Awareness of Turkish Female Adolescents and Young Women about HPV and their Attitudes Towards HPV Vaccination

  • Background: The aim of the study was to assess the knowledge about HPV and HPV vaccines and attitudes towards vaccination among the females aged 9-24 years in Turkey. Materials and Methods: Self-administered questionnaires were filled out individually by the participants covering demographic information, knowledge about HPV infection and HPV vaccines, attitudes towards vaccination, and the perceptions of them about their parental attitudes about vaccination. Results: Of the 408 subjects participating in the study, 41.6% (n=170) had heard of HPV. Thirty-three percent (n=136) knew the causal relationship between HPV and cervical cancer. Only 27.9% (n=114) of them knew that HPV vaccines can prevent cervical cancer. Eleven percent (n=46) of the females participating in the study were willing to be vaccinated, and only 1.4% (n=6) were already vaccinated at the current time. The main reason listed among the participants who were not willing to be vaccinated was lack of information. Conclusions: Awareness and knowledge of Turkish female adolescents and young women about HPV, relation with cervical cancer and prevention of cervical cancer by Pap smear and vaccine are still limited. If the most important barrier to vaccination, which is reported as lack of information, were to be addressed, it would greatly impact the decision-making and vaccine acceptance.

Development of a Risk Index for Prediction of Abnormal Pap Test Results in Serbia

  • Background: Serbia is one of the countries with highest incidence and mortality rates for cervical cancer in Central and South Eastern Europe. Introducing a risk index could provide a powerful means for targeting groups at high likelihood of having an abnormal cervical smear and increase efficiency of screening. The aim of the present study was to create and assess validity ofa index for prediction of an abnormal Pap test result. Materials and Methods: The study population was drawn from patients attending Departments for Women's Health in two primary health care centers in Serbia. Out of 525 respondents 350 were randomly selected and data obtained from them were used as the index creation dataset. Data obtained from the remaining 175 were used as an index validation data set. Results: Age at first intercourse under 18, more than 4 sexual partners, history of STD and multiparity were attributed statistical weights 16, 15, 14 and 13, respectively. The distribution of index scores in index-creation data set showed that most respondents had a score 0 (54.9%). In the index-creation dataset mean index score was 10.3 (SD-13.8), and in the validation dataset the mean was 9.1 (SD=13.2). Conclusions: The advantage of such scoring system is that it is simple, consisting of only four elements, so it could be applied to identify women with high risk for cervical cancer that would be referred for further examination.

Prevalence and Predicting Factors for Anxiety in Thai Women with Abnormal Cervical Cytology Undergoing Colposcopy

  • Aim: To compare prevalence of anxiety in women with abnormal cervical cytology (Pap) undergoing colposcopy to that of women attending the outpatient clinic for check-up and to examine predicting factors. Materials and Methods: In this cross-sectional analytical study, 100 women with abnormal cervical cytology (abnormal Pap group) and 100 women who attended our outpatient clinic for check-up (control group) were recruited from June 2013 to January 2014. The Hospital Anxiety and Depression Scale (HADS) was employed to determine anxiety in the participants with the score of ${\geq}11$ suggestive of clinically significant anxiety. The prevalence of anxiety and the mean HADS scores for anxiety were compared between the groups. For those with abnormal Pap, association between clinical factors and anxiety was assessed. A p-value of < 0.05 was considered significant. Results: Median age was different between the groups, 44.0 years in the abnormal Pap group and 50.0 years in the control group (p=0.01). The proportion of participants who had more than one sexual partner was higher in the abnormal Pap group, 39.2% vs. 24.7% (p=0.03) and the prevalence of anxiety was significantly higher 14/100 (14.0%) vs. 3/100 (3.0%) (p < 0.01). The prevalence of depression was comparable between the groups. The mean HADS scores for anxiety and depression subscales were significantly higher in the abnormal Pap group, 6.6 vs. 4.8 (P < 0.01) and 3.9 vs. 3.1 (p=0.05), respectively. For the abnormal Pap group, no definite association between clinical factors and anxiety was demonstrated. Conclusions: The prevalence of anxiety in women with abnormal Pap awaiting colposcopy was significantly higher than that of normal controls. Special attention including thorough counselling, with use of information leaflets and psychological support, should be directed to these women.

Comparison of Detection Sensitivity for Human Papillomavirus between Self-collected Vaginal Swabs and Physician-collected Cervical Swabs by Electrochemical DNA Chip

  • Background: Human papillomavirus (HPV) DNA testing is an effective method to screen for precancerous changes in the cervix. Samples from self-collection rather than Pap smear can potentially be used to test for HPV as they are more acceptable and preferred for use in certain settings. The objective of this study was to compare HPV DNA testing from self-collected vaginal swabs and physician-collected cervical swabs. Materials and Methods: A total of 101 self-collected vaginal and physician-collected cervical swabs of known cytology from Thai women were tested by electrochemical DNA chip assay. The specimens were divided into 4 groups: 29 with normal cytology, 14 with atypical squamous cells of undetermined significance (ASCUS), 48 with low-grade squamous intraepithelial lesion (LSIL), and 10 with high-grade squamous intraepithelial lesion (HSIL). Results: Positive detection rates of HPV from self-collected swabs were similar to those from physician-collected swabs. Among specimens with abnormal cytology, HPV was found in 50% of self-collected swabs and 47.2% of physician-collected swabs. In specimens with normal cytology, 17.2% of self-collected swabs and 24.1% of physician-collected swabs were positive for HPV. Concordance was relatively high between results from self-collected and physician-collected samples. The most common HPV genotype detected was HPV 51. Conclusions: HPV DNA testing using self-collected swabs is a feasible alternative to encourage and increase screening for cervical cancer in a population who might otherwise avoid this important preventive examination due to embarrassment, discomfort, and anxiety.

Comparison Between Two Detection Methods for HPV16, HPV18 and P16Ink4a Biomarkers in Diagnosis of Abnormal Cervical Cytology

  • Background: Cervical cancer is the second most common cancer among women in many populations. While the Pap smear is a well established screening test it suffers from both false-positive and false-negative results in diagnosis of cancers and precancerous states. In this study, immunocytochemistry of the P16 biomarker and HPV-PCR were compared for their diagnostic potential. Materials and methods: In the study, we obtained pairs of specimens from 45 women with cervical dysplasia. One sample was placed in a liquid-based solution, and processed for staining of sections with antibodies to P16. HPV-PCR was performed on the other and the results obtained were analyzed by T-test using SPSS v. 15. Results: Using HPV-PCR 71% of the samples were found to be infected with either HPV 16 or HPV 18, and the rate of infection did not have a statistically significant relationship with higher grades of dysplasia (p= 0.253). In contrast, with immunocytochemistry evaluation of P16, 64% of the specimens were positive, but the percentage of positive results significantly increased with higher grades of dysplasia (p= 0.0001). Conclusion: Employment of the P16 marker as an optional test might be preferable over HPV-PCR for cervical dysplasia in our geographical region.

결핵성 경부 임파선염의 임상적 고찰 (Clinical Investigation of Cervical Tuberculous Lymphadenitis)

  • 연구배경 : 폐결핵은 세계적으로 발병율이 점차 추세에 있으나 결핵성 임파선염은 그 빈도가 그대로 유지되고 있다. 하지만 아직도 치료기간을 어느 정도로 해야할 지, 임파선이 줄지 않을 경우 치료에 반응이 있는 것인지 판단하기가 어렵다. 그리고 다른 부위의 결핵과의 연관성도 많이 알려져있지 않아 결핵성 임파선염의 임상양상과 치료기간, 타 장기의 결핵과 관련성에 대하여 후향적 고찰을 시행하였다. 방 법 : 1990년 1월 1일부터 1996년 8월 30일 까지 본원 흉부내과에 내원하여 임파선의 세침홉인검사를 통해 결핵성 경부임파선염으로 진단 받은 환자 120명을 대상으로 후향적 고찰을 시행하여 다음과 같은 결과를 얻었다. 결 과 : 1) 결핵성 경부 임파선염은 20 대에서 53.3% 로 가장 호발하였으며 여지에서 2.5배 더 많았다. 경부의 종괴촉진을 주소로 내원하는 경우가 80.0%로 가장 많았다. 경부 임파선염은 후경부에 가장 호발하였으며 좌우측에 침범하는 빈도는 비슷하였다. 2) 침범된 임파선염의 세침홉인검사를 시행하여 조직학적 검사상 결핵성 임파선염으로 진단된 경우는 82.3%이었고 홉인검체물로 항산균도말검사를 시행하여 양성은 38.6%, 결핵균배양에서 17.6%가 양성이었다(p<0.001). 3) 폐결핵은 79예(65.8%)에서 동반되었으며 대부분 경증폐결핵이었다(53.8%). 폐외결핵은 14명(11.7%)에서 동반되었으며 그중 결핵성 늑막염이 6명(42.8%)으로 가장 많았다. 4) 치료는 항결핵제복용(1차약, EHRZ)을 원칙으로 하였으며 평균치료기간은 18.5개월이었다. 치료기간동안 75.8%는 점차 크기가 줄어들었으며 30.4%에서 새로운 임파선이 발생하였고 26.8% 에서 농양이 형성되었으며 17.0%에서 누공이 형성되었다. 결 론 : 결핵성 임파선염은 젊은 여자에서 호발하는 질병으로 경부 임파선을 가장 많이 침범한다. 결핵성 임파선염은 전신적인 질환의 한 표현으로 여겨지며 타 장기의 결핵과 동반되는 경우도 빈번하다. 이는 대부분 항결핵제치료로 완치될 수 있으며 치료기간은 약 18~24개월을 요한다. 항결핵제를 계속 복용함에도 불구하고 크기가 커지거나 새로운 임파선염이 생가고 농양, 누공이 형성될 수 있으나 항결핵제로 지속적인 치료를 요하며 임파선의 세침흡인검사를 반복하여 결핵균이 있는지 여부를 보는 것도 치료효과를 판정하는데 도움을 주리라 생각한다. 충분한 결핵약 치료에도 불구하고 계속 커지거나 농양, 누공이 생겨 환자에게 불편을 초래하는 경우에는 완전절제생검를 고려해본다.

Susceptible and Protective Associations of HLA Alleles and Haplotypes with Cervical Cancer in South India

  • Background: Human leukocyte antigen (HLA) genes have been implicated in cervical cancer in several populations. Objectives: To study the predispositions of HLA alleles/haplotypes with cervical cancer. Materials and Methods: Clinically diagnosed and PAP smear confirmed cervical cancer patients (n 48) and age matched controls (n 47) were genotyped for HLA-A,-B,-DRB1* and DQB1* alleles by PCR-SSP methods. Results: The frequencies of alleles DRB1*04 (OR=2.57), DRB1*15 (OR=2.04), DQB1*0301 (OR=4.91), DQB1*0601 (OR=2.21), B*15 (OR=13.03) and B*07 (OR=6.23) were higher in cervical cancer patients than in the controls. The frequencies of alleles DRB1*10 (OR=0.22) and B*35 (OR=0.19) were decreased. Strong disease associations were observed for haplotypes DRB1*15-DQB1*0601 (OR=6.56; p< $3.5{\times}10^{-4}$), DRB1*14-DQB1*0501 (OR=6.51; p<0.039) and A*11-B*07 (OR=3.95; p<0.005). The reduced frequencies of haplotypes DRB1*10-DQB1*0501 (OR=0.45), A*03-B*35 (OR=0.25) and A*11-B*35 (OR= 0.06) among patients suggested a protective association. HLA-C* typing of 8 patients who possessed a unique three locus haplotype 'A*11-B*07-DRB1*04' (8/48; 16.66%; OR=6.51; p<0.039) revealed the presence of a four locus haplotype 'A*11-B*07-C*01-DRB1*04' in patients (4/8; 50%). Amino acid variation analysis of susceptible allele DQB1*0601 suggested 'tyrosine' at positions ${\beta}9$ and ${\beta}37$ and tyrosine-non-tyrosine genotype combination increased the risk of cervical cancer. Conclusions: Strong susceptible associations were documented for HLA alleles B*15, B*07, DRB1*04, DRB1*15, DQB1*0301, DQB1*0601 and haplotypes DRB1*15-DQB1*0601 and DRB1*14-DQB1*0501. Further, protective associations were evidenced for alleles B*35 and DRB1*10 and haplotypes A*11-B*35 and DRB1*10-DQB1*0501 with cervical cancer in South India.

Glutathione S-transferase M1 and T1 Polymorphisms, Cigarette Smoking and HPV Infection in Precancerous and Cancerous Lesions of the Uterine Cervix

  • Glutathione S-transferases (GSTs) play an important role in detoxification of carcinogenic electrophiles. The null genotypes in GSTM1 and GSTT1 have been implicated in carcinogenesis. Present study was planned to evaluate the influence of genetic polymorphisms of GSTM1 and GSTT1 gene loci in cervical carcinogenesis. The study was conducted in Lok Nayak hospital, New Delhi. DNA from clinical scrapes of 482 women with minor gynaecologic complaints attending Gynaecology OPD and tumor biopsies of 135 cervical cancer cases attending the cancer clinic was extracted. HPV DNA was detected by standard polymerase chain reaction (PCR) using L1 consensus primer pair. Polymorphisms of GSTM1 and GSTT1 were analysed by multiplex PCR procedures. Differences in proportions were tested using Pearson's Chi-square test with Odds ratio (OR) and 95% confidence interval (CI). The risk of cervical cancer was almost three times in women with GSTM1 homozygous null genotype (OR-2.62, 95%CI, 1.77-3.88; p<0.0001). No association of GSTM1 or GSTT1 homozygous null genotypes was observed in women with normal, precancerous and cervical cancerous lesions among ${\leq}35$ or >35 years of age groups. Smokers with null GSTT1 genotype had a higher risk of cervical cancer as compared to non-smokers (OR-3.01, 95% CI, 1.10-8.23; p=0.03). The results further showed that a significant increased risk of cervical cancer was observed in HPV positive smoker women with GSTT1 (OR-4.36, 95% CI, 1.27-15.03; p=0.02) and GSTM1T1 (OR-3.87, 95% CI, 1.05-14.23; p=0.04) homozygous null genotypes as compared to HPV positive non smokers. The results demonstrate that the GST null genotypes were alone not associated with the development of cervical cancer, but interacted with smoking and HPV to exert effects in our Delhi population.

도말층 제거와 상아질의 부위가 치수강 내부 상아질에 대한 수종 상아질 결합제의 전단결합성질에 미치는 영향 (THE EFFECT OF SMEAR LAYER REMOVAL AND POSITION OF DENTIN ON SHEAR BOND PROPERTIES OF DENTIN BONDING SYSTEMS TO INTERNAL CERVICAL DENTIN)

  • The aim of this study was to determine the shear bond properties of four dentin bonding systems to internal cervical dentin, and to investigate the effect of the pretreatment for removing smear layer and position of dentin on shear bond strength of dentin bonding agents. The materials tested in this study were consisted of four commercially available dentin bonding systems[Allbond 2(AB), Clearfil Linerbond 2(CL), Optibond FL(OP), Scotchbond Multi-purpose(SB)], a restorative light-cured composite resin[Z100]J and a chelating agent[RC-prep(RC)]. Fifty-six freshly extracted human molars were used in this study. Dentin specimens were prepared by first cutting the root of the tooth 1mm below the cementoenamel junction with a diamond bur in a high speed handpiece under air-water coolant, and then removing occlusal part at pulp horn level by means of a second parallel section, The root canal areas were exposed by means of cutting the dent in specimens perpendicular to the root axis. Dentin specimens were randomly assigned to two groups(pretreated group, not-pretreated group) based on the pretreatment method of dentin surface. In pretreated group, RC was applied to dentin surface for 1minute and then rinsed with NaOCl. In not-pretreated group, dentin surface was rinsed with saline Each groups were subdevided into four groups according to dentin bonding systems. Four dentin bonding systems and a restorative resin were applied according to the directions of manufacturer. The dentin-resin specimens were embedded in a cold cure acrylic resin, and were cut with a low speed diamond saw to the dimension of $1{\times}1mm$. The cut specimens were divided into three groups according to the position of internal cervical dentin. The shear bond properties of dentin-resin specimens were measured with Universal testing machine (Zwick, 020, Germany) with the cross head speed of 0.5mm/min. From this experiment. the following results were obtained : 1. In case of shear bond strength, there was no significant difference among dentin bonding systems in not-pretreated groups, whereas in pretreated groups, the shear bond strengths of AB and of SB were statistically significantly higher than those of CL and of OP. 2. The shear bond strengths of AB and of SB in pretreated groups were significantly higher than those in not-pretreated groups. 3. The shear bond strengths of radicular layer of OP were higher than those of occlusal layer of OP in not-pretreated groups, and of AB in pretreated groups. The shear bond strengths of radicular layer of AB and of CL in not-pretreated groups were higher than those in pretreated group.

자궁경부질 도말상 선암종의 세포학적 분석 (Cytologic Analysis of Adenocarcinoma of the Cervicovaginal Smears)

  • We have investigated the cervicovaginal smears in order to define the cytologic features of uterine adenocarcinomas. Total 22 cases were reviewed(12 cases from the Sanggye Paik hospital, Inje university and 10 cases from the Korea Cancer Center Hospital) from January 1992 to December 1997. Five cases were endometrial adenocarcinomas and the remaining 17 cases were cervical adenocarcinomas. Seventeen cases of cervical adenocarcinomas were divided into endometrioid(6 cases), endocervical(7 cases), mixed endometrioid and endocervical(1 case), papillary (2 cases), and adenosquamous(1 case) carcinomas. The background of endocervical adenocarcinoma was hemorrhagic or inflammatory. The tumor diathesis was less prominent than that of the squamous cell carcinoma. The prominent features of the endocervical type adenocarcinomas were large and loose clusters, large intracytoplasmic vacuoles, and prominent overlapping and peripheral palisading of nuclei. In contrast, the endometrioid adenocarcinomas showed small and compact clusters, and small intracytoplasmic vacuoles. The detection rate of endometrial adenocarcinoma was lower than that of the endocervical adenocarcinoma.
