). And the CETE was significantly higher than the CSE. Neck disability index were significant (p<.05) in the CETE post test. There was no significant difference between the two groups. Conclusion: Our results of this study showed that applying cervical extension-traction exercise to patients with mild turtle syndrome improved cervical alignment, pain and neck dysfunction.

The Effects of Cervical Traction on the Pain, Range of Motion and the Quality of Sleep of the Older Women with Tension Headache (목뼈 견인치료가 긴장성 두통을 가진 노인 여성의 통증, 관절가동범위 및 수면의 질에 미치는 영향)

  • Purpose : The purpose of this work was to ascertain the effects of cervical traction on the pain, the range of motion and the quality of sleep of the older women with the tension headache. Method : 25 subjects between ages 66 and 82 who agreed on this experiment and diagnosed with tension headache participated in this study. All subjects were treated with air-pressure cervical traction while 20 minutes a day, 3 times per week, for 4 weeks. The effects of air-pressure cervical traction were evaluated by PSQI(Pittsburgh sleep quality index), a pain last a day, VAS, and four ROMs. In order to investigate the effect of age on the treatment, subjects were divided into two groups, of less and of over than age 75. The measured data were analyzed by using one-way repeated ANOVA and two-way repeated ANOVA. Result : 1) The group of less than age 75 showed the significant improvements in all measurement variables, PSQI, a pain last a day, VAS and four ROMs. 2) The group of over than age 75 showed the significant improvements in all measurement variables excluding the flexion ROM of cervical. 3) No significant differences between two groups were showed in all measurement variables, but the interaction effect between group and period was significant in only VAS. Conclusion : From the results of this study, a cervical traction intervention using an air-pressure cervical traction proved to be effective on the reduction of tension headache and the improvement of the quality of sleep.

Effect of nerve mobilization with intermittent cervical segment traction on pain, range of motion, endurance, and disability of cervical radiculopathy

  • Objective: This study aimed to evaluate the effects of the Kaltenborn-Evjenth concept of nerve mobilization combined with intermittent cervical segment traction (ICST) on pain, Neck Disability Index (NDI) scores, range of motion (ROM) and endurance in persons with cervical radiculopathy (CR). Design: Two-group pretest-posttest design. Methods: Thirty subjects participated in this study and were randomly assigned to two groups. The ICST group (n=15) was performed simultaneously with nerve mobilization and cervical traction for the segment with cervical pain at the same time. The intermittent cervical total traction (ICTT) group (n=15) performed nerve mobilization and cervical traction for the whole cervical area at the same time. In this study, outcome measures such as the Visual Analog Scale (VAS), NDI, ROM, endurance (cranio-cervical flexion test), and passive intervertebral motion performed before and 4 weeks after the experiment were compared to investigate the effects of each intervention. Results: In both groups, there were significant differences in the VAS, NDI scores, and endurance, and there were significant differences between the two groups except for endurance (p<0.05). In the ICST group, significant differences were found in all ROM, and in the ICTT group, significant differences were found in only extension, and there were significant differences between the two groups (p<0.05). Conclusions: The ICST group showed more improvement than the ICTT group in pain, NDI scores and ROM. Moreover, our findings show that the ICST could be used as a new strategy for manual therapy in persons with CR.

  • Joint mobilization and mechanical traction are common treatment forms for mechanical cervical spine problem. The purpose of the study was to investigate the effectiveness of cervical mobilization and mechanical traction on active range of motion of cervical spine. Sixty volunteers, aged between 21 and 24 years (mean age 22), were recruited. Each subject was divided into one of three groups; mechanical traction, general coordinative manipulation, and mobilization group. Active range of motions in the cervical were measured before and after each treatment technique from each subject on the three occasions. In the cervical range of motion, all subjects regardless treatment technique showed significantly increasing ranges after applied treatment technique in all directions except extension and left rotation in the mobilization group.

  • Background: This study examined the effects of the muscle energy technique and cervical traction after applying conservative physical therapy to patients with cervical radiculopathy. Methods: Twenty-six patients with cervical radiculopathy were randomized into two groups: the experimental group (n=13) and the control group (n=13). The two groups performed the muscle energy technique and for 4 weeks, 3 times a week, in 70-minute sessions. Pain, range of motion, function, and muscle performance were assessed using the visual analogue scale (VAS), cervical range of motion (CROM) goniometer, neck disability index (NDI), and stabilizer pressure biofeedback, before and after training. Results: After four weeks of therapy, the VAS (p<.001), NDI (p<.01), and the maximum strength of the deep neck flexion muscles significantly decreased (p<.01) and CROM significantly increased in both groups (p<.05). Conclusion: The muscle energy technique and therapeutic modalities such as cervical traction are effective in reducing VAS and NDI and increasing CROM and muscle performance in patients with cervical radiculopathy.

  • Objectives: The object of this study is to report a clinical effect of chuna manual theraphy for the soft herniated intervertebral cervical disc. Methods: The patient was treated by cervical traction technique which is one of the chuna manual theraphy with other conservative treatments including acupuncture, herbal mixture, bee-venom theraphy and TENS(Transcutaneous Electrical Nerve Stimulation). Visual analogue scale(VAS) have been used for result report. Results: After the cervical traction technique, the VAS was significantly improved. Conclusions: It is thought that the chuna manual theraphy might be effective to the patient with soft herniated intervertebral cervical disc.

  • Background: Cervical radiculopathy is the result of cervical nerve root pathology that may lead to chronic pain and disability. Although manual therapy interventions including cervical traction and neural mobilization have been advocated to decrease pain and disability caused by cervical radiculopathy, their analgesic effect has been questioned due to the low quality of research evidence. The purpose of this paper is to present the effect of manual therapy on pain, ROM, disability in a patient experiencing cervical radiculopathy. Methods: In this study, 30 participants who met the diagnostic criteria for cervical radiculopathy were randomized into two groups: group I (nerve mobilization group) & group II (Mckenzie exercise group). The experimental group was performed manual cervical traction and neural mobilization technique. The control group was performed manual cervical traction and cervical retraction, extension exercise. Assessments were performed to 30 participants before and after 4 weeks therapy. The components of assessments were pain intensity (PI), cervical rotation ROM (CR) and neck disability index (NDI). Results: After 4 weeks therapy, PI, CR and NDI were significantly reduced in both groups (p<.01). The PI, CR and NDI were no significantly reduced between group I and group II (p>.01). Conclusions: Manual therapy could reduce the symptoms of cervical radiculotpathy.

  • Background: Effect of cervical and lumbar tractions on the reduction in the angle of curvature and the effect of a correction exercise or a general traction method on balance, muscle strength, pain, and body alignment, however insufficient research has been undertaken on self traction exercises targeting patients with scoliosis. Purpose: To determine the effect of cervical and lumbar tractions on the reduction in the angle of curvature and the effect of a correction exercise or a general traction method on balance, muscle strength, pain, and body alignment. Design: Randomized controlled clinical trial (single blinded) Methods: Twelve adults(20s) with scoliosis were included in this study and performed a traction program that was composed of a 5-min warm-up exercise, a 15-min main exercise, and a 5-min cool-down exercise (25 minutes in total), three times a week for four weeks. The Chiro traction machine was used for the self-traction exercise. Vertebral alignment, muscle strength, and flexibility were compared before and after the intervention using the paired T-test. Results: The scoliosis angle, pelvic torsion, and lumbar extensor were significantly changed by intervention; however, there was no significant difference in flexibility. Conclusion: The results revealed that self-traction exercise activated blood flow through the extension and contraction of muscles, effectively increasing the function of the muscles around the vertebrae.

Effect of Cervical Spinal Decompression on the Cervical Muscle Tone and Disc Height in Patients with Cervical Intervertebral Disc Herniation

  • The purpose of this study was to investigate the effect of traction and decompression therapies on the cervical muscle tone and disc height. The decompression group (n=16) received decompression therapy and Mckenzie exercises once a day and four times a week respectively, for three weeks. The traction group (n=15) also received traction therapy and Mckenzie exercises for the same period. Muscle tone was measured with a myotonometer, and the disk height was measured using magnetic resonance imaging (MRI), before the interventions. Three weeks later, we investigated the therapeutic effect by repeating these measurements. The difference in disk height between the two groups was not significant. There was a significant difference in the disk herniation index (p<.05). A significant difference was found only in the upper trapezius muscle after comparison of muscle tone and stiffness between the groups (p<.05). Findings from this study suggest that the decompression therapy is a more effective intervention for patients with cervical intervertebral disc herniation.

The Effects of Neck Traction and Foot Type on Plantar Pressure Distribution during Walking (경추 견인기 부착 여부에 따른 발 형태별 보행 시 족저압에 미치는 영향)

  • Objective: The purpose of this study was to investigate the effect of neck traction and foot type on plantar pressure distribution during walking. Method: Total of 24 data were collected from women working with a computer for more than 6 hours every day. Three groups by foot type were divided: Pes Planus, normal foot, and Pes Cavus. Depending on the foot type and cervical traction, plantar pressure variables were measured; CA, MF, PP, and CT. Each variable was divided into 12 masks. MANOVA was performed for the difference of plantar pressure variables by foot type, and a paired t-test was performed for the cervical traction within groups. Results: The total CA decreased in the Pes Planus (p<.001) and Pes Cavus (p<.05) groups. MF increased in the big toe (p<.01) and 2nd toe (p<.05) of the normal foot, and MF-3rd metatarsal decreased (p<.01). The MF-2nd toe (p<.01) and 3rd toe (p<.05) of Pes Cavus decreased. The PP decreased in 2nd toe (p<.05), 3rd toe (p<.01), and 4th toe (p<.05) of the Pes Cavus. In normal foot, the PP-3rd metatarsal (p<.05) and PP-4th metatarsal (p<.01) reduced. In Pes Planus, PP decreased in the hindfoot (p<.05). In Pes Cavus group wearing a neck-tractor, the CT-hindfoot increased (p<.05). Conclusion: There was a significant change in the plantar pressure change by foot type after neck traction. When walking with a neck-tractor, the heel impact was alleviated in the Pes Planus, and the Pes Cavus showed the smooth and effective propulsion in the push-off. Overall, weight acceptance was effectively performed when walking with neck-traction. It was also found that the neck-tractor corrects the alignment of the neck, thereby creating a more stable gait pattern.
