Gait Initiation in a Patient With Spastic Hemiplegia Cerebral Palsy With and Without a Dynamic Ankle Foot Orthosis: A Pilot Study (강직성 편측 뇌성마비 환자의 보행 기시에 미치는 동적 단하지 보조기의 효과: 사전연구)

  • 본 연구는 강직성 편측 뇌성마비 환자의 보행 기시(gait initiation)의 특성을 파악하고, 동적 단하지 보조기(dynamic ankle foot orthosis : DAFO)가 이런 환자들의 보행 기시에 미치는 영향을 알아보기 위하여 실시하였다. 연구대상자는 19세 선천성 강직성 우측 뇌성마비를 가진 여자 대학생이었다. 연구대상자는 4개의 다른 보행 기시 조건에서 각 조건당 7회의 보행 기시를 하였는데, 각각의 보행 기시 조건은: 1) 보조기 없이 좌측 하지로 보행 기시; 2) 우측 하지 보조기 착용 상태에서 좌측하지로 보행 기시; 3) 보조기 없이 우측 하지로 보행 기시; 4) 우측 하지 보조기 착용 상태에서 우측 하지로 보행 기시였다. 두개의 힘 판(force plate) 위에서 본인이 선택한 가장 편안한 방법과 속도로 2 m를 걷도록 하였다. 연구대상자가 보행 기시를 하는 동안 양쪽 가자미근과 앞정강근에 부착된 4개의 표면 전극으로부터 근 전도 신호와 보행 기시를 하는 연구 대상의 양 발 아래 놓인 두개의 힘 판의 자료를 분석하여 체중(body weight : BW)으로 정량화 하였다. 실험결과는 각 조건마다 7회 실시된 측정값들의 평균을 이용하였다. 본 사전연구의 결과는 다음과 같다. 1. 보행 기시 전 양다리로 선 상태에서 환측보다 건측으로 많은 체중지지를 하였으나(환측 42.57%BW, 건측 58.03%BW) 보조기의 착용이 환측으로의 체중지지를 향상시켰다(환측 44.46%BW, 건측 55.54%BW). 2. 보행 기시 중 가자미근과 앞정강근의 동시수축(coactivation)이 계속되었는데 이러한 양상은 보조기의 착용 유무에 따라 크게 달라지지 않았다. 3. 힘 판에서 들어온 지면 반발력(ground reaction force)의 수직 분력(vertical element, Fz)과 전후 분력(anterior-posterior element, Fx)을 분석한 결과, 건측 다리로 보행 기시를 할 때 보조기 착용은 가속을 촉진시켜 역학적으로 유리하였다. 이상의 연구 결과는 보조기의 착용이 건측 다리로 보행 기시를 할 때 미치는 신경학적, 역학적 장점들에 대한 구체적 연구의 필요성 및 근거를 제시하고, 환자들의 보행훈련 접근에 보다 효율적인 보행방법을 소개하는데 이용될 수 있다.

Region Segmentation and Volumetry of Brain MR Image represented as Blurred Gray Value by the Partial Volume Artifact (부분체적에 의해 번진 명암 값으로 표현된 뇌의 자기공명영상에 대한 영역분할 및 체적계산)

  • This study is to segment white matter, gray matter, and cerebrospinal fluid(CSF) on a brain MR image and to calculate the volume of each. First, after removing the background on a brain MR image, we segmented the whole region of a brain from a skull and a fat layer. Then, we calculated the partial volume of each component, which was present in scanning finite thickness, with the arithmetical analysis of gray value from the internal region of a brain showing the blurring effects on the basis of the MR image forming principle. Calculated partial volumes of white matter, gray matter and CSF were used to determine the threshold for the segmentation of each component on a brain MR image showing the blurring effects. Finally, the volumes of segmented white matter, gray matter, and CSF were calculated. The result of this study can be used as the objective diagnostic method to determine the degree of brain atrophy of patients who have neurodegenerative diseases such as Alzheimer's disease and cerebral palsy.

An Evaluation of Significance of Oriental Medicine on Treating Speech Impediment and Limb Impediment Among the Children with Development Disabilities (발달장애아동의 언어장애와 수족장애에 대한 한방치료의 유의성 평가)

Objectives This study was conducted to establish a standard and classify suitability in the treatment of speech impediment and limb impediment among the children with development disabilities using Oriental Medicine treatment on the ground of Sasang Constitutional Medicine. 2. Methods 43 patients with pediatric development disabilities with speech impediment and 10 patients with pediatric development disabilities with limb impediment as the main symptoms had been chosen in this study. Oriental Medicine treatments on the ground of Sasang Constitutional Medicine were rendered and evaluated responses as well as pursuing most proper treatment methods. 3. Results & Conclusions 1) For pediatric development disabilities with speech impediment, Oriental Medicine treatment showed a significant effect on similarity of Autism and Cerebral Palsy, but insignificant effect on innate Autism. 2) For pediatric development disabilities with limb impediment, Bee venom was effective for severe limb impediment and limb asthenia, while as eight principles herbal acupuncture effective for mild limb impediment and spasticity. 3) Sasang Constitutional Medicine for treating speech impediment and limb impediment among the children with development disabilities showed general improvement as a fundamental healing through coordinating inner organ activities.

An Alternative Surgical Technique for Repair of Anomalous Origin of the Left Coronary Artery from the Pulmonary Artery

  • Background: For the surgical management of anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA), there have been various techniques that reduce the tension and kinking of the coronary artery during reimplantation to the aorta. The aim of this study is to describe the results of our modified technique of coronary reimplantation for the treatment of ALCAPA. Methods: Between October 2003 and February 2011, seven patients underwent coronary reimplantation with the modified technique (tubing formation with the sinus wall of the pulmonary artery and trapdoor formation at the site of implantation in the aorta). The median follow-up duration was 52 months (range, 4 to 72 months). Clinical outcomes and serial echocardiographic data were reviewed. Results: There was no mortality. One patient had a small amount of cerebral hemorrhage postoperatively and improved without any sequelae. Another patient had left diaphragm palsy and underwent diaphragm plication. Follow-up echocardiogram showed that all patients had normal ventricular function without chamber enlargement. Conclusion: Our modified technique (tubing formation with the sinus wall of the pulmonary artery and trapdoor formation at the site of implantation in the aorta) demonstrated successful clinical outcomes. We conclude that this surgical technique can be a potential alternative for the treatment of ALCAPA.

Case Report of Cerebellar Artery Infarction Patients Treated by Traditional Korean Medicine (소뇌혈관별 경색에 따른 환자 증례보고)

  • Objective: This study reports on three cases of cerebellar artery infarction patients treated by traditional Korean medicine.Method: Three patients with cerebellar artery infarction according to cerebellar artery were treated by traditional Korean medicine at the traditional Korean medicine hospital of Daejeon University. The posterior cerebral artery (PCA) infarction patient had ataxia, dysarthria, gait disturbance, and dizziness; the anterior inferior cerebellar artery (AICA) infarction patient had facial palsy, dizziness, and hearing loss; and the posterior inferior cerebellar artery (PICA) infarction patient had gait disturbance and dizziness. Acupuncture, herbal medicine, and pharmacopuncture were used as traditional Korean medicine treatments during hospitalization. We then observed any improvement in the symptoms.Results: The scale for the assessment and rating of ataxia (SARA) score (gait, stance) of the PCA infarction patient was reduced from 8.6 to 2.2, while the dizziness numerical rating scale (NRS) score was reduced from 10 to 2 after treatment. The Yanagihara score of the AICA infarction patient was reduced from 27 to 14, while the dizziness NRS score was reduced from 10 to 2 after treatment. The SARA score (gait, stance) of the PICA infarction patient was reduced from 7.4 to 1.0, and the dizziness NRS score was reduced from 10 to 1 after treatment.Conclusion: Traditional Korean medicine appears to be effective in treating cerebellar infarction symptoms.

A Study on the Evacuation Model Considering Occupancy Load in Child Care Center with Disabilities (장애아전문 어린이집 재실자 피난 부하를 고려한 피난 모델 연구)

  • The purposes of this study are to propose evacuation recommendations through evacuation model considering occupancy load model in child care center with disabilities. To achieve these purposes, the survey on the occupancy load and egress model and evacuation experiments have carried out, and evacuation occupancy load model for child care center with disabilities are suggested. The results of this study are as follows : (1) The evacuation plan for children with disabilities are necessary special considerations according the type and levels of disability, and various egress means and method for children in child care center with disabilities conditions. (2) The evacuation occupancy loads are different with non-impaired children and evacuation occupancy loads and evacuation corridors width are necessary considering impaired children's evacuation behaviors. (3) The temporal evacuation area(refuge area) is essential for children with disability who are impossible vertical evacuation such as impaired children using wheelchair and cerebral palsy.

Effects of Cervical Instability on Function of Deep Neck Flexor Muscle and Muscle Tonus of Neck Muscles (목 불안정성이 깊은목굽힘근의 기능과 목 근육들의 긴장도에 미치는 영향)

  • Purpose : The time spent using smart devices is constantly increasing, particularly in recent times. Using smart devices for a long time with an incorrect posture may lead to cerebral palsy (CP), instability, and abnormal muscle tone. Therefore, we aimed to investigate the relationships among cervical instability, deep neck flexor (DNF) activity, range of motion (ROM), and muscle tonus. Methods : Fifty subjects with CP participated in this study, and they were physiotherapists at W Hospital in Daejeon. Those who voluntarily participated in the research were selected as candidates who fulfilled the selection criteria. According to an instability test, 25 subjects were assigned to the instability and control groups. All subjects first underwent the instability test to be allocated to the appropriate group. Those in the instability group tested positive on the instability test. The Neck Disability Index (NDI), ROM, muscle tone, and DNF activity were measured to evaluate their relationships. The DNF strength and endurance were measured using a cranio-cervical flexion test. The upper trapezius (UT), sternocleidomastoid (SCM), and suboccipital (SO) muscle tones were measured using a contact soft tissue tone measuring instrument. The statistical significance level was set to .05. Results : There were significant differences in the flexion, extension, and rotation of the cervical ROM (CROM) between the two groups (p<.05). The SCM, UT, and SO muscle tones were significantly different between the two groups (p<.05). The DNF strength and endurance showed a significant difference between the two groups (p<.05). Conclusion : We found that there were significant increases in the CROM and muscle tone and decrease in the DNF strength and endurance in the instability group. This indicated that cervical instability is affected by the DNF strength and endurance. We may recommend DNF exercises in cases of cervical instability in clinical environments.

A Case report of Thalamic pain Syndrome Patient who were Constitutionally Typed as Taeeumin improved by Yeoldahanso-tang(熱多寒少湯) (열다한소탕(熱多寒少湯) 가감방(加減方)을 이용한 태음인(太陰人) 시상통증증후군 치험 1례)

  • A 67 year-old Taeeumin(太陰人) female patient with Thalamic pain Syndrome(Dejerine-Roussy Syndrome) was treated. She was suffering from rt. side numbness, pain and weakness, rt. facial palsy, dizziness, dysarthria and lassitude. She was diagnosed as Cerebral lacunar infarction(Lt. thalamus) by Brain MRI(Magnetic Resonance Imaging) and adminstered Yeoldahanso-tang(熱多寒少湯). After taking Yeoldahanso-tang(熱多寒少湯), her symptoms had improved. Result suggest that Yeoldahanso-tang(熱多寒少湯) is an effective for Taeeumin Thalamic pain Syndrome. Though, results support a role for oriental medical treatment for Cerebrovascular Accident(CVA) connected Sasang Constitution, further research into the effectiveness of oriental medical treatments for CVA connected Sasang Constitution is needed.

  • Tongue acupuncture is not only a body acupuncture treatment for the tongue but also a new microacupuncture treatment. Tongue acupuncture is a very promising treatment from a diagnostic and therapeutic point of view. According to several studies, Guan's tongue acupuncture and tri-tongue acupuncture are frequently used. Tongue acupuncture can be used for dysphagia, dysarthria, aphasia, insomnia, autism spectrum disorder, burning mouth syndrome, and cerebral palsy.

Evidence of Cortical Reorganization in a Monoparetic Patient with Cerebral Palsy Detected by Combined Functional MRI and TMS

  • The motor recovery mechanism of a 21-year-old male monoparetic patient with cerebral palsy, who had complained of a mild weakness on his right hand since infancy, was examined using functional Magnetic Resonance Imaging (fMRI) and Transcranial Magnetic Stimulation (TMS). The patient showed mild motor impairment on the right hand. MRI located the main lesion on the left precentral knob of the brain. fMRI was performed on this patient as well as 8 control subjects using the Blood Oxygen Level Dependent technique at 1.5 T with a standard head coil. The motor activation task consisted of finger flexionextension exercises at 1 Hz cycles. TMS was carried out using a round coil. The anterior portion of the coil was applied tangentially to the scalp at a 1.0 cm separation. Magnetic stimulation was carried out with the maximal output. The Motor Evoked Potentials (MEPs) from both Abductor Pollicis Brevis muscles (APB) were obtained simultaneously. fMRI revealed that the unaffected (right) primary sensori-motor cortex (SM1), which was centered on precentral knob, was activated by the hand movements of the control subjects as well as by the unaffected (left) hand movements of the patient. However, the affected(right) hand movements of the patient activated the medial portion of the injured precentral knob of the left SM1. The optimal scalp site for the affected (right) APB was located at 1 cm medial to that of the unaffected (left) APB. When the optimal scalp site was stimulated, the MEP characteristics from the affected (right) APB showed a delayed latency, lower amplitude, and a distorted figure compared with that of the unaffected (left) APB. Therefore, the motor function of the affected (right) hand was shown to be reorganized in the medial portion of the injured precentral knob.
