Effects of a Combination of Scapular Stabilization and Thoracic Extension Exercises on Respiration, Pain, Craniovertebral Angle and Cervical Range of Motion in Elementary School Teachers with a Forward Head Posture: A Randomized Controlled Trial

  • PURPOSE: This study examined the effects of an exercise program for the thoracic spine and scapula rather than the neck, which is the primary site of pain. METHODS: Thirty-two elementary school teachers with a forward head posture (FHP) were assigned randomly to either the experimental group (n = 16) or the control group (n = 16). The experimental group performed scapular stabilization exercise (SSE) and thoracic extension exercise (TEE), and the control group performed cervical self-myofascial release exercise and stretching exercise. The pulmonary functions, pain, craniovertebral angle (CVA), and cervical range of motion (CROM) were measured before the intervention and six weeks after. RESULTS: The within-group comparisons showed that the VAS and CROM (except for extension) in both groups were significantly different before and after the intervention (p < .05). The changes in the maximum inspiratory pressure (MIP), maximum expiratory pressure (MEP), forced vital capacity (FVC), forced expiratory volume at 1 sec (FEV1), and CVA were significant only in the experimental group (p < .05). The between-group comparisons showed a significant difference in the FVC, FEV1, VAS, CVA, and left lateral flexion (p < .05). CONCLUSION: The combination of SSE and TEE in the experimental group was more effective in improving the FHP and breathing ability. Moreover, the experimental group and control combination appeared to be effective in reducing pain and improving the CROM. The combination of SSE and TEE, which are exercises that do not target the cervical spine directly, was effective in improving the posture, respiration, neck pain, and CROM in elementary school teachers with FHP.

Effects of Combined Cervical Stabilization and Stretching Exercises on Craniovertebral Angle, Respiration, Disability, and Range of Motion in Office Workers with Forward Head Posture : A Randomized Control Trial

  • Purpose : The purpose of this study was to investigate the effects of combined cervical stabilization exercise (CSE) and stretching exercise (SE) on office workers with forward head posture (FHP). Methods : A total of 32 office workers with forward head posture were randomly assigned to experimental (n=16) and control (n=16) groups. The experimental group underwent combined CSE and SE, and the control group underwent cervical self-myofascial release and SE. Both groups performed exercises for 40 min per day, thrice per week for a total of 6 weeks. Craniovertebral angle (CVA), respiration, disability, and joint range of motion (ROM) before and 6 weeks after intervention were measured and compared. Results : There was no significant between-group difference in the general characteristics (p>.05). The intra-group comparison showed significant differences in the visual analog scale (VAS) and neck disability index (NDI) of both groups post-intervention (p<.05). CVA and forced expiratory volume in 1 seconds (FEV1) were significantly improved post-intervention in the experimental group only (p<.05). In the experimental group, all ROM variables were significantly improved post-intervention. In contrast, in the control group, all ROM variables improved significantly post-intervention, except for extension (p<.05). The inter-group comparison showed significant differences in NDI, left lateral flexion, right lateral flexion, and left rotation between the two groups (p<.05). Conclusion : The combination of CSE and SE, which stabilizes the cervical spine, had positive effects on cranial rotation angle, respiration, disability, and joint ROM in office workers with forward head posture. Therefore, the combination of the two exercises may be an effective option to reduce symptoms and prevent postural problems in office workers with FHP.

Effects of Occipital Bone Stimulation by Cervical Stabilizing Exercise on Muscle Tone, Stiffness, ROM and Cervical Lordosis in Patient with Forward Head Posture: Single System Design

  • The purpose of this study was to investigate the effects of occipital bone stimulation by cervical stabilizing exercise on the muscle tone, stiffness, ROM, and cervical lordosis in patient with forward head posture(FHP). This study was a case study of a single patient with forward head posture. This study used a ABA' design, A and A' were the baseline phases and B was the intervention phase. The intervention was occipital bone stimulation by cervical stabilization exercise. It was administered once daily for 7 days. The therapist kept hands together, and placed the two index fingers under the subject's occipital bone. The subject performed the chin-in exercise with a maximum isometric contraction for 20 sec.The exercise was implemented by performing the movements 10 times as a set and repeating the set three times. The muscle tone was not significantly changed after intervention. However, the stiffness was decreased and lasted the effect lasted without intervention. The cervical flexion angle was increased, but the cervical extension angle was not significantly changed after the intervention. The left and right lateral flexion angles were increased and the effect lasted without any intervention. However, the left and right rotation angles were significantly changed after the intervention. Cervical lordosis increased not from $37^{\circ}$ to $41^{\circ}$ after the intervention. These results suggest that occipital bone stimulation by cervical stabilizing exercise had a positive effect on cervical stiffness, flexion and lateral flexion ROM, and lordosis in a patient with forward head posture.

8주간의 척추 안정화 운동 프로그램이 만성 경부통 환자의 NDI, 균형 능력 및 자세 변화에 미치는 영향 (The effects of 8-week spinal stabilization exercise program on NDI, postural balance and body shape change in patients with chronic neck pain)

  • 이 연구는 8주간의 척추 안정화 운동프로그램이 경부 장애 지수, 균형 능력 및 자세 변화에 어떠한 영향을 주는지를 알아보는 것이다. 운동 프로그램은 8주간 주 3회 척추 안정화 운동을 시행하였으며, 총 60분으로 구성하였다. 연구의 대상자는 6개월간 경부통을 호소하는 만성 경부통 환자 16명을 대상으로 운동 집단(n=8명)과 통제 집단(n=8명)으로 분류하여 운동 프로그램 사전, 사후의 결과를 관찰하여 다음과 같은 결과를 도출하였다. 경부 장애 지수는 시기, 집단, 상호작용에서 유의차가 나타났다(p<.05). 균형 능력은 집단 및 시기 간에서 유의한 상호작용 효과가 나타났다(p<.05). 자세 변화는 골반 경사각에서 시기, 상호작용에서 유의차가 나타났고(p<.05), 경부 및 어깨 위치각은 집단, 시기, 집단, 상호작용 모두 유의차가 나타났다(p<.05). 이상의 결과 척추 안정화 운동이 만성 경부통 환자에게 경부 장애 지수, 균형 능력 변화, 자세 변화에서 유의하게 개선된 결과를 나타내었다. 추후 연구에서는 방사선 영상을 통한 척추 구조 변화에 대한 구체적인 변화를 분석하여 척추 안정화 운동의 타당성을 높이는 연구가 필요할 것으로 사료된다.

  • 본 연구는 전방머리자세 대학생을 대상으로 목뼈안정화운동이 머리뼈척추각, 깊은목굽힘근 지구력 및 뒤통수밑근 근긴장도의 변화를 알아보고자 하였다. 이 연구 결과 CSEG에서 CVA, DNFET에서 유의하게 증가하였으며 뒤통수근 근긴장도에서는 경직도에서만 유의하게 감소하였다. 목뼈 안정화운동은 목뼈의 중립자세를 유지하면서 깊은목 굽힘근 활성화되고 근지구력이 향상되었으며, 턱을 당기는 동작으로 튀통수근의 경직이 감소한 것으로 생각된다. 또한 지구력 증가시키고 근 긴장도에 감소는 통증을 조절함에 있어서도 도움이 될 것이다. 따라서 목뼈안정화운동은 깊은목굽힘근과 뒤통수근의 개선을 위한 중재방법임을 확인할 수 있었으며, 전방머리자세 예방하기 위해 필요할 것으로 생각된다.

Effects of Stabilization Exercise Applying Sling Neurac Method on Pain and Cervical Alignment in Young Adults with Forward Head Posture

  • 이 연구의 목적은 전방머리자세를 가진 젊은 성인을 대상으로 슬링뉴렉안정화운동이 중재방법이 통증 및 목뼈 앞굽음각, 중력중심선에 어떠한 영향을 미치는지 알아보고자 실시되었다. 연구의 대상자는 슬링뉴렉안정화운동그룹(SNSEG) 10명, 통제그룹(CC) 10명으로 선정하였으며, 8주간 주3회 70분간 실시하였다. 이 연구 결과 슬링뉴렉안정화운동은 통증은 감소(p<.001), 목뼈 앞굽음각은 증가(p<.001)와 중력중심선은 감소에(p<.01) 유의한 효과가 나타났다. 이상의 결과 종합해 보면 슬링뉴렉안정화운동은 속근육(LM)군의 고유수용성감각 개선 및 근활성화에 효과적이며, 대근육(GM)군과 공동활성화하여 앞굽은각과 중력중심선에 개선에 효과적인 것으로 나타났다. 따라서 깊은목굽힘근과 뒤통수밑근의 재활성화는 통증 조절과 자세 정렬에 중요한 요소로 전방머리자세 개선시킬 수 있는 효과적인 중재방법으로 제안한다.

Effects of the Cranio-cervical Static Stabilization Exercises among the Using Small Tools

  • PURPOSE: The purpose of this study is to determine the differences between the muscle activity of cranio-cervical flexion and extension muscles according to the types of tools used through a short-term intervention of cranio-cervical static stabilization exercises using small tools. METHODS: A total of fifteen male and female adults in their 20s who showed forward head posture in the overall body posture measurement system participated in this study. Each subject performed cranio-cervical static stabilization exercises about flexion and extension while using a sling, a foam roller, a TOGU ball, and without tools separately, and the muscle activity of the sternocleidomastoid muscle, scalenus anterior and splenius capitis was measured. Each value was measured for 10 seconds a total of three times. The maximum voluntary isometric contraction value was computed using the average during the middle four seconds. RESULTS: Cranio-cervical flexion exercises using various tool types, the average activity of the sternocleidomastoid and scalenus anterior muscles was significantly higher when applying the TOGU ball (p<.05). According to the results of implementing cranio-cervical flexion exercise using various tools, the maximum muscle activity of the sternocleidomastoid muscle was significantly higher for the TOGU ball (p<.05). CONCLUSION: Based on these results, the provision of an unstable surface using small tools rather than a stable surface is recommended as an exercise scheme for proprioceptive stimulation in a forward head posture. Particularly, we recommend using the TOGU ball for the provision of an unstable surface to increase the muscle activity of the sternocleidomastoid muscle and scalenus anterior.

전방머리자세에서 목근육 근활성도에 대한 맥켄지 신장운동과 어깨 안정화운동의 효과 비교 (Comparison between McKenzie Stretch Exercise and Scapula Stability Exercise on Neck Muscle Activation in the Forward Head Posture)

  • Purpose : The purpose of this study was to compare the effect of exercise on the neck muscles activities between scapular stability and McKenzie stretch exercise in the forward head posture subjects. Method : After measuring cervical alignment using the Global Posture System, 20 volunteers with forward head posture posture were selected and divided into two groups. The experimental group A(n=15) and Experimental group B(n=15) participated in respectively and McKenzie neck stretching exercise, three times per week for 4 weeks. The scapular stabilization exercise program was comprised middle and lower trapezius strength exercises and the stretching exercise program was comprised levator scapulae and upper trapezius stretching exercise. The activities of the muscles of the posterior neck was then measured using electromyography. Result : After the intervention, there was significant difference of a electromyography activity changes between the pre-test and post-test in the experimental group. Conclusion : This study showed that both scapular stabilization and McKenzie neck stretching exercises are more effective for reducing neck muscles activities.

경추안정화운동과 경추관절가동술이 경부의 최대근력과 정적근지구력에 미치는 영향 (The effect of cervical stabilized exercise and joint mobilization on maximum muscle strength and static muscle endurance of cervical region)

  • 경추안정화 운동과 경추관절가동술이 경부의 최대근력과 정적근지구력에 미치는 영향을 알아보기 위해 60명을 경추안정화운동군, 경추관절가동술군, 대조군의 3개의 군으로 나누어 각 그룹당 20명씩 무작위 배치하였다. 경추관절가동술과 경추안정화운동은 일주일에서 3회씩 총 3주 동안 시행되었고, 경추안정화운동군은 선자세, 바로누운자세, 엎드려누운자세에서 경추심부근육의 등척성운동을 적용하였다. 경추관절가동술군은 칼텐본 기법을 이용한 관절내 운동을 적용하였다. 경추안정화운동과 경추 관절가동술 모두 최대근력과 정적근지구력이 증가하였고 그 중 경추안정화운동이 경추관절가동술보다 최대근력과 정적근지구력에 더 많은 영향을 미쳤다.

목 안정화 운동이 경추성두통 환자의 근육특성과 근활성도 및 자세에 미치는 영향 (Effects of Neck Stabilizing Exercise on Muscle Characteristics, Muscle Activity and Posture in Patients with Cervicogenic Headache)

  • Purpose : The purpose of this study was to provide an effective method of exercise therapy for patients with cervicogenic headache. Methods : The subjects were divided into the following two groups according to the intervention received: cervical stretch exercise (n=15, control group) cervix-stabilizing exercise (n=15, experimental group) tone (Hz) and stiffness (N/m) of the suboccipital and upper trapezius muscles were measured. T1 slope angle and neck tilt angle were measured. After the exercise program intervention, a greater amount of change in muscle tone and stiffness of suboccipital and upper trapezius muscles was found in the experimental group, as compared to the control group. Greater amount of change in posture was found in the experimental group, as compared to the control group (p<0.05). Results : After the exercise program intervention, a greater amount of change in muscle tone and stiffness of suboccipital and upper trapezius muscles was found in the experimental group, as compared to the control group. Greater amount of change in posture was found in the experimental group, as compared to the control group (p<0.05). Conclusion : The neck-stabilizing exercise were shown to be effective in decreasing the tone of the cervical muscles by stabilizing the cervical bone and improving muscle activity, and in improving the posture by decreasing muscle tone and stiffness.
