경추 추간판탈출증에 의한 브라운-시쿼드 증후군 : 수술 후 도수치료와 운동치료 효과-사례연구 (Brown-Sequard Syndrome Produced by Cervical Disc Herniation : Manual and Exercise Therapy after Operation-Case Studies)

  • 목적 : 브라운시쿼드는 대부분 척추손상과 수질외 척추 종양에서 주로 나타난다. 드물게 경추 디스크가 브라운시쿼드의 원인이 된다. 경추디스크에 의한 브라운시쿼드 증세의 수술후 물리치료 및 도수치료 결과를 보고하기 위함이다. 방법 : 50세 남자로써 브라운시쿼드 증세로 진단을 받고 수술후 좌측 팔과 다리에 운동신경에 의한 마비증세가 있었으며(팔>다리), 우측으로 감각과 온도감각이 저하된 경우이다(팔>다리). 측정방법은 통증지수(VAS), 근력(Distal PowerTracII$^{TM}$ test), 지구력(Ergometer) 측정과 심리상태(설문)를 치료전과 후를 비교하였다. 물리치료에서는 기능적 전기자극 치료와 도수치료 및 운동치료 방법을 실시하였다. 도수치료는 통증완화와 근력강화를 위한 MET, MFR, Mobilization 등을 실시하였으며, 운동은 슬링시스템 등을 이용한 운동과 견관절의 불안정을 위해 안정화운동을 실시하였다. 결과 : 이 케이스는 수술후 이상 징후가 척수압박으로 인하여 보다 넓게 통증이 나타났으며, 운동 및 감각신경이 둔해지고 온도에 대한 감각이 반대편 결손으로 나타났으며, 좌측 어깨, 팔 견갑부의 근육 마비와 우측의 감각이 떨어진 현상이 나타났다. 물리치료 후 단기목표와 장기목표에 있어서 통증과 운동 및 감각 기능이 회복되어 각각 팔 통증에서는 VAS 8 ${\rightarrow}$ 1, 상지 하지의 운동기능은 Trace ${\rightarrow}$ Good 로 평가 회복되었으며, 근력측정에서 모두 유의한 차이를 보였다. 모든 치료과정 결과에서 심리적 상태의 설문에서도 높은 점수를 얻어 긍정적 신뢰가 높아 진 것으로 나타났다. 검사결과 다리의 근력이 증가는 걷기 운동 및 에르고메터의 지구력 및 균형이 레벨1의 10분 수행능력이 레벨 20에서 30분 수행능력으로 향상되어 일상적인 활동이 가능해졌다. 결론 : 예상하지 못했던 수술 후유증(side effects)에 대한 치료과정이 환자의 심리에 심각한 부정적인 생각이 신체의 기능과 감정의 손상에 영향을 미치기 때문에 체계적이고 장기적인 치료 과정에서 기능적 향상과 더불어 정신적인 심리의 정서 안정이 매우 필요하다고 사료된다.

전방 경추 수술 후 발생한 세균성 뇌수막염과 동반된 척수염 (Bacterial Meningitis Complicated by Myelitis Following Anterior Cervical Spinal Surgery)

  • 척추 수술 후 뇌수막염은 매우 드물게 발생하지만 치명적일 수 있다. 49세 남자 환자가 제5, 6 경추부의 추간판 탈출증에 의한 압박성 척수증 진단하에 전방 경추부 감압술 및 유합술을 시행받았다. 술 후 심한 경부통 및 경부 강직, 발열이 발생하였고 수술 일주일 째 갑자기 사지 마비 증상을 보였다. 추시 자기공명영상 검사 및 뇌척수액 검사상 세균성 뇌수막염과 동반된 척수염이 확인되었다. 환자는 항생제 및 스테로이드 치료를 시행받았으나 결과는 불량하였다. 저자들은 전방 경추 수술 이후 발생한 뇌수막염과 동반된 척수염 증례에 대하여 문헌 고찰과 함께 보고하고자 한다.

  • Objective : It Is known that depression disorder has been related to chronic pains such as HIVD and physical harm. We propose that chronic cervical and lumbar herniations of intervertebral disc patients have emotional and psychiatric problems, therefore we compared it to Beck's Depression Inventory scores. Method : We divided them into two groups: The groups consisted of cervical and lumbar HIVD patients. We then requested them to fill out BDI research questionnaires, and evaluated patients according to the information and results. Result : The Beck's Depression Inventory mean score for the cervical - lumbar HIVD group was $14.00{\pm}5.80$, $10.83{\pm}$5.64 each. Higher Scores were recorded for cervical HIVD group than the lumbar HIVD group. Conclusion : There is statistical significance among two groups.(p<.05) however, the two groups' BDI score were lower than the defined boundary line of Depression disorder(16 points).

목덜미 통증 환자의 방사선 소견 (Radiologic Finding of Patients with Neck Pain)

  • Disorders related to the cervical spine can present a variety of signs and symptoms because of the many tissues and systems associated with this region. Therefore the challenge for the clinician is to organize the information from examination into a treatment plan that addresses the potential influences of the problem. the examination process should gather pertinent information from the patients, relating the pain pattern resulting loss of function. Our patients' X-rays showed absence of any abnormalities except loss of curvature. We therefore concluded that the main cause of posterior neck and shoulder pain was caused by muscle spasm, not cervical disc herniation not degenerative change.

Spondylodiscitis after Cervical Nucleoplasty without Any Abnormal Laboratory Findings

  • Infective spondylodiscitis is a rare complication that can occur after interventional spinal procedures, of which symptoms are usually back pain and fever. Early diagnosis of infective spondylodiscitis is critical to start antibiotics and to improve prognosis. Laboratory examinations including complet blood cell count (CBC), erythrocyte sedimentation rate (ESR), and C-reactive protein (CRP) are conventional tools for the early detection of infectious spondylitis. However, we experienced infective spondylodiscitis after cervical nucleoplasty which did not display any laboratory abnormalities, but was diagnosed through an MRI. A patient with cervical disc herniation received nucleoplasty at C5/6 and C6/7. One month later, the patient complained of aggravated pain. There were neither signs of chill nor fever, and the laboratory results appeared normal. However, the MRI findings were compatible with infectious spondylodiscitis at the nucleoplasty site. In conclusion, infectious spondylodiscitis can develop after cervical nucleoplasty without any laboratory abnormalities. Therefore, an MRI should be taken when there is a clinical suspicion for infection in order to not miss complications after interventional procedures, even if the laboratory findings are normal.

적외선체열촬영을 통한 편측 경항상지통환자의 임상적 고찰 (Analysis about DITI of the Patients having a Neck and Upper Extremity Pain on One Side)

  • Objectives : We studied to discover the patterns of DITI of the patients having a both neck and upper extremity pain on one side without any nerve root compression sign and the history of cervical disc herniation. Method : We selected 26 patients as an experimental group, who had visited for a both neck and upper extremity pain on one side without any nerve root compression sign, at the department of acupunture and moxibustion in Conmaul oriental medical hospital, during 2001. 01. - 2003. 09. And 31 normal adults were selected as a control group, not having any neck or arm pain and any history of cervical disease. We analyzed the segmental temperature statistically with t-test. Results & Conclusion : There was a significant change of temperatures in the scapula, posterior brachium, lateral forearm and dorsal hand area. And the experimental group showed cold spot and disruption of normal thermographic shape that were usually shown on the cervical sprain.

Effects of Nonsurgical Spinal Decompression Treatment on the Level of Pain and Quality of Life in Patients with Cervical or Lumbar Disc Herniation: A Retrospective Observational Study

  • Background: This study aims to statistically analyze and compare the curative effect and satisfaction level between typical traditional Korean medicine treatment and nonsurgical spinal decompression treatment. Methods: Of the patients who were diagnosed with the cervical or lumbar herniated intervertebral disc at the Department of Acupuncture and Moxibustion Medicine at the Daejeon Korean Medicine Hospital from April 14th to August 25th, 2019, this study retrospectively analyzed the medical records of 31 patients who underwent nonsurgical spinal decompression treatment and traditional Korean medicine (assigned to Group A) and another 31 patients who received typical traditional Korean medicine alone (assigned to Group B). The clinical data were analyzed using IBM SPSS Version 23.0. Results: No statistically significant differences appeared in terms of sociodemographic, condition, and therapeutic characteristics, except whether the patient received Western medicine treatment, before or after a treatment with traditional Korean medicine. Group A exhibited higher variations in numeric rating scale, EuroQol- 5 dimension and EuroQol visual analogue scale scores compared to Group B as determined by independent sample t test and analysis of covariance. In addition, the satisfaction score of Group A was higher than that of Group B The result of cross analysis revealed that desire for continued treatment in Group A was higher than that of Group B. Conclusion: This retrospective observational study showed that the patients with nonsurgical spinal decompression treatment reported a greater reduction in pain, improved quality of life and satisfaction than patients receiving typical traditional Korean medicine.

교통사고 이후 디스크 탈출이 없이 상지 저림을 호소하는 환자의 경추 측굴과 상지 저림에 관한 연구 (The Study on Relation between Cervical Lateroflexion and Upper Limb Numbness of Patients without Disc Herniation after Traffic Accident)

  • Objectives : The purpose of this study is to investigate the relation between cervical spine lateroflexion and upper limb numbness after whiplash injury by traffic accident. Methods : Outpatients who visited Jaseng korean medicine hospital after traffic accident took cervical MRI. Patients who had Normal disc and bulging disc were reviewed to measure the cervical lateroflexion by C2-C7 Cobb's angle & scalenus muscle's length through neutrality AP X-ray views. For statistics, we used SPSS version 18.0 for windows. Results : Groups classified into difference of scalenus muscle's length were showen statistical significance than into cobb's angle. Means of numbness group's length difference are $4.18{\pm}2.26mm$ and that of non-numbness group is $1.59{\pm}1.17mm$. Unilateral numbness group had greater angle and longer of length's difference than non-numbness group. Conclusions : The more severe the lateroflexion of the upper extremity numbness occurs well. Group classifed into difference of scalenus muscle's length has more tendency of occurrence of upper limb numbness than that into cobb's angle. And upper limb numbness occurs more frequently at the same direction of lateroflexion.
