Through so that accident of semiconductor industry deduces unsafe factor of the person center on unsafe behaviour that incident history and questionnaire and I made starting point that extract very important factor. It served as a momentum that make up base that analyzes factors that happen based on factor that extract factor cause classification for the first factor, the second factor and the third factor and presents model of human error. Factor for whole defines factor component for human factor and to cause analysis 1 stage in human factor and step that wish to do access of problem and it do analysis cause of data of 1 step. Also, see significant difference that analyzes interrelation between leading persons about human mistake in semiconductor industry and connect interrelation of mistake by this. Continuously, dictionary road map to human error theoretical background to basis traditional accidental cause model and modern accident cause model and leading persons. I wish to present model and new model in semiconductor industry by backbone that leading persons of existing scholars who present model of existent human error deduce relation. Finally, I wish to deduce backbone of model of pre-suppression about accident leading person of the person center.

Reactor protection system to keep nuclear safety and operational economy of plants requires high reliability. Such a high reliability of the system can be achieved through the redundant design of components. However, common cause failures of components reduce the benefits of redundant design. Thus, the common cause failure analysis, to accurately calculate the reliability of the reactor protection system, is carried out using alpha-factor model. Analysis results to 24 operating months are that 1) the system reliability satisfies the reliability goal of EPRI-URD and 2) the common cause failure contributes 90% of the system unreliability. The uncertainty analysis using alpha factor parameters of 0.05 and 0.95 quantile values shows significantly large difference in the system unreliability.
In Everland Zoological Gardens, the mortality by extrinsic cause in non-human primates during 1976∼1999 were retrospectively analyzed based on the clinical charts and/or autopsy reports. The number of deaths from extrinsic factor was 61 among a total of 161 monkeys which were died during that period. Among 61 monkeys of death from extrinsic factor, the number at a detailed cause were as follows: strangulation, 17(27.87%); accident fall, 15(24.59%); suffocation, 13(21.31%); drowning, 7(11.48%); death from pressure, 2(3.28%); collision, 2(3.28%); sunstroke, 1(64%); starvation, 1(1.64%); freezing to death, 1(1.64%); contusion, 1(1.64%). The number of deaths from extrinsic factor was 39 among a total of 81 squirrel monkeys which were died during that period. Among 39 squirrel monkeys of death from extrinsic factor, the number at a detailed cause were as follows; suffocation, 11(28.21%); accident fall, 8(20.51%); strangulation, 7(17.95%); drowning, 7(17.95%); death from pressure, 2(5.13%); starvation, 1(2.56%); collision, 1(2.56%). The number of deaths from extrinsic factor was 14 among a total of 50 Japanese macaque died during that period. Among 14 Japanese macaque from extrinsic factor, the number at a detailed cause were as follows; strangulation, 7(50.55%); accident fall, 6(42.85%); suffocation, 1(7.14%). It was considered that far facilities, adequate space and suitable indoor temperature are needed for the prevention of deaths of extrinsic cause at the monkey raising in zoological gardens or research center.
To develop a Root Cause Analysis Map which determines the cause of the accident in chemical laboratory, The Root Cause Analysis(RCA) Map for the laboratory areas was sketched from Phase 1 of the accident element to Phase 3 of the accident element, based on the RCA Map which is applied in the petrochemical industry. On the basis of laboratory RCA Map which was classified by using such method. The root causes of the 211 accident cases in laboratories were classified from Phase 4 to Phase 5 by the Cause Factor Charting technique and The cause of the accident data were inputted to EXCEL program. After that, The causes of the accident data were sorted and classified by type and each step. So 'Approximate Primary RCA Map Draft' was written. In addition, it was reaffirmed whether the root causes of 211 accidents of laboratory were appropriate to 'Primary RCA Map Draft'. By complementing the cause which was expected to cause future accidents, the RCA Map for chemical laboratories was developed. Based on 'RCA Map' proposed in this study, the causes of accidents were analysed management systems 35%, monitoring 12.2%, Human Factor Eng. 15.1% and education training 12.1% by the size of the frequency from Phase 1 to Phase 5.

The study on semiconductor industrial accident in korea has been focused on frequencies of each type, employee, characteristics, cause and un-safety condition, behaviour and so on. Those attributes of semiconductor industrial accidents were usually analyzed independently, so that it was hard to provides a well-process and systematic guide lines for efficient safety management. There fore, there were a few studies based on comprehensive survey in terms of sharp-type of safe management. The questionnaire survey carried out for the workers(284) who were responsible for safety management in to center with corporate company with semiconductor industry the factor analysis showed that there were three factor of safety management. They were 1) Investment and operation and management for accident prevention, 2) Unsafe, safety management 3) General human error and behavior the industries of respondents were correlative with three group. Three Groups showed a statistically significant differences on the number of cases. Actually, the group with the larger investment and the better unsafe cause, human error a of accident prevention had a smaller cause of accident cases.

Enlargement and speed-up of a ship and diversification of ship's type have served to greatly increase the importance of marine transport means. It's reported that accident occurrence frequency of collision is high next to engine damage among the ship accident types, and that the accident ratio according to human factors is also high. In addition, ship accidents come to occur caused by complex cause factors rather than a sole cause factor, it is necessary to investigate the cause factors through the written verdict. This study proposed the cause factors of collision ship accident on the basis of human factors in collision ship accident among the written verdicts provided by the Korean Maritime Safety Tribunal, and inquired into the cause factor and effect through the correlation analysis of accident occurrence factors. Also, this study predicted the collision accident through analyzed the major cause factor of the occurrence at the zero minute when collision on the basis of the time taken from the time point of detecting collision of ships to the time point of collision occurrence. This study used commercial software-Statistical Package for Social Sciences (SPSS Ver21.0) to do correlation analysis. For time analysis, this study analyzed the cause factor and time by analyzing the time taken from the time point of detected ships to the time point of collision occurrence on the basis of the written verdicts. The study analysis showed that there were many cases of collision ship accidents occurrence caused by more than two sorts of cause factors, and that the case (zero minute) where there is no time to spare for collision avoidance accounted for 36.1 %, and negligence in guard or surveillance of the other ship, and sailing while drowsy, or drinking was a contributor to an accident. Poor watch keeping is very strong relationship with pool ready for sail.

Based on the results of Probabilistic Safety Assessment(PSA) for a Nuclear Power Plant (NPP), Common Cause Failure(CCF) events have been recognized as one of the main contributors to the risk. Also, the CCF data and estimation method used in domestic PSA models have been pointed out as an issue with respect to the quality. The existing method of MGL and non-staggered testing even widely used were considered conservative in estimating the safety and had a limited capability in uncertainty analyses. Therefore, this paper presents the CCF estimation using a new generic data source and Alpha factor method. The analyses showed that Alpha factor and staggered method are effective in estimating the CCF contribution and risk insights of reference plant. This method will be a common bases for the optimization of new design for the construction plants as well as for the updating of safety assessment on the operating nuclear power plants.
Background: This study used the receiver operating characteristic curve (ROC) to analyze Surveillance, Epidemiology and End Results (SEER) bronchioaveolar carcinoma data to identify predictive models and potential disparity in outcomes. Materials and Methods: Socio-economic, staging and treatment factors were assessed. For the risk modeling, each factor was fitted by a Generalized Linear Model to predict cause specific survival. The area under the ROC was computed. Similar strata were combined to construct the most parsimonious models. A random sampling algorithm was used to estimate modeling errors. Risk of cause specific death was computed for the predictors for comparison. Results: There were 7,309 patients included in this study. The mean follow up time (S.D.) was 24.2 (20) months. Female patients outnumbered male ones 3:2. The mean (S.D.) age was 70.1 (10.6) years. Stage was the most predictive factor of outcome (ROC area of 0.76). After optimization, several strata were fused, with a comparable ROC area of 0.75. There was a 4% additional risk of death associated with lower county family income, African American race, rural residency and lower than 25% county college graduate. Radiotherapy had not been used in 2/3 of patients with stage III disease. Conclusions: There are socio-economic disparities in cause specific survival. Under-use of radiotherapy may have contributed to poor outcome. Improving education, access and rates of radiotherapy use may improve outcome.
This study puts an emphasis on the importance clothing and its reuse, dealing with the unmarried women workers who will be main domestic consumers. Questionnaire survey method was used in this research. The sample was taken from 546 unmarried women workers. The major findings are summarized as follows, it was found that Factor I was the cause of unplanned clothing management, Factor II was the cause of clothing modification and body type changes and Factors III was the cause of fashionmongering. Second, it was shown that as for experiences in hoarded clothing recycling disposition behavior subjective social strate had the high level, the group of sale and technical jobs / self-employed jobs had the higher level than the group of specialized skil / management jobs, the group in office-going and leaving clothing had the higher level than the group of people in separate uniform as working dress and the group of those experienced in environmental education had the high level.
