Antimicrobial Susceptibility of Pathogens Isolated from Clinical Sources 1973 (1973년에 분리된 병원성세균의 항균제에 대한 감수성)

  • During March to October 1973 at National Medical Center, one thousand four hundred sixty four pathogens that exclude penicillin sensitive organisms such as gonococcus, pneumococcus, streptococcus; ampicillin sensitive H. influenzae and gentamicin sensitive pseudomonas spp, were isolated. These strains identified as follows: Sta. aureus 399 E. coli 359 Sta. epidermidis 183 Klebsiella spp 171 Proteus spp 103 B. anitratum 80 Salmonella spp 68 Enterococcus 53 Shigella spp 48 In general, Sta. epidermidis is regarded as nonpathogen, however, author included pure culture of this bacteria in this study. 1. High susceptibility to staphylococcus reveals cephalosporin, methicillin, gentamicin & leucomycin. 2. Ampicillin is only one susceptible antibiotic to Enterococcus. 3. Cephalosporin and gentamicin reveal susceptible antibiotics to E. coli. 4. Gentamicin is only one susceptible antibiotic to Klebsiella spp. 5. Gentamicin & carbenicillin reveal moderately susceptible antibiotics to Proteus spp. 6. There is no susceptible antibiotic to B. anitratum. 7. About thirty percent strains of salmonella are resistant to chloramphenicol. 8. Carbenicillin, cephalosporin & gentamicin reveal susceptible to shigella spp. 9. Multiple resistant rate to more than two antibiotics which includes tetracycline are as follows. Sta. aureus 56.5% E. coli. 80.9% Shigella spp 93.8%

Stimulation of Cephalosporin C Production by Acremonium chrysogenum M35 with Fatty Acids

  • Supplementation with rice oil and its major components (oleic acid and linoleic acid) was found to have a significant influence on cephalosporin C (CPC) production and cell growth by A. chrysogenum M35 in shake flask cultures. Five percent (v/v) rice oil had the most robust effect and 5% (v/v) oleic acid was the second most efficient on cell growth, whereas 3% (v/v) linoleic acid was found to be optimal for CPC production. Rice oil, oleic acid, and linoleic acid also significantly improved the rates of glucose consumption. When glucose was almost consumed, CPC production was initiated and, on the addition of rice oil, lipase activity increased steadily to 1.56 U/ml for 4 days. These results suggest that rice oil and fatty acids are used as carbon source to produce CPC by A. chrysogenum M35. Moreover, a mixture, composed of 40% (v/v) oleic acid and 60% (v/v) linoleic acid, had the strongest stimulatory effect on CPC production, due to a synergistic effect of the two fatty acids. Consequently, the maximum CPC titer (7.44 g/l) was improved about 4.5-fold.

  • Pseudomonas cepacia BY21 was found to produce glutaryl acylase that is capable of deacylating glutaryl-7-aminocephalosporanic acid (glutaryl-7-ACA) to 7-aminocephalosporanic acid (7-ACA), which is a starting material for semi-synthetic cephalosporin antibiotics. Amino acids of the reported glutaryl acylases from various Pseudomonas sp. strains show a high similarity (>93% identity). Thus, with the known nucleotide sequences of Pseudomonas glutaryl acylases in GenBank, PCR primers were designed to clone a glutaryl acylase gene from P. cepacia BY21. The unknown -subunit gene of glutaryl acylase from chromosomal DNA of P. cepacia BY21 was cloned successfully by PCR. The -subunit amino acids of P. cepacia BY21 acylase (GenBank accession number AY948547) were similar to those of Pseudomonas diminuta KAC-1 acylase except that Asn408 of P. diuminuta KAC-1 acylase was changed to Leu408.

Immobilization of Trigonopsis variabilis and Conversion of Cephalosporin C to 7$\beta$-(4-Caboxybutanamido)Cephalosporanic Acid (Trigonopsis variabilis의 고정화 및 Cephalosporin C로부터 7$\beta$-(4-Carbohybutanamido)Cephalosporanic Acid의 전환)

  • An immobilized Trigonopsis variabilis cells having an high activity of D-amino acid oxidase(DAO) was used to convert CPC into GL-7-ACA. The optimal pH of the reaction system was 8.0-8.5, and the optimal temperature was 40$\circ$C. When immobilized cell was used repeatedly in semi-batchwise reaction, the system retained 80% of the initial activity after used of 12 times for over 12 hours. The storage stability of the immobilized cell was maintained for 30 days at 4$\circ$C. The CPC concentration for the maximal reaction rate was about 30 mM and 40 mM for free and immobilized cells, respectively. Substrate inhibition of CPC concentration more than 50 mM was overcomed by 20~25% by immobilization. Pure oxygen supply into reaction system was most efficient in D-amino acid oxidase reaction. Continuous conversion to GL-7-ACA from CPC has been developed with an bioreactor system containing immobilized T variabilis cells. By opera- tion of the reactor for 5 hours, the average conversion yield of >80% and GL-7-ACA production of 40~45 mM per hour could be obtained.

  • Cephalosporium acremonium MAR-80, a strain of methionine analogue-resistant mutant, showed good activity of sulfate utilization as only sulfur source. The effect of methionine on the sulfate uptake system was investigated by using $Na_{2}^{35}SO_{4}$ as a tracer in the resting cell system. From this result, it was revealed that sulfate permease of this strain was less repressed and/or less inhibited by methionine than parent type. This deregulation was due to low actibity of methionine uptake, which was operated by somewhat simple diffusion. From these studies, it could by anticipated that the improved productivity of cephalosporin C and lower dependence of cephalosporin C production on methionine were related to increased uptake rate of sulfate.

  • To formulate the parenteral delivery of a new cephalosporin derivative, 7-${\beta}$-[(2)-2-(2-arninothiazol-4-yl)-2methoxyiminoacetamido]- 3- [(2,3-cyclopenteno-4-carbamoyl-l-pyridinium)methyl]- 3-cephem-4-carboxylate sulfate( CKD604), the stability and solubility of CKD-604 in various aqueous media were investigated. The degradation kinetics of CKD-604 in aqueous solutions (ionic strength 0.1, pH 1-8) were studied at $37^{\circ}C$. The observed degradation rates followed pseudo first order kinetics. The pH-rate profile exhibited a minimum degradation rate at pH 5. The Arrhenius activation energy was 14.2 kcal/mol in pH 5 buffer solution. Excellent agreement between the cephalosporins' theoretical pH-rate profile and the experimental data indicated that the degradation pathway of CKD-604 could be predicted according to the general pathway of cephalosporins. The solubility of CKD-604 was 8.16 mg/ml at $25^{\circ}C$. To enhance the solubility and adjust the suitable pH, CKD-604 was solubilized by using sodium ascorbate, ascorbic acid and urea. The compositions were obtained to satisfy optimum pH and concentration, and the total amount of additives was several times of the active ingredient, CKD-604.

  • New cephalosporin antibiotics, 7-[(3,4-dihydro-6-methoxycarbonyl-2,2-dimethyl-2H-1,4-thiazin-3-yl)acetamido]-3-substituted-3-cephem-4-carboxylic acid derivatives 2a-2d, 7 -[(3,4-dihydro-6-ethoxycarbonyl-2,2-dimethyl-2H-1,4-thiazin-3-yl)acetamido] -3-substituted-3-cephem-4-carboxylic acid derivatives 3a-3d and 7-[(3,4-dihydro-6-methoxycarbonyl-2,2-dimethyl-2H-1,4-thiazin-3-yl-1-(S)-oxide)acetamido]-3-substituted-3-cephem-4-carboxylic acid derivatives 4a-4d were synthesized. Antibacterial activities of these new cephalosporin derivatives and the relationship between their structures and their activities were examined. Among them, 7-[(3,4-dihydro-6-methoxycaronyl-2,2-dimethyl-2H-1,4-thiazin-3-yl-1-(S)oxide)-acetamido]-3-[(1,2,3-triazol-5-yl)thiomethyl]-3-cephem-4-carboxylic acid 4d exhibited the antibacterial activities against Gram(+)and Gram(-) bacteria.

Peritoneal Dialysis Associated Peritonitis and Empirical Antibiotics Therapy in Korean Children with Chronic Renal Failure (소아 복막 투석 환자에서 발생한 복막염의 경험적 항생제 치료에 관한 연구)

  • Purpose : This study aims to verify the effectiveness of initial empirical antibiotic choice recommended by the International Society for Peritoneal Dialysis(ISPD) guide among Korean children. Methods : We have collected data on peritonitis from January 2001 to December 2007 in Samsung Medical Center. Results : Of the 42 patients, 48 episodes of peritonitis had occurred in 21 patients. The rate of peritonitis was one episode over 35.3 patient-months. Mean dialysis duration before peritonitis was 18.06$\pm$15.81 months. Gram-positive organisms accounted for 58.3% of all episodes. Of the gram-positive organisms, the most common pathogen was Staphylococcus aureus(29.2%), the next common pathogens were Coagulase negative staphylococcus(14.6%) and Streptococcus species(6.3%). 35.7% of gram-positive pathogens were resistant to 1st cephalosporin. However, in patients younger than 4 years old, 50% of gram-positive pathogens were resistant to 1st generation cephalosporin. 10 episodes of peritonitis were methicillin-resistant and were treated by vancomycin. Of the gram-negative organisms, E. coli was the most common (8.3%). 64.8% of all pathogens were sensitive to cephalothin or ceftazidime. Conclusion : The empirical therapy with 1st generation cephalosporin and ceftazidime can be also effective to peritoneal dialysis associated peritonitis in Korean children. However, in patients younger than 4 years old, glycopeptide should be considered as the first empirical therapy in Korean children.
