Effects of Cervical Stabilization Exercise in patients with Cervical Artificial Disc Replacement

  • Purpose: This study was to identify the effect of cervical stabilization exercise on pain and structure in patients with cervical artificial disc replacement. Methods: Forty-four individuals with cervical artificial disc replacement volunteered to participate from FEB 2012 to MAR 2013 in this study. They were allocated to either Experimental Group (EG) or Control Group (CG), with 22 subjects in each group. Subjects from the EG performed cervical stabilization exercise program and subjects from the CG performed isometric exercise program. Assessment tools were made with the Visual Analogue Scale (VAS), Neck Disability Index (NDI), and Cervical Lordosis Angle (CLA). Results: In this study, in within-group and between-group comparison, the EG and CG showed significant differences in all parameters(p<0.05). But EG showed more improvement than CG at all parameters. Conclusion: These findings suggest that cervical stabilization exercise may be favorably used to improve VAS, NDI and CLA in patients with cervical artificial disc replacement. Further studies with larger sample and long-term follow-up period need to generalize the results of this study.

  • Cervical disc herniations can press on the spinal cord and cause a problem called cervical myelopathy. The most common symptom of cervical disc herniation is neck pain that spreads down to the upper limb in various locations. There can also be associated with weakness and movement disorders of upper limb. In Oriental medicine, cervical disc herniation is treated with herb-med, physical therapy, acupuncture, Chuna, etc, but the Bee Venom therapy is the most common and effective. In this case, we used the Bee Venom therapy at cervical hyeopcheokhyeol for about 2 months to a patient who was diagnosed with disc herniation at Cervical 5-6, 6-7 and appealed weakness, limited elevation and abduction of the left upper limb. As a result, left upper limb disability was improved. Using the Bee Venom therapy at cervical hyeopcheokhyeol that are effective on movement disorders and neurological diseases of upper limb is an effective treatment to upper limb disability diagnosed with cervical disc herniation, and suggests the direction of the treatment to upper limb weakness and movement disorders diagnosed with cervical disc herniation.

초음파 영상을 통한 머리-목 굽힘 운동의 적정 수준 제시에 관한 연구 (The suggestion of Proper Pressure level in Cranio-Cervical Flexion Exercise for Deep Cervical Flexor by Ultrasonographic Measurement)

  • Purpose : Chronic cervical pain is a common source of disability in society. There has been no research regarding the amount of pressure for the deep cervical flexors during cranio-cervical flexion exercise (CCFE). The purpose of this study is to determine optimal pressure levels to facilitate deep cervical muscles (longus colli & longus capitis) during cranio-cervical flexion exercise, using ultrasound measurement of the muscles. Methods : Using a cross-sectional design, the study was performed in an experimental group of 19 subjects (7 men, 12 women) with no cervical pain. Participants were instructed to perform the CCFE, and during the five incremental stages of the test, changes in thickness, as compared to resting baseline values, were measured using ultrasonography for sternocleidomastoid muscle (SCM) and deep cervical muscles (DCF). Results : Both DCF and SCM muscles demonstrated an increase in recruitment with each progressive phase of the test. In comparing the different pressure increments, the most significant changes found in DCF thickness were between phase 2 and phase 3 (p<.05). However no differences were found between pressure increments for SCM thickness (p>.05). Conclusion : The results suggest 26 mm Hg as the optimal pressure level during cranio-cervical flexion exercise for facilitation of deep cervical flexor.

경추 견인과 신경가동운동이 경추 신경근병증을 가진 컴퓨터 종사자의 통증, 기능장애, 근력에 미치는 영향 (The Effects of Cervical Traction and Nerve Mobilization Exercise on Pain, Disability and Muscle Strength in Computer Workers with Cervical Radiculopathy)

  • Background: This study examined the effects of cervical traction group and cervical traction & nerve mobilization exercise group after applying conservative physical therapy to Computer Workers with Cervical Radiculopathy. Methods: They were randomly divided into two groups: 18 subjects were cervical traction group, 22 subjects were cervical traction and nerve mobilization exercise group. Each group performed its own exercise 30 minutes per day, three times per week, for 4 weeks. Pain intensity was measured by the visual analogue scale (VAS) and neck disability index (NDI). Cervical extensor muscles strength (CEMS) was measured by the Pressure biofeedback unit. Grasping power (GP) was measured by the Grip Track Commander. Results: After 4 weeks therapy, VAS and NDI were significantly reduced in both groups (p<.05) and CEMS and GP were significantly increased in both groups (p<.05). Significant differences were also evident between the two groups for these three measurements (p<.05). Conclusions: cervical traction and nerve mobilization exercise group is more effective than cervical traction group for reducing VAS and NDI and increasing GP in computer workers with cervical radiculopathy.

Cost-Effectiveness Analysis of Cervical Anterior Fusion and Cervical Artificial Disc Replacement in the Korean Medical System

  • Objective : This study is a retrospective cost-benefit analysis of cervical anterior interbody fusion and cervical artificial disc replacement, which are the main surgical methods to treat degenerative cervical disc disease. Methods : We analyzed 156 patients who underwent anterior cervical disc fusion and cervical artificial disc replacement from January 1, 2008 to December 31, 2009, diagnosed with degenerative cervical disc disorder. In this study, the costs and benefits were analyzed by using quality adjusted life year (QALY) as the outcome index for patients undergoing surgery, and a Markov model was used for the analysis. Only direct medical costs were included in the analysis; indirect medical costs were excluded. Data were analyzed with TreeAge Pro $2015^{TM}$ (TreeAge Software, Inc, Williamstown, MA, USA). Results : Patients who underwent cervical anterior fusion had a total cost of KRW 2501807/USD 2357 over 5 years and obtained a utility of 3.72 QALY. Patients who underwent cervical artificial disc replacement received 4.18 QALY for a total of KRW 3685949/USD 3473 over 5 years. The cumulative cost-effectiveness ratio of cervical spine replacement surgery was KRW 2549511/QALY (USD 2402/QALY), which was lower than the general Korean payment standard. Conclusion : Both cervical anterior fusion and cervical artificial disc replacement are cost-effective treatments for patients with degenerative cervical disc disease. Cervical artificial disc replacement may be an effective alternative to obtain more benefits.

Analysis of HPV Prevalence in Pairs of Cervical and Urine Samples from the Same Woman

  • The main cause of cervical cancer is a persistent infection with high-risk human papillomavirus (HR-HPV). Cervical cancer is reported as a preventable cancer in more than 80% of cases with early diagnosis and appropriate treatment. Papanicolaou test (Pap test) has been a global strategy to prevent cervical cancer, and recently, HPV test has been reported to be effective against cervical cancer and precancerous lesions. However, pelvic examinations give patients anxiety, discomfort, pain, distress, and psychological stress. HPV test via a urine sample caused less physical and psychological stress and more advantage than the Pap test. Therefore, it is necessary to study the usefulness of the HPV test for easy-to-collect urine samples. A total of 220 samples were collected from a pair of cervical and urine samples from 110 women and only 108 pairs of samples out of 110 were used because 2 cases were not amplified by β-globin. Among 108 pairs of cervical and urine samples, the prevalence of HPV was 37.0% (40/108) in cervical samples, 34.3% (37/108) in urine samples and HR-HPV was 22.2% (24/108) in cervical samples, 18.5% (20/108) in urine samples. In this study, urine samples showed a lower positive rate of HPV than cervical samples. There were many variables that could affect the condition of the urine sample. However, the HR-HPV agreement rate of the cervix and urine samples was 94.44% and the Kappa value was 0.823, which was "almost perfect". Through these results showed the significance of cervical cancer screening using a urine sample. Cervical screening is crucial, as cervical cancer can be prevented in more than 90% of cases. Urine samples collected by non-invasive methods may have the potential advantage of increasing acceptance of cervical cancer screening. Therefore, it is necessary to develop a new cervical cancer screening strategy using urine samples through further study based on the results of this study.

한국인의 치경부마모증 경험도에 관한 조사연구

  • In order to collect some necessary data to promote correcting the toothbrushing technic on the Korean public, the author had examined the number of present teeth on which had cervical abrasion and the most basic home dental care in 2,000 Korean male and female adults from 19 to 52 years in the age. Then, the cervical abrasion experience rate and the cervical abrasion experience teeth rate were calculated and evaluated. The obtained results were as follows: 1. Cervical abrasion experience rate was 32.45% 2. Cervical abrasion experience rate was gradually increased by ageing. 3. Cervical abrasion experience rate in male adults was higher than that in female adults. 4. Cervical abrasion experience rate in the lower jaw was higher than that in the upper jaw. 5. Cervical abrasion experience rate of the teeth at the right side was higher than that of the teeth at the left side. 6. Cervical abrasion experience teeth rate was 3.82%. 7. Cervical abrasion experience rate was highest on the first bicuspid, and followed in the sequence of the second bicuspid, canine, first molar, incisors, and 2nd and 3rd molars.

경추부 견인이 경추부 통증 환자의 증세 및 통증에 미치는 영향 (The Effect of Cervical Traction on Pain & Symptom for Patients with Cervical Pain)

  • The purpose of this study was to investigate the influence of cervical pain and radiating pain after cervical traction for patients with cervical pain. This evaluation was made 81 persons who cervical pain or radiating pain. The result of this study were as following ; 1. There were statistically significant decrease in cervical pain and radiating pain after cervical traction. 2. A type group(only neck pain ; n=5) and B type group(neck to elbow radiating pain ; n=11), there were pain decreased but there were not significant difference (p>0.05), C type group(neck to shoulder radiating pain group ; n = 14) and D type group(neck to hand radiating pain group ; n = 50), there were pain decreased before test then after test by VAS and significant difference(p<0.05).

The Role of Knowledge and Self-Efficacy in Behavioral Intention to Prevent Cervical Cancer among Female College Students

  • In order to promote cervical cancer prevention behaviors of female college students, this study aims to explore factors that influence cervical cancer prevention behavior of female college students. Specifically, this study examined mediating roles of knowledge and self-efficacy in predicting behavioral intention to prevent cervical cancer. An online survey was conducted on female college students. Analysis of 236 female college students' responses revealed that health status influenced behavioral intention to prevent cervical cancer indirectly through knowledge and self-efficacy. Exposure to Internet media influenced behavioral intention to prevent cervical cancer directly and indirectly through knowledge and/or self-efficacy. Knowledge influenced behavioral intention to prevent cervical cancer directly and indirectly through self-efficacy. Based on the findings of this study, we provided implications for planning of educational programs and campaigns to promote the cervical cancer prevention behaviors of female college students.

  • This study was undertaken to investigate the interrelationships between the degrees of skeletal maturity of cervical vertebrae and the hand-and-wrist in skeletal Class III malocclusions. In 185 skeletal Class III malocclusions (male 62, female 123) having the lateral cephalogram and hand-wrist radiogram which were taken on the same day, 6 skeletal maturity stages of cervical vertebrae were compared with 11 skeletal maturity indicators of the hand-and-wrist. On the basis of findings of this study, the following results were obtained: 1. The stages of cervical vertebral maturity are one of the methods possible to assess the individual maturity. 2. Mean ages of male and female were obtained in each cervical vertebral stage. 3. Cervical vertebral stages 1 and 2 are considered to the accelerative growth phase, cervical vertebral stages 3 and 4 are corresponded to the peak height velocity, and cervical vertebral stages 5 and 6 were observed to occur during the decelerative phase of growth after peak height velocity in both sexes. 4. In cervical vertebral stages 1, 2, 3, 4, and 5, the degrees of skeletal maturity of cervical vertebrae in males were more retarded than females. 5. There was the high correlation between the degrees of skeletal maturity of cervical vertebrae and hand-and-wrist.
