The Effect of Protective Socks with Functional Insoles on Plantar Foot Pressure in Diabetes Patients

  • Purpose: The most common cause of plantar ulceration is an excessive plantar pressure in patients with peripheral neuropathy. Foot orthosis and therapeutic footwear have been used to decrease the plantar pressure and prevent the plantar ulceration in in diabetes patients. We investigated whether protective sock with functional insoles reduce plantar pressure while walking in 17 diabetes patients. Methods: An in-shoe measurement device was used to measure the peak plantar pressure while walking. Peak plantar pressure data were collected while walking under two conditions: 1) wearing diabetic sock and 2) wearing the protective sock with functional insoles. Each subject walked 3 times in 10-m corridor under three conditions, and data were collected in 3 steps in the middle of corridor with in right and left feet, respectively. Pared t-test was used to compare the peak plantar pressures in three plantar areas under these two conditions. Results: The protective sock with functional insoles significantly reduced the peak plantar pressure on the lateral rearfoot, but significantly increased the peak plantar pressure on the middle forefoot, and medial midfoot (p<0.05). However, there were not significant in medial and lateral forefoot, lateral midfoot, and medial rearfoot between diabetic sock and the protective sock conditions (p>0.05). Conclusion: The protective sock with functional insoles reduced plantar pressures in the rearfoot and supported the medial longitudinal arch. However, it is necessary to change the position of metatarsal pad in the insole design of forefoot area to prevent diabetic foot ulceration.

당뇨병의 연구 경향에 대한 고찰 -한의 학술 논문 검색을 중심으로- (The Review on the Study of Diabetes Mellitus in Oriental Medicine Journals)

  • Objectives : The purpose of this study is to research trends in the study of diabetes mellitus and to establish the future direction for its study. Methods : We reviewed oriental medicine papers published in the last twenty-three years(1980-2002). Results : 1. We researched 94 papers in 14 journals and the patterns of study were as follows: experimental studies were 53 (56.3%), reviewed studies were 20 (21.2%), clinical studies were 12 (12.7%) and case reports were 9 (9.8%). 2. The experimental studies (53) were divided into papers on efficiency testing of herbal medications (40) and acupuncture & moxibustion (13). 3. The reviewed studies showed that research has been carried out in the fields of cause, pathogenesis, principles for treating disease, acupuncture and moxibustion, herbal medication, exercise, and relation of sasang (사상) to diabetes mellitus. 4. The clinical studies showed that the research was carried out in the fields of follow up surveys for herbal medication efficiency testing, the relativity of diabetes mellitus to other diseases and sasang (사상). 5. In consequence of researching the case reports, there was no common prescription used meaningfully. Conclusions : We need more studies of diabetes mellitus by reaching for specific therapy.

소아청소년기 당뇨병성 신병증(I); 임상 소견을 중심으로 (Diabetic Nephropathy in Childhood and Adolescence (I) : Clinical Features)

  • 소아청소년기의 당뇨병은 대부분 제1형 당뇨병이나 최근 우리나라를 포함한 서구 사회에서는 제2형 당뇨병의 빈도가 증가하고 있다. 임상소견 상 제1형 당뇨병은 여러 위험인자에 의하여 비교적 전형적인 단계를 거치면서 미세알부민뇨와 당뇨병성 신병증으로 진행하면서 만성 신질환으로 발전하게 되며, 제2형 당뇨병은 비전형적 임상경과를 거치나 신병증 진행율이 높아서, 실제로 당뇨병성 신병증은 전세계 신장대체요법이 필요한 말기 신질환의 가장 많은 원인이며 국내에서도 꾸준히 원인 질환으로서 증가 중이다. 당뇨병이 사춘기 전에 발생하는 경우보다 사춘기나 그 이후에 발생하는 경우에 혈관합병증의 발생이 증가하므로, 사춘기가 위험인자로 작용하며, 이것은 유병기간과 함께 사춘기 전에 소아 당뇨병성 신병증이 발생하는 경우는 매우 드문 이유이다. 제1형과 제2형 당뇨병에서 신병증은 비슷하게 15-25%에서 발병하며, 당뇨병성 신병증과 만성 신질환으로 진행하는 과정 중에 가장 중요한 표식자인 미세알부민뇨는 위험인자이고 병리학적 소견과 관련이 있다.

Cellular and Molecular Roles of $\beta$ Cell Autoantigens, Macrophages and T Cells in the Pathogenesis of Automimmune Diabetes

  • Type I diabetes, also known as insulin-dependent diabetes mellitus (IDDM) results from the destruction of insulin-producing pancreatic $\beta$ cells by a progressive $\beta$ cell-specific autoimmune process. The pathogenesis of autoimmune IDDM has been extensively studied for the past two decades using animal models such as the non-obese diabetic (NOD) mouse and the Bio-Breeding (BB) rat. However, the initial events that trigger the immune responses leading to the selective destruction of the $\beta$ cells are poorly understood. It is thought that $\beta$ cell auto-antigens are involved in the triggering of $\beta$ cell-specific autoimmunity. Among a dozen putative $\beta$ cell autoantigens, glutamic acid decarboxylase (GAD) has bee proposed as perhaps the strongest candidate in both humans and the NOD mouse. In the NOD mouse, GAD, as compared with other $\beta$ cell autoantigens, provokes the earliest T cell proliferative response. The suppression of GAD expression in the $\beta$ cells results in the prevention of autoimmune diabetes in NOD mice. In addition, the major populations of cells infiltrating the iselts during the early stage of insulitis in BB rats and NOD mice are macrophages and dendritic cells. The inactivation of macrophages in NOD mice results in the prevention of T cell mediated autoimmune diabetes. Macrophages are primary contributors to the creation of the immune environment conducive to the development and activation of $\beta$cell-specific Th1-type CD4+ T cells and CD8+ cytotoxic T cells that cause autoimmune diabetes in NOD mice. CD4+ and CD8+ T cells are both believed to be important for the destruction of $\beta$ cells. These cells, as final effectors, can kill the insulin-producing $\beta$ cells by the induction of apoptosis. In addition, CD8+ cytotoxic T cells release granzyme and cytolysin (perforin), which are also toxic to $\beta$ cells. In this way, macrophages, CD4+ T cells and CD8+ T cells act synergistically to kill the $\beta$ cells in conjunction with $\beta$ cell autoantigens and MHC class I and II antigens, resulting in the onset of autoimmune type I diabetes.

Design and Implementation of Salivary Electrical Stimulator for xerostomia

  • After 40 years of age, the saliva glands are aged and the saliva is not made enough to cause xerostomia symptoms. Side effects such as hypertension medication or diuretics that the elderly take mainly can cause xerostomia syndrome. In addition, autoimmune diseases, diabetes, anemia, depression and other common diseases that cause xerostomia symptoms. If the saliva secretion is insufficient, tooth decay and gum disease are likely to occur, and the digestive ability of the saliva is also reduced due to the lack of amylase, which is a digestive element. Once the degenerated salivary gland is restored to its normal state, it is difficult to recover. In this paper, we give electrical stimulation to the masseter which is in contact with the large pituitary gland, and stimulate the salivary gland to the utmost by using speech recognition using words corresponding to oral gymnastics. Use the STM32F407VG to implement a system to relieve xerostomia.

Remission of Progesterone-induced Diabetes Mellitus after Ovariohysterectomy in an Intact Female Dog

  • A 10-year-old, 8.28 kg, intact female Cocker Spaniel was presented with continuous polyuria and polydipsia. The dog had proestrus bleeding 5 weeks earlier, and hyperglycemia, glucosuria, ketouria, and high level of serum fructosamine in laboratory findings. Based on hyperglycemia, glucosuria, ketouria, and ketosis, the patient was tentatively diagnosed as diabetes mellitus (DM) with ketouria. After diagnosis, 5 to 7 U/body porcine lente insulin was administered during 11 days from initial presentation as the remission of DM for the dog. But, blood glucose was still high level. Because there was no reaction to porcine lente insulin, it was replaced by 4-10 U/body neutral protamine Hagedorn (NPH) during 3 days. But, NPH also did not regulate blood glucose level. Because insulin therapy failed to regulate blood glucose level, the dog was considering insulin-resistant diabetes. The dog was tentatively diagnosed with progesterone-resistant DM on the basis of the history that had revealed proestrus bleeding 5 weeks earlier. Progesterone level was moderate high (43.7 ng/ml; reference range, 15.0-90.0 ng/ml). Ovariohysterectomy (OHE) was performed to remove the cause of the dog's diabetes. After OHE 11 days, blood glucose was gradually declined by insulin treatment. Consequently, blood glucose was well controlled in reference range without insulin treatment after 2 months. This case is a report on progesterone-induced DM treated with OHE and insulin treatment during the diestrus.

복합만성질환의 융합적 관련요인에 관한 연구 (Factors associated with Multiple Chronic Diseases among One Metropolitan Citizens)

  • 이 연구는 일개 광역시 지역의 2015년도 지역사회건강조사 응답자 4,590명의 자료를 분석하였다. 만성질환의 대표질환인 고혈압, 당뇨, 복합만성질환자에 대한 단변량 및 다변량 분석을 실시하였다. 주요 연구결과로 복합만성질환과 유의한 관련변수로 학력, 소득, 소금섭취 및 BMI 등이 이었으며, 고혈압은 학력, 소득, 소금섭취 등이었고, 당뇨는 연령, 학력, 음주빈도, BMI 등 이었다. 복합만성질환과 고혈압, 당뇨는 인구, 사회통계적 특성과 건강행태적 특성, 건강관리적 특성에 따라 유의한 차이를 있음을 재확인하였다. 추후 복합만성질환 및 고혈압, 당뇨 등의 만성질환의 예방관리프로그램 기획과 정책개발에 이러한 점들을 고려해야 할 것이다.

단삼투여가 고지방식이로 유발된 비만형 당뇨병 동물모델에 미치는 영향 (The Effects of Salvia miltiorrhiza on High Fat Diet-induced Obese Diabetic Mouse Model)

  • Objectives : Obesity is an important cause of diabetes, and lipotoxicity causes insulin resistance. In this study, we investigated the effects of Salvia miltiorrhiza on high fat diet-induced obese type 2 diabetic mouse models. Methods : Diabetes was induced in ICR male mouse (23~25 g) with Surwit's high fat, high sucrose diet. Mice were divided into 4 groups (n=10) of normal, control, Salvia miltiorrhiza, and metformin. After 8 weeks, body weight, OGTT, fructosamine, lipid profile, serum level of adiponectin and leptin, epididymal fat pad, liver weight and epididymal adipocyte size were measured. Results : Salvia miltiorrhiza significantly reduced oral glucose tolerance levels, fructosamine serum level, epididymal fat weight, and epididymal adipocyte size. Salvia miltiorrhiza also increased HDL-cholesterol, adiponectin and leptin serum levels. Conclusions : These results show that Salvia miltiorrhiza improves insulin resistance. Therefore we suggest that Salvia miltiorrhiza would be an effective treatment for obese type 2 diabetic patients.

Diabetes affects Peripheral Nerve and Heart Function

  • Diabetes mellitus (DM) leads to a variety of complications and thus we have retrospectively studied to investigate problems of nerve conduction velocity (NCV) study and the heart in the patients with type-II DM. Blood glucose and blood pressure levels were higher in DM group than in Non-DM group. We found that several latencies were delayed in motor conduction study of upper (median and ulnar nerve) and lower extremities (peroneal and tibial nerve), whereas amplitudes and NCVs were decreased in DM group compared with Non-DM group. Latencies of sensory conduction study in upper and lower extremities (sural nerve) were delayed, while amplitudes and NCVs were lower in DM group than in Non-DM group. Abnormal percent of the electrocardiogram was higher in DM group than in Non-DM group. This retrospective study suggests that type-II DM can cause a damage effect on the peripheral nerve and the heart function.

Type 1 diabetes genetic susceptibility markers and their functional implications

  • Type 1 diabetes (T1D) is a chronic autoimmune disease characterized by selective destruction of pancreatic ${\beta}$-cells resulting in insulin deficiency. The genetic determinants of T1D susceptibility have been linked to several loci, in particular to the human leukocyte antigen (HLA) region, which accounts for 50% of the genetic risk of developing T1D. Multiple genes in the HLA region, which are in strong linkage disequilibrium, are thought to be involved. Another important locus, with a smaller effect on genetic predisposition to T1D, is the insulin gene. The advent of numerous single nucleotide polymorphism markers and genome screening has enabled the identification of dozens of new T1D susceptibility loci. Some of them appear to predispose to T1D independently of the HLA and may be important in families with T1D who lack strong HLA susceptibility. Other loci may interact with each other to cause susceptibility. The autoimmune response against ${\beta}$-cells can also be triggered by environmental factors in the presence of a predisposing genetic background. Deciphering the environmental and genetic factors involved should help to understand the origin of T1D and aid in the design of individualized prevention programs.
