일반 측방 두부규격 방사선사진과 측방 추부규격 전산화 방사선사진에서의 계측점의 신뢰도에 대한 비교 연구 (A COMPARATIVE STUDY OF COMPUTED RADIOGRAPHIC CEPHALOMETRY AND CONVENTIONAL CEPHALOMETRY IN RELIABILITY OF HEAD FILM MEASUREMENTS (LANDMARKS IDENTIFICATION))

  • The purpose of this study was to compare & to find out the variability of head film measurements (and marks identification) between Fuji computed radiographic cephalometry and conventional cephalometry. 28 Korean adults were selected. Lateral cephalometric FCR film and conventional cephalometric film of each subject was taken. Four investigators identified 24 cephalometric landmarks on lateral cephalometric FCR film and conventional cephalometric film. The comparable measurements between lateral cephalometric FCR film and conventional cephalometric film were statistically analysed. The results were as follows : 1. In FCR film & conventional film, coefficient of variation (C.V.) of 24 landmarks was taken horizonta1ly & vertically. There is no significant difference of rank order of landmarks in C.V. between two films. 2. In comparison of significant differences of landmarks variability between FCR film & conventional film, horizontal value of coefficient of variation, showed significant differences in four landmarks among twenty-four landmarks, but vertical value of coefficient of variation showed significant differences in sixteen landmarks among twenty-four landmarks. FCR film showed significantly less variability than conventional film in 17 subjects among 20(4+16) subjects that showed significant difference.

코골이의 역학 및 측방두부규격방사선학적 특징에 관한 연구(청년층을 중심으로) (Epidemiological Study and Cephalometric Features of Snoring(In the Young Adults))

  • The Purpose of this study were to examine the epidemiology of snoring, its associated factors and anatomic features on cephalogram according to the frequency of snoring in young adults. Epidemiological survey using questionaire was made to the 438 students (320 male, 118 female) aged 19 - 28 years, and cephalometric study of anatomic features on 14 habitual snorers, 31 occasional snorers and 30 non-snorers among men was done. The obtained results were as follows : 1. The prevalence snoring was 11.4% in the young adults, 15.0% in male and 1.7% female. 2. Of the young adults, 3.7% were habitual snorers and 7.8% were occasional snorers. 3. Smoking and drinking increased the frequency of snoring(p < 0.01), but didn't affec the differences in the frequency between habitual and occasional snoring. 4. No significant correlation was made between the frequency of snoring and the factors such as overweight, nasal disease and hypertension. 5. In the cephalometric comparison between snorers and non-snorers, snorers had inferiorly positioned hyoid bone(p < 0.05), longer soft palate(p < 0.01), steeper soft palate(p < 0.05) and narrower nasopharyngeal(p < 0.05) and oropharyngeal(p < 0.001) airway. 6. In the cephalometric comparison between habitual snorers and occasional snorers, habitual snorers had narrow nasopharyngeal airway(p < 0.05).

3-Dimensional analysis for class III malocclusion patients with facial asymmetry

  • Objectives: The aim of this study is to investigate the correlation between 2-dimensional (2D) cephalometric measurement and 3-dimensional (3D) cone beam computed tomography (CBCT) measurement, and to evaluate the availability of 3D analysis for asymmetry patients. Materials and Methods: A total of Twenty-seven patients were evaluated for facial asymmetry by photograph and cephalometric radiograph, and CBCT. The 14 measurements values were evaluated and those for 2D and 3D were compared. The patients were classified into two groups. Patients in group 1 were evaluated for symmetry in the middle 1/3 of the face and asymmetry in the lower 1/3 of the face, and those in group 2 for asymmetry of both the middle and lower 1/3 of the face. Results: In group 1, significant differences were observed in nine values out of 14 values. Values included three from anteroposterior cephalometric radiograph measurement values (cant and both body height) and six from lateral cephalometric radiographs (both ramus length, both lateral ramal inclination, and both gonial angles). In group 2, comparison between 2D and 3D showed significant difference in 10 factors. Values included four from anteroposterior cephalometric radiograph measurement values (both maxillary height, both body height) and six from lateral cephalometric radiographs (both ramus length, both lateral ramal inclination, and both gonial angles). Conclusion: Information from 2D analysis was inaccurate in several measurements. Therefore, in asymmetry patients, 3D analysis is useful in diagnosis of asymmetry.

Reproducibility of lateral cephalometric landmarks on conventional radiographs and spatial frequency-processed digital images

  • Purpose : Computed radiography (CR) has been used in cephalometric radiography and many studies have been carried out to improve image quality using various digital enhancement and filtering techniques, During CR image acquisition, the frequency rank and type affect to the image quality. The aim of this study was to compare the diagnostic quality of conventional cephalometric radiographs to those of computed radiography. Materials and Methods : The diagnostic quality of conventional cephalometric radiographs (MO) and their digital image counterparts were compared, and at the same time, six modalities (M1-M6) of spatial frequency-processed digital images were compared by evaluating the reproducibility of 23 cephalometric landmark locations. Reproducibility was defined as an observer's deviation (in mm) from the mean between all observers. Results and Conclusion: In comparison with the conventional cephalometric radiograph (MO), Ml showed statistically significant differences in 8 locations, M2 in 9, M3 12, M4 in 7, M5 in 12, and M6 showed significant differences in 14 of 23 landmark locations (p < 0.05). The number of reproducible landmarks that each modality possesses were 7 in M6, 6 in M5, 5 in M3, 4 in M4, 3 in M2, 2 in Ml, and 1 location in MO. The image modality that observers selected as having the best image quality was M5.

골격성 제 III급 부정교합환자와 구순구개열환자의 두부방사선계측치의 비교 (A cephalometric comparison of Skeletal Class III malocclusion and Cleft lip and palate patients)

  • A cephalometric study was performed to reveal differences between skeletal Class III malocclusion patients and cleft lip and palate patients, The material for this study consisted of 16 males (mean age 19.8, range 17-29) and 9 females(mean age 19.4, range 16-27) with cleft lip and palate, and 222 Skeletal Class III malocclusion patients(males 106, females 116), Cephalometric tracing and measurements were done by one investigator. Results were followed: 1. Cleft lip and palate group had more retrusive maxilla than the skeletal Class III malocclusion group. 2, Cleft lip and palate group had smaller effective maxillary and mandibular length than skeletal Class III malocclusion group, and the difference was more prominent in the mandible than in the maxilla. 3. Dental compensation was not observed in the upper incisors of cleft lip and palate group and in the lower incisors it was smaller than skeletal Class III group. 4, In the Gonial angle and lower anterior facial height values, there was no significant difference between cleft lip and palate and skeletal Class III malocclusion group. These results can be used in orthodontic treatment planning and orthognathic surgery for the cleft lip and palate patients.

  • Clinical examination, photometric evaluation, & radiographic analysis have been used to evaluate the asymmetry of the face. Commonly used skull radiographs to assess skeletal asymmetry include poatero-anterior cephalometries, submentovertex view, & panoramic view. The purpose of this study is the comparison of the reliance of the postero-anterior cephalometric view, submentovertex view, & panoramic view in the asymmetry evaluation. All measurements were performed on the each radiographs of 31 control group & 30 asymmetric group. The measurements are MSL(midsagittal plane)-Co, MSL-Go, MSL-Me, MSL-Al, MSL-Bl, Mn. Ramus Height(Co-Go), Mn. Body Length(Go-Me), and Total Mn. Length(Co-Me). The results were as follows: 1. The lack of either a right-sided or left-sided asymmetric dominant was found. 2. The postero-anterior cephalometric view & submentovertex view relatively agreed with each other in the result. The postero-anterior cephalometric view & submentovertex view had the clinical reliability & effectiveness in the diagnosis of the skeletal asymmetry(p<0.05). 3. The panoramic view showed more magnification compared to the other radiographs. In the vertical measurements the panoramic view had clinical reliability relatively(p<0.05). But we cannot rely on the horizontal measurements in the panoramic view(p>0.05).

디지털 측방두부규격방사선사진과 MDCT의 3차원 재구성 영상을 이용한 합성측방두부규격방사선사진의 계측치 비교 및 머리 위치가 미치는 효과 (Comparison of measurements from digital cephalometric radiographs and 3D MDCT-synthetized cephalometric radiographs and the effect of head position)

  • Purpose : To investigate the reproducibilities and compare the measurements in digital and MDCT-synthesized cephalometric radiograph, and to investigate the effect of head position on the measurement during imaging with MDCT. Materials and Methods : Twenty-two dry skulls (combined with mandible) were used in this study. Conventional digital cephalometric radiograph was taken in standard position, and MDCT was taken in standard position and two rotated position ($10^{\circ}$ left rotation and $10^{\circ}$ right tilting). MDCT data were imported in $OnDemand^{(R)}$ and lateral cephalometric radiograph were synthesized from 3D virtual models. Two types of rotated MDCT data were synthesized with default mode and with corrected mode using both ear rods. For all six images, sixteen angular and eleven linear measurements were made in V-$Ceph^{(R)}$ three times. Reproducibility of measurements was assessed using repeated measures ANOV A and ICC. Linear and angular measurements were compared between digital and five MDCT-synthesized images by Student t-test. Results : All measurements in six types of cephalometric radiograph were not statistically different under ICC examination. Measurements were not different between digital and MDCT-synthesized images (P>.05). Measurements in MDCT-synthesized image in $10^{\circ}$ left rotation or $10^{\circ}$ right tilting position showed possibility of difference from digital image in some measurements, and possibility of improvement via realignment of head position using both ear rods. Conclusion : MDCT-synthesized cephalometric radiograph can substitute conventional cephalometric radiograph. The error on head position during imaging with MDCT have possibility that can produce measurement errors with MDCT-synthesized image, and these position error can be corrected by realignment of the head position using both ear rods.

  • Purpose : To compare cephalometric measurement between measuring methods in digital and conventional lateral cephalometric radiograph. Materials and Methods : Twenty digital and conventional lateral cephalometric radiographs were selected. In digital group, cephalometric measurements were performed manually using hardcopies and automatically using $V-Ceph^{TM}$ program on the monitor. In conventional group, the same measurements were performed manually on conventional films, and for automatic measurement conventional films were digitized by scanner. All measurements were performed twice by 4 observers, and 24 cephalometric variables were calculated and the time spent for each measurement was recorded. The differences in measurements data and the time spent for each measurement were compared within each group. Intra-observer and inter-observer comparisons were performed. Results : In both groups, no statistically significant difference between manual and automatic measurements was observed and most of the variables didn't show statistically significant differences between methods. The observer with less experience tended to show statistically significant differences of measurements between methods, and differences from other observers. The differences of measurements between methods in digital group were lesser than those of conventional group with statistical significance in 8 variables out of 24. With automatic method and in digital group, the spent time was shorter. Conclusion : With direct digital radiograph, automatic method using manually idenitified landmarks can be preferable in cephalometric analysis. (Korean J Oral Maxillofac Radiol 2005; 35 : 15-23)

일반두부방사선계측사진과 디지털방사선계측사진의 계측점 식별의 오차 및 재현성에 관한 비교 연구 (The comparison of landmark identification errors and reproducibility between conventional lateral cephalometric radiography and digital lateral cephalometric radiography)

  • 본 연구의 목적은 일반두부방사선계측사진과 디지털두부방사선계측사진의 계측점 식별의 오차를 구하여 각각의 영상에서 오차의 특징을 살펴보고 재현성을 비교 평가하는 것이다. 연구 대상은 서울대학교병원 치과진료부 교정과에 내원한 교정 환자 중 18세에서 29세 사이의 성인 환자 중에서 일반두부방사선계측사진군과 디지털두부방사선계측사진군 각각 15명씩 30명을 무작위로 선택하여 연구 대상으로 하였으며 남녀의 구별이나 두개 안면 구조의 형태는 고려하지 않았다. 계측점은 동일인이 시간차를 두고 식별 하였다. 식별 후 각 계측점은 좌표 (x, y)로 표시하였으며, 처음 계측점을 식별한 두부방사선계측사진군을 T1으로, 1 주 후 동일 계측점을 재식별한 두부방사선계측사진군을 T2로, 1 달 후 동일계측점을 재식별한 두부방사선계측사진군을 T3로 분류하였다. 오차의 평균과 표준편차는 x좌표, y좌표로 구분하여 계산하였다. 초기 식별 1주 후 재식별시 오차는 T2-T1(x), T2-T1(y)로, 초기 측정 1달 후 재시별시 오차는 T3-T1(x), T3-T1(y)로 표시하였으며 일반두부방사선계측사진과 디지털두부방사선계측사진으로 각각 나누었다. 재현성의 평가를 위한 오차간의 통계학적인 검정은 independent t-test를 사용하였으며 다음과 같은 결론을 얻었다. 1. 디지털두부방사선계측사진이 일반두부방사선계측사진보다 일반적으로 오차의 평균 및 표준편차가 작았다. 2. 일반두부방사선계측사진의 오차와 디지털두부방사선계측사진의 오차가 통계학적으로 유의성 있는 차이를 보인 항목은 드물었다. 3.상의 향상을 통한 오차의 개선은 한계가 있었으며 상이 향상되더라도 각 계측점의 오차 의 경향은 크게 변하지 않았다.
